1.Research progress on influencing factors of prognosis in elderly patients with breast cancer

Breast cancer is a malignant tumor that occurs in the glandular epithelium of the breast, and it is one of the most common tumors that seriously affect the physical and mental health of women. With the aggravation of population aging, the incidence of breast cancer in the elderly has increased year by year in recent years. Elderly patients with breast cancer often have a variety of underlying diseases, and their prognosis is usually related to many factors such as cancer staging, cancer classification, treatment status and health status, with a significant difference in survival rate among patients. Due to the unique clinical and pathological characteristics of elderly patients with breast cancer compared to young and middle-aged patients, there are many studies on the treatment of elderly breast cancer patients in the past, and there are few reviews on the influencing factors of prognosis in elderly patients. This paper reviews the research progress of influencing factors of prognosis in elderly patients with breast cancer from the aspects of clinicopathological factors, treatment options and prognosis factors, in order to provide a reference for clinical determination of treatment options for elderly patients with breast cancer in the future.
2.Impact of visceral fat area on significant liver fibrosis in patients with nonalcoholic fatty liver disease and establishment of a predictive model

ObjectiveTo investigate whether visceral fat area (VFA) is an independent risk factor for significant liver fibrosis in patients with nonalcoholic fatty liver disease (NAFLD) based on clinical data, and to establish an effective diagnostic model. MethodsA total of 222 NAFLD patients who attended Department of Hepatology, Guangdong Provincial Hospital of Traditional Chinese Medicine, from January 2021 to April 2025 were enrolled, and according to liver stiffness measurement (≥8 kPa or not), they were divided into significant fibrosis group and non-significant fibrosis group. Propensity score matching (PSM) was performed at a ratio of 1∶1 to balance the baseline data between the two groups. The independent-samples t test or the Mann-Whitney U test was used for comparison of continuous data between two groups; the chi-square test was used for comparison of categorical data between groups. A Spearman correlation analysis was used to determine the correlation of VFA and other indicators with significant liver fibrosis; univariate and multivariate logistic regression analyses were used to identify whether VFA was an independent risk factor for significant liver fibrosis in NAFLD patients, and the receiver operating characteristic (ROC) curve was plotted to assess the predictive performance of related indicators. ResultsA total of 45 patients with significant liver fibrosis and 177 patients without significant liver fibrosis were enrolled, and after PSM, 90 patients (45 pairs) were finally included in analysis. Compared with the non-significant fibrosis group, the significant fibrosis group had significantly higher levels of body mass index (BMI), fasting blood glucose (FBG), glycated hemoglobin (HbA1c), uric acid (UA), alanine aminotransferase (ALT), aspartate aminotransferase (AST), gamma-glutamyl transpeptidase (GGT), controlled attenuation parameter (CAP), and VFA, as well as a significantly higher proportion of patients with visceral fat obesity or three or more metabolic risk factors (all P<0.05). VFA, BMI, AST, and HbA1c were strongly correlated with significant liver fibrosis (all r>0.5, all P <0.05), and ALT, GGT, UA, FBG, and CAP were significantly positively correlated with significant liver fibrosis (r=0.3 — 0.5, all P<0.05). VFA (odds ratio [OR]=1.040, 95% confidence interval [CI]: 1.018 — 1.062, P<0.05), FBG (OR=2.372, 95%CI: 1.199 — 4.691, P<0.05), and AST (OR=1.032, 95%CI: 1.003 — 1.058, P<0.05) were independent risk factors for significant liver fibrosis in NAFLD patients. The new diagnostic model based on VFA, FBG, and AST (with an area under the ROC curve [AUC] of 0.907) had a significantly better performance than aspartate aminotransferase-to-platelet ratio index (AUC=0.834), fibrosis-4 (AUC=0.660), triglyceride-glucose index (AUC=0.656), and NAFLD fibrosis score (AUC=0.768) in predicting significant liver fibrosis in NAFLD patients (all P<0.05). ConclusionVFA is an independent risk factor for significant liver fibrosis in NAFLD patients, and the noninvasive diagnostic model based on VFA, FBG, and AST can effectively predict the onset of significant liver fibrosis in NAFLD patients.
3.Role of TLR4/ERK1/2 signaling pathway in progesterone regulating IL-8 expression in decidual stromal cells

Objective:To explore the mechanism of TLR4/ERK1/2 signaling pathway in progesterone regulating expression of IL-8 in decidual stromal cells(DSCs).Methods:Human DSCs on early pregnancy were isolated and cultured in vitro.The cells were treated with 0.01 μmol/L,0.1 μmol/L and 1 μmol/L progesterone,respectively.Expressions of IL-8,TLR4,ERK1/2 and p-ERK1/2 were detected by Western blot.TLR4 inhibitor TAK-242 and ERK1/2 inhibitor U0126 were further treated,and expression of IL-8 in progesteron+inhibitor group,progesterone group and control group was detected by Western blot.Results:With the increase of pro-gesterone concentration,expression of IL-8 in DSCs decreased gradually.Expressions of DSCs IL-8,TLR4 and p-ERK1/2 in 0.01 μmol/L progesterone group were lower than those in control group(P=0.035 8,P=0.019 4 and P=0.047 1).Expressions of IL-8,TLR4 and p-ERK1/2 in DSCs in 0.1 μmol/L progesterone group were significantly lower than those in control group(P=0.003 5,P=0.004 0 and P=0.009 9).Expressions of IL-8,TLR4 and p-ERK1/2 in DSCs in 1 μmol/L progesterone group were significantly lower than those in control group(P=0.003 3,P=0.001 6 and P=0.005 0).Expression of IL-8 in progesterone+TAK-242 group was signifi-cantly lower than that in progesterone group(P=0.009 1),and expression of IL-8 in progesterone+U0126 group was significantly lower than that in progesterone group(P=0.004).Conclusion:TLR4/ERK1/2 signaling pathway is involved in the physiological process of progesterone regulating expression of IL-8 in DSCs,and progesterone reduces IL-8 expression by inhibiting expression of TLR4 and phosphorylation activation of ERK1/2.
4.A family study of cataplexy and epilepsy due to mutations in the KCNA1 gene and literature review

Objective:To investigate the clinical and genetic characteristics of a family with cataplexy and epilepsy caused by KCNA1 gene mutations. Methods:The clinical data of a family with KCNA1 gene mutations leading to cataplexy and epilepsy who hospitalized in the Department of Pediatric Neurology at Anhui Children's Hospital in August 2022 were collected,and their clinical manifestations,imaging,electroencephalogram,gene testing results and treatment were analyzed. A total of 68 pathogenic or potentially pathogenic variants of the KCNA1 gene were identified by searching the database of CNKI,Wanfang Data Knowledge Service Platform, ClinVar, dbSNP and PubMed using the keyword‘KCNA1’between the establishment and August 2023. Results:The proband was a 9 years and 8 months old boy who initially presented with cataplexy induced by strenuous exercise or fatigue,followed by focal epilepsy. The whole exome sequencing detected heterozygous variation of exon 2 c.1006G>A(p.Gly336Arg)of KCNA1 gene,which was a missense mutation and was not reported in the country or abroad. Both the mother and brother of the proband detected heterozygous mutations at the same locus,and both had cataplexy induced by strenuous exercise or fatigue,but the type of seizure was generalized tonic-clonic seizure. The proband's grandmother,aunt,and brother all had seizures or cataplexy. Affected patients receiving different or the same anti-seizure drugs(sodium valproate,lamotrigine,phenytion sodium and carbamazepine)had varying degrees of relief,and those treated with sodium channel blockers had varying degrees of relief. A total of 68 mutation sites of KCNA1 gene were retrieved from domestic and foreign literature,mainly missense mutations,and most patients showed episodic ataxia type 1(EA1),and there was genetic heterogeneity between the genotype and phenotype of the variable patients. Conclusion:We have reported a heterozygous mutation in the KCNA1 gene c.1006G>A(p.Gly336Arg),which is a missense mutation and is easy to misdiagnose in patients with cataplexy and epilepsy as the main phenotypes. Patients with the KCNA1 mutation have different degrees of efficacy on sodium channel blockers.
5.Analysis of the construction status and framework of the standard system of smart health and elderly care in China

Objective:To analyze the current status of the standard system construction for smart health and elderly care in China,analyze the framework of the standard system,and propose policy recommendations for the construction of the standard system.Methods:The standard system of smart health and elderly care were searched on the websites of the State Council of China,the National Standard Information Public Service Platform,and the sub-websites of national,industry,local,group,and enterprise standards.Policy text analysis was used to explore the current status and framework of standard system construction.Results:Currently,67 standards related to smart health and elderly care in China were included,including 0 national standards,2 industry standard,10 local standards,31 group standards,and 24 enterprise standards.These standards formed a subsystem that covered general,data,product,platform,and service standards.Conclusion:The construction of the standard system of smart health and elderly care in China has achieved certain results,but there are still shortcomings,and it is necessary to strengthen the top-level design and build a perfect framework of the standard system.
6.Correlation of changes in inner retinal irregularity index after idiopathic macular epiretinal membrane stripping with visual function

Objective To investigate the correlation between changes in the inner retinal irregularity index(IRII)af-ter macular epiretinal membrane(ERM)stripping and visual function in patients with idiopathic macular epiretinal mem-brane(IMEM).Methods A retrospective analysis was conducted on the clinical data of 134 patients(162 eyes)with IMEM who visited our hospital from March 2022 to December 2024.Based on fundus color photography and OCT findings,the patients were divided into the cellophane macular reflex(CMR)group and the preretinal macular fibrosis(PMF)group.Differences in central macular thickness(CMT),best-corrected visual acuity(BCVA),inner retinal thickness(IRT),and IRII were analyzed between the two groups.The diagnostic efficacy of these indicators for IMEM at admission was analyzed using receiver operating characteristic(ROC)curves.The differences in visual function indicators and IRII before and 3 months after ERM stripping were compared in IMEM patients.The correlation between IRII levels and visual function parameters at 3 months postoperatively was analyzed using Pearson correlation analysis.Results Among the 134 patients(162 eyes)with IMEM,93 patients(103 eyes)were in the CMR group and 41 patients(59 eyes)were in the PMF group.The CMT,IRT,and IRII were significantly lower in the CMR group than in the PMF group(all P<0.05).The areas under the curve(AUC)for CMT,IRT,and IRII were 0.616,0.609,and 0.862,respectively(all P<0.05).Among the 134 patients(162 eyes)with IMEM,79 patients(86 eyes,53.86%)underwent surgical treatment,and 55 patients(76 eyes,46.91%)received non-surgical treatment.Three months after surgery,the CMT,IRT,and IRII in IMEM patients were sig-nificantly lower than those before surgery(all P<0.05).At 3 months postoperatively,CMT and IRT were significantly pos-itively correlated with IRII in IMEM patients(all P<0.05).Conclusion CMT,IRT,and IRII all have good diagnostic ef-ficacy for IMEM staging.Moreover,IRII changes in IMEM patients who underwent macular ERM stripping are positively correlated with visual function.

9.Effect of p38MAPK signaling pathway mediating progesterone down-regulation of IL-8 secretion by decidual stromal cells on early spontaneous miscarriage

Objective:To investigate the effect of p38MAPK signaling pathway mediating progesterone regulation on IL-8 protein secretion by decidual stromal cells(DSCs)on early spontaneous miscarriage.Methods:IHC and Western blot were applied to detect protein expressions of p38MAPK and p-p38MAPK in decidual tissues of miscarriage group and control group.Human DSCs in early pregnancy were isolated in vitro and cultured to be treated with different concentrations of progesterone(0.01 μmol/L,0.1 μmol/L,1 μmol/L and 10 μmol/L),with ELISA measuring IL-8 protein secretion from DSCs and Western blot measuring protein expressions of p38MAPK and p-p38MAPK in DSCs.After treatment with p38MAPK inhibitor SB203580,IL-8 protein secretion was detected by ELISA in progesterone+inhibitor group,progesterone group and control group.Results:Protein expression of p-p38MAPK in decidual tissues of miscarriage group was significantly higher than that of control group(P=0.002 3).Protein secretion of IL-8 in DSCs of 0.01 μmol/L progesterone group was lower than that of control group(P=0.027 6),protein expression of p-p38MAPK in DSCs of 0.1 μmol/L proges-terone group was lower than that of control group(P=0.025 3),IL-8 protein expression was significantly lower than that in control group(P=0.007 0),and protein expressions of both IL-8 and p-p38MAPK from DSCs in 1 μmol/L and 10 μmol/L progesterone groups were significantly lower than those in control group(P=0.003 2,P=0.001 9;P=0.002 2,P=0.001 3).IL-8 protein secretion in proges-terone+p38MAPK inhibitor group was further reduced compared to progesterone group(P=0.046 6).Conclusion:Abnormal activation of p38MAPK signaling pathway is involved in early spontaneous miscarriage,and progesterone may down-regulate IL-8 expression in DSCs by inhibiting p38MAPK phosphorylation to correct early spontaneous miscarriage caused by Th1/Th2 cytokine imbalance.
10.Analysis of differences on contraindication information for children in domestic and foreign drug instructions

Objective:To explore the differences on contraindication information for children in domestic and foreign drug instructions, and provide reference for improving the relevant information in Chinese drug insert sheets.Methods:Chinese drug insert sheets of chemicals and biological products contained in the China Pharmacopoeia 2020 and those of the western medicines in the 2023 China′s Basic Medical Insurance, Work-related Injury Insurance and Childbirth Insurance Drug Catalog were collected; drugs that were marked as contraindication for children were selected and relevant contraindication information in the Chinese drug insert sheets was collected. Instructions of the above-mentioned drugs approved by the U.S. Food and Drug Administration (English labels) were also collected, and the information on pediatric medication was reviewed and compared with the Chinese drug insert sheets. Results:A total of 222 drugs were labeled as contraindication for children in the Chinese drug insert sheets, of which 149 were available for their English labels; 123 drugs (17.5%) were not labeled as contraindication for children in English labels, and 26 (82.5%) were labeled. The 123 drugs that were not labeled as contraindication for children in the English labels included the following conditions: 58 were labeled as contraindication for children of some age in the Chinese drug insert sheets but not in the English labels, and relevant medication information was provided; 40 were labeled as contraindication for children of some age group in the Chinese drug insert sheets but was described as the effectiveness and safety of the use for children have not yet been determined for this age group in the English labels; 13 were labeled as contraindication for children in the Chinese drug insert sheets, but the medication information on children in the English labels was not clear or missing; 12 were labeled as contraindicated for children in Chinese drug insert sheets but not in the English labels, only expressed as not yet determined or not recommended for use, etc., with inconsistent age group. Among the 26 drugs labeled as contraindication for children in both Chinese and English instructions, the contraindication age group were the same in above 2 instructions for 20 drugs, and were inconsistent for the other 6 drugs; reasons for contraindication were described in both the 2 instructions for 17 drugs (13 were consistent, 4 were inconsistent), only in English labels for 8 drugs, and only in Chinese drug insert sheets for 1 drug.Conclusions:Many drugs are labeled as contraindication for children in Chinese drug insert sheets, but reasons for contraindication are rarely explained. Differences in children′s age in contraindications exist for some drugs between the Chinese drug insert sheets and English labels. The information on contraindications for children in Chinese drug insert sheets still needs to be further improved.
