1.Application of loosening the inferior pulmonary ligament in thoracoscopic right upper lobe apical segment resection for non-small cell lung cancer: A prospective study

Objective To compare the effects of preserving and releasing the inferior pulmonary ligament (IPL) during thoracoscopic right upper lobe apical segment resection for non-small cell lung cancer patients, and to explore appropriate management methods for intraoperative IPL. Methods According to the prospective and open principle, the patients with non-small cell lung cancer who underwent thoracoscopic right upper lobe apical segment resection in the Cardiothoracic Surgery Department of Ningbo Second Hospital from January 2020 to November 2022 were selected and divided into two groups: a release group (receiving IPL release during thoracoscopic right upper lobe apical segment resection) and a retention group (receiving the same right upper lobe apical segment resection, but retaining IPL during operation). The clinical data of the two groups were compared. Results A total of 70 patients were included in this study, including 33 males and 37 females, aged 35-76 years. There were 35 patients in the release group with an average age of (57.02±9.25) years; 35 patients in the retention group with an average age of (56.81±9.94) years. The surgical time in the release group was statistically longer than that in the retention group (P=0.017). There was no statistical difference between the two groups in terms of intraoperative bleeding, duration of postoperative air leakage, time to achieve lung recruitment, drainage flow rate of the chest tube, retention time of the chest tube, incidence of postoperative complications, postoperative hospitalization days, or residual cavity rate one month after the surgery (P>0.05). Conclusion Compared to releasing IPL during thoracoscopic right upper lobe apical segment resection, preserving IPL can simplify surgical procedures, shorten surgical time and reduce trauma, and does not increase postoperative adverse effects. This further reflects the concept of minimally invasive surgery and can be applied to clinical practice.

Pelvic fractures, the most severe bone trauma, account for approximately 3% of all fractures. As they are caused by high-energy injuries, their rates of mortality and disability are high. Over the past two centuries, the treatment strategies for these fractures have evolved from conservative therapy to open reduction and plate fixation, then to closed reduction and percutaneous screw fixation, and recently to screw fixation assisted by artificial intelligence. In the past 40 years, constant progress has been made in the treatment of pelvic fractures. It is generally acknowledged that a personalized treatment plan should be formulated based on each patient's age, fracture type, comorbidities, functional requirements and other factors when an appropriate treatment mode is chosen. The primary aim of treatment is to accelerate functional recovery, decrease the rate of disability after injury, and improve the quality of daily life of the patient. This paper reviews the treatment history of pelvic fractures, discusses the advantages and disadvantages of current treatment options, and looks ahead to future prospects, aiming to offer valuable references for related clinical practice.
3.Risk analysis of adverse immune reconstitution in HIV/AIDS patients after antiviral therapy based on random forest model

Objective A random forest model was used to analyze the risk of immune reconstitution dysfunction(INR)after antiviral therapy(HAART)in HIV/AIDS patients.Methods A total of 67 HIV/AIDS patients treated in our hospital from January 2019 to December 2022 were selected and all of them received HAART treatment.After 18 months of follow-up,patients were divided into INR group and non-INR group according to prognosis.The clinical data of the two groups were compared,and the importance of the indicators affecting INR in patients was ranked by random forest model,and the related factors affecting INR in HIV/AIDS patients after HAART were analyzed by Logistic regression model.Results During the follow-up period of 18 months,a total of 13(19.40%)of 67 HIV/AIDS patients developed INR.There were statistically significant differences in the time interval from diagnosis to treatment and baseline CD4+T lymphocyte count between INR group and non-INR group(P<0.05).Random-forest model results showed that variables were ranked in descending order of importance as baseline CD4+T lymphocyte count and time interval between diagnosis and treatment.Logistic multivariate regression analysis showed that more than 1 year between diagnosis and treatment and baseline CD4+T lymphocyte count≤ 200/μl are all risk factors for INR in HIV/AIDS patients after HAART treatment(P<0.05).Conclusion More than 1 year between diagnosis and treatment and baseline CD4+T lymphocyte count≤200/μl are all risk factors for INR in HIV/AIDS patients after HAART.In addition,the random forest model is of great significance for health care workers to identify high-risk groups of INR after HAART and formulate intervention plans.

Objective To investigate the effects of digital platform construction in departmental Party branches on the or-ganizational stronghold role of primary-level Party organizations.Methods Under the leadership of the hospital Party committee,the Radiology Department Party Branch launched the development of a"Party building+Services"digital platform in July 2021,facilitating a comprehensive integration of political and operational activities.This study retrospectively compared the differences in the indicators of party-building and the disciplinary development,mainly including Party-building effectiveness,team cohe-sion,management efficiency,clinical services,scientific research outputs,and educational performances between three-years pre-and post-implementation.Additionally,it also summed up the impact of an electronic platform development on the role of primary-level Party organizations as a combat bastion.Results Post-implementation metrics demonstrated statistically significant improvements in such indicators of Party organization building and disciplinary development as primary-level Party building effec-tiveness,team cohesion,management efficiency,scientific research outputs,and educational performances(all P<0.05).Conclusion The establishment of an electronic platform within the department fosters deep integration and intelligent empower-ment of Party building and operational activities,enhances the quality of Party building,and promotes the advancement of disci-plines,thereby reinforcing the role of primary-level Party organizations as effective combat bastions.

8.Efficacy of adjusted NI value guidance combined with small-dose esketamine for program-controlled closed-loop target-controlled infusion system

Objective:To evaluate the efficacy of adjusted Narcotrend Index (NI) value guidancecombined with small-dose esketamine for program-controlled closed-loop target-controlled infusion (TCI) system.Methods:Forty-eight American Society of Anesthesiologists Physical Status classificationⅠ or Ⅱpatients, regardless of gender, aged 18-55 yr, with body mass index of 18-25 kg/m 2, scheduled for elective laparoscopic surgery under general anesthesia, were assigned to control group (group C, NI baseline value median 36) and esketamine group(group E, NI baseline value median 46) using a random number table method, with 24 cases in each group. Anesthesia induction and maintenance were carried out using effect-site concentration TCI(Schnider model for propofol infusion and Minto model for remifentanil infusion). After the NI value was maintained at 26-46 during anesthesia maintenance, a small dose of esketamine was given (as an intravenous bolus 0.2 mg/kg, followed by an infusion of 5 μg·kg -1·min -1for 30 min) in group E, and the equal volume of normal saline was given instead in group C. Program-controlled closed-loop TCI was then started, and the target effect-site concentrations of propofol and remifentanil were adjusted every 5 min according to the corresponding preset NI baseline value. The main outcome measures were the percentage of time of NI value maintained in the target range within 1 h after administration of esketamine. Secondary outcome measures were the consumption of propofol and remifentanil, postoperative recovery time, incidence of nausea and vomiting, pain and shivering within 1 h after surgery. Patients were followed for intraoperative awareness on 2nd day after operation. Results:The performance of the program-controlled closed-loop TCI systems was within the safe clinical threshold, with no intraoperative awareness occurred in both groups. The consumption of propofol and remifentanil was significantly reduced in group E as compared to group C( P<0.05). There were no statistically significant differences in the percentage of time of NI value maintained in the target range, postoperative recovery time and incidence of adverse reactions between the two groups ( P>0.05). Conclusions:Adjusted NI value guidance combined with small-dose esketamine provides better efficacy when used for program-controlled closed-loop TCI system.
9.Differential expression and bioinformatics analysis of microRNAs in exosomes of sheep poxvirus-infected cells

Sheep pox is widespread worldwide and is the most severe animal pox virus infection. This study aimed to identify the key microRNAs (miRNAs) differentially expressed in the exosomes of sheep poxvirus-infected cells and their target genes and related pathways and provide a theoretical basis for an in-depth understanding of the molecular mechanisms of sheep poxvirus-infected cells. In this study, the differentially expressed miRNAs were verified by quantitative polymerase chain reaction (qPCR), and the target genes of miRNAs were predicted and analyzed by bioinformatics. The qPCR results showed that the expression trends of oar-miR-21, oar-miR-10b, oar-let-7f, oar-let-7b, and oar-miR-221 were consistent with the sequencing results. The Gene Ontology and Kyoto Encyclopedia of Genes and Genomes results showed that differentially expressed miRNAs were mainly involved in the immune system processes of the Arf6 downstream pathway. The target genes Reactome pathways were mainly enriched in the RAC1 GTPase cycle, CDC42 GTPase cycle, RHO GTPase cycle, RHOV GTPase cycle, and post-transcriptional silencing of small RNAs. The transcription factors SP4, NKX6-1, MEF2A, SP1, EGR1, and POU2F1 that may be connected to sheep pox virus (SPPV)-infected cells were discovered by transcription factor annotation screening. In conclusion, this study screened for differentially expressed miRNAs in SPPV-infected cells and performed a series of bioinformatic analyses of their target genes to provide a theoretical basis for the molecular mechanism of sheep pox virus infections of cells. The data can be used as basic information in future studies on the defense mechanisms against poxvirus infections.
