1.Optimization of the Forming Process of Mingmu Yijing Pills Based on AHP-CRITIC Mixed Weighting Method

OBJECTIVE
To clarify the weight index and optimize the technology of compound Mingmu Yijing pills.
METHODS
Water content, dissolution time limit and bolus-forming pass rate were included as evaluation indexes. The analytic hierarchy process(AHP) and the criterion importance through inter-criteria correlation(CRITIC) method based on the objectivity of indicators were adopted, AHP-CRITIC mixed weighting method. In this way, the relative coefficients of each index were calculated for weight distribution and comprehensive score, and the molding process was optimized and verified by Box-Behnken response surface test.
RESULTS
Compared with the single weighting method, the AHP-CRITIC mixed weighting method was more reasonable and stable. This method finally optimized the forming process conditions of Mingmu Yijing pills as follows: adding water amount of 0.8 times, the damp mass infiltration for 0.5 h, drying temperature of 60 ℃, drying time of 8 h. The mean comprehensive score of validation experiment was 88.21, RSD was 0.29%.
CONCLUSION
AHP-CRITIC method scientifically determines the weight of multiple indicators, and the verification test confirms that the optimized molding process is stable and feasible, which can be used in practical large-scale production.
2.Diagnostic application of SpyGlass in indeterminate stricture of hilar bile duct

Objective: To investigate the diagnostic value of SpyGlass choledochoscopy system in indeterminate stricture of hilar bile duct. Methods: A retrospective analysis was conducted on 146 patients with indeterminate stricture of the hilar bile duct who underwent endoscopic retrograde cholangio-pancreatography (ERCP) and SpyGlass examinations at our hospital between January 2018 and December 2022. The study recorded the visual impressions of the lesions, direct visualization-guided biopsy pathology, and conventional brush cytology results obtained using SpyGlass system. The sensiti-vity, specificity, and accuracy of different diagnostic methods were compared, taking pathological diagnosis as the gold standard and considering long-term follow-up results as well. Results: Out of the 146 patients, the procedure was successful in 145 cases (99.3%). The average procedure time was (51.5±16.9) min. The sensitivity, specificity, and accuracy of SpyGlass visual impression diagnosis for indeterminate biliary stricture were 83%, 84%, and 83%, respectively, outperforming brush cytology results (47%, 100%, and 52%). The combination of visual diagnosis and SpyBite biopsy yielded sensiti-vity of 91%, specificity of 63%, and accuracy of 85%. Adverse events occurred in 24.7% of cases, all of which were clinically mild. Conclusions: SpyGlass choledochoscopy has a high accuracy and safety in differential diagnosis of indeterminate stricture of the hilar bile duct. Its widespread application is warranted.
3.Improvement Effects of α-lipoic Acid on Glucose Metabolism Disorder of Insulin Resistant HepG 2 Cells

OBJECTIVE:To study the i mprovement effects of α-lipoic acid on glucose metabolism disorder of insulin resistant HepG2 cells. METHODS :The effects of 25-1 000 µmol/L α-lipoic acid on survival rate of human hepatoma cell HepG2 were determined by MTT assay so as to determine the concentration of α-lipoic acid. Negative control group ,insulin resistance group (1× 10-7 mol/L insulin ),combination resistance group (30 µmol/L sodium arsenite+ 1×10-8 mol/L insulin ),α-lipoic acid low- concentration,medium-concentration and high-concentration groups were set up. HepG 2 cells were treated with α-lipoic acid for 12 h and then cultured with corresponding concentration of sodium arsenite or/and insulin for 24 h. The glucose oxidase method was used to detect the glucose consumption ,colorimetric method was used to detect hexokinase activity and pyruvate kinase activity , and anthrone method was used to detect glycogen content. Western blot assay was used to detect the protein expression of GLUT 4, p-GSK3β and GSK3β as well as the ratio of p-Akt/Akt and p-GSK3β/GSK3β. RESULTS:25,50,100 µmol/L α-lipoic acid had no significant effect on the survival rates of HepG 2 cells(P>0.05),and survival rates of H epG2 cells were higher than 96%,so they were used as the low ,medium and high concentration for follow-up study. Compared with negative control group ,glucose consumption,the activities of hexokinase and pyruvate kinase ,glycogen content ,protein expression of GLUT 4 and p-GSK 3β,the ratio of p-Akt/Akt and p-GSK 3β/GSK3β were decreased significantly in insulin resistance group and combined resistance group, while the protein expression of GSK 3β was increased significantly(P<0.05). Compared with combination resistance group ,the glucose consumption (except for α-lipoic acid low- concentration group ),the activities of h exokinase(except for α-lipoic acid low-concentration and medium-concentration groups ) andpyruvate kinase (except for α-lipoic acid low-concentration com and medium-concentration groups ), glycogen contents , protein expression of GLUT 4 (except for α-lipoic acid mail:bliang163@163.com low-concentration group )and p-GSK3β,the ratio of p-Akt/ Akt(except for α-lipoic acid low-concentration and medium-concentration groups )and p-GSK 3β/GSK3β(except for α-lipoic acid low-concentration groups )were increased significantly in α-lipoic acid groups ,while protein expression of GSK 3β(except for α-lipoic acid low-concentration and medium-concentration groups ) was decreased significantly (P<0.05);glycogen content , protein expression of GLUT 4 and the ratio of p-GSK 3β/GSK3β in α-lipoic acid high-concentration group as well as the protein expression of p-GSK 3β in α-lipoic acid medium-concentration and high-concentration groups were improved significantly (P<0.05). CONCLUSIONS:α-lipoic acid can improve the disorder of glucose metabolism in insulin resistant HepG 2 cells,the mechanism of which may be associated with the increase of glucose consumption ,the activities of glucose metabolism related enzymes and glycogen content ,and expression up-regulation of the phosphorylation levels of Akt and GSK 3β protein,the expression of GLUT 4 and p-GSK 3β proteins,down-regulation of the expression of GSK 3β protein.
4.Migraine and risk of hemorrhagic stroke: a meta-analysis

Objective:To comprehensively evaluate the correlation between migraine and the risk of hemorrhagic stroke using Meta-analysis.Methods:The published observational studies on migraine and the risk of hemorrhagic stroke in PubMed, EMbase, Cochrane library, Chinese Biomedical Database, China Journal Full-text Database, Wanfang Database and VIP Database were retrieved by computers. The retrieval time limit was from the establishment of the databases to December 31, 2019. Two reviewers independently conducted the literature screening and data extraction, and evaluated the quality according to Newcastle Ottawa scale. Stata SE 12.1 software was used for Meta-analysis.Results:Six case-control studies and 7 cohort studies met the inclusion criteria, all of which were in English. The results of Meta-analysis showed that exposure to migraine increased the risk of hemorrhagic stroke (odds ratio [ OR] 1.47, 95% confidence interval [ CI] 1.23-1.76; P<0.001). Sensitivity analysis showed that the results were robust. Subgroup analysis showed that migraine with aura ( OR 1.38, 95% CI 1.05-1.81; P=0.019), migraine without aura ( OR 1.46, 95% CI 1.19-1.80; P<0.001), male ( OR 2.10, 95% CI 1.72-2.56; P<0.001) and female ( OR 1.53, 95% CI 1.22-1.92; P<0.001) migraine could increase the risk of hemorrhagic stroke. Conclusion:Regardless of the gender of patients and presence or absence of migraine aura, migraine can significantly increase the risk of hemorrhagic stroke.
5.HIF-1α mediates energy metabolism of pancreatic exocrine cells after radiation

Objective To study the radiation-induced mitochondrial damage and energy metabolic alteration in pancreatic exocrine cells,and to explore underlying mechanism.Methods Rat pancreatic exocrine cells (AR42 J) were divided into control group and experimental group irradiated with 6 Gy of X-rays.Mitochondrial membrane potential was detected at 24,48,72 and 96 h,the lactic acid and ATP production were detected at 24 h and 48 h,and reactive oxygen species (ROS) were detected at 24 h after irradiation.The expressions of energy metabolism related factors of HIF-1α,LDHA and PDH were detected by Western blot.The animal experiments were conducted to confirm the changes.According to random number table,eight rats were randomly divided into two groups with 4 rats in each.The irradiated group was exposed to 12 Gy of X-rays,while the control group sham-irradiated.Results Compared with the nonirradiated control group,the mitochondrial membrane potential (△Ψm) of the experimental group was progressively decreased at 24-96 h after irradiation (t =5.438-17.687,P<0.05).The ATP content in the experimental group decreased at 24 h and 48 h (q=17.300,8.328,P<0.05),the lactic acid increased (q =21.790,16.250,P<0.05),and the ROS level increased (t =7.935,P<0.05).The expressions of HIF-1α and LDHA were significantly increased,but PDH was reduced after radiation.Silencing HIF-1α by siRNA eliminated radiation-induced energy metabolic alteration.These changes were confirmed with animal experiments by locally irradiating rats.Conclusions The expression of HIF-1α is upregulated by irradiation,which leads to the change of energy metabolism as the enhancement of glycolysis pathway and the inhibition of aerobic oxidation of mitochondria in pancreatic exocrine cells.
6.Comparison of the vertigo etiology between flying personnel patients and ordinary patients

Objective To compare the etiology of vertigo patients between flying personnel group and ordinary group and to provide evidence for the accurate diagnosis of vertigo or dizziness and the aeromedical assessment. Methods One thousand and sixteen ordinary vertigo patients,who were diagnosed by the Vertigo Center of Former Air Force General Hospital of PLA in the period from January 2017 to April 2018,were assigned to ordinary group.One hundred and fifty-three vertigo inpatients and outpatients of flying personnel,who were diagnosed by the hospital in the period from September 1984 to Augut 2018,were selected as flying personnel group.The etiological stratification analysis was conducted upon the gender and age in ordinary group.The disease spectrum was analyzed in flying personnel group.The etiological characteristics were compared between two groups. Results①In ordinary group,1 265 etiological factors were found in 1 016 vertigo patients (some of them had 2 or more etiologies).The top 7 etiological factors of the vertigo patients in ordinary group were benign paroxysmal positional vertigo (BPPV)that took 32.73 % (414/1 2 6 5 ),vestibular migraine (VM),31.46% (398/1 265 ),hypertension 12.96% (164/1 265 ),diabetes 4.03% (51/1 265 ), Meniere,s disease(MD)3.87%(49/1 265),vestibular neuritis(VN)3.87%(49/1 265),and cerebral infarction 3.24%(41/1 265).1.43%(1 7/1 265)cases were unable to categorize clearly.②The patients of ordinary group,aged from 8 to 89 yrs,were categorized to <21 yrs,21-40 yrs,41-60 yrs and >60 yrs subgroups.The top 4 etiological factors for <21 yrs group were VM (43.37%),BPPV (15.79%),VN(10.53%),and sudden deafness with vertigo(10.53%);for 21-40 yrs group were VM (46.86%),BPPV(29.7 1 %),VN (5 .86%),and hypertension (4.60%);for 41-60 yrs group were BPPV(35 .1 7%),VM(3 1 .54%),hypertension(12.30%),and MD(4.73%);and for >60 yrs group were BPPV (3 1 .28%),VM (20.5 9%),hypertension (20.05%),and cerebral infraction (6.68%). Detection rate of the top 7 etiological factors had significant difference in different age groups except VN and MD (χ2=1 3 .45-5 3 .1 6 ,P<0.0 1 ).③The etiological stratification by gender showed that the ratio of male to female was 1.00︰1.57.Detection rate of VM in female was higher than that in male. Detection rate of hypertension,MD and VN in male was higher than that in female,the difference was satistically significant (χ2=3.87-8.93,P<0.05).④In flying personnel group,184 etiological factors were found in 153 vertigo patients (some of them had 2 or more etiologies).Top 3 etiological factors were motion sickness(MS)that took 1 1 .41%(21/184),VN that took 10.32%(1 9/184)and MD that took 9.78% (18/184 ).The dizziness or vertigo with no accurate etiology took 57.61 % (106/184 ).⑤The BPPV and VM detection rate in flying personnel group were less than those in ordinary group (χ2=78.34,78.7 7 ,P<0.0 1 ).The dizziness or vertigo cases with no accurate etiology and MS,MD, VN and alternobaric vertigo cetection rate in flying personnel group were higher than those in ordinary group (χ2=6 .02-645 .5 9 ,P<0.05 ). Conclusions The causes of vertigo/dizziness are complex and diverse and patients may have more than one etiology.The etiological stratification analysis for patients is conducive to accurate diagnosis and treatment.Studying the difference of etiology between flying personnel and ordinary people will contribute to exploring the pathogenesis and prevention measures in flight environment and providing proof for aeromedical assessment.
7.Detection and reference value of cervical and ocular vestibular evoked myogenic potential in military pilots

Objective To obtain the detection method and reference constants of cervical vestibular evoked myogenic potential (cVEMP ) and ocular vestibular evoked myogenic potential (oVEMP)in military pilots and to provide reference basis for evaluating pilot,s otolith function in aeromedical assessment. Methods Thirty-three (6 6 ears )healthy male active duty fighter pilots were selected as study subj ects.They were between the age of 25 to 42 years old (30.3 ± 4.8 years). cVEMP and oVEMP were induced unilaterally by means of 500 Hz air-conducted tone burst stimulation.The VEMPs threshold of each ear was examined and the reference range of amplitude, latency and wave interval were calculated. Results The composite wave of cVEMP and oVEMP was observed in 33 pilots.The evoked rate was 100%.The latency of p13 and n23,p13-n23 interval,peak-to-peak p1 3-n2 3 amplitude and asymmetry ratio of cVEMP in these pilots were (1 3 .94 ± 1 .5 9 )ms, (20.95±1.59)ms,(7.01 ± 1.34)ms,(224.70 ±66.15)μV,(0.1 1±0.10).The amplitude had slight difference between the ears while the cVEMP p1 3 latency,n23 latency and wave interval showed very small difference between the ears.The latency of N1 and P1 ,N1-P1 interval,peak-to-peak N1-P1 amplitude and asymmetry ratio of oVEMP in the pilots was (1 1 .1 ± 0.9 9 )ms,(1 5 .41 ± 0.85 )ms, (4.31±0.84)ms,(2.23±0.59)μV,(0.10 ±0.08)respectively.The amplitude of oVEMP,latency of N1 ,latency and wave interval of P1 showed unobvious difference between left and right ears. Conclusions This study has established the conventional detection methods and constants of cVEMP and oVEMP of military pilots and these provide the obj ective reference to evaluate the functions of utricle and sacculus in aeromedical assessment.

10.Content Determination of Sinalbin in Sinapis alba Decoction Piece by HPLC

OBJECTIVE:To establish a method for the content determination of sinalbin in Sinapis alba decoction piece. METHODS:HPLC was performed on the column of Sepax HP-C18 with mobile phase of 50% acetonitrile-water solution(contain-ing 5 mmol/L tetraoctyl ammonium bromide)at a flow rate of 1.0 ml/min,detection wavelength was 227 nm,column temperature was 30 ℃,and the injection volume was 10 μl. RESULTS:The linear range of sinalbin was 0.010 1-0.202 4 mg/ml(r=0.999 7);RSDs of precision,stability and reproducibility tests were lower than 2%,recovery was 98.51%-104.62%(RSD=1.90%,n=9). CONCLUSIONS:The method is simple,accurate and reliable,and can be used for determination of sinalbin in Sinapis alba decoc-tion piece.
