With the change of infectious disease incidence pattern and the development of related technologies, progresses have been made in the research of infectious disease epidemiology. In recent years, due to the change in the requirements of infectious disease prevention and control, the research focus has expanded from common infectious diseases to diseases which have been eliminated or might be eliminated, as well as emerging and re-emerging infectious diseases. Infectious disease data has been characterized by multiple sources and modalities. Along with the rapid development of pathogen detection methods, infectious disease surveillance has shifted from a single disease-targted one to a comprehensive one. Moreover, novel technologies such as multi-omics and artificial intelligence have been applied in infectious disease epidemiology research. The international cooperation in this field has become increasingly crucial, and the revision of the International Health Regulations and the negotiation of pandemic agreement will have a profound impact. In the future, infectious disease epidemiology research will develop with more powerful tools to improve its capabilities.
2.Optical coherence tomography angiography and microvessel density quantification in penumbra after traumatic brain injury in rats.

OBJECTIVE:
To observe the dynamic changes of microvascular injury and repair in the penumbra of traumatic brain injury (TBI) rats with effective cerebral perfusion microvascular imaging using optical coherence tomography angiography (OCTA).
METHODS:
Transparent closed cranial windows were placed in craniotomy rats after TBI caused by weight drop. All the rats in TBI group and control group underwent head MRI examination on the first postoperative day, and the changes of cerebral cortical microvessel density were measured by OCTA through cranial windows on d0, d2, d4, d6, and d8. On the second day after the operation, the same number of rats in the two groups were selected to complete the immunohistochemical staining of brain tissue with pimonidazole, an indicator of hypoxia.
RESULTS:
MRI T2W1 and immunohistochemical staining demonstrated that edema and hypoxia in the traumatic brain tissue extended deeply throughout the entire cortex. OCTA showed that the cortical surface veins of the rats in both groups were significantly dilated and tortuous after operation, and recovered to the postoperative day level on d8. The effective perfusion microvessel density of the rats in both groups gradually recovered after a temporary decrease, and the TBI group decreased from 39.38%±4.48% on d0 to 27.84%±6.01% on d2, which was significantly lower than that on d0, d6, and d8 (P < 0.05). The highest value was 61.71%±7.69% on d8, which was significantly higher than that on d0, d2, and d4 (P < 0.05). The control group decreased from 44.59%±7.78% on d0 to 36.69%±5.49% on d2, which was significantly lower than that on d0, d6, and d8 (P < 0.05). The highest value was 51.92%±5.96% on d8, which was significantly higher than that on d2, and d4 (P < 0.05). Comparing the two groups, the effective perfusion microvessel density in the TBI group was significantly lower than that in the control group on d2 (P=0.021), and significantly higher than that in the control group on d8 (P=0.030).
CONCLUSION
OCTA can be used as a method of imaging and measurement of effective perfusion microvessels in the injured cerebral cortex of TBI rats. After TBI, the effective perfusion microvessel density in the wound penumbra gradually recovered after decreasing, and increased significantly on d8.

3.Impact of neoadjuvant chemotherapy combined with immunotherapy for the postoperative survival rate in patients with esophageal cancer

Objective:To summarize and analyze the application value of neoadjuvant chemotherapy combined with immunotherapy in the surgical treatment of esophageal cancer.Methods:A retrospective case analysis method was used to summarize and analyze the clinical data of 73 esophageal cancer surgery patients admitted to Beijing Chuiyangliu Hospital from April 2018 to December 2023, including 60 males and 13 females, with an age of (63.9±9.29) years, ranged from 38 to 84 years. Among them, 58 patients with stage II or above received neoadjuvant chemotherapy and immunotherapy. The survival rates at 1, 3, and 5 years after surgery were statistically analyzed. Measurement data with normal distribution were represented as mean±standard deviation( ± s), and the comparison between groups was conducted using the t-test; the comparison of count data were represented as case and percentage (%) and was conducted by chi-square test. Kaplan-Meier method was applied to plot survival curves, and the Log-rank test was used for comparing survival rates. Results:In clinical staging, the 3 and 5 years survival rates of 73 patients in stage Ⅰ were 100%, the 3 years survival rate of stage Ⅱ was 88.2%, the 3 years survival rate of stage Ⅲ was 81.3%, and the 3 years survival rate of stage Ⅳ was 57.7%. There was no statistically significant difference in 3 years survival rates between patients in stage Ⅱ and stage Ⅰ( P>0.05), while there were statistically significant differences in 3 years survival rates between patients in stage Ⅲ, Ⅳ, and stage Ⅰ ( P<0.05). Among the 58 patients with pathological staging after neoadjuvant therapy, the 3 years survival rate of stage I was 80.8%, stage Ⅱ was 100%, stage Ⅲ was 81%, and stage Ⅳ was 27.8%. There was no statistically significant difference in 3 years survival rates among stage Ⅰ, Ⅱ, and Ⅲ patients ( P>0.05), but the 3 years survival rates of stage Ⅰ, Ⅱ, and Ⅲ patients were significantly higher than those of stage Ⅳ patients, and the differences were statistically significant ( P<0.05) when compared with stage Ⅳ. The 3 years and 5 years survival rates of patients with lymph node metastasis of stage N 0 were 91.2% and 68.4%, respectively. The 3 years survival rate of stage N 1 patients was 85.7%, stage N 2 patients was 72.2%, and stage N 3 patients was 0. There was no statistically significant difference in 3 years survival rate among patients with lymph node metastasis of stage N 0, N 1, and stage N 2( P>0.05). However, there was a significant statistical difference in 3 years survival rate between patients with lymph node metastasis of stage N 0, N 1, and stage N 3 ( P<0.01). Conclusion:Neoadjuvant chemotherapy combined with immunotherapy significantly improves the 3 years survival rate of patients with esophageal cancer, but the 3 years survival rate of stage Ⅳ patients and patients with lymph node metastasis of stage N 3 is still relatively low.
4.Clinical Efficacy of Bushen Tongmai Prescription in Treating Type 2 Diabetes Mellitus Complicated with Atherosclerotic Cardiovascular Disease of Kidney Deficiency and Blood Stasis Type and Its Influence on Serum Mir-126

Objective To observe the clinical efficacy of Bushen Tongmai Prescription in treating type 2 diabetes mellitus(T2DM)complicated with atherosclerotic cardiovascular disease(ASCVD)of kidney deficiency and blood stasis type,and to investigate its effect on serum microRNA-126(miR-126).Methods Sixty patients with T2DM complicated with ASCVD of kidney deficiency and blood stasis type who admitted to the outpatient and inpatient departments of the Department of Geriatric Medicine of Shenzhen Traditional Chinese Medicine Hospital from March 2023 to October 2023 were randomly divided into the study group and the control group according to random number table method,with 30 patients in each group.All patients in the two groups were required to take low-salt and low-fat diabetic diet and exercise for intervention,and were also given the conventional western medical treatment with hypoglycemic,lipid-lowering,antihypertensive,anti-platelet aggregation agents.Additionally,the study group was treated with Bushen Tongmai Prescription.The course of treatment for the two groups covered eight weeks.Before and after treatments,the two groups were observed in the changes of C-reactive protein(CRP),interleukin 1β(IL-1β),interleukin 6(IL-6),vascular cell adhesion molecule 1(VCAM1),miR-126,carotid intima-media thickness(CIMT),carotid plaque Crouse scores,and blood lipid indicators of total cholesterol(TC),triglycerides(TG),low-density lipoprotein cholesterol(LDL-C)and high-density lipoprotein cholesterol(HDL-C).After treatment,the therapeutic efficacy on carotid artery and drug safety in the two groups were evaluated.Results(1)After treatment,the serum miR-126 level in the study group was significantly increased compared with that before treatment(P<0.01),but the level in the control group only showed a rising trend and the difference was not statistically significant(P>0.05).The comparison between the two groups showed that the increase of serum miR-126 level in the study group was significantly superior to that in the control group(P<0.05).(2)After treatment,the serum VCAM1 level in the study group was decreased significantly compared with that before treatment(P<0.01),but the level in the control group only showed a rising trend and the difference was not statistically significant(P>0.05).The comparison between the two groups showed that the decrease of serum VCAM1 level in the study group was significantly superior to that in the control group(P<0.01).(3)After treatment,the serum CRP,IL-1 β,and IL-6 levels in the two groups were decreased compared with those before treatment(P<0.05 or P<0.01),and the decrease of serum IL-1β and IL-6 levels in the study group was significantly superior to that in the control group(P<0.05).(4)After treatment,the TC,TG and LDL-C levels in the study group were decreased(P<0.01)and the HDL-C level was increased compared with that before treatment(P<0.01),but in the control group,the changes in the TC,TG,LDL-C,and HDL-C levels were not significant(P>0.05).The comparison between the two groups showed that the decrease of TC and LDL-C levels in the study group was significantly superior to that in the control group(P<0.05).(5)After eight weeks of treatment,the total effective rate of the study group was 90.00%(27/30),while that of the control group was 63.33%(19/30).The intergroup comparison(tested by chi-square test)showed that the therapeutic efficacy of the study group was significantly superior to that of the control group(P<0.05).(6)During the treatment,no serious adverse reactions or toxic-side effects occurred in the two groups.Conclusion Bushen Tongmai Prescription combined with conventional western medicine treatment can up-regulate the level of miR-126,inhibit the expression of VCAM1,and decrease the levels of serum CRP,IL-1β,and IL-6,so as to inhibit inflammatory response and retard the progression of T2DM complicated with ASCVD.The combined therapy exerts certain clinical efficacy and higher safety in treating T2DM complicated with ASCVD.
5.Mechanism of Liangfang Wenjing Decoction in Regulating Endoplasmic Reticulum Stress to Reduce Apoptosis and Alleviate Ovarian Microvascular Injury in Rats with Cold Coagulation and Blood Stasis Syndrome

ObjectiveTo observe ovarian microvascular damage in rats with cold coagulation and blood stasis syndrome and to explore the mechanism by which Liangfang Wenjing decoction improves this condition in rats. MethodsFifty SPF female SD rats were randomly divided into a blank group, a model group, low-dose (8.1 g·kg-1) and high-dose groups (16.2 g·kg-1) of Liangfang Wenjing decoction, and a 4-phenylbutyric acid (0.1 g·kg-1) group, with 10 rats in each group. The ice-water bath method was employed to establish the rat model of cold coagulation and blood stasis syndrome. Concurrent with modeling, Liangfang Wenjing decoction was administered continuously for 21 days, once daily. The rats' syndrome manifestations and estrous cycles were recorded. The enzyme-linked immunosorbent assay (ELISA) was used to detect serum reproductive hormone levels and levels of endothelin-1 (ET-1), nitric oxide (NO), thrombomodulin (TM), and von Willebrand factor (vWF) in ovarian tissue. Prothrombin time (PT), activated partial thromboplastin time (APTT), thrombin time (TT), and fibrinogen (FIB) were measured. The ovarian microcirculatory blood perfusion was detected by laser speckle contrast imaging. Hematoxylin-eosin (HE) staining was performed to observe the ovarian histopathology, flow cytometry to detect ovarian apoptosis rate, and transmission electron microscopy to observe the ultrastructure of ovarian microvascular endothelial cells. Western blot was employed to detect the protein expression of endothelial nitric oxide synthase (eNOS), phosphorylated eNOS (p-eNOS), Caspase-3, B-cell lymphoma-2 (Bcl-2), Bcl-2-associated X protein (Bax), glucose-regulated protein 78 (GRP78), C/EBP homologous protein (CHOP), inositol-requiring enzyme1α (IRE1α), p-IRE1α, apoptosis signal-regulating kinase 1 (ASK1), p-ASK1, c-Jun N-terminal kinase (JNK), and p-JNK. Immunofluorescence was used to detect ovarian Bax and Bcl-2 expression in microvascular endothelial cells. ResultsCompared with the blank group, the model group showed signs of cold coagulation and blood stasis syndrome, prolonged estrus cycles, and reproductive hormone disorders. Histopathological results revealed a decrease in follicle counts at all stages and disorganized granulosa cell arrangement. Ovarian microcirculatory perfusion was significantly decreased (P<0.01). PT, APTT, and TT were reduced (P<0.05, P<0.01), while FIB levels were increased (P<0.05). In ovarian tissue, NO content was decreased, while ET-1, vWF, and TM levels were increased significantly (P<0.01). The apoptosis rate of ovarian cells was markedly increased (P<0.01). Furthermore, p-eNOS/eNOS and Bcl-2 were decreased (P<0.05), whereas Bax, cleaved-Caspase-3/Caspase-3, GRP78, CHOP, p-IRE1α/IRE1α, p-ASK1/ASK1, and p-JNK/JNK expression showed significant increases (P<0.05, P<0.01). Compared with the model group, Liangfang Wenjing decoction intervention alleviated the symptoms of cold coagulation and blood stasis, gradually restored the estrus cycle, and improved ovarian histopathology and endothelial cell ultrastructure. Microcirculatory blood perfusion was significantly elevated (P<0.05). NO content in ovarian tissue was elevated, while ET-1, vWF, and TM levels were significantly decreased (P<0.05, P<0.01). The p-eNOS/eNOS ratio and Bcl-2 expression were significantly elevated (P<0.05), while the expression of Bax, cleaved-Caspase-3/Caspase-3, GRP78, CHOP, p-IRE1α/IRE1α, p-ASK1/ASK1, and p-JNK/JNK was significantly decreased (P<0.05, P<0.01). ConclusionLiangfang Wenjing decoction may regulate the IRE1α/ASK1/JNK signaling pathway to inhibit endoplasmic reticulum stress, attenuate apoptosis, and improve microvascular endothelial injury in ovaries of rats with cold coagulation and blood stasis syndrome.
6.Quality Evaluation of Uncaria Hook in the Market Based on Evaluation Sampling

Objective To evaluate the quality of Uncaria Hook(UH)in the market.Methods Twenty-nine batches of UH were tested by legal standards(character,identification,inspection,and extract detection).The TLC method was used to explore the identification of the control herbs,pilocarcinine,dehydropilocarcinine,pilocarcinine and isocarcinine.The HPLC method was used to establish the characteristic spectrum of UH.Then cluster analysis,PCA-X,and OPLS-DA analysis methods were used to perform statistical analysis on the common peak area of unguttine,and determine the contents of heicoside,iso-dehydrounguttine,iso-unguttine,iso-unguttine,and unguttine,to screen out the key quality control components of UH and evaluate their quality.Results Among the 29 batches of samples,23 batches were qualified,and the qualified rate was 79.31%.The results of the TLC analysis showed that the chemical constituents of different base sources were very different.No pilocarcinine was detected in Uncaria macrophylla no hydropilocarcinine was detected in Uncaria sinensis,and four components were not detected in Uncaria hirsuta.Only the samples with the base source of the plant showed corresponding spots in the chromatographic position with the control drug and the control substance.The results of HPLC showed that the chemical composition and content of different base sources of UH were very different.No hydrouncinine and pilocarcinine were detected in Uncaria macrophylla.No iso-dehydrouncinine,dehydrouncinine,and dehydropilocarcinine were detected in Uncaria sinensis.And only heicoside was detected in Uncaria sinensis,and the content of heicoside was low.Based on statistical analysis,there were differences among different batches of UH,and three key quality control ingredients,iso-dehydrounguocarcinoma,dehydrounguocarcinoma,and iso-unguocarcinoma that may lead to quality differences of unguocarcinoma were found.Conclusions Overall,the UH is of poor quality,the current processing standards and testing standards can not ensure the quality of the product,affecting the safety and effectiveness of clinic use.The chemical composition categories and contents of different base sources of UH are very different,which increases the difficulty of formulating a unified quality standard.
7.Design and implementation of online continuing education platform for transfusion medicine

To provide in-service medical technicians and nurses with convenient access to continuing education resources in transfusion medicine, reduce transfusion-related adverse events, and ensure the safety, rationalization, and effectiveness of clinical transfusion, we designed and developed an online transfusion continuing education platform. The platform was based on the new managed code programming model.NET Core and the powerful functions of hypertext preprocessor PHP 7.4, addressing current issues in transfusion online continuing education. Through in-depth analysis of student attributes, learning behaviors, and teaching behaviors, a comprehensive online continuous teaching quality evaluation index system was established. This system not only facilitates the quantitative assessment of teaching quality but also successfully integrates the two core functions of teaching and management, thereby achieving unified online teaching.
