Objective To investigate the role of matrix GLA protein(MGP)in the pathogenesis of idiopathic calcium oxalate kidney stones(ICOS),and to find potential biomarkers for early diagnosis and disease evaluation.Methods A total of 120 patients admitted to our hospital from September 2021 to September 2023 were prospectively included,of which 60 patients with ICOS were in the calculus group and 60 patients without calculus were in the control group.Serum biochemical indexes and immunohistochemical scores of the two groups were detected,urinary MGP levels were determined by ELISA,and MGP mRNA and protein expression in renal papilla tissues were detected by qPCR and Western blot.The independent risk factors of ICOS were screened by Logistic regression analysis,and the prediction model was drawn by nomogram.Results Compared with control group,urinary MGP content in calculus group was decreased[(1 805.91±244.44)pg/ml vs.(2 014.79±252.14)pg/mnl,P＜0.05).Expression of MGP mRNA and MGP protein in renal papillae decreased(0.89±0.15 vs.1.00±0.00,P=0.001)and decreased(0.87±0.18 vs.1.00±0.00,P＜0.05).MGP immunohistochemical scores of renal tissue were decreased[4(2-6)scores vs.6(4-8)scores,P＜0.001].Multivariate analysis showed that urinary calcium(OR=1.370),urinary MGP(OR=1.127),renal papilla MGP relative expression level(OR=27.532)and renal tissue MGP immunohistochemical score(OR=1.359)were independent risk factors for ICOS.Area under ROC curve of the nomogram prediction model built based on the above factors is 0.839,indicating that the model has good differentiation ability in risk prediction.Conclusion MGP is closely related to the pathogenesis of ICOS.Urinary and renal tissue MGP levels may be potential biomarkers for early diagnosis and disease assessment of ICOS.

African swine fever(ASF)is a virulent,hemorrhagic disease of swine caused by African swine fever virus(ASFV),which seriously affects the healthy development of Chinese pig indus-try.The genome of ASFV is large and encodes more than 165 proteins.Among them,the p54 pro-tein is encoded by the E183L gene,which has various functions such as participating in viral as-sembly,inducing apoptosis and inducing immune response.The conventional Chinese vaccine(C-strain)is a safe and effective attenuated vaccine developed by Chinese scientists.It can efficiently protect against attacks from various genotypes of classical swine fever virus(CSFV).The aim of this study was to investigate whether C-strain can express ASFV p54 protein serve as a delivery vector for ASF genetically engineered vaccines.An infectious clone of pHCLV-p54 was constructed by homologous recombination,the recombinant virus rHCLV-p54 was rescued by transfecting it into SK6 cells by blind passaging.Its genetic stability and growth curve were determined in vitro,while rabbits were immunized to evaluate its immunity effect.The results showed that the E183L gene remained genetically stable in the recombinant virus,indicating that the E183L gene could be stably inherited in recombinant viruses,but the inserted exogenous gene affected the replication of the C-strain.The results of the rabbit immunization test showed that the recombinant virus rHCLV-p54 was able to induce ASFV-specific antibodies.The above results indicated that we have successfully constructed a recombinant C-strain that stably expresses the ASFV p54 protein.In summary,the recombinant virus rHCLV-p54 has a good immunogenicity and is warranted for fur-ther evaluation as a vaccine candidate.

Objective: To analyse the effects of physical activity on neurocognitive function in children with autism spectrum disorder (ASD),so as to provide a theoretical basis and specific guidance for the clinical cognitive function rehabilitation practice of ASD children. Methods: Relevant literature on the effects of physical activity on neurocognitive function in children with ASD were searched in the China National Knowledge Infrastructure (CNKI), Wanfang, Web of Science, and EBSCO databases from their inception to March 2024 using specific subject terms. Information on authors, countries, publication dates, study populations, diagnostic criterion, interventions, measurement tools and study results were extracted postscreening. The methodological quality of the articles was assessed using the PEDro Scale, followed by a systematic review. Results: A total of seventeen articles and were included in the study and English literature, most of them were of good quality. The study included 10 randomized controlled trials, 4 quasiexperimental designs, and 3 preand posttest designs. The rehabilitative effects of physical activity on children with ASD were primarily observed in enhanced attention and executive function, as well as improvements in brain structure and function. Children with ASD showed varying cognitive effects in physical and skillbased programs, with skillbased activities like basketball and table tennis notably improving cognitive function. Conclusion Physical activity is an effective intervention that enhances cognitive abilities such as attention and executive function, as well as brain structure and function in children with ASD.

Objective To review the application of artificial intelligence(AI)in the identification,monitoring and intervention of sleep disorders in the elderly and the effect. Methods A scoping review was conducted by searching relevant literature ahout the application of AI in the field of sleep disorders among the elderly from databases including PubMed,Web of Science,CNKI,and Wanfang data,covering literatures from the establishment to June,2024. Results A total of ten articles were included,originating from seven countries and involving 36 344 elderly participants.The publication dates ranged from 2020 to 2024.The study types included six cross-sectional studies,one pro-spective study,one self-controlled study and two randomized controlled trials.The articles mainly came from the fields of clinical medicine,sleep research,rehabilitation medicine and information engineering.AI was primarily used for monitoring the entire sleep process of the elderly,predicting and identifying sleep disorders,and inter-ventions such as biofeedback,online consultations and cognitive-behavioral therapy based on mobile platforms. Conclusion AI not only improves the accuracy of diagnosing sleep disorders,but also provides robust data support for clinical intervention.Online sleep interventions based on big data and intelligent algorithms can offer effective health management for the elderly.

Objective:To screen and diagnose cervical lesions in middle-aged and elderly postmenopausal women and analyze the characteristics of cervical lesions in middle-aged and elderly women.Methods:A retrospective analysis was conducted on the screening data of 1 337 postmenopausal women aged between 50 and 86 years who visited the outpatient clinics of the gynecology department of Beijing Hospital between January, 2015 and December, 2019.Participants were divided into groups based on age, with 787 in the 50-59 years old group, 483 in the 60-69 years old group, 60 in the 70-79 years old group, and 7 in the 80-89 years old group.Exfoliated cervical cells were collected, and high-risk human papillomavirus(HPV)detection and liquid-based cervical cytology examination were performed to confirm the diagnosis based on the pathological results of cervical biopsy.Cervical lesions were analyzed.Results:53.0%(708)of the 1 337 enrolled patients made their hospital visits because of symptoms.There were 666 patients(49.8%)with cervical lesions, including 357 with low-grade squamous intraepithelial lesions(LSIL), 252 with high-grade squamous intraepithelial lesions(HSIL)and 57(4.3%)with cervical cancer.The 70-79 group showed the highest incidence of HSIL at 30.0%(18/60), while the 50~59 group had the highest incidence of LSIL at 30.6%(241/787), and there were significant differences in different types of cervical lesions between different age groups( χ2=28.919, P<0.01). Liquid-based cervical cytology examination revealed a positive rate of 51.6%(166/322)for LSIL, a positive rate of 70.8%(143/202)for HSIL, and a rate of 71.4%(25/35)for cervical cancer.There were significant differences in LSIL, HSIL and other above-mentioned positive rates of lesions( P<0.05). In the cervical malignancy group, cytological high-grade squamous epithelial lesions made up 51.4%(18/35). Intraepithelial lesions or cancerous lesions represented 29.2%(59/202)in the HSIL group.The rate of intraepithelial lesions or cancerous lesions in the LSIL group was 48.4%(156/322), with statistically significance in these differences( χ2=118.806, P<0.01). Chronic cervicitis was found in 52.9%(109)of 206 patients with atypical squamous cells and positive HPV, and in 76.2%(16)of 21 patients without HPV.HPV examination was completed in 1 301 patients, with 69.4%(903)having infections by one HPV strain and 30.6%(398)having infections by mixed HPV strains.Biopsy results showed that the rates of HPV infections in LSIL, HSIL and cancer patients were 84.6%(302/357), 95.9(185/193)and 100%(54/54), respectively, and the differences in these pathological results were statistically significant( P<0.01). HPV16 was the most common subtype in single HPV strain infections.The most common subtypes of HPV infections in cervical lesions with HSIL and above were HPV16 and HPV18.The agreement rate was 52.4%(187/357)for LSIL, 60.7%(153/252)for HSIL, and 64.9%(37/57)for cervical cancer between diagnoses based on cervical biopsy and those based on colposcopy.As the severity of lesions increased, the accuracy of colposcopy also increased.Differences between colposcopic diagnosis and pathological diagnosis on different types of cervical lesions were statistically significant( χ2=550.382, P<0.01).1 310 patients underwent endocervical curettage, identifying 120 cases of HSIL and 39 cases of cervical cancer, with a detection rate of 55.8%(159/285). Conclusions:The incidence of cervical cancer is high in elderly women.The incidences of high-grade lesions and cervical cancer in patients with atypical squamous cells plus HPV infections in the cervix are also high.Postmenopausal patients with cervical lesions are mainly infected by a single HPV subtype, with subtype 16 being the most common one.With increasing severity of lesions, the accuracy of colposcopy-based diagnosis increases and the rate of missed diagnosis decreases.Cervical curettage is an important means to reduce missed diagnosis with colposcopy.

Pulmonary arterial hypertension (PAH) is a severe, progressive disease leading to right ventricular failure and finally death. Lung transplantation is recommended for PAH patients who do not respond to targeted drug combination therapy or World Health Organization functional class (WHO FC) Ⅲ or Ⅳ. However, only 3% of PAH patients can recieve the lung transplantation. A novel implantable interatrial shunt device (ISD) can create a relatively fixed right-to-left shunt established by balloon atrial septostomy (BAS). The device may decompress the right sided chambers, facilitate left heart filling, improve organ perfusion and reduce the likelihood of syncope, acute pulmonary hypertensive crisis and death. The systemic oxygen transport improves despite hypoxemia. Implantation is simple, feasible and safe, and the X-ray time and operation time are short. There is no severe complication or thrombosis during the mid-term follow-up of the clinical studies and the device remained patent. The syncope symptoms, six-minute walk distance, cardiac index and systemic oxygen transport improve significantly in the patients. ISD may be currently the last alternative treatment to improve symptoms and prolong survival in currently drug-resistant patients with severe PAH.

Objective:To investigate the efficacy and safety of modular flexible ureteroscopic lithotripsy (M-FURL) in treatment of diabetic patients with kidney stones.Methods:Sixty patients with diabetes and kidney stones admitted to the Department of Urology, Xindu District People’s Hospital of Chengdu were selected as the observation group, and 78 patients with diabetes without kidney stones who were treated in our hospital during the same period were selected as the control group. M-FURL was performed to compare the operation of the two groups of patients.Results:Compared with the control group, fasting blood glucose and blood glucose level at 2h after meals in the observation group were significantly increased ( P<0.05) ; and the ratio of uric acid stones and calcium oxalate stones in the observation group was significantly higher than that ine the control group ( P<0.05) ; The two groups of patients were treated with M-FURL, and there was no statistically significant difference in stone clearance rate, total stone clearance rate, or complication rate ( P>0.05) ; At the same time, the difference in operation time and intraoperative blood loss between the two groups had no statistical significance ( P>0.05) ; while the hospital stay of the observation group was significantly longer than that of the control group ( P<0.05) . Conclusions:Patients with kidney stones and diabetes are more likely to have calcium oxalate stones and uric acid stones. M-FURL treatment in patients with diabetes and kidney stones is ideal and safe. Combining diabetes will not affect the effect of surgery.

Objective Analyzed surgical outcome following Flex-3D thoracoscopy among 429 cases with lobectomy and segmentectomy in this paper to define its safety and efficacy.Methods From the completion of the Olympus Flex-3D integrated operation room in Shanghai Chest Hospital in June 2015 up to December 2016,a single surgeons team carried out 429 cases of Flex-3D anatomic video-assisted thoracic surgery.Demography,preoperative condition,perioperative period complications and pathology for these patients were analyzed and discussed.Results There was a total of 429 patients including 258 males and 171 females.The age at diagnosis was ranged 21-81 yds.Lobectomy was performed in 313 cases,segmentectomy in 116 cases.Among those with 389 primary malignant tumors,39 benign tumors and 1 MALT were anatomically resected.The mean number of lymph nodes resected was 11.10 ±4.58(1-30) and mean sampled lymph node stations 6.10 ± 1.34(1-10).1patient was converted to thoracotomy because of vessel injury.The average operation time was 98.00 ±24.61 min(range,35-274 min) and the average blood loss was(97.9 ±24.6)ml(range,50-400 ml).The postoperative hospital stay was(5.6 ± 1.3) days on average.There was no operative death,and operative complications occurred in 18 patients(4.1％).The 1-year overall survival and 1-year disease-free survival for the lung cancer group were 100％ and 99.8％,respectively.Conclusion Flex-3D video-assisted thoracic surgery is a safe and effective surgical procedure featured by its added depth perception to facilitate operation and short learning curve.

OBJECTIVETo explore the curative effect of surgery and selerotherapy for massive venous malformations of the tongue.

METHODSFrom January 2005 to December 2014, subtotal resection or debulking for 15 cases of massive venous malformation in the tongue was undertaken with multiple sessions of pre- and post-operative injection therapy of pingyangmycin, lauromacrogol and absolute ethanol.

RESULTSAll signs associated with the lesions including eating, sleep and speech disorders disappeared after treatment. Complete or near complete resolution was achieved in 9 cases, and a significant reduction in size in a further 6 cases after surgical excision and peri-operative sclerotherapy.

CONCLUSIONSFor massive venous malformations of the tongue, surgical excision combined with multiple sessions of sclerotherapy is a good treatment option.
