1.Risk factors associated with postoperative adjuvant therapy for resectable esophageal squamous cell carcinoma

The benefit of postoperative adjuvant therapy for patients with resectable esophageal squamous cell carcinoma (ESCC) is not yet supported by high-level evidence. This review analyzes the role of adjuvant therapy by examining the discrepancy between clinical needs and guidelines, its historical evolution, recent advances in high-risk factors, and future outlooks. We provide a detailed discussion of high-risk factors used for patient selection, including lymph node positivity, and for node-negative patients, features such as tumor length, location, T stage, extent of lymph node dissection, differentiation, vascular and neural invasion, laboratory indices, and molecular markers. The goal is to inform the development of individualized precision treatment strategies for resectable ESCC.
2.Improvement effect and mechanism of Shengmai powder on heart failure mice with qi-yin deficiency

OBJECTIVE To study the improvement effect and mechanism of Shengmai powder on heart failure (HF) mice with qi-yin deficiency. METHODS The mice were randomly divided into blank group (water), model group (water), Shengmai powder low-, medium-, and high-dose groups [2.61, 5.22 and 10.44 g/kg (based on crude drug dosage)] and positive control group (metoprolol, 30 mg/kg), with 10 mice in each group. Except for the blank group, all other groups were subcutaneously injected with D-galactose, and a qi-yin deficiency HF mice model was established by continuous food restriction and weight-bearing swimming. At the same time of modeling, the corresponding medicine/water was gavaged once a day for five weeks. The general state of mice was recorded and the traditional Chinese medicine (TCM) syndrome score was evaluated. Behavioral experiments were conducted to investigate the total distance of open field action, the percentage of immobility time, and the swimming exhaustion time of mice. The contents of aspartate transaminase (AST), creatine kinase (CK) and lactate dehydrogenase (LDH) in the serum of mice were detected; cardiac function indexes [heart rate, left ventricular ejection fraction (LVEF), left ventricular end systolic diameter (LVESD), left ventricular end diastolic diameter (LVEDD), left ventricular mass index and whole heart mass index] were all detected; the histopathological morphology of mice myocardium was observed; the level of cardiomyocyte apoptosis in mice was detected; mRNA expression levels of B-cell lymphoma 2 (Bcl-2), Bcl-2 associated X protein (Bax), and Cleaved-caspase-3 in myocardial tissue of mice were detected; the phosphorylation levels of sarcoplasmic reticulum calcium regulatory related proteins [ryanodine receptor 2 (RyR2) and phospholamban (PLB)] in myocardial tissue of mice were detected. RESULTS Compared with the blank group, the body weight, total distance of open field action, swimming exhaustion time, LVEF, LVEDD, Bcl-2 mRNA expression level in myocardial tissue and PLB protein phosphorylation level in the model group were significantly reduced/shortened (P<0.05); TCM syndrome score, the percentage of immobility time, heart rate, LVESD, left ventricular mass index, whole heart mass index, cardiomyocyte apoptosis rate, the contents of CK, LDH and AST in serum, mRNA expression levels of Cleaved-caspase-3 and Bax and the phosphorylation level of RyR2 protein in myocardial tissue were significantly increased (P<0.05); there were inflammatory cell infiltration, disordered cell arrangement and obvious myocardial interstitial fibrosis in myocardial tissue. After the intervention of Shengmai powder, most of the above quantitative indexes in mice were significantly reversed (P<0.05), the inflammatory cell infiltration in myocardial tissue was reduced, and the degree of fibrosis was significantly reduced. CONCLUSIONS Shengmai powder can improve cardiac function, reduce the level of cardiomyocyte apoptosis and myocardial fibrosis in HF mice with qi-yin deficiency. Its mechanism may be related to the regulation of the expression of sarcoplasmic reticulum calcium regulation related proteins.

4.Advances in the Treatment of Multiple Primary Lung Cancer.

In recent years, the incidence of multiple primary lung cancer (MPLC) has been increasing, and it cannot be ignored in clinical practice. The treatment of MPLC is still controversial, but surgical treatment is recognized as the most important treatment. However, current studies have shown that the treatment of MPLC needs to develop multimodal treatment according to different patients. This review summarizes multiple treatment method for MPLC, including surgery, ablation, chemotherapy, radiotherapy, targeted therapy, and immunotherapy in order to enhance understanding of MPLC treatment.

6.Validating Multicenter Cohort Circular RNA Model for Early Screening and Diagnosis of Gestational Diabetes Mellitus

Background:
Gestational diabetes mellitus (GDM) is a metabolic disorder posing significant risks to maternal and infant health, with a lack of effective early screening markers. Therefore, identifying early screening biomarkers for GDM with higher sensitivity and specificity is urgently needed.
Methods:
High-throughput sequencing was employed to screen for key circular RNAs (circRNAs), which were then evaluated using reverse transcription quantitative polymerase chain reaction. Logistic regression analysis was conducted to examine the relationship between clinical characteristics, circRNA expression, and adverse pregnancy outcomes. The diagnostic accuracy of circRNAs for early and mid-pregnancy GDM was assessed using receiver operating characteristic curves. Pearson correlation analysis was utilized to explore the relationship between circRNA levels and oral glucose tolerance test results. A predictive model for early GDM was established using logistic regression.
Results:
Significant alterations in circRNA expression profiles were detected in GDM patients, with hsa_circ_0031560 and hsa_ circ_0000793 notably upregulated during the first and second trimesters. These circRNAs were associated with adverse pregnancy outcomes and effectively differentiated GDM patients, with second trimester cohorts achieving an area under the curve (AUC) of 0.836. In first trimester cohorts, these circRNAs identified potential GDM patients with AUCs of 0.832 and 0.765, respectively. The early GDM prediction model achieved an AUC of 0.904, validated in two independent cohorts.
Conclusion
Hsa_circ_0031560, hsa_circ_0000793, and the developed model serve as biomarkers for early prediction or midterm diagnosis of GDM, offering clinical tools for early GDM screening.
7.Deep learning-based fusion of color and spectral features from hyperspectral imaging for the origin identification of Salvia miltiorrhiza

Background: Salvia miltiorrhiza Bunge, commonly known as “Danshen” in China due to the distinctive red color of its roots, is one of the most widely used traditional Chinese medicines. It is cultivated in various regions across China, and environmental differences among these regions can affect the secondary metabolites of plants, thereby influencing the quality of S. miltiorrhiza. In recent years, increasing demand for S. miltiorrhiza has exacerbated the problem of origin fraud. Therefore, ensuring the authenticity of its geographical origin is crucial for the sustainable development of the industry. Objective: The red coloration of S. miltiorrhiza is closely associated with the content of its primary active compounds, particularly tanshinones. Therefore, both its internal chemical composition and external color characteristics serve as key indicators for quality assessment. This study utilized hyperspectral imaging technology to evaluate its potential in classifying the geographical origin of S. miltiorrhiza. Methods: Spectral data reflecting the internal chemical properties of S. miltiorrhiza were integrated with color information representing its external features through 3 levels of data fusion. These fused datasets were then combined with deep learning algorithms to achieve accurate origin classification. Results: The results demonstrated that the Transformer model combined with soft-voting decision-level fusion achieved the highest classification accuracy of 98.72% by integrating image color and short-wave infrared spectral data. Conclusion: This study demonstrates that integrating hyperspectral imaging spectral data with color information provides a reliable and innovative approach for verifying the authenticity and traceability of S. miltiorrhiza.
8.Clinical analysis of diagnosis and treatment in patients with bile duct injuries associated with laparoscopic cholecystectomy

Objective:To explore optimal timing and surgical approaches for bile duct repair in patients with bile duct injuries associated with laparoscopic cholecystectomy (LC-BDI).Methods:A retrospective analysis was conducted on the clinical data of 13 patients with LC-BDI treated at Chuxiong Yi Autonomous Prefecture People's Hospital from October 2012 to October 2023. The cohort included 6 males and 7 females, aged (51.2±7.6) years. Clinical parameters such as gender, age, Strasberg classification, timing of surgery, surgical techniques, and complications were recorded. Postoperative follow-up was conducted via telephone to assess complications and reoperation status.Results:Among the 13 LC-BDI patients, 4 cases were diagnosed intraoperatively during LC, while 9 cases were diagnosed postoperatively. The Strasberg classification was as follows: 5 cases of E2, 6 cases of E3, and 2 cases of E4. Except for 2 patients who underwent immediate repair during LC, the remaining 11 patients initially received proximal bile duct or subhepatic space drainage, followed by definitive biliary reconstruction after 6-8 weeks. All 13 patients underwent repair via side-to-side hepaticojejunostomy (SSHJ). Over a mean follow-up period of (68.5±30.8) months, only 1 patient with Strasberg E4 LC-BDI was judged as repair failure at 21 months postoperatively; the remaining 12 cases achieved successful repair.Conclusion:For LC-BDI patients with Strasberg E2 or higher classifications, in the absence of intra-abdominal infection, biliary reconstruction via SSHJ at 6-8 weeks postoperatively demonstrated superior therapeutic efficacy.
9.Quality Evaluation of Hibisci Mutabilis Folium with Multi-index Component Quantitative Combined with Chemical Pattern Recognition,Weighted TOPSIS and Grey Correlation Degree Fusion Model

Objective To establish a method for 11 main components of Hibisci Mutabilis Folium;To evaluate the quality of Hibisci Mutabilis Folium using the chemical pattern recognition,weighted TOPSIS and grey correlation degree fusion model.Methods A total of 18 batches of Hibisci Mutabilis Folium from main producing areas were collected,and the contents of caffeic acid,ferulic acid,salicylic acid,fraxetin,hyperoside,rutoside,isoquercitrin,kaempferol-3-O-rutinoside,tiliroside,β-sitosterol and eleucheroside were detected by external standard method.The extract,total ash and acid-insoluble ash were detected.The quality was systematically evaluated using chemical pattern recognition,weighted TOPSIS and grey correlation degree fusion model.Results The methodological verification showed that the linear relationship of each component was good,the repeatability and accuracy of the established method were good,the instrument precision was high,and the stability of the test solution of Hibisci Mutabilis Folium ultrasonically extracted with 70%methanol was good within 24 h.The content determination results showed that the quality difference between batches was large.Chemical pattern recognition analysis showed that 18 batches of Hibisci Mutabilis Folium were clustered obviously,and the quality difference compounds were identified as rutoside,caffeic acid,kaempferol-3-O-rutinoside,fraxetin and hyperoside.The relative closeness of 18 batches of samples was between 0.253 8 and 0.694 3.Conclusion The established multi-index quantitative method can reflect the quality differences of Hibisci Mutabilis Folium from different habitats.Chemical pattern recognition combined with weighted TOPSIS and grey correlation degree fusion model can comprehensively analyze the quality conditions of different batches of Hibisci Mutabilis Folium,which can establish a foundation for the quality analysis and evaluation of Hibisci Mutabilis Folium.

Thoracolumbar spine fracture often leads to severe pain, functional impairments, and neurological deficits, for which open reduction and internal fixation can effectively restore the spinal structural stability. Open decompression and reduction with internal fixation can help relieve spinal cord compression and improve spinal function in cases of concomitant cord injury. Although spinal stability can be restored through surgery, patients often face chronic pain and functional impairments postoperatively. A postoperative rehabilitation program is critical in optimizing therapeutic outcomes, reducing complications, and minimizing the risk of secondary injuries. However, current rehabilitation methods, such as physical therapy, functional training, and pain management, are confronted with problems in clinical practice, including significant variation in efficacy, poor patient adherence, and prolonged rehabilitation period. There is an urgent need for a unified rehabilitation strategy to address these problems. To this end, the Spinal Trauma Group of the Orthopedic Physicians Branch of the Chinese Medical Association and the Spine Health Professional Committee of the Chinese Human Health Technology Promotion Association organized experts from relevant fields to formulate Evidence-based guidelines for rehabilitation treatment after internal fixation of thoracolumbar spine fracture in adults ( version 2025) by integrating evidences from clinical researches and advanced rehabilitation concepts at home and abroad. A total number of 14 recommendations concerning the rehabilitation treatment with multimodal analgesia, psychological intervention, deep vein thrombosis prevention, core muscle and extremity exercise, appropriate use of braces, early weight-bearing, device-aided rehabilitation exercise, neuroregulatory therapy, rehabilitation team were put forward, aiming to standardize the post-operative rehabilitation process following internal fixation, promote the functional recovery, and enhance patients′ quality of life.
