1.Perioral myoclonia with absences: report of 3 cases and literature review

Objective:To explore the clinical characteristics and therapeutic effect of perioral myoclonia with absences (PMA), in order to improve the clinicians′ understanding of the disease.Methods:The clinical manifestations during seizures, as well as interictal and ictal electroencephalogram (EEG) features of 3 PMA patients admitted to the Epilepsy Unit, Neurology Center, Beijing Tiantan Hospital, Capital Medical University from May to October 2024 were analyzed. The clinical manifestations, EEG characteristics, differential diagnosis, treatment experience of PMA patients were summarized by retrospective analysis combined with previous literature review.Results:The onset age of these 3 PMA cases was 15, 15, and 7 years, respectively. The prominent manifestation of seizure was perioral myoclonus without disturbance of consciousness. The duration of seizures was brief, less than 10 seconds. PMA can occur several times a day and was prone to present as continuous state of perioral myoclonus. Three patients had rare generalized tonic-clonic seizures (GTCS). The seizures could be controlled by sodium valproate, lamotrigine, levetiracetam, or combined treatment. Fifteen literatures about PMA were retrieved (4 in Chinese and 11 in English). A total of 31 patients (including these 3 patients) were included in the study, of whom 17 were males and 14 were females. The onset age was from 9 months to 34 [7.63 (2.19, 11.00)] years. Perioral myoclonus with varying degrees of disturbance of consciousness was the main seizure type, and 12 patients exhibited continuous state of perioral myoclonus, 24 patients had GTCS. Eight patients had a history of fever convulsion, 6 patients had mental or learning disabilities, and 11 patients had a family history of epilepsy or febrile convulsion. The ictal EEG showed around 3 Hz generalized spike-wave/multiple spike-wave discharges accompanied with synchronous myoelectric bursts. There were no specific abnormalities in physical examination of nervous system and cranial imaging in all the patients. Ten patients were initially diagnosed as focal epilepsy. Nine patients′ seizures were increased or worsened due to improper medication. Valproate, lamotrigine and levetiracetam were the main treatments of PMA, and the seizures of 21 patients were effectively controlled.Conclusions:PMA is clinically characterized by perioral myoclonia with varying degrees of disturbance of consciousness and is easy to be misdiagnosed as focal epilepsy, so it is necessary to use video EEG combined with synchronous electromyography to confirm the diagnosis. PMA should be treated with broad-spectrum antiseizure drugs.

5.Establishment of early warning control charts based on the syndromic surveillance data of outpatient diarrhea in Beijing

Objective:To establish the control charts for early warning of diarrhea based on the syn-dromic surveillance data from enteric clinic in Beijing .Methods:The outpatient data from enteric clinic of a Grade Three General hospital in Haidian district , Beijing from April 1 to Oct.31, 2009 and from May 1 to Nov.10, 2010 were collected, according to the moving average method , the baseline calcula-ted, the value of probability αand μα, the early warning value based on the formula “w=Xj +μαSj”calculated and the early warning control charts drew at last .Results:According to the harmfulness , the severity and controllability of diarrheal diseases , the value of probability αwas determined as 0.01, thenμα( unilateral) as 2, based on the early warning value , the control charts of diarrheal diseases , bacillary dysentery and other infectious diarrhea were established .Conclusion:The enteric clinic requires to fur-ther collect baseline data to evaluate and continuously adjust the established control charts for the best early warning model in accordance with the enteric clinic .
6.Study on the levels of serum organochlorine pesticides residues in breast cancer patients from Tangshan areas

Objective To evaluate the levels of serum organochlorine pesticide residues in breast cancer patients and benign breast disease patients from Tangshan areas. Methods One hundred fifty-eight breast cancer patients and 129 benign breast disease patients from two districts of TangShan were entrolled in the study. The levels of serum organochlorine pesticides ( DDTs and HCHs) were detected through gas chromatography/electron capture. Spearman's rank correlation was used between age, BMI and the level of serum organochlorine pesticide residues in patients. Results The serum levels of a-HCH, p-HCH and PP'-DDE between breast cancer patients and benign breast disease patients were statistically different(P <0. 05). The serum levels of p-HCH and PP'-DDE were positively correlated with age and BMI in both groups, with correlation coefficients of 0. 272 and 0. 330 for age and 0. 207 and 0. 313 for BMI in the breast cancer patients,as well as correltion coefficients of 0. 339 and 0. 260 for age and 0. 227 and 0. 209 for BMI in the benign breast disease patients. Conclusions The levels of organochlorine pesticide residues in serum in breast cancer patients were higher than those in benign breast disease patients and were positively correlated with age and BMI. The results offer more information to the pathogenesis, prevention and control measures of breast cancer.
7.Study on serum organochlorine pesticides and residues levels in breast cancer patients between the seriously and lightly earthquake-striken areas

Objective To evaluate serum organochlorine pesticides and residues levels in breast cancer pa-tients between the seriously and lightly earthquake-striken areas of Tangshan. Methods 93 patients of breast cancer from seriously earthquake-striken areas and 65 patients from lightly earthquake-striken areas were selected as study subjects,which serum organochlorine pesticides (DDTs and HCHs) levels were detected and compared with gas chromatography/electron capture. Results Serum organochlorine pesticides and residues levels in 158 patients be-tween the seriously and lightly earthquake-striken areas of Tangshan were not normal distributed. The order of detec-tion rate of HCH isomer in breast cancer patients between the seriously and lightly earthquake-striken areas were β-HCH>δ-HCH >α-HCH >γ-HCH. The orders of detection rate of DDT isomer in breast cancer patients between two areas of Tangshan were PP'-DDE > PP'-DDT > OP'-DDT > PP'-DDD. The serum levels of α-HCH,β-HCH, PP'-DDE and PP'-DDD in breast cancer patients between the seriously and lightly earthquake-striken areas were statistically different. Conclusion The levels of organechlorine pesticides and residues in serum with high detection rate in seri-ously earthquake-striken area are higher than that in lightly earthquake-striken area. It is likely that there is an asso-ciation with a large number of organechlorine pesticides sprayed.
