Journal of Public Health and Preventive Medicine 2025;36(4):102-105
Objective To analyze the relationship between cardiac autonomic neuropathy (CAN) and bone mineral density (BMD) reduction and fracture risk in patients with type 2 diabetes mellitus (T2DM). Methods A total of 396 patients with T2DM admitted to Nanchong Mental Health Center of Sichuan Province were selected, and all of them underwent detection of BMD of hip, lumbar vertebra and femoral neck. Fracture risk was evaluated using the probability of major osteoporotic fracture (PMOF) and ten-year probability of hip fracture (PHF). According to the degree of fracture risk, the patients were divided into low-risk group and high-risk group. Clinical data and CAN condition of the two groups were compared. Factors influencing fracture risk in patients with T2DM were analyzed. According to CAN condition, the patients were divided into early group, diagnosed group, and severe group. The correlation between CAN score and BMD was analyzed. Results The proportion of CAN in the high-risk group was significantly higher than that in the low-risk group (P<0.05). The BMD of hip, lumbar vertebra and femoral neck was significantly lower than that in the low-risk group (P<0.05). Logistic regression analysis showed that BMD of hip (OR=0.143, 95%CI: 0.102-0.201), BMD of lumbar vertebra (OR=0.047, 95%CI: 0.022-0.100), BMD of femoral neck (OR=0.208, 95%CI: 0.168-0.257), and CAN (OR=39.409, 95%CI: 14.704-105.623) were risk factors for fracture (P<0.05). The BMD of hip, BMD of lumbar vertebra and BMD of femoral neck in the severe group, the diagnosed group, and the early group increased in order (P<0.05). CAN score was negatively correlated with the BMD of hip, BMD of lumbar vertebra and BMD of femoral neck in patients with T2DM (P<0.05). Conclusion The condition of CAN in patients with T2DM is closely related to BMD reduction, and CAN is a risk factor for fracture.

Objective:To investigate the impact of different CT reconstruction kernels on the quantitative analysis of small pulmonary vessels in patients with chronic obstructive pulmonary disease (COPD) and high-risk patients. Methods:This study was a cross-sectional study. Clinical and imaging data of 73 COPD and high-risk patients visiting the China-Japan Friendship Hospital between March and April 2024 were retrospectively analyzed. All patients underwent high-resolution CT of the chest and pulmonary function tests, with the ratio of forced expiratory volume in one second to forced vital capacity (FEV 1/FVC) obtained. The raw CT data were reconstructed using different kernels: the FC86 group used the adaptive iterative dose reduction(AIDR) 3D standard lung sharp reconstruction algorithm, the FC18 group used the AIDR 3D standard Body standard reconstruction algorithm, the advanced intelligent clear-IQ engine(AiCE) Lung group used the AiCE deep learning reconstruction algorithm for lung, and the AiCE Body group used the AiCE deep learning reconstruction algorithm for body. Image signal-to-noise ratio (SNR), contrast-to-noise ratio (CNR) and image noise were calculated. The pulmonary vessel segmentation & Measurement software was used to segment and extract pulmonary arteries and veins of four groups in thin-slice whole-lung CT imaging, obtaining the ratio of small pulmonary arteries (%V artery<5) and the ratio of small pulmonary veins (%V vein<5). The One-way repeated measures ANOVA or Friedman test was used to compare the differences in SNR, CNR, image noise, %V artery<5, and %V vein<5 among the four groups, followed by Bonferroni post hoc or Bonferroni-Dunn test with P-value correction to analysis differences between subgroups. The correlations between %V artery<5 and FEV 1/FVC, as well as between %V vein<5 and FEV 1/FVC were analyzed using Spearman rank correlation analysis in all four groups. Results:The overall differences in image noise, SNR, and CNR in the AiCE Lung, AiCE Body, FC18, and FC86 groups were statistically significant ( P<0.001). Except for the difference in CNR values between the AiCE Lung group and the FC18 group, which was not statistically significant ( P=0.192), all differences were statistically significant ( P<0.016 7). The overall differences in %V artery<5 values and %V vein<5 values in the AiCE Lung, AiCE Body, FC18, and FC86 groups were statistically significant ( P<0.001). The %V artery<5 and %V vein<5 values in the FC18 group were lower than those in the AiCE Lung, AiCE Body, and FC86 groups ( P<0.016 7), and the rest of the differences were not statistically significant ( P>0.016 7). %V artery<5 and %V vein<5 were positively correlated with FEV 1/FVC in all 4 groups ( P<0.05), with the highest correlation coefficient between %V vein<5 and FEV 1/FVC in the AiCE Body group ( r=0.501, P=0.001). Conclusions:DLR-AiCE-based kernel reconstruction optimizes image quality and significantly affects the results of quantitative parameters of small pulmonary vessels. The reconstruction kernel prioritized for quantitative analysis of small vessels within the lungs in COPD based on the CT scanner in this study is AiCE Body.

BACKGROUND:Steroid-induced osteonecrosis of the femoral head is a key risk factor for non-traumatic femoral head necrosis, and the incidence of it has gradually increased in recent years, but its specific pathogenesis is still unclear, and finding a reasonable animal model is essential for disease research and treatment.OBJECTIVE:To review the commonly used animal models of steroid-induced osteonecrosis of the femoral head in recent years, and to analyze the advantages and disadvantages of different modeling methods and evaluation criteria,so as to provide a reference for follow-up research. METHODS:The articles published from 2013 to 2023 were searched in CNKI,WanFang Data, and PubMed with the keywords "femoral head necrosis, osteonecrosis of the femoral head,steroid-induced osteonecrosis of the femoral head, animal model." Finally,61 articles were involved for comprehensive analysis according to the inclusion criteria, including 38 English articles and 23 Chinese articles.RESULTS AND CONCLUSION:(1) Rabbits, rats, and chickens are the animals that are widely used in the study of steroid-induced osteonecrosis of the femoral head models. (2) Hormone combined with lipopolysaccharide or horse serum modeling has low mortality, high success rate, and strong stability. (3) Histopathology is the gold standard for the evaluation model, but the experiment needs to be terminated, which is not conducive to subsequent experimental research,so finding a non-invasive alternative method is still the direction of future efforts. (4) An ideal model of steroid-induced osteonecrosis of the femoral head has not yet been explored, and future researchers need to continue to strive to achieve breakthroughs in this field as soon as possible.

Objective:To explore the effectiveness of shared decision-making intervention with nurses as "decision coaches" in perioperative decision management of bladder cancer patients. Methods:From March 2023 to February 2024, 152 bladder cancer patients in the Department of Urology Surgery of Xinxiang Central Hospital for surgical treatment were selected by convenience sampling. The patients were divided into control group and observation group of 76 cases each using the randomized numerical table method. Control group implemented conventional bladder cancer surgical decision management, and observation group implemented a shared decision-making intervention with nurses as "decision coaches" on this basis, and the intervention period was one week before surgery. Decisional Conflict Scale (DCS), Preparation Decision Making (PrepDM), Patients Satisfaction with Participation in Medical Decision-making Scale, and Decision Regret Scale (DRS) scores were compared between the two groups of patients before and after intervention. Results:Finally 74 patients in control group and 75 patients in observation group completed the study. After the intervention, the DCS scores of observation group were lower than those of control group and before the intervention, and the differences were statistically significant (all P<0.05). After the intervention, the PrepDM scores of observation group were higher than those of control group and before the intervention, and the differences were statistically significant (all P<0.05). One week after surgery, observation group had higher decision-making participation satisfaction scores and lower DRS scores than control group, and the differences were statistically significant (all P<0.05) . Conclusions:Preoperative shared decision-making interventions with nurses as "decision coaches" in bladder cancer patients can effectively reduce preoperative decision conflicts, increase preoperative decision readiness and participation satisfaction with decision-making, and decrease patients' postoperative decision regret.
