1.Imaging characteristics of pediatric growing skull fracture
Journal of Practical Radiology 2025;41(7):1194-1197
Objective To investigate the imaging characteristics of pediatric growing skull fracture(GSF).Methods A retro-spective analysis was conducted on the clinical and imaging data from 14 cases pediatric GSF.Results At the time of initial trauma, cranial CT examinations were performed in 10 cases, with 7 presenting as linear fractures and 3 as comminuted fractures. The median separation distance between fracture edges was 5 mm.At the time of GSF diagnosis, all 14 patients underwent cranial CT examina-tions,and 9 underwent cranial MRI examinations. The morphological changes in GSF skulls displayed two distinct patterns:The first pattern, observed in 12 cases, involved widening of the fracture gap with simultaneous separation of the inner and outer skull tables and diploic layer, leading to skull defects. The median separation distance of fracture edges in this group was 10 mm.The second pat-tern,seen in 2 cases, showed significant expansion and destruction of the diploic layer at the fracture site.Additional imaging findings associated with GSF included brain herniation, encephalomalacia, subdural effusion, subgaleal effusion, cerebrospinal fluid accumula-tion in the diploic layer, brain perforation malformations, and lateral ventricular enlargement. Conclusion The typical imaging mani-festation of GSF is widening of the fracture gap leading to skull defects, though a small proportion may present with diploic layer expan-sion.Serial cranial CT and MRI are essential imaging modalities for diagnosing GSF.

Improving the entire outpatient clinic experience is an important measure in the new era to enhance patients'sense of gain from medical treatment.Beijing Tongren Hospital Affiliated to Capital Medical University explores the establishment of a full-process outpatient service management system based on continuous improvement of medical services.Through the concept of forward service,it focuses on the patient's pre-diagnosis experience;the process is simplified and intelligently guided to optimize the patient's in-diagnosis experience;and continuous diagnosis and treatment Model innovation improves patients'post-diagnosis experience and creates a Chinese-style modern outpatient medical service model to continuously meet the people's growing needs for a better life.

Objective:To investigate the clinicopathological characteristics of acidophil stem cell pituitary neuroendocrine tumors (PitNET)/adenoma.Methods:Five cases of acidophil stem cell PitNET/adenoma were diagnosed between May 2022 and July 2023 at the Second Hospital of Hebei Medical University, Shijiazhuang, China. The clinicopathological features of the tumor were analyzed by using histology, immunohistochemistry, and electron microscopy. The relevant literature was reviewed. Results:There were 1 male and 4 females, aged from 23 to 69 years. Patient 3 was 55 years old at the time of diagnosis and first surgery, and relapsed 5 years later. The patients′ median age was 32 years. Patients 1 and 5 showed elevated blood prolactin, with various degrees of hormonal symptoms except Patient 3, who showed only tumor compression symptoms. Imaging studies showed that all cases involved the sellar floor. The tumors of Patients 1, 2 and 5 were closely related to the cavernous sinus segment of the internal carotid artery. The tumors exhibited a diffuse growth pattern with chromophobic to slightly acidophilic cytoplasm. A few of tumor cells showed chromophobic cytoplasm. The nucleoli were conspicuous. Intranuclear inclusion bodies and variably-sized clear vacuoles were observed occasionally. Under electron microscope, marked mitochondrial abnormalities were observed, including increased mitochondria number, expanded hypertrophy, and absence of mitochondrial ridge fracture. Some mitochondrial matrices were dense, while some were vacuolated. Conclusions:Acidophil stem cell PitNET/adenoma is a rare type of pituitary adenomas/PitNETs. It often has a more clinically aggressive manner with immature cells, diffuse expression of PIT1, prolactin, and varying degrees of growth hormone expression. Because of the obvious diversity of their clinical hormone status and hormone immune expression, the diagnosis of this type tumor is still a challenge.

Objective To investigate the safety and feasibility of peripheral vascular intervention via the ipsilateral transulnar access(TUA)due to failure of transradial access(TRA)puncture. Methods The clinical data of 2546 peripheral vascular interventions via TRA,which were performed at authors'hospital between January 2019 and December 2021,were retrospectively analyzed.Among the 2546 interventions,TRA puncture failed in 37 procedures, and in 27 of these patients the ipsilateral TUA puncture had to be adopted. The puncture success rate, surgical success rate and puncture approach-related complications of TUA of the 27 patients receiving ipsilateral TUA puncture were analyzed. Results The success rate of ipsilateral TUA puncture after TRA puncture failed was 96.3%(26/27),and in one patient transfemoral access(TFA)puncture had to be substituted because of the ulnar artery spasm. The total success rate of interventional procedures was 96.3%(26/27).No serious complications occurred, and the incidence of minor complications was 19.2%(5/26).Conclusion Preliminary results indicate that for the experienced TRA operators, using ipsilateral TUA puncture due to failure of TRA puncture is a safe and feasible strategy choice.
