1.Isolation and identification of yak-derived Enterococcus from four provinces of Tibetan plateau and detection of virulence resistance genes and biofilms

This study aims to understand the epidemic distribution characteristics,antimicrobial resistance,virulence genes,and biofilm adhesion ability of Enterococcus in yaks on the Tibetan plateau.Three hundred and forty-six fresh yak fecal samples and 311 milk samples were collected from four provinces on the Tibetan plateau(Xizang,Sichuan,Gansu,Qinghai),totaling 657 sam-ples.Bacterial isolation and identification were conducted,followed by 16S rDNA gene detection and the construction of a systematic evolutionary tree.The isolated strains were tested for antimi-crobial resistance and virulence genes using PCR,and sensitivity tests were performed using 18 types of antibiotics.The biofilm adhesion ability of the isolated bacteria was determined using an improved semi-quantitative crystal violet staining method.The results showed that the total isola-tion rate of Enterococcus was 32.27%,with Sichuan having the highest at 60.23%,followed by Gansu,Qinghai,and Tibet autonomous region at 42.70%,23.47%,and 18.31%respectively.In terms of sample types,the isolation rate in fecal samples was 36.71%,and in milk samples,it was 27.33%.Through PCR amplification,bands of approximately 1 400 bp were obtained,and 5 strains were selected for evolutionary analysis,forming a separate cluster.Among the 212 isolated strains,a high resistance to clindamycin,quinupristin-dalfopristin,linezolid,levofloxacin,and erythromycin was observed,with various resistance phenomena,accounting for 60.85%.Only 5 out of 12 resist-ant genes were detected,namely erm(B),tet(L),tet(O),tet(M),and ant(6)-Ia.All 13 virulence genes were detected in Enterococcus,with detection rates in the range of 5.19%to 95.76%,where cpd was 95.75%,gelE was 91.98%,efaA was 86.79%,asal was 86.32%,and the rest ranged from 5.19%to 55.66%.The fsr virulence gene was not detected in Enterococcus from milk sources.Among the isolated strains,3.30%showed medium adhesive ability,48.58%showed weak adhesive ability,and 48.11%showed no adhesive ability.The above research revealed the preva-lence of yak derived Enterococcus,the carrying status of resistance and virulence genes,and the correlation between biofilm phenotypes,laying the foundation for mastering research data on yak-derived Enterococcus in the Tibetan plateau.
2.Clinical observation of thunder-fire moxibustion combined with fire-dragon cupping in the treatment of acute-phase peripheral facial palsy with co-morbid anxiety and depression

Objective:To evaluate the effect of thunder-fire moxibustion combined with fire-dragon cupping on patients with acute peripheral facial paralysis with co-morbid anxiety and depression.Methods:A total of 80 patients with acute peripheral facial paralysis with co-morbid anxiety and depression were selected and divided into two groups according to the random number table method,with 40 patients in each group.In the control group,acupuncture therapy combined with routine care was given;in the observation group,thunder-fire moxibustion and fire-dragon cupping therapy were added.Both groups were treated once a day,6 times a week,with 1 d of rest,for 4 consecutive weeks(a total of 24 treatments).The House-Brackmann(H-B)grading,facial clinimetric evaluation(FaCE),self-rating anxiety scale(SAS),and self-rating depression scale(SDS)were used before and after the intervention to evaluate the facial nerve function,quality of life,mental health,and clinical efficacy of the two groups.Results:After treatment,patients in both groups showed improvement in H-B grading,scores of all dimensions of the FaCE scale,and SAS and SDS scores compared to those before treatment,and the differences within the groups were statistically significant(P<0.05).After treatment,the total effective rate was 95.0%in the observation group and 75.0%in the control group,and the difference between the two groups was statistically significant(P<0.05).After treatment,the H-B grading of the observation group was better than that of the control group(P<0.05);the scores of all dimensions of the FaCE scale and SAS and SDS scores of the observation group were better than those of the control group(P<0.05).Conclusion:Adding thunder-fire moxibustion and fire-dragon cupping therapy to acupuncture and routine care can promote the recovery of facial nerve function,reduce anxiety and depression,and improve the patients'living standard,and thus is suitable for popularization and application.

5.Enhancement Effect of Pre-Treatment 18F-FDG PET/CT Radiomic Features in Prognostic Evaluation of Diffuse Large B-Cell Lymphoma

Purpose To investigate the predictive value of 18F-FDG PET/CT radiomic features before treatment for progression free survival(PFS)and overall survival(OS)in diffuse large B-cell lymphoma(DLBCL).Materials and Methods A total of 135 patients with pathologically proven DLBCL in Chinese PLA General Hospital from January 2016 to December 2018 and 18F-FDG PET/CT imaging prior to treatment were retrospectively collected.Patients were randomly divided into training set and test set using 8∶2,and then the training set was divided into training set and verification set using 8∶2 to construct the model.Semi-automatically delineated patients'lymphoma lesions as regions of interest and extracted the features.Univariate COX and least absolute shrinkage and selection operator regression were used to screen the features,and the non-zero radiomic features were obtained and the weight coefficients were used to calculate the Radscore value of each patient,and the predictive value of Radscore on PFS and OS was analyzed.Three models were established using traditional prognostic indicators(metabolic parameters and clinical factors),Radscore and combination.C-index,time-dependent area under the curve and decision curve were used to evaluate the model prediction efficiency.Finally,based on the optimal model,a column diagram was drawn,and the calibration curve was used to verify the efficiency of the column diagram.Results The combined model predicted PFS and OS at 3 and 5 years better than the traditional prognostic index model and Radscore model(Z=0.962 1-2.253 9,all P<0.05).Decision curve showed that the combined model achieved the greatest clinical net benefit.The calibration curve showed that the predicted values of the nomogram were in good agreement with the observed values.Conclusion Radscore is an independent prognostic factor for survival in DLBCL patients.The combined model has great application value in guiding the formulation of clinical individualized treatment plan.
6.11C-methionine PET/CT radiomics model for evaluating isocitrate dehydrogenase1 status of glioblastoma

Objective To explore the value of 11C-methionine(MET)PET/CT radiomics model for evaluating isocitrate dehydrogenase1(IDH1)status of glioblastoma.Methods Data of 157 patients with glioblastoma who underwent 11C-MET PET/CT examination,including 68 cases of IDH1 mutation and 89 cases of IDH1 wild-type were retrospectively analyzed.The patients were divided into training set(n=125)and validation set(n=32)at the ratio of 8:2.Based on PET/CT images,lesions ROI were delineated and radiomics features were extracted and screened to establish radiomics models with logistic regression(LR),support vector machine(SVM)and decision trees(DT),respectively.Meanwhile,the nomogram based on patients'age and radiomics features was drawn.The efficacy of radiomics models and clinical-radiomics nomogram for evaluating IDH1 status were comparatively observed.Results The area under the curve(AUC)of DT radiomics model for evaluating IDH1 status of glioblastoma in training set was 0.910,higher than that of LR(0.697)and SVM(0.698)models(both P<0.05).In validation set,the AUC of DT model for evaluating IDH1 status of glioblastoma was 0.805,which was higher than that of LR model(0.740)and clinical-radiomics nomogram(0.704)(both P<0.05).Conclusion 11C-MET PET/CT radiomics model based on DT was helpful for evaluating IDH1 status of glioblastoma.
7.Application of cardiac magnetic resonance imaging in diagnosing troponin increase with non-obstructive coronary arteries

Objective:To investigate the utility of cardiac magnetic resonance (CMR) multiparametric imaging in the etiological and differential diagnoses of troponin increase with non-obstructive coronary arteries (TINOCA).Methods:A retrospective analysis was conducted on patients diagnosed with TINOCA and confirmed by coronary angiography in Dongguan Kanghua Hospital from January 2018 to June 2023. CMR examinations were performed within 7 days of onset. The examination sequences included "black blood" single-shot balanced turbo field echo with breath-hold, balanced turbo field echo with breath-hold, T 2-weighted short tau inversion recovery black blood, modified gradient and spin echo black blood with SENSE, dynamic balanced turbo field echo, and phase-sensitive inversion recovery [late gadolinium enhancement (LGE)]. Based on the imaging findings, patients were categorized into 4 groups: myocardial infarction with non-obstructive coronary arteries (MINOCA) group, acute myocarditis group, Takotsubo syndrome group, and CMR negative group. The observed indices included left ventricular function, myocardial edema, first perfusion, and LGE of contrast enhancement. The differences in these parameters among the aforementioned disease groups were statistically compared. The categorical data were analyzed between groups using the chi-square test or Fisher′s exact probability method. The data in line with normal distribution were statistically described by xˉ±s. The independent-sample t test was used to compare the means of the 2 samples. The data with skewed distribution were described by M ( Q1, Q3). The Mann-Whitney U test was used for intergroup analysis. Results:A total of 33 patients were enrolled in this study, the 4 groups comprised 14(42.4%), 12(36.4%), 0, and 7(21.2%) patients, respectively. The positive rate of CMR diagnosis was 78.8%. The onset age in the MINOCA group significantly differed from that in the acute myocarditis group ( Z=3.32, P=0.001). No significant differences were observed in left ventricular function, number of myocardial edema segments, and T 2 value of the diseased myocardium between the 2 groups ( P>0.05), but the number of abnormal first perfusion was significantly distinct ( P<0.001). Significant differences were observed in the quality and volume of LGE between the 2 groups ( P<0.05 for all). Conclusion:CMR multiparameteric imaging technology plays a unique role in the etiological diagnosis of TINOCA, accurately distinguishing TINOCA caused by MINOCA, acute myocarditis, and other cardiac causes.
8.Porphyromonas gingivalis bacteremia increases the permeability of the blood-brain barrier via the Mfsd2a/Caveolin-1 mediated transcytosis pathway.

Bacteremia induced by periodontal infection is an important factor for periodontitis to threaten general health. P. gingivalis DNA/virulence factors have been found in the brain tissues from patients with Alzheimer's disease (AD). The blood-brain barrier (BBB) is essential for keeping toxic substances from entering brain tissues. However, the effect of P. gingivalis bacteremia on BBB permeability and its underlying mechanism remains unclear. In the present study, rats were injected by tail vein with P. gingivalis three times a week for eight weeks to induce bacteremia. An in vitro BBB model infected with P. gingivalis was also established. We found that the infiltration of Evans blue dye and Albumin protein deposition in the rat brain tissues were increased in the rat brain tissues with P. gingivalis bacteremia and P. gingivalis could pass through the in vitro BBB model. Caveolae were detected after P. gingivalis infection in BMECs both in vivo and in vitro. Caveolin-1 (Cav-1) expression was enhanced after P. gingivalis infection. Downregulation of Cav-1 rescued P. gingivalis-enhanced BMECs permeability. We further found P. gingivalis-gingipain could be colocalized with Cav-1 and the strong hydrogen bonding between Cav-1 and arg-specific-gingipain (RgpA) were detected. Moreover, P. gingivalis significantly inhibited the major facilitator superfamily domain containing 2a (Mfsd2a) expression. Mfsd2a overexpression reversed P. gingivalis-increased BMECs permeability and Cav-1 expression. These results revealed that Mfsd2a/Cav-1 mediated transcytosis is a key pathway governing BBB BMECs permeability induced by P. gingivalis, which may contribute to P. gingivalis/virulence factors entrance and the subsequent neurological impairments.

9.Current Status and Challenges of Biological Sample Management in Clinical Trials

With the rapid development of clinical trials, the relevant medical research and molecular detection based on biological samples are closely related to the progress of clinical trials, making the role of biological samples in clinical trials increasingly obvious. The standardized supervision mode of biological samples is an important prerequisite for carrying out high-quality clinical trials. Although the laws and regulations related to clinical trials are becoming more and more perfect, there are still a large number of adverse events related to biological samples, which seriously affects the progress and results of clinical trials, and is one of the important challenges currently facing. Therefore, it is urgent to enhance the supervision of biological samples and improve the management methods of biological samples in clinical trials at this stage. Through in-depth discussion of the current status of biological sample management in clinical trials at home and abroad, this paper analyzed the issues existed during the supervision of biological samples, and supplemented the biological sample management methods by further combing the existing relevant laws and regulations and the Guidelines for the Ethical Management of Biological Samples in Clinical Trials, with a view to providing suggestions and ideas for optimizing the management mode of biological samples in clinical trials.
10.Meta-synthesis of qualitative research on experience of type 2 diabetes patients using self-management APP

Objective:To integrate qualitative research on the experience of type 2 diabetes patients using self-management APP, so as to provide evidence support for the design and promotion of highly accepted diabetes self-management APP.Methods:This study is a Meta-synthesis. Qualitative research on experience of type 2 diabetes patients using self-management APP was retrieved through computer in China National Knowledge Infrastructure (CNKI) , WanFang, VIP, China Biology Medicine disc Literature Database, PubMed, Web of Science, Cochrane Library, Embase, CINAHL, PsycINFO, Ovid (LWW Journals) , Scopus Chinese and English databases. The retrieval time limit was from the establishment of the database to April 30, 2022. According to the quality evaluation criteria for qualitative research of the Joanna Briggs Institute (JBI) Evidence-Based Health Care Center, the methodology of the included article was evaluated, and the included article was integrated with the method of pooled Meta-synthesis.Results:A total of 11 articles were included, 48 research findings were extracted and summarized into 3 integration results and 14 new categories. The integration results included that self-management APP promoted patients to form continuous self-management behavior and improved physical and mental health, obstacle factors that affected patients' use of APP, and patients' suggestions for improving the function of APP.Conclusions:Patients feel the benefits of self-management APP, such as correct understanding of diabetes, disease control, and improvement of psychological status. When developing relevant APP, we should pay attention to the change of patient behavior, design the APP function based on user needs, conduct cultural adjustment, improve training and technical support, and improve its acceptance and promotion.
