1.Analysis of nutritional status of primary and middle school students in Baodi district,Tianjin

Objective To analyze the nutrition status of primary and middle school students in Baodi district,Tianjin in 2021 and 2023,as to provide scientific basis for improving the nutritional status of students.Methods A stratified random cluster sampling method was adopted to select 12 schools in Baodi district,Tianjin.Students' nutrition status were assessed from September to November of every year in 2021 and 2023,including questionnaire survey,physical examination and biochemical indicators.Results Compared with the same gender and age students in 2021,the height of boys aged 7-13 and girls aged 7-12 will significantly increase in 2023,while the body mass of boys and girls aged 8 and 11 will significantly increase.Compared with 2021,the overweight and obesity rates among males have significantly decreased in 2023,while the emaciation rates among males and females have significantly increased.The anemia rate,serum ferritin and vitamin A,and vitamin D deficiency rate among students in 2023 were 12.25%,3.77%,15.70%,and 50.31%,respectively,with no statistically significant difference compared to 2021(P>0.05).The awareness rate of nutrition knowledge and the proportion of related behaviors among students in 2023 have significantly increased compared to 2021.Conclusion Compared with 2021,the overweight and obesity rates among primary and secondary school students will decrease in 2023,while anemia,serum ferritin,vitamin A,and vitamin D deficiencies will not improve.Targeted intervention activities are urgently needed.

Objective:To analyze a novel intronic variant in the SPTB gene and explore its effect on SPTB mRNA splicing.Methods:Clinical data of a child diagnosed with hereditary spherocytosis (HS) and admitted to the First Affiliated Hospital of Xi′an Jiaotong University in February 2022 were analyzed retrospectively. Whole genome sequencing was used to identify disease-causing variantions and the results were validated with Sanger sequencing, mRNA sequencing was used to determine the SPTB gene′s mRNA expression level, and bioinformatics tools were used for splicing site prediction and analysis.Results:The proband is a 2-month-old Han male child, clinically presenting with anemia and jaundice. In the past, jaundice appeared early and was severe during the neonatal period, with significantly elevated indirect bilirubin (203.5 μmol/L), accompanied by moderate anemia. This family consisted of four generations, eight of whom suffered from splenomegaly, jaundice, and anemia. In their peripheral blood, the percentage of microglobular erythrocytes was between 5% and 10%. Under scanning electron microscopy analysis of the proband's father's peripheral red blood cells, about 6% exhibited a mouth-shaped morphology, about 4% were spherical, and about 3% were oval. Following the splenectomy, the father′s anemia and jaundice recovered to normal level. Whole genome sequencing analysis of the proband identified a heterozygous variant in the SPTB gene (NM_ 001355436.2 (SPTB):c.6022+4_6022+18delinsTGGCTCCTCCGTGAAGGGACAGTCCTGC), which was verified to be co-segregating with the disease in this family line by Sanger sequencing. The results of the SPTB gene mRNA expression level detection showed that the expression levels of the SPTB variant gene were statistically increased in the proband and affected family members (father, grandmother, cousin, second cousin, great-grandmother, great-aunt) (all P<0.05). The SPTB gene′s intron can undergo selective splicing, as demonstrated by analysis using the bioinformatics program ESE Finder. Additionally, predictions from the SpliceAI and SpliceTool software indicated that activation of a new covert splicing donor can result in a code-shift variantion that introduces an early termination codon and nonsense-mediated degradation of the mRNA, which prevents the synthesis of proteins. Conclusion:A new variantion site c.6022+4_6022+18delinsTGGCTCCTCCGTGAAGGGACAGTCCTGC was found in SPTB gene. This variantion was the pathogenic factor of HS. By affecting the splicing process, this variantion triggers the nonsense mediated mRNA degradation pathway, resulting in inactivation of gene function.
4.Survival analysis of microsatellite instability and microsatellite stable right-sided colon cancer

Objective Compare the clinicopathological factors of microsatellite unstable(dMMR)and microsatellite stable(pMMR)in right colon cancer and analyze the factors affecting overall survival(OS)and disease-free survival(DFS)between the two groups.Methods Clinical data with right colon cancer of patients who underwent radical resection from January 1,2016 to December 31,2023 were retrospectively analyzed(a total of 247 patients were included,including 43 dMMR and 204 pMMR).Through propensity score matching,the baseline difference between the two groups was matched 1∶1,and a total of 86 cases were obtained,43 cases in dMMR and 43 cases in pMMR.The difference of relevant indicators between the two groups was analyzed,and univariate and multivariate Cox analysis was performed for OS and DFS.The survival curve was plotted,and the comparison of survival rates was conducted using the Log rank test.Results According to age,location,tumor length,shape,histological type,T,N,TNM stage,nerve invasion,and differentiation degree with 1∶1 matching(P<0.05),there was no statistical difference in baseline data between dMMR and pMMR patients with right colon cancer(P>0.05).It was found that 5-year OS of dMMR patients with right colon cancer was significantly better than pMMR(P<0.05),and 5-year DFS dMMR was better than pMMR(P<0.05).Protective factors affecting OS of dMMR right colon cancer included tumor length<5 cm,T1-3,M0,Stage Ⅰ-Ⅲ,absence of intravascular cancer thrombi and absence of nerve invasion(P<0.05).In survival analysis,M0 was compared with M,(95%vs.60%,P<0.05).Stage Ⅰ-Ⅲ compared with stage Ⅳ(95%vs.54%,P<0.05);No vascular cancer thrombus vs.cancer thrombus(94%vs.88%,P<0.05).Protective factors affecting DFS of dMMR right colon cancer included age<50 years,no mucous or sig-ring cells,M0,StageⅠ-Ⅲ,and no intravascular cancer thrombus(P<0.05).In survival analysis,M0 was compared with M,(90%vs.63%,P<0.05).Stage Ⅰ-Ⅲ compared with stage Ⅳ(92%vs.57%,P<0.05);There was a significant difference between non-vascular cancer thrombus and vascular cancer thrombus(91%vs.77%,P<0.05).Conclusion The patients with dMMR had higher OS and DFS than pMMR,which were not affected by clinical factors.
5.Development of an exercise program for patients with osteoporotic vertebral compression fractures

Objective To develop a rehabilitation exercise program for the patients with osteoporotic vertebral compression fractures.Methods An online search was performed across both Chinese and English databases and websites to retrieve literature on rehabilitation exercises and related therapeutic measures for patients with osteoporotic vertebral compression fractures.Two researchers independently screened the retrieved literature,evaluated the quality,and extracted relevant evidence.Semi-structured interviews were conducted among 15 patients at various rehabilitation phases about the requirements of rehabilitation.A preliminary exercise program was developed and verified for the validity and feasibility using Delphi method.Results A rehabilitation exercise program was formulated.The program included five periods of preoperative period,bedridden period after surgery,ambulation period(1 week after surgery),weeks 2-8 after surgery and weeks 9-12 after surgery covering six domains of exercise type,exercise items,exercise duration,exercise frequency,exercise intensity and exercise safety.The response rate and effective rate over the two rounds of expert consultation were both 100.00%,with an authority coefficient of 0.94.The Kendall's W coefficients for secondary indicators in the two rounds of expert consultation were 0.184 and 0.334,respectively(both P<0.001).Conclusion The exercise program developed in this study for the patients with osteoporotic vertebral compression fractures is scientifically reliable and reasonable.The two features of continuity and staged characteristics in rehabilitation process are fully considered,thereby it offers a guidance for clinical healthcare professionals as well as the patients in development of practical and effective rehabilitation exercise plans.

7.Mechanism of NPPA/NPR1 signaling promoting ovarian follicle expan-sion in mice

AIM:To explore the role and mechanism of natriuretic peptide A(NPPA)/natriuretic peptide re-ceptor 1(NPR1)signaling in luteinizing hormone(LH)-induced ovarian follicle expansion in mice.METHODS:Three-week-old female mice were injected with pregnant mare serum gonadotropin/equine chorionic gonadotrophin(eCG)48 h before use to stimulate follicle development,or injected with eCG followed by human chorionic gonadotropin(hCG)treat-ment for 4 or 8 h,with 6 mice in each group.Periodic acid-Schiff(PAS)staining in ovarian sections was used to observe the follicular diameter,and qPCR was used to detect the expression of Npr1 mRNA levels in granulosa cells.Wild-type(WT)and Npr1 knockout(Npr1-/-)mice were injected with eCG,or injected with eCG followed by hCG treatment for 4 h.PAS staining was used to observe the follicular diameter,with 6 mice in each group.Superovulation and fertility assays were performed to count the number of ovulated oocytes and pups,with 6 mice in each group subjected to superovulation and 7 mice in each group subjected to fertility assays.The isolated follicles were randomly divided into control,NPPA,LH,LH+NPPA,LH+NPPA+KT5823[protein kinase G(PKG)inhibitor],and LH+NPPA+PPQ-102[cystic fibrosis transmembrane conductance regulator(CFTR)inhibitor]groups,with 20 follicles in each group.In addition,WT and Npr1-/-mice were injected with eCG 48 h or eCG 48 h/hCG 4 h,with 3 mice in each group.In vitro follicle culture and qP-CR were used to explore the effects of PKG and CFTR signalling on NPPA/NPR1-induced follicular expansion.The prolif-eration of granulosa cells in WT and Npr1-/-mice was detected via immunofluorescence staining,with 5 mice in each group.RESULTS:The results revealed that hCG treatment significantly increased the follicle expansion and Npr1 mRNA expression in granulosa cells(P<0.01).The capacity for ovarian follicle expansion,the number of ovulations,and the number of pups per litter were significantly lower in Npr1-/-mice compared with WT mice(P<0.01).In in vitro follicular culture,the addition of PKG inhibitor KT5823 and CFTR inhibitor PPQ-102 significantly inhibited NPPA/NPR1-induced ovarian follicle expansion(P<0.01),and the expression of Pkg mRNA and Cftr mRNA in granulosa cells of Npr1-/-mice was significantly lower than that in WT mice(P<0.01).In addition,the proliferation ability of granulosa cells in Npr1-/-mice was significantly lower than that in WT mice(P<0.01).CONCLUSION:LH promotes mouse ovarian follicle ex-pansion by NPPA/NPR1 signalling-induced PKG and CFTR pathways and granulosa cell proliferation.
