1.Neoadjuvant Sintilimab Combined with Gemcitabine and Cisplatin for Muscle-Invasive Bladder Cancer Patients Followed by Selective Bladder Sparing Surgery: A Phase 2 Trial

Purpose:
This study aimed to evaluate the safety and efficacy of gemcitabine and cisplatin (GP) regimen in combination with immune checkpoint inhibitor sintilimab as neoadjuvant therapy for muscle-invasive bladder cancer (MIBC) patients and the feasibility of the following selective bladder sparing surgery.
Materials and Methods:
Patients with histopathologically confirmed urothelial carcinoma without distant metastases (T2-4a, N ≤ 1, M0, American Joint Committee of Cancer 8th) and with adequate organ function will be enrolled. The therapeutic regimen was sintilimab 200 mg once on day 8, gemcitabine 1,000 mg/m2 and cisplatin 35 mg/m2 once on days 1 and 8, every 21 days for four cycles. The primary endpoint was pathologic complete response (pCR, pT0N0) rate. The secondary end points were ypT < 2 rate, R0 resection rate, event-free survival, and safety.
Results:
From May 4, 2020, to May 20, 2023, 55 patients were enrolled. Forty-six patients were evaluated for efficacy. Among the 42 patients who underwent surgery, 16 patients (38.0%) achieved pCR. Thirty-three patients (78.6%) achieved pT < 2. With a median follow-up of 15.7 months, the 1-year event-free survival was 91.3%. Notwithstanding the poor pathological baseline characteristic of a high T3-T4a proportion (39.1%), a promising bladder preservation (including 22 patients transurethral resection of bladder tumor, 5 patients partial cystectomy, and 4 surveillances) rate was achieved (67.4%). The most common grade ≥ 3 treatment-related adverse events was neutropenia (n=15, 27.3%), which was related to chemotherapy. There were no grade 3 immune-related adverse events.
Conclusion
Neoadjuvant GP plus sintilimab is a promising regimen for MIBC patients, with relatively high pT < 2 rate and triggering the emerging roles for the multi-disciplinary team decision-making for bladder sparing surgery.
2.Effectiveness of diabetes education via mobile application (t-manis) in improving glycemic control among patients with Type 2 Diabetes

Introduction:
Current education on diabetes self-management is time-consuming as it is conducted face-to-face. This study aimed to
evaluate the effectiveness of diabetes education via mobile application on hemoglobin A1c (HbA1c).
Methodology:
This was a double-blinded randomized controlled study conducted in the Department of Endocrinology, Hospital Pulau
Pinang and Hospital Raja Permaisuri Bainun. Participants were stratified randomized according to HbA1c to intervention
group (using T-Manis) and control group. The primary objective was to assess the difference in HbA1c between the groups.
Secondary objectives were to assess the difference in body weight, hospitalization related to diabetes, diabetes knowledge
between groups and usability and satisfaction of T-Manis. Laboratory investigations and questionnaires were conducted
at baseline, 4 months and 8 months.
Results:
A total of 112 participants were randomized into two groups each with 56 participants. Mean age was 51 years with similar
diabetes duration of 16 years in both groups. Participants comprised of 61.6% females with ethnic distribution of 49.1%
Malay, 30.4% Chinese, 19.6% Indian and 0.9% Siamese. Baseline HbA1c levels were comparable between intervention
and control groups (10.04% versus 9.91%; mean difference 0.13%; 95% CI −0.56 to 0.81; p = 0.718). At 4 months, HbA1c of
the intervention group demonstrated a significant reduction compared to the control group (8.93% versus 9.80%; mean
difference −0.86; 95% CI −1.59 to −0.14; p = 0.02). Sustained reduction of HbA1c in the intervention group was observed
at 8 months (mean difference −1.01; 95% CI −1.85 to −0.16; p = 0.02). Baseline body weight was 77 kg, with no betweengroup differences in body weight, hospitalization related to diabetes and diabetes knowledge scores, although diabetes
knowledge scores improved at 4 months in the intervention group. 89.3% of participants reported satisfaction with T-Manis
Conclusion
The T-Manis mobile application aided in significantly improving glycemic control with high user satisfaction, supporting
its feasibility as an adjunct to standard diabetes care.

3.Innovating a"three-dimensional integration"mechanism for fostering professional identity among newly contracted staff in traditional Chinese medicine hospitals under Party building leadership

In the new era,the requirements for Party building in public hospitals stipulate that the leadership of the Com-munist Party of China(CPC)should be deeply integrated into all fields and links of hospital governance,among which talent cul-tivation is one of the core areas.Aiming at the key dilemmas in the cultivation of professional identity among new employees in Traditional Chinese Medicine(TCM)hospitals,such as disconnected values,fragmented competencies,and disjointed culture,this study explores a systematic solution guided by Party building.With the Third Affiliated Hospital of Zhejiang Chinese Medical University as the research and practice setting,this study innovatively constructed a"content immersion-mechanism linkage-tech-nology empowerment"three-dimensional integration mechanism.It reshaped value identity through scenario-based cultural infil-tration,facilitated unimpeded pathways for professional competence development by relying on the Party organization resource co-ordination network,and enabled responsive and targeted service delivery through intelligent means.This mechanism transformed Party building from a one-way indoctrination model to an internally motivated identity-driven approach,which effectively enhanced new employees' sense of identification with the hospital's public welfare mission and TCM culture.This approach provides a repli-cable paradigm for cultivating TCM-featured talents under the guidance of Party building in public hospitals.
4.Application of a multicomponent exercise and cognitive stimulation program in elderly patients with mental disorders and sarcopenia

Objective To explore the effect of a multicomponent exercise and cognitive stimulation program in elderly patients with mental disorders and sarcopenia,so as to provide references for reducing the risk of falls,preventing and improving sarcopenia,and enhancing cognitive function in patients.Methods The multi-component exercise and cognitive stimulation program was formulated through literature review and expert meeting.In this quasi-experimental study,76 elderly patients with mental disorders and sarcopenia who were hospitalized in a tertiary mental health center in Shanghai from January 2023 to February 2024 were selected as research subjects.They were randomly divided into an experimental group and a control group according to their hospitalization building number(38 cases in each group).The experimental group was treated with multicomponent exercise combined with cognitive stimulation program based on routine nursing,and the control group was treated with routine nursing.The risk of falls,skeletal muscle mass,muscle strength,physical function,cognitive function,and incidence of adverse events were compared between 2 groups after 12 weeks of intervention.Results A total of 75 patients with 37 in the control group and 38 in the experimental group completed the study.The TUG time,6M walking speed and the score of Short Physical Performance Bettery of the experimental group were all lower than those of the control group(P<0.05),and the scores of skeletal muscle mass,muscle strength,calf circumference,physical function and cognitive function of the experimental group were all higher than those of the control group(P<0.05).Neither group experienced any adverse events.Conclusion The application of this multicomponent exercise combined cognitive stimulation program developed for elderly patients with mental disorders and sarcopenia can effectively reduce the risk of falls,enhance the skeletal muscle mass and muscle strength and improve the cognitive function in elderly patients with mental disorders and sarcopenia.
5.Characteristics and advantages in finite element analysis techniques in knee biomechanics

BACKGROUND:Finite element analysis is an advanced computer-based engineering technique that uses mathematical approximations to simulate the human body.This method accurately reflects the biomechanical characteristics within the knee,providing a powerful tool for understanding knee disease pathogenesis,optimizing surgical protocols,and developing new implant materials.OBJECTIVE:To review the establishment of finite element modelling of the knee joint and its application in the study of knee joint diseases,and look forward to the future development trend.METHODS:The first author searched the PubMed and EI databases in April 2024 by applying a computer with English search terms"finite element analysis,FEA,knee joint,finite element model,knee biomechanics,knee osteoarthritis,knee prosthesis,knee ligaments,meniscus"and searched CNKI and WanFang databases with Chinese search terms"finite element analysis,finite element model,knee joint,biomechanics,osteoarthritis,computational model,knee prosthesis,knee ligament,meniscus."Finally,75 papers were included in the analysis.RESULTS AND CONCLUSION:(1)Finite element analysis method uses medical imaging data to obtain a three-dimensional human model,simplifies the complex human joint structure into finite and interconnected units,and visually displays the internal stress distribution of the knee joint by applying external loads to the model.(2)The researchers deeply study the internal stress and strain distribution of the knee joint under different working conditions by means of finite element analysis,revealing the overloading of the articular cartilage and the decrease of load in some areas when the balance of the internal load distribution of the knee joint is changed,and that such long-term abnormal stresses cause deformation,wear and tear,and eventual loss of cartilage,which is crucial for understanding how biomechanical factors cause degenerative changes of the knee joint.(3)The effect of physical therapy methods such as Tai Chi and gait adjustment in patients with osteoarthritis of the knee joint was evaluated by finite element analysis,and the results showed that these treatments reduced the overloading of the cartilage,which provided a scientific theoretical basis for clinical treatment.(4)Clinicians are able to optimize surgical treatment strategies by performing three-dimensional reconstruction,data measurement,and simulation of surgery before surgery through finite element analysis.Furthermore,the mechanical characteristics of different prostheses can be simulated to improve the shape,material,and fixation of the prostheses,reduce patient complications,and improve patient outcomes.(5)The combination of artificial intelligence and finite element analysis makes the construction of finite element models more accurate and easy to operate,greatly contributing to the efficiency of clinicians'medical practice and patient outcomes.(6)Finite element analysis is only a digital simulation,which is still somewhat different from the real physical state.
6.Study on the mechanism of allogeneic renal subcapsular transplantation of CD24+renal epithelial cells in the alleviation of ischemia-reperfusion-induced acute kidney injury in mice

Objective To investigate the therapeutic effect and potential mechanisms of allogeneic renal subcapsular transplantation of CD24+renal epithelial cells for the treatment of acute kidney injury(AKI)induced by ischemia-reperfusion(I/R).Methods CD24+renal epithelial cells were isolated from mouse kidneys using flow cytometric sorting and expanded by passaging.C57BL/6N mice were randomly divided into three groups:the normal control group(n=8,sham surgery only),the model control group(n=8,unilateral kidney I/R plus contralateral nephrectomy),and the CD24+cell treatment group(n=8,AKI model followed by renal subcapsular transplantation of CD24+cells).Mice were euthanized at 24 h after modeling and serum was collected to measure biochemical markers[serum creatinine(Scr),blood urea nitrogen(BUN),tumor necrosis factor-α(TNF-α),and interleukin-6(IL-6)].Renal tissues were subjected to pathological evaluation and macrophage staining.An M1-polarized macrophage model was established using mouse bone marrow-derived macrophages co-cultured with CD24+renal epithelial cells.The polarization state of macrophages was assessed by quantitative real-time polymerase chain reaction(qPCR)and flow cytometry.Results CD24+renal epithelial cells were successfully isolated and passaged stably.Compared with the normal control group,the model control group exhibited significantly elevated Scr and BUN levels and renal pathological damage.In contrast,the CD24+cell treatment group showed significant reduction in serum biochemical markers and pathological injury compared with the model control group,along with reduction in M1 macrophage infiltration in the kidneys(P<0.05,P<0.01).In vitro co-culture experiments demonstrated that in the CD24+co-culture group,the expression of M1 polarization-related markers in macrophages was significantly lower than that in the non-co-culture group,and the proportion of CD80+M1 macrophages in the co-culture group decreased(P<0.05,P<0.01).Conclusion Allogeneic renal subcapsular transplantation of CD24+renal epithelial cells can alleviate I/R-induced AKI by inhibiting M1 macrophage polarization through paracrine mechanisms.

9.Dopamine receptor 3 knockout inhibits acquisition and recall of fear memory induced by electric shocks

OBJECTIVE To investigate the effect of dopamine receptor 3(D3R)on fear memory induced by intense electric shocks and the possible neurobiological mechanism.METHODS ① To prevent pain threshold differences from influencing the effect of intense electric shocks,wild-type(WT)and D3R knockout mice(D3R-/-)were used in the Hargreaves test to evaluate their basal pain threshold,with the paw withdrawal latency(PWL)as the observation index.② WT and D3R-/-mice were divided into control groups and model groups,respectively.On the training day(the first day,D 1),the model groups received inescapable electric shocks(1.5 mA,10 s,10 s interval,15 cycle)while the control groups did not.Contextual fear tests were conducted on D2,D7,D10,D14,and D16 after training,with the percentage of freezing time(FT)as the observation index to evaluate fear memory acquisition induced by contextual cues.On D17,after the model groups showed no more fear responses to contex-tual cues,they were re-stimulated with low-intensity current(0.5 mA,10 s,10 s interval,15 cycle)to evoke fear memory.The two control groups did not receive any shocks.Contextual fear tests were conducted on day 18,and the FT%of each group was observed to evaluate fear memory retrieval induced by contextual cues.③ Another cohort of WT and D3R-/-mice was used to further investigate the underlying neural mechanism,with the same grouping and treatment as in ②.Real-time dynamic changes in calcium signals of dopamine(DA)neurons in the ventral tegmental area(VTA)of WT and D3R-/-mice were detected using fiber photometry during electric shocks.The fluorescence area under the curve(AUC)was used as the indicator to quantify the excitability of DA neurons.RESULTS ① In the Hargreaves test,there was no significant difference in PWL between D3R-/-mice and WT mice,indi-cating the two genotype mice had no significant differences in the basal pain threshold.② Compared with the WT control group,the percentage of FT of the WT model group significantly increased on D2,D7,D10,and D14(P<0.05).Compared with the D3R-/-control group,the percentage of FT of the D3R-/-model group significantly increased only on D2 and D7(P<0.01).Meanwhile,the percentage of the FT of D3R-/-model group was significantly lower than in the WT control group on D2,D7,D10,and D14(P<0.05,P<0.01).During memory recall(D18),the percentage of FT of the WT model and D3R-/-model groups significantly increased compared to their respective control groups(P<0.01,P<0.05),while the percentage of FT of D3R-/-model mice was significantly lower than that of WT model mice(P<0.01).③ In the fiber photometry test,during the shock period,the calcium signals of DA neurons in the VTA of WT model and D3R-/-model mice rapidly increased within the first 2 s,and then gradually decreased between 2 to 10 s.The AUC within the 2 to 10 s interval was significantly lower in D3R-/-model mice compared to WT model mice(P<0.05),indicating that the excitability of DA neurons in the VTA of D3R-/-model mice was significantly lower than that of WT-model mice.CONCLUSION D3R knockout inhibits the acquisition and recall of long-term fear memory in mice,and its neurobiological mechanism may be related to the decreased excitability of DA neurons during electric shock.
