1.Expert recommendations on vision friendly built environments for myopia prevention and control in children and adolescents

Abstract
The prevention and control of myopia in Chinese children and adolescents has become a major public health issue. While maintaining increased outdoor activity as a cornerstone intervention, there is an urgent need to explore new complementary approaches that can be effectively implemented in both indoor and outdoor settings. In recent years, environmental spatial frequency has gained increasing attention as one of the key environmental factors influencing the development and progression of myopia. Both animal studies and human research have confirmed that indoor environments lacking mid to high spatial frequency components, often characterized as "visually impoverished", can promote axial elongation and myopia through mechanisms such as disruption of retinal neural signaling, impaired accommodative function, and altered expression of related molecules. Based on the scientific consensus, it is recommended that "enriching of environmental spatial frequency" should be integrated into the myopia prevention and control framework. Following the principles of schoolled organization, family cooperation, community involvement, and student participation, specific measures are put forward in three areas:optimizing school visual settings, improving home spatial environments, and promoting healthy visual behavior. The aim is to create "visually friendly" indoor environments as an important supplement to outdoor activity, thereby providing a novel perspective and strategy for comprehensively advancing myopia prevention and control among children and adolescents.
2.Progress in artificial intelligence for predicting therapeutic efficacy of intravitreal injection

Intravitreal anti-vascular endothelial growth factor(anti-VEGF)therapy has been widely used, but the variability in its therapeutic efficacy limits individualized treatment. In recent years, the application of artificial intelligence(AI)has opened up new avenues for personalized treatment response prediction, and its core branches include machine learning(ML)and deep learning(DL). This review systematically retrieved and analyzed 41 relevant studies published up to April 2025. Comprehensive analysis reveals that AI predictive models are evolving from forecasting single endpoints(such as visual acuity or central retinal thickness)to integrating multi-dimensional endpoints(encompassing anatomical, functional, and treatment demand parameters)and generating predictive imaging outputs. In terms of technical approaches, DL models(28 studies, accounting for 68.3%)dominate this field due to their robust image interpretation capabilities, while ML models(10 studies, 24.4%)retain significant value in the analysis of structured clinical data. Cross-disease comparisons indicate that research efforts are most concentrated on age-related macular degeneration(ARMD)and diabetic macular edema(DME), with shared conceptual frameworks for model construction, yet distinct anatomical and functional indicators are prioritized for each disease. Currently, the field confronts several key challenges, including insufficient prospective clinical validation, limited model interpretability(the “black box problem”), and a scarcity of high-quality multi-center datasets. Moving forward, it is imperative to advance real-world validation and develop explainable AI techniques to expedite the clinical translation of these predictive models.
3.Disease burden of influenza like illness among student populations in Shenzhen

Objective:
To understand the epidemiological characteristics and disease burden of influenza like illness (ILI) among student populations, so as to provide data support for policy formulation and optimal allocation of health resources.
Methods:
From January 2024 to February 2025, a questionnaire survey was conducted among parents of kindergarten, primary school, junior and senior high school students in 9 districts of Shenzhen, including Longhua, Futian, Bao an, Longgang, Luohu, Nanshan, Guangming, Pingshan and Yantian. Parents were asked to complete the questionnaire based on whether their children had fever, cough, vomiting, diarrhea, rash and other common symptoms in 2024. A total of 3 537 parents were investigated, and 444 ILI cases were included as study subjects. The epidemiological burden, including incidence rate of influenza, visitation rate, years lived with disability (YLDs) and economic burden (including direct economic burden, indirect economic burden and intangible burden) were analyzed.
Results:
The incidence rate of influenza among students in Shenzhen in 2024 was 12.55%. The ILI incidence rates in kindergarten, primary school, junior and senior high school were 14.01%, 11.69% and 5.23%, respectively, with a statistically significant difference ( χ 2= 45.20, P <0.01). The ILI consultation rate among students was 85.36%, and the consultation rates in kindergarten (87.36%) and primary school students (84.62%) were higher than those in junior and senior high school students ( 56.52 %) ( χ 2=16.47, P <0.01). A total of 78.88% of cases did not receive etiological detection.The median total economic burden per ILI case was 2 354.62 yuan, including direct medical costs of 300.00 yuan, direct non medical costs of 212.50 yuan, indirect costs of 1 000.00 yuan, and intangible burden of 500.00 yuan.
Conclusions
Schools are high risk environment for influenza, and younger students are a high risk group for ILI. The disease burden caused by student ILI remains substantial.
4.Clinical Observation on Huatan Quyu Formula Improving Cerebral Small Vessel Disease with Phlegm and Blood Stasis Blocking Collateral Pattern via Regulating Glymphatic System Circulation

ObjectiveTo observe the clinical efficacy and safety of Huatan Quyu formula in treating cerebral small vessel disease (CSVD) with phlegm and blood stasis blocking collateral pattern via randomized controlled trial, and explore its mechanism of improving CSVD by regulating glymphatic system (GS) circulation. MethodsSixty-eight CSVD patients with phlegm and blood stasis blocking collateral pattern in the Department of Encephalopathy, Affiliated Hospital of Shaanxi University of Chinese Medicine from April to December 2024 were selected and randomly divided into an experimental group (34 cases) and a control group, with 34 cases in each group. Both groups received basic Western medicine treatment, while the experimental group additionally received Huatan Quyu formula. After a course of 12 weeks, the following parameters were compared between the two groups before and after treatment. Clinical outcomes were assessed using the Tinetti performance-oriented mobility assessment (POMA), Montreal Cognitive Assessment (MoCA), Scales for Outcomes in Parkinson's Disease-Autonomic (SCOPA-AUT), and traditional Chinese medicine (TCM) syndrome scores of phlegm and blood stasis blocking collateral pattern. Perivascular space (PVS) in the frontal lobe/basal ganglia and cerebrospinal fluid (CSF) flow parameters in the cerebral aqueduct were evaluated by 3.0T brain MRI, cerebrospinal fluid flow imaging, and phase-contrast magnetic resonance imaging (PC-MRI). Then, safety indicators were monitored, and SPSS 25.0 was used for statistical analysis. ResultsSixty-four patients completed the study (32 in each group). ①Baseline data: No statistically significant difference was found between the two group. ②Efficacy indicators: After treatment, the experimental group exhibited significantly improved total POMA, SCOPA-AUT, and TCM syndrome scores (P<0.01), outperforming the control group (P<0.05). No significant change was observed in MoCA scores between the two groups. ③Imaging indicators: The experimental group showed a reduced PVS area alongside significantly increased CSF flow parameters (including downward flow during the systolic period, and upward flow during the diastolic period) (P<0.01), which were superior to the control group (P<0.01). ④Safety: The laboratory indicators were normal in both groups, with no drug-related adverse reactions. ConclusionFor CSVD patients with phlegm and blood stasis blocking collateral pattern, Huatan Quyu formula can safely and effectively improve motor function, autonomic nerve function, and TCM syndromes, with potential mechanisms related to pulsatile CSF flow enhancement and GS circulation efficiency improvement.
5.Mechanism of Jianfu mixture in the treatment of erectile dysfunction based on network pharmacology analysis, molecular docking and in vitro experimental validation

Objective To explore the molecular mechanism of Jianfu mixture in the treatment of erectile dysfunction (ED) by network pharmacology and molecular docking techniques, and validate its core targets and mechanisms through in vitro experiments. Methods The active components and corresponding molecular targets of Jianfu mixture were searched by searching TCMSP and Batman-TCM databases, and the disease targets of ED were searched by using GeneCards database. Find the intersection of drug ingredient target and disease target. The interaction between intersected targets was described and analyzed by String database, and the analysis results were visualized by Cytoscape software to determine the core target and the corresponding active components. GO functional enrichment analysis and KEGG pathway enrichment analysis were performed for intersection targets; the core target within the intersection were found through MCODE plug-in on Cytoscape software and molecular docking was performed with the corresponding active ingredients. An endothelial dysfunction model was established by transfecting HUVECs with si-eNOS. Intervene with different concentrations of the Jianfu mixture for the model cells for 24 h. QPCR was used to detect mRNA expression of core targets (MAPK1, MAPK3, JUN, ESR1, MAPK8); Western blot was used to analyze protein expression (eNOS, JUN, p-JUN, MAPK, p-MAPK) and phosphorylation levels. Results 144 effective active components and 168 active components target-disease targe intersection of Jianfu mixture were obtained. GO analysis revealed 200 5 biological processes, 151 molecular functions, and 63 cellular components. KEGG analysis yielded 181 pathways. 5 core targets including MAPK1, MAPK3, JUN, ESR1 and MAPK8 were screened out. The active components such as β-sitosterol, kaempferol, astapterocarpan had good binding affinity with the core target. In vitro experiments confirmed successful construction of the endothelial dysfunction model (eNOS expression significantly decreased after si-eNOS transfection). Jianfu mixture dose-dependently inhibited mRNA expression of MAPK1, MAPK3, JUN, ESR1, and MAPK8. Additionally, it reduced phosphorylation levels of JUN and MAPK, indicating inhibition of the JNK/c-Jun and ERK/MAPK signaling pathways to improve endothelial function. Conclusion Jianfu mixture treats ED by suppressing abnormal activation of multi-target signaling pathways (MAPK/JUN/ESR1), reducing endothelial apoptosis, and promoting NO synthesis. This mechanism aligns with the traditional Chinese medicine principle of “activating blood circulation, resolving stasis, tonifying Qi, and strengthening cardiovascular function.” The study provided molecular-level evidence for the therapeutic efficacy of Jianfu mixture in ED management.

7.Mechanism of tanshinone ⅡA in ameliorating acetaminophen-induced liver injury based on network pharmacology and molecular docking techniques

Objective To investigate the potential protective mechanism of tanshinone ⅡA against acetaminophen (APAP)-induced acute liver injury. Methods Thirty mice were randomly divided into the normal group, model group, low-dose tanshinone ⅡA group (5 mg/kg), medium-dose tanshinone ⅡA group (10 mg/kg) and high-dose tanshinone ⅡA group (20 mg/kg). All groups were intragastrically administered once daily for 7 consecutive days. On the 7th day, except for the normal group, mice in the remaining groups were intraperitoneally injected with 400 mg/kg APAP to establish the acute lung injury (ALI) model. The protective effect of tanshinone ⅡA was evaluated based on the liver weight ratio, the levels of aspartate aminotransferase (AST) and alanine aminotransferase (ALT), and HE staining. Targets of tanshinone ⅡA were predicted by the Traditional Chinese Medicine Systems Pharmacology (TCMSP), and a shared target protein-protein interaction (PPI) network was constructed by Search Tool for the Retrieval of Interacting Genes/Proteins (STRING) 11.5 in combination with disease targets from GeneCards. The Gene Ontology (GO) and Kyoto Encyclopedia of Genes and Genomes (KEGG) enrichment analysis was performed by Database for Annotation, Visualization and Integrated Discovery (DAVID), and the component-target-pathway network was constructed. The molecular docking between tanshinone ⅡA and the core targets was performed by AutoDock and visualised. Results Compared with the normal group, the liver weight ratio and serum aminotransferase level of mice with acute liver injury were significantly elevated (P<0.05), and the pathological injury of liver tissue was obvious. After tanshinone ⅡA treatment, the above index were significantly reduced (P<0.05), and the pathological injury was significantly improved. Further network pharmacological analysis was carried out and found that KEGG pathway enrichment analysis focused on IL-17 signaling pathway and tumor necrosis factor (TNF) signaling pathway, etc. In addition, the PPI core network showed that the key targets of tanshinone ⅡA to ameliorate acute liver injury mainly included TP53, AKT1, SRC, TNF and JUN. The molecular docking results showed that tanshinone ⅡA had high binding scores with the targets of MMP9, NFKB1, TNF, EP300 and SMAD3; and was able to bind to the targets of TP53, AKT1, SRC and JUN. Conclusion Tanshinone ⅡA may play a protective role against acute liver injury by regulating genes such as AKT1, JUN and TNF, and participating in signaling pathways such as TNF and IL-17.
8.The Effect of Qishao Tongbi Capsule (芪芍通痹胶囊) on the Wnt/β-catenin Pathway in a Rat Model of Intervertebral Disc Degeneration

ObjectiveTo explore the possible mechanism of Qishao Tongbi Capsule (芪芍通痹胶囊, QTC) in the treatment of intervertebral disc degeneration (IDD). MethodsSeventy-five rats were randomly divided into control group, model group, low-dose QTC group, high-dose QTC group, high-dose QTC +agonist group, with 15 rats in each group. Except for the control group, all other groups were subjected to a fibrous ring puncture to prepare an IDD model. After modeling, rats in low-dose QTC group and high-dose QTC group were given QTC at doses of 0.2 and 0.8 g/(kg·d) by gavage, respectively. Rats in high-dose QTC+ agonist group was given QTC at 0.8 g/(kg·d) and SKL2001 solution at 10 mg/(kg·d) by gavage. The control group and model group were given 10 ml/(kg·d) distilled water by gavage. All treatments were given once a day for 4 consecutive weeks. After treatment, X-ray and magnetic resonance imaging (MRI) were used to detect IDD degree. Hematoxylin-eosin (HE) staining and Safranin O-Fast Green staining were used to observe the morphological changes of the intervertebral disc tissue. Immunohistochemical staining was performed to examine the levels of proteoglycan, type Ⅱ collagen (COL Ⅱ), and matrix metalloproteinase-3 (MMP-3) in the intervertebral disc tissue. Western blotting was used to detect the extracellular matrix (ECM)-related proteins (proteoglycan, COL Ⅱ, MMP-3, MMP-9, MMP-13), aging-related proteins (P53, P21, P16), apoptosis related proteins, including B-cell lymphoma/leukemia 2 (BCL-2), BCL-2 related X protein (BAX), Cleaved Caspase-3, and Wnt/β-catenin pathway related proteins such as Wnt3a, glycogen synthase kinase-3β (GSK-3β) and β-catenin in the intervertebral disc nucleus pulposus (NP) tissue. Reverse Transcription Quantitative Polymerase Chain Reaction (RT-qPCR) was used to assess the mRNA expression of Wnt3a, GSK-3β, and β-catenin in intervertebral disc tissue. ResultsCompared with the model group, rats in the low-dose QTC group and high-dose QTC group exhibited improved DHI, decreased Pfirmann grading, and alleviated IDD. The structural integrity of the NP and annulus fibrosus increased, and the number of the NP increased. The levels of proteoglycan, COL Ⅱ, BCL-2 and GSK-3β increased, while the levels of MMP-3, MMP-9, MMP-13, P53, P21, P16, BAX, Cleaved Caspase-3, Wnt3a and β-catenin protein decreased. The mRNA expression of Wnt3a and β-catenin mRNA decreased, while GSK-3β mRNA expression increased (P<0.05). Compared with the low-dose QTC group, the high-dose QTC group showed further improvements in DHI, decrease in Pfirrmann grading (P<0.05), and greater alleviation of IDD. The structural integrity of NP and annulus fibrosus was further enhanced, and the number of NP cells further increased. The levels of proteoglycan, COL Ⅱ, BCL-2 and GSK-3β were higher, while the levels of MMP-3, MMP-9, MMP-13, P53, P21, P16, BAX, Cleaved Caspase-3, Wnt3a and β-catenin were lower. The mRNA expression of Wnt3a and β-catenin decreased, while GSK-3β mRNA expression increased (P<0.05). Compared with the high-dose QTC group, the high-dose QTC +agonist group showed a decrease of DHI, an increase of Pfirmann grading (P<0.05), significant aggravation of IDD, reduction in structural integrity of the NP and annulus fibrosus, a decrease of NP cell count, lower levels of proteoglycan, COL Ⅱ, BCL-2 and GSK-3β, and higher levels of MMP-3, MMP-9, MMP-13, P53, P21, P16, BAX and Cleaved Caspase-3. Additionally, GSK-3β mRNA expression decreased (P<0.05). ConclusionQTC can inhibit NP cell aging, apoptosis, and ECM degradation in IDD rats, and its therapeutic effect may be mediated through the inhibition of the Wnt/β-catenin pathway.
9.Analysis on the use of medicines in treatment of benign prostatic hyperplasia in sample hospitals under the national volume-based procurement policy

Objective To explore the impact of national volume-based procurement policies on the use of medicines in treatment of benign prostatic hyperplasia (BPH) and provide data support for the rational clinical use of medicines in BPH treatment. Methods Data on the use of BPH treatment medications from 2019 to 2023 were extracted from the Chinese Medicine Economic Information Network (CMEI), covering 892 hospitals (including 645 tertiary hospitals and 247 secondary hospitals). The changes in various indicators, including the consumption sum, Defined daily doses (DDDs), Defined daily dose cost (DDDc), and the ranking ratio (B/A) of these drugs were analyzed and compared. Results From 2019 to 2023, due to the influence of relevant policies, the overall consumption sums of medicines used in the sample hospitals in BPH treatment showed a trend of decreasing first and then rising steadily. The DDDs showed an overall upward trend, while the DDDc demonstrated a gradual decline. Tamsulosin and finasteride consistently ranked first and second in DDDs. The B/A value for tamsulosin was significantly higher than that of other BPH treatment medications. Conclusion The implementation of national centralized drug volume-based procurement policies and other policies from 2019 to 2023 had effectively reduced the economic burden of patients with benign prostatic hyperplasia. Tamsulosin and finasteride, which had the highest B/A in the two categories of α-blockers and 5α-reductase inhibitors, dominated the market for BPH treatment. The clinical use of BPH treatment medications was relatively rational.
10.Guben Kechuan Granules (固本咳喘颗粒) for the Treatment of Chronic Bronchitis with Lung Qi Weakness Pattern:A Multi-Centre Randomised Controlled Trial

ObjectiveThis study aimed to evaluate the clinical effectiveness and safety of Guben Kechuan Granules (固本咳喘颗粒) in treating chronic bronchitis (CB) with lung qi weakness pattern. MethodsA multicenter, randomized controlled trial was conducted, and 180 patients with CB of lung qi weakness pattern were randomly divided into 120 cases in the treatment group and 60 cases in the control group according to a 2∶1 ratio. The control group received health education for 24 weeks, while conventional symptomatic treatment was given when acute exacerbation of CB occurred. Treatment group was treated with the oral administration of Guben Kechuan Granules, 2 g each time, 3 times a day, for a total of 24 weeks on the basis of treatment of the control group. Both groups were followed up for 24 weeks after 24 weeks of treatment. Primary effectiveness indicators included the number of CB acute exacerbations occurence during the treatment and follow-up period, and the total number of CB acute exacerbations from the start of treatment to the end of follow-up. Secondary effectiveness indicators included the details of CB acute exacerbations, i.e., time to first acute exacerbation, time between acute exacerbations, duration of each time of acute exacerbation, and acute exacerbation symptom severity scores, and lung function indices. The scores of cough, sputum, and wheeze and total symptom scores were compared prior to treatment, at 4, 8, 12, 16, 20, and 24 weeks of treatment, and at 24 weeks of follow-up. The occurrence of adverse events during the study period was recorded and safety indices including blood routine, liver function, kidney function, and urine routine were tested. ResultsA total of 179 participants completed the trial including 119 in the treatment group and 60 in the control group. Compared to pre-treatment scores within the group, the treatment group showed reductions in cough, sputum, and wheeze scores, and total symptom scores at weeks 4, 8, 12, 16, 20, and 24 of treatment, as well as at 24 weeks of follow-up; in the control group, cough scores decreased at weeks 16, 20, and 24, sputum and wheeze scores decreased at week 24 of treatment and at 24 weeks of follow-up, and total symptom scores decreased at weeks 20, 24 of treatment, and at 24 weeks of follow-up (P<0.05). Compared with the control group, the treatment group showed reductions in the number of CB acute exacerbations occurence during the treatment and follow-up period, and the total number of CB acute exacerbations from the start of treatment to the end of follow-up, the duration of acute exacerbations, the acute exacerbation symptom severity scores, and the scores for cough, sputum, wheeze, and total symptoms at weeks 8, 12, 16, 20, 24 of treatment, and at 24 weeks of follow-up; while the time to the first acute exacerbation of CB was significantly prolonged in the treatment group (P<0.05 or P<0.01). There were no statistically significant differences in lung function indicators between groups before treatment and at 24 weeks after treatment (P>0.05). Safety indicators showed no significant abnormalities before or after treatment in either group, and the incidence of adverse events during the treatment period showed no significant differences between the groups (P>0.05).
