1.Analysis of the relationship between stromal fibrosis degree and targeted therapy resistance and prognosis in EGFR mutant lung adenocarcinoma

Objective To assess stromal fibrosis in epidermal growth factor receptor(EGFR)mutant lung adenocarcinoma and its association with resistance to targeted therapy and patient prognosis.Methods Medical records of 207 patients diagnosed with EGFR-mutant advanced lung adenocarcinoma who received treatment at a hospital between January 2021 and December 2022 were reviewed.A total of 86 patients were ultimately included based on their prognosis and survival duration.These patients were categorized into a resistance group(32 cases)and a non-resistance group(54 cases),depending on whether they developed resistance to targeted therapy within one year.Additionally,patients were classified into mild,moderate,and severe fibrosis groups according to the extent of fibrosis observed.Clinical and pathological characteristics,as well as fibrosis levels,were compared between the two groups.Factors influencing the development of resistance to targeted therapy in patients with EGFR-mutant lung adenocarcinoma were analyzed,and the survival outcomes of patients with varying degrees of fibrosis were evaluated during follow-up.Results In the resistance group,the prevalence of EGFR exon 20 insertion mutations,elevated CA125 levels,and the presence of moderate-to-severe fibrosis were significantly higher compared to the non-resistance group(P<0.05).Multivariate logistic regression analysis revealed that EGFR exon 20 inser-tion mutation(OR=3.691,95%CI:1.043~13.057),elevated CA125 levels(OR=4.104,95%CI:1.160~14.517),and moderate-to-severe fibrosis(OR=3.959,95%CI:1.410~11.115)were independent risk factors associated with resistance to targeted therapy among patients with EGFR-mutant lung adenocarcinoma(P<0.05).The Cox proportional hazards model demonstrated a C-index of 0.72(95%CI:0.65~0.79),with area under the curve(AUC)values for 1-year and 2-year survival predictions of 0.781 and 0.734,respectively.EGFR exon 20 insertion mutation(HR=3.691),moderate-to-severe fibrosis(HR=3.959),and elevated CA125 levels(HR=4.104)were identified as independent prognostic factors for overall survival in these patients following targeted therapy.The median progression-free survival(PFS)for patients with mild,moderate,and severe fibrosis was 10.5 months,7.2 months,and 3.9 months,respectively,while the median overall survival(OS)was 21.4 months,16.1 months,and 11.5 months,respectively.Statistically significant differences in both PFS and OS were observed across the three fibrosis severity groups.(P<0.05).Conclusion The extent of stromal fibrosis in EGFR-mutant lung adenocarcinoma influences resistance to targeted therapy,and the progression of fibrosis is correlated with an unfavorable prognosis.

3.Divergent chondro/osteogenic transduction laws of fibrocartilage stem cell drive temporomandibular joint osteoarthritis in growing mice.

The anterior disc displacement (ADD) leads to temporomandibular joint osteoarthritis (TMJOA) and mandibular growth retardation in adolescents. To investigate the potential functional role of fibrocartilage stem cells (FCSCs) during the process, a surgical ADD-TMJOA mouse model was established. From 1 week after model generation, ADD mice exhibited aggravated mandibular growth retardation with osteoarthritis (OA)-like joint cartilage degeneration, manifesting with impaired chondrogenic differentiation and loss of subchondral bone homeostasis. Lineage tracing using Gli1-CreER+; Tmfl/-mice and Sox9-CreER+;Tmfl/-mice showed that ADD interfered with the chondrogenic capacity of Gli1+ FCSCs as well as osteogenic differentiation of Sox9+ lineage, mainly in the middle zone of TMJ cartilage. Then, a surgically induced disc reposition (DR) mouse model was generated. The inhibited FCSCs capacity was significantly alleviated by DR treatment in ADD mice. And both the ADD mice and adolescent ADD patients had significantly relieved OA phenotype and improved condylar growth after DR treatment. In conclusion, ADD-TMJOA leads to impaired chondrogenic progenitor capacity and osteogenesis differentiation of FCSCs lineage, resulting in cartilage degeneration and loss of subchondral bone homeostasis, finally causing TMJ growth retardation. DR at an early stage could significantly alleviate cartilage degeneration and restore TMJ cartilage growth potential.

4.Effects of "1+N" extended nursing on medication compliance and self-management ability among type 2 diabetes patients

Objective? To explore the effects of "1+N" extended nursing on medication compliance, self-management ability and metabolic indicators among discharged type 2 diabetes patients. Methods? From October to December 2017, we selected 300 discharged type 2 diabetes patients from Department of Endocrinology from Shaanxi Provincial People's Hospital as subjects. All of the patients were divided into intervention group and control group with the method of random number table, 150 cases in each group. Control group carried out telephone follow-up post discharge. On the basis of telephone follow-up, intervention group implemented "1+N" extended nursing. We compared the fasting blood glucose (FBG), bisphosphoglyceric acid (BPG), glycosylated hemoglobin (HbA1c), body mass index (BMI), scores of 8-item Morisky Medication Adherence Scale (MMAS-8) and the scoring rate of Diabetes Self-care Scale (2-DSCS) of patients between two groups 6 months after follow-up. Results? After intervention, the FBG, BPG and BMI of intervention group were lower than those of control group with statistical differences (P<0.05). The number of patients with the high level of compliance of MMAS-8 in intervention group was more than that in control group with a statistical difference (P<0.05).The item scoring rate of 2-DSCS of patients in intervention group were higher than those in control group with statistical differences. Conclusions "1+N" extended nursing can improve metabolic indicators, medication compliance and self-management ability among type 2 diabetes patients.
5.Effect of MIG1 and SNF1 deletion on simultaneous utilization of glucose and xylose by Saccharomyces cerevisiae.

Mig1 and Snf1 are two key regulatory factors involved in glucose repression of Saccharomyces cerevisiae. To enhance simultaneous utilization of glucose and xylose by engineered S. cerevisiae, single and double deletion strains of MIG1 and SNF1 were constructed. Combining shake flask fermentations and transcriptome analysis by RNA-Seq, the mechanism of Mig1 and Snf1 hierarchically regulating differentially expressed genes that might affect simultaneous utilization of glucose and xylose were elucidated. MIG1 deletion did not show any significant effect on co-utilization of mixed sugars. SNF1 deletion facilitated xylose consumption in mixed sugars as well as co-utilization of glucose and xylose, which might be due to that the SNF1 deletion resulted in the de-repression of some genes under nitrogen catabolite repression, thereby favorable to the utilization of nitrogen nutrient. Further deletion of MIG1 gene in the SNF1 deletion strain resulted in the de-repression of more genes under nitrogen catabolite repression and up-regulation of genes involved in carbon central metabolism. Compared with wild type strain, the MIG1 and SNF1 double deletion strain could co-utilize glucose and xylose, and accelerate ethanol accumulation, although this strain consumed glucose faster and xylose slower. Taken together, the MIG1 and SNF1 deletions resulted in up-regulation of genes under nitrogen catabolite repression, which could be beneficial to simultaneous utilization of glucose and xylose. Mig1 and Snf1 might be involved in the hierarchical regulatory network of genes under nitrogen catabolite repression. Dissection of this regulatory network could provide further insights to new targets for improving co-utilization of glucose and xylose.
6.Research on the relationship between psychological health and social support and coping style in general hospitats

Objective To study the relationship between psychological health and social support and coping style in general hospitals. Methods 756 nurses from 3 general hospitals were enrolled by cluster sampling and were investigated by Symptom Checklist 90(SCL-90),Social Support Rate Scale(SSRS)and Simplified Coping Style Questionnaire(SCSQ).The investigation results were analyzed. Results The general level of psychological health of nurses was not optimistic with a positive rate Of 37.04%. The somatizatlon factor of the head nurses was different from that Of normal nurses. Significant difference existed in the following aspects such as somatization, depression and terror factors between night shift nurses and day shift nurses. Relativity could be seen between social support and coping style and psychological health. Conclusion Hospital managers should attach importance to the psychological health of nurses. At the same time they should supply positive social support and instrucitons of coping style to improve the general level of psychological health.
7.Effects of Electrical Stimulation on Dysphagia after Stroke

Objective To investigate the effects of electrical stimulation on dysphagia after stroke.Methods 40 patients with dysphagia after stroke were divided into control group and research group randomly.Both groups were treated with rehabilitation training and medicine.Research group was managed with electrical stimulation in addition.They were assessed with the classification of dysphagia.Results Dysphagia was reduced in both groups,but was better in the research group than in the control group(P<0.05).Conclusion Electrical stimulation can improve the recovery of deglutition.

BACKGROUNDTo observe the suppressive effects of exogenous p27 gene on human lung cancer cell line A549.
METHODSAn adenovirus expression vector (pAd CMV-p27) containing 570 bp human full-length p27 cDNA was transfected into human lung cancer cell line A549. Expression of exogenous p27 gene was detected by dot-blot hybridization and laser co-focal system. MTT was adopted to measure the effects of exogenous p27 gene on cell cycle progression and cell features of the infected A549.
RESULTSThe mRNA and protein expression level of p27 was remarkably increased after transfecting with exogenous p27 gene. The apoptosis of infected A549 occurred and the progression of cell cycle was arrested in G1 phase.
CONCLUSIONSp27(kipl) gene transfer may play a therapeutic role in the treatment of lung cancer.
