1.Association between exposure to entertainment screen content on mobile phones and symptoms of anxiety-depression co-morbidity among college students

Objective:
To determine the association between exposure to entertainment screen content on mobile phones and symptoms of anxiety-depression co-morbidity among college students,so as to provide evidence for mental health interventions.
Methods:
A baseline survey was conducted from April to May 2019. A total of 1 135 college students were selected from one university each in Shangrao City, Jiangxi Province and Hefei City, Anhui Province using cluster random sampling method. A follow up study was conducted in November 2019, resulting in 1 110 matched valid responses. Self rating questionnaires were used to assess the exposure of entertainment screen content. The Depression Anxiety Stress Scale-21(DASS-21) and the Patient Health Questionnaire-9 (PHQ-9) were used to evaluate the anxiety symptoms, depressive symptoms, and symptoms of anxiety-depression co-morbidity among college students. A multivariate binary Logistic regression model was constructed following initial intergroup comparisons with Chi-square test to determine the association between baseline exposure to mobile entertainment screen content and the risk of symptoms of anxiety depression co-morbidity at baseline and the 6 month follow up.
Results:
The prevalence rates of symptoms of anxiety-depression co-morbidity among college students were 25.4% and 20.6% at baseline and follow up, respectively.After adjusting for confounding factors such as gender, self rated family economic status and self rated health status, the results of multivariate binary Logistic regression analysis showed that compared with the appropriate exposure level group, the exposure of entertainment screen content on mobile phones at baseline, including frequent exposure to reading( OR =1.65,95% CI =1.14-2.39), occasional exposure to other entertainment screen content ( OR =1.46,95% CI =1.01-2.10)and frequent exposure to other entertainment screen content( OR =1.76,95% CI =1.20-2.60), increased the co-occurrence risk of symptoms of anxiety-depression co-morbidity among college students during the follow up period (all P <0.05).
Conclusion
Occasional or frequert exposure to mobile entertainment screen content can increase the risk of symptoms of anxiety depression co-morbidity among college students.
2.Axillary management after neoadjuvant therapy for breast cancer and optimization of sentinel lymph node diagnosis and treatment

The axillary management of breast cancer patients after neoadjuvant therapy(NAT)is undergoing optimization.Sentinel lymph node biopsy(SLNB)has become an important means for evaluating the status of axillary lymph node(ALN),especially in early-stage breast cancer patients with initially clinically negative ALN(cN0),where it can replace traditional ALN dissection(ALND)to reduce unnecessary surgical risks and complications.However,SLNB has some limitations in terms of false negative rate(FNR)and variability when applied to breast cancer patients with initially clinically positive ALN(cN+)after NAT.By removing≥3 SLN,using dual tracers(such as radioactive isotopes combined with blue dye),and conducting pathological assessment in combination with immunohistochemistry(IHC),the FNR can be significantly reduced to an acceptable range(4.9%-9.1%).Moreover,various optimization schemes have been developed,such as marking ALN with radioactive iodine(MARI),targeted axillary dissection(TAD),and radioactive iodine seed placement in the axilla with SLNB(RISAS),all of which demonstrate low FNR.Not only that,non-invasive imaging techniques such as positron emission tomography and computed tomography(PET/CT),magnetic resonance imaging(MRI),conventional ultrasound and contrast-enhanced ultrasound(CEUS)can all be used to evaluate the axillary response after NAT,with varying diagnostic efficacies.This article summarized recent studies on the optimization of axillary management and SLNB diagnosis and treatment for breast cancer patients with cN+after NAT.
3.Establishment and optimization of combined model of influenza and wind-heat syndrome in mice

Objective To establish a mouse model of H1N1 influenza wind-heat syndrome by combining climate intervention with influenza virus nasal drops.Methods Seventy-two BALB/c mice were divided randomly into nine groups:a Control group,wind-heat(FR)groups(FR-3Day,FR-5Day),and Model groups(1LD-3Day,2LD-3Day,3LD-3Day,1LD-5Day,2LD-5Day,2LD-5Day,3LD-5Day)(n=8 mice per group).Mice in the Control group were housed in a normal environment,while mice in the FR and Model groups were kept in wind-heat conditions for 7 d.Mice in the Model groups received nasal PR8 influenza virus infection on the 8th day,and mice in the Control and FR heat groups received equal amounts of physiological saline nasal drops.After virus challenge,each group was housed in a normal environment and samples were taken on days 3 and 5.The appearance of the mice was observed and recorded and the lung index,routine blood parameters,lung tissue pathology,serum interleukin(IL)-6 levels,and virus titers were detected in each group based on their behavioral status,stools,and body temperature.Results After 7 d of wind-heat intervention,mice in the FR groups showed no significant abnormalities in terms of appearance,stools,body temperature,routine blood parameters,or lung tissue pathology compared with the Control group.The appearance,lung index,red blood cell count,hemoglobin,hematocrit,pathological result,and body temperature in the Model groups worsened progressively with increasing time and toxin dosage,while the neutrophil percentage,lymphocyte percentage,virus titer,and serum IL-6 levels peaked on day 3 after viral attack,for the same viral dose,and then decreased slightly on day 5.Conclusions PR8 nasal drops and 7 d of wind-heat climate intervention can be used to establish a mouse model of influenza wind-heat syndrome.
4.Research on high-throughput detection of plasma cell-free DNA for targeted therapy-related genes screening and prognosis prediction in non-small cell lung cancer patients

Background and purpose:High-throughput detection of plasma cell-free DNA(cfDNA)is widely used for multi-cancer targeted therapy drug screening,and this study investigated the relationship between the type and number of plasma cfDNA class Ⅰ and Ⅱ targeted therapy-related gene variants and cancer survival in patients with non-small cell lung cancer(NSCLC).Methods:The sequencing results and clinical data of NSCLC patients who underwent tumor plasma cfDNA high-throughput sequencing projects in Sun Yat-sen University Cancer Center from 2021 to 2023 were collected.The survival follow-up of enrolled patients was carried out from the day of plasma collection on June 1,2021 to May 27,2024,and GraphPad Prism 8.0 and SPSS Statistics 25.0 were used.Univariate and multivariate statistical analyses were conducted on the types and numbers of class Ⅰ and class Ⅱ targeted therapy-related genes in the survival and clinical data of patients and sequencing results(Ethical approval:B2024-359-01).Results:A total of 313 patients included in this study with NSCLC were categorized into stage Ⅰ 25 patients(7.98%),stageⅡ 20 patients(6.39%),stage Ⅲ 38patients(12.14%),and stage Ⅳ 230 patients(73.48%).Pathological diagnosis results showed that adenocarcinoma accounted for 90.10%,squamous cell carcinoma accounted for 5.11%,large cell carcinoma accounted for 2.87%and other classifications accounted for 1.92%.The number and the percentage of class Ⅰ and class Ⅱ targeted therapy drug-related genes in the plasma cfDNA NSCLC patients were 0(25.24%),1(17.57%),2(19.17%),3(14.38%),4(8.31%),and 5 or more(15.34%).The results of statistical analysis showed that 3 genes with the highest mutation frequencies were EGFR,TP53 and ERBB2,and the mutation frequency of EGFR gene was 36.04%.The mutation frequency of TP53 gene was 30.63%.The mutation frequency of ERBB2 gene was 4.95%.The survival time of patients is related to not only the expression of hotspot targeted genes,but also the number of class Ⅰ and Ⅱ target-related gene variants detected by plasma cfDNA high-throughput sequencing.The survival time of the patients with no targeted therapy-related locus variants after treatment was longer compares with targeted therapy-related locus variants,which can reduce the risk of death by 63.2%.However,patients with a single gene locus variant had longer survival time and lower risk of death than those with multiple driver locus variants,and the measured class Ⅰ and Ⅱ targeted therapy drugs were within 3 genes.Overall,the smaller the number of genes,the longer the survival.Conclusions:The number of class Ⅰ and class Ⅱtargeted therapy-related gene variants in plasma cfDNA high-throughput sequencing also has an effect on the survival of patients after treatment.Plasma cfDNA level detected by high-throughput sequencing could be a prognostic factor for the NSCLC patients.

Coronary heart disease with angina pectoris,characterized by myocardial ischemic injury as its fundamental pathological mechanism,represents a prevalent cardiovascular condition.The"State-Target Differential Diagnosis and Treatment"presents a holistic regulatory framework,integrating macroscopic state regulation with microscopic targeting.Guided by this approach,the pathological evolution of coronary heart disease is examined through four dimensions:"state-target-cause-effect."The"cause"encompasses both the pathogenesis and etiological factors of traditional Chinese and Western medicine.The"effect"manifests as adverse cardiovascular events,including myocardial infarction and heart failure.The"state"delineates the progressive development pattern of qi to blood to deficiency,beginning with qi stagnation and cold congealment in the initial stage,followed by blood stasis and phlegm obstruction in the intermediate stage,and culminating in qi-blood deficiency in the advanced stage.The"target"encompasses multi-level therapeutic interventions addressing both symptomatic manifestations and clinical indicators.Building on this theoretical foundation,this research focuses on the pattern of qi sinking and blood stasis commonly observed in late-stage angina,systematically elucidating its state regulation and targeting therapeutic strategies.Using the clinical empirical formula Shengxian Quyu Decoction as the baseline state prescription,an in-depth investigation was conducted to determine optimal combination patterns of symptom-and biomarker-targeted medications.This study aims to establish a modernized differential treatment system for angina pectoris with the pattern of qi sinking and blood stasis,providing novel research perspectives and theoretical foundations for enhancing clinical efficacy and reducing the risk of cardiovascular events.
6.Comprehensive clinical evaluation of Shuangdan Mingmu Capsule in diabetic retinopathy treatment

Objective To evaluate the clinical evidence and overall value of Shuangdan Mingmu Capsule in treating diabetic retinopathy(DR),highlight its advantages and characteristics,clarify the exact clinical positioning for DR,and propose a reference that can aid the National Medical Management Department in its decision-making.Methods This study combined qualitative and quantitative evaluation methods.Multiple method ologies were used to analyze the"6+1"dimensions.Each dimension was measured using the Multi-criteria Decision Analysis Model and calculated using CSC software V2.0.Results SRS monitored since the launch of Shuangdan Mingmu Capsule were general adverse reactions and those reported in the literature;phase Ⅱ,Ⅲ,and Ⅳ clinical studies primarily focused on the gastrointestinal system,no serious adverse reactions were reported,and the safety was rated as A.When Shuangdan Mingmu Capsule was combined with calcium hydroxybenzenesulfonate or laser photocoagulation in the treatment of DR in a meta-analysis,it further enhanced ophthalmologic parameters such as macular thickness,hemorrhagic spot area,leakage area,best-corrected visual acuity,and the degree of visual field defect.It also increased serum vascular endothelial growth factor and platelet-derived growth factor levels,the scores from the low vision quality of life scale and ultrasound indexes,and the validity was rated as grade A.A pharmacoeconomic evaluation study of Shuangdan Mingmu Capsule combined with calcium hydroxybenzenesulfonate compared to calcium hydroxybenzenesulfonate alone for DR treatment indicated that assuming the per capita disposable income in 2023 was applied as the willingness-to-pay threshold for patients,with an ICER of 3,730.98—which is approximately 2.85%of the per capita disposable income in 2023—the treatment regimen of Shuangdan Mingmu Capsule combined with calcium hydroxybenzenesulfonate was more economical than the calcium hydroxybenzenesulfonate treatment plan alone.The economy was rated as grade B.Therefore,Shuangdan Mingmu Capsule can meet clinical needs,and the formula includes medicines to tonify the liver and kidney,activate blood,and clear the channels,which is a combination of both symptomatic and fundamental treatments.Shuangdan Mingmu Capsule can also tonify the kidney,nourish the liver,stimulate blood circulation,and improve eyesight,and it is rated as grade A for innovativeness.Therefore,the drug is widely used,has no special technical and administrative requirements,is convenient to carry,and does not need to be administered at special times.The comprehensive questionnaire survey indicated that the appropriateness of the drug is good(grade B),that the production capacity is sufficient,and that the accessibility has been rated as grade B.This drug originates from ancient prescriptions of Er Zhi Pill and Liuwei Dihuang Pill,which have a long-standing reputation in traditional medicine.The combination of disease and evidence embodies the Chinese medicine characteristics of holistic concept and identification of evidence-based treatment,and the special features of Chinese medicine are rated as grade A.Conclusion The clinical value of Shuangdan Mingmu Capsule was evaluated as Class A by synthesizing the"6+1"dimensions and calculated using CSC V2.0 software.The clinical value of Shuangdan Mingmu Capsule is Class A.However,clinical needs must be focused upon and transformed into relevant clinical medication policies,providing a strong reference for the efficacy research of clinical trials.
7.Clinical characteristics of co-infection of Talaromyces marneffei and non-tuberculous Mycobacterium in HIV-negative patients

Objective To analyze the clinical characteristics of co-infection of Talaromyces marneffei(TM)and non-tuberculous Mycobacterium(NTM)in human immunodeficiency virus(HIV)-negative patients.Methods Clinical data of 8 HIV-negative patients with co-infection of TM and NTM in a hospital from 2019 to 2022 were co-llected.Clinical manifestations,auxiliary examination,treatment and prognosis were retrospectively analyzed.Results Among the 8 patients,5 were females and 3 were males,with an average age of(52.25±12.31)years old.All patients presented TM and NTM disseminated infection.The major involved organs were lung(100%),lymph nodes(87.5%),and skin(75.0%).Clinical symptoms mainly included cough and expectoration(87.5%),fatigue(62.5%),joint and lumbosacral pains(62.5%),fever(50.0%),as well as skin and soft tissue abscess(50.0%),etc.Anti-interferon-γ(INF-γ)autoantibodies were detected in 4 patients and the results were positive.All 8 patients(100%)had pulmonary lesions,with chest CT mainly showing spots,patches,and striped shadows in both lungs.Among them,7 cases(87.5%)had increased and enlarged mediastinal lymph nodes,4 cases(50.0%)had pleural thickening and pleural effusion,2 cases each(25.0%for each)were accompanied by pulmonary mass shadows,bronchial stenosis,as well as increased and enlarged hilar lymph nodes.One case each(12.5%for each)had pulmonary cavity formation,bronchiectasis,and pericardial effusion.Conclusion The co-infection of TM and NTM in non-HIV patients presents disseminated infection,with multiple clinical symptoms.Chest imaging shows a wide variety of pulmonary lesions.It is prone to miss diagnosis in clinic,and the effect is not ideal after treatment for single pathogen infection.
8.A follow-up study on the bidirectional associations between smartphone multitasking behavior and comorbid symptoms of anxiety and depression among college students

Objective:To explore the direction of the association between smartphone multitasking behavior and comorbid symptoms of anxiety and depression (CAD) among college students.Methods:College students from one college located in Shanxi, Chongqing, and Shenzhen were selected between October and December 2021 using a multistage random cluster sampling method, and a follow-up visit was conducted in May 2022. The Assessment of Smartphone Multitasking for Adolescents, the Patient Health Questionnaire-9 Items, and the Generalized Anxiety Disorder Questionnaire-7 Items were used to assess the smartphone multitasking behaviors and CAD of college students. Multivariate logistic regression models were used to analyze the associations between smartphone multitasking behavior and CAD. Autoregressive cross-lagged models (ARCLM) were used to analyze the bidirectional associations between smartphone multitasking behavior and CAD.Results:A total of 953 college students were included in this study, including 323 (33.9%) males and 630 (66.1%) females, with an age of (18.89±1.33) years. The detection rates of CAD at baseline and follow-up were 25.5% and 27.5%, respectively. The mean of the total smartphone multitasking index at baseline was 2.11±0.48. The mean of smartphone and non-media activity multitasking index was 2.00±0.55. The mean of smartphone and other media activity multitasking index was 1.92±0.67. The mean of smartphone functional use multitasking index was 2.18±0.54. The multivariate logistic regression models showed that smartphone and non-media activity multitasking index ( OR=1.85, 95% CI: 1.39-2.45), smartphone and other media activity multitasking index ( OR=1.42, 95% CI: 1.10-1.83), and total smartphone multitasking index ( OR=1.66, 95% CI: 1.21-2.28) were positively associated with the risk of CAD among college students at baseline. The smartphone and non-media activity multitasking index ( OR=1.74,95% CI: 1.30-2.32), smartphone and other media activity multitasking index ( OR=1.37, 95% CI: 1.04-1.79) and total smartphone multitasking index ( OR=1.48, 95% CI: 1.06-2.06) were positively associated with the risk of CAD at follow-up. There was no statistical association between smartphone functional use multitasking index at baseline and CAD. The ARCLM showed that smartphone and non-media activity multitasking index ( β=0.974, P=0.029), smartphone and other media multitasking index ( β=0.798, P=0.003), and the total smartphone multitasking index ( β=1.379, P=0.011) at baseline could positively predict the risk of CAD at follow-up. The CAD at baseline could positively predict smartphone and other media multitasking index ( β=0.004, P=0.016) and smartphone functional use multitasking index at follow-up ( β=0.004, P=0.016). Conclusion:There is a bidirectional association between smartphone and other media activity multitasking and CAD among college students.

10.Analysis of Vaginal Microflora Examination Results in 19322 Initial Visit Infertil-ity Women

Objective:To investigate the characteristic distribution of vaginal microbiota in infertile women.Methods:We collected the results of vaginal microbiological examinations from 19322 initial visit infertile women who visited the Reproductive Medicine Center of the Department of Obstetrics and Gynecology,the Second Affili-ated Hospital of Air Force Medical University from March 1,2023 to July 31,2024.The vaginal microbiota infection status of patients was compared in different age groups(<25 years old,25-<30 years old,30-<35 years old,35-<40 years old,≥40 years old)and different seasons(spring,summer,autumn,winter).Results:①Among 19322 women,6027 cases(31.19%)showed abnormal vaginal microecology.Pathogenic microorganisms were detected in 3093 cases,including 2882 cases of single vaginitis,211 cases of mixed vaginitis,3764 cases with vagi-nal cleanliness grade Ⅲ-Ⅳ,and 3965 cases with abnormal lactobacilli.Among patients with single vaginitis,1349 cases(46.81%)were diagnosed with vulvovaginal candidiasis(VVC),which was the highest proportion.Aerobic vaginitis(AV)followed with 752 cases(26.09%),and bacterial vaginosis(BV)had 671 cases(23.28%),trichomonal vaginitis(TV)with 110 cases(3.82%).Among patients with mixed vaginitis,AV+BV was the most common with 96 cases(45.49%).②The detection rates of cleanliness grade Ⅲ-Ⅳ,abnormal lactobacilli,abnor-mal microorganisms(unclear pathogen),single vaginitis(BV,VVC and TV),and mixed vaginitis showed statisti-cally significant differences across different seasons(P<0.05).Specifically,the detection rates of cleanliness grade Ⅲ-Ⅳ and abnormal microorganisms(unclear pathogens)were significantly higher in autumn than in other seasons(P<0.05),while the detection rate of abnormal lactobacilli was higher in spring than in other seasons(P<0.05).③The detection rates of abnormal lactobacilli,abnormal microorganisms(unclear pathogen),single vaginitis(BV,VVC and AV),and mixed vaginitis showed significant differences among different age groups(P<0.05).Specifically,the detection rate of abnormal microorganisms(unclear pathogen)was higher in the age group<25 years than in other age groups(P<0.05),while the detection rate of BV among single vaginitis cases was higher in the age group≥40 years than in other age groups(P<0.05).Conclusions:The vaginal microecol-ogy of infertile women varies in terms of infection rates across different age groups and seasons.Patients with simple vaginitis have the highest rate of VVC,while those with mixed vaginitis have the highest proportion of AV+BV infection.
