1.Osler’s view of the physician and physician’s narrative literacy in narrative medicine

In the era of evidence-based medicine, the progress of medical science and technology has enriched medical diagnostic tools and treatment methods, but it has also led to the loss of medical warmth and the alienation of the doctor-patient relationships. William Osler emphasized that while medical technology advances, attention should also be paid to the practice of narrative medicine and the development of physician’s narrative literacy. The view of the physician he advocated reminds us that the core of medicine still lies in the narrative connection between doctors and patients, as well as a deep understanding of human nature. By exploring the relationship between Osler’s view of the physician and narrative medicine as well as physician’s narrative literacy, this paper analyzed the methods of cultivating physician’s narrative literacy, providing references for modern medical education and practice, and assisting in the harmony and unity of science and technology and humanity.
2.Effects of prostaglandin D2 on cytokine secretion and phagocytosis and killing function in cow bone marrow-derived macrophages induced by E.coli

In order to study the effects of prostaglandin D2(PGD2)on cow bone marrow-derived macrophages induced by E.coli,cultured cow bone marrow-derived macrophages were taken as the research object.The effects of endogenous and exogenous PGD2 on the secretion and phagocytosis of E.coli induced proinflammatory cytokines(IL-1β,IL-6 and TNF-α)in macrophages were ana-lyzed.The results showed that the synthesis of PGD2 in macrophages induced by E.coli is depend-ent on the natural pattern recognition receptors TLR2,TLR4 and NLRP3.Inhibition of endogenous PGD2can down-regulate the secretion of pro-inflammatory cytokines(IL-1β,IL-6 and TNF-α)in E.coli induced macrophages(P<0.001),and inhibition of endogenous PGD2 can enhance the kill-ing function of macrophages to a certain extent(P<0.01).In addition,exogenous PGD2 could up-regulate the secretion of pro-inflammatory cytokines(IL-1β,IL-6 and TNF-α)in macrophages af-ter E.coli stimulation(P<0.01),and exogenous PGD2 could weaken the killing function of mac-rophages within a certain concentration range(P<0.01).Results indicated that PGD2 had certain effects on the secretion of cytokines and phagocytosis and killing function of macrophages induced by E.coli.

4.Screening Pathways and Practical Research on Primary Care Disease Groups under the DIP Payment

Objective:To explore screening pathways and practical implementation strategies for primary care disease groups un-der the Diagnosis-Intervention Packet(DIP)payment.Methods:Matching and expert consultation methods were used to select primary care conditions,and experts included DIP National Steering Group experts,DIP experts from a sample municipal health-care security bureau,health policy researchers,coders from a tertiary hospital in Beijing,and big data engineers.Results:The DIP primary disease of"conservative treatment groups"in primary and secondary medical institutions were 104 and 105,repective-ly.The DIP primary disease of"primary can be completed groups"were 74 and 171,respectively.The final number of primary diseases in primary and secondary hospital was determined to be 178 and 276,respectively.Conclusions:The number of primary care disease groups varies from place to place,but it still needs to be dynamically adjusted and optimized in conjunction with big data methods and expert consultation,evaluated and corrected in a timely manner to ensure scientificity and effectiveness.The pri-mary care disease groups implement"same price for the same disease"across different levels of medical institutions,eliminating the differentiation of medical institution grade coefficients to promote hierarchical diagnosis and treatment.Cost-value measurement of disease group,scientific development of payment criteria for disease group,lack of regulatory tools for big data,and high sets of codes are key barriers to primary care implementation.

6.Analysis of the current application of the Consolidated Framework for Implementation Research in the field of public health

Evidence-based public health, as the forefront of modern public health practice, has increasingly important in public health field. However, a significant gap remains between the available evidence and its practical application. Effectively disseminating and implementing evidence-based public health practice in real-world settings has become a key challenge in contemporary public health research. In this context, Implementation Science has emerged as a vital discipline. This paper explores the critical role of Implementation Science in public health, reviews the origins and core components of the Consolidated Framework for Implementation Research (CFIR), and analyzes the current application of CFIR in public health through bibliometric methods. Additionally, it discusses specific examples to further elucidate the steps involved in using the CFIR and its application contexts. The findings indicate that since 2015, research on CFIR in public health has progressively increased, showing a continuous upward trend. CFIR applications mainly address context-specific facilitators, health decision-making, barrier and facilitator identification, and community-based participatory evaluation, predominantly employing qualitative and mixed-methods research. This paper not only reviews and analyzes the current use of CFIR in public health but also provides a detailed discussion on its application. The goal is to offer valuable insights for the development of Implementation Science research within China's public health sector.

Osteoporosis is a common senile bone metabolism disease, clinically characterized by decreased bone mass, destruction of bone microstructure, increased bone fragility, and easy fracture. It tends to occur in the elderly and postmenopausal women, seriously threatening the quality of life and physical and mental health of the elderly. At present, the treatment of osteoporosis is mainly based on oral western medicines, such as calcium, Vitamin D, and bisphosphonates. Still, there are drawbacks such as a long medication cycle and many adverse reactions. In recent years, due to the advantages of multi-component, multi-pathway, and multi-target, some traditional Chinese medicines and effective ingredients can regulate the osteogenic and osteoclastic differentiation process in both directions and are widely used in the prevention and treatment of osteoporosis. Hippophae rhamnoides is a commonly used herbal medicine, and its fruits are rich in flavonoids, polyphenols, fatty acids, vitamins, and trace elements, which have been proven to have a good anti-osteoporosis effect. Isorhamnetin is the main effective ingredient of Hippophae rhamnoides fruits, which has many pharmacological effects such as anti-inflammation, anti-oxidative stress, anti-aging, and anti-tumor. Studies have shown that isorhamnetin can participate in the regulation of bone metabolism and has a good anti-osteoporosis effect. However, the pharmacological effects and related mechanisms of isorhamnetin against osteoporosis have not been systematically summarized. Therefore, this paper reviewed the pharmacological effects and related mechanisms of isorhamnetin against osteoporosis by referring to relevant literature to provide more basis for the development and application of isorhamnetin.
