1.Effect of Optimized New Shengmai Powder (优化新生脉散方) on Exercise Tolerance in Patients with Chronic Heart Failure of Qi Deficiency,Blood Stasis and Fluid Retention Syndrome:A Randomized,Double-Blind,Placebo-Controlled Trial

ObjectiveTo evaluate the effects and safety of the optimized new Shengmai Powder (优化新生脉散方) on exercise tolerance in patients with chronic heart failure (CHF) of qi deficiency, blood stasis, and fluid retention syndrome. MethodsA randomized, double-blind, placebo-controlled trial was conducted. A total of 78 CHF patients with qi deficiency, blood stasis, and fluid retention syndrome were recruited and randomly assigned to a treatment group (39 cases) and a control group (39 cases). On the basis of conventional western medical therapy, patients in the treatment group additionally received the optimized new Shengmai Powder granules, while the control group was given an oral placebo of optimized new Shengmai Powder granules. Patients in both groups took 30.6 g each time, twice a day, mixed with water for administration, with a total treatment course of 4 weeks. The primary outcomes were 6-minute walk distance (6MWD) and peak oxygen uptake (Peak VO2) measured by cardiopulmonary exercise testing. Secondary outcomes included New York Heart Association (NYHA) functional classification, B-type natriuretic peptide (BNP) levels, cardiac function indexes including left ventricular ejection fraction (LVEF), left ventri-cular end-systolic diameter (LVESD) and left ventricular end-diastolic diameter (LVEDD), Minnesota Living with Heart Failure Questionnaire (MLHFQ) scores, and scores of four diagnostic information of traditional Chinese medicine (TCM). All indicators were assessed once before and after treatment respectively. Safety indicators were evaluated, and adverse events during the trial were recorded. ResultsAll patients in both groups were included in the full ana-lysis set (FAS) and safety set (SS). Compared with baseline, the 6MWD and Peak VO₂ of cardiopulmonary exercise test in the treatment group significantly increased after treatment, while the MLHFQ scores, serum BNP levels and scores of TCM four diagnostic information significantly decreased, and the NYHA cardiac function grade significantly improved (P<0.01). After treatment, the 6MWD and Peak VO₂ of cardiopulmonary exercise test, as well as their changes from baseline in the treatment group were higher than those in the control group; the MLHFQ scores, serum BNP levels and scores of TCM four diagnostic information in the treatment group were lower than those in the control group; and the improvement of NYHA cardiac function grade in the treatment group was superior to that in the control group (P<0.01). There was no statistically significant differences in all indicators after treatment in the control group (P>0.05). The incidence of adverse events was 5.1% (2/39) in the treatment group and 2.6% (1/39) in the control group, with no statistically significant difference between groups (P>0.05). ConclusionOn the basis of conventional western medicine treatment, the addition of the optimized new Shengmai Powder can further improve exercise tolerance, cardiac function and quality of life in patients with CHF of qi deficiency, blood stasis and fluid retention syndrome, and show good safety.
2.A Case of One-stop Procedure for Cryoballoon Ablation and Left Atrial Appendage Occlusion of Atrial Fibrillation Under Electroacupuncture Anesthesia

Atrial fibrillation is the most common type of chronic cardiac arrhythmia. Catheter ablation and left atrial appendage occlusion are effective treatment methods for atrial fibrillation, but these procedures require anesthesia support. However, anesthetic drugs often cause side effects such as nausea, vomiting, involuntary movements, and respiratory depression. This paper presents a case of a successful one-stop procedure for cryoballoon ablation and left atrial appendage occlusion of atrial fibrillation performed entirely under acupuncture anesthesia. Thirty minutes before the procedure, acupuncture needles were inserted perpendicularly at bilateral Neiguan (PC 6), Lieque (LU 7), Ximen (PC 4) and (LU 6). After obtaining the deqi (得气) sensation, an electroacupuncture device was connected, and electroacupuncture anesthesia was used for pain control throughout the procedure. The patient exhibited good tolerance and cooperation, with electroacupuncture anesthesia completely replacing intravenous anesthetics, ensuring the smooth completion of the surgery. Postoperative follow-up showed favorable outcomes.
3.Influence of long-term spaceflight on human speed perception characteristics

Objective Accurate speed perception is crucial for tasks such as man-controlled rendezvous and docking,and teleoperation of space manipulator.Therefore,it is necessary to conduct in-orbit experiments to explore the influence of long-term spaceflight on human speed perception characteristics.Methods The Time-to-Collision(TTC)paradigm was selected to develop experimental software,using a tablet computer for stimulus presentation.Human speed perception characteristics were evaluated based on the subjects'keystroke response data on the keyboard.Through ground-based experiments,the usability and reliability of the paradigm were explored,and the gravity internal model effect was quantitatively analyzed.Through in-orbit experiments on space station tasks,the influence of long-term spaceflight on human speed perception characteristics was further investigated.Results Under the 1G environment on the ground,the TTC paradigm has a high test-retest reliability(r>0.8),and indicators such as average deviation rate and absolute value of average deviation rate show no practice effect.In addition,ground experiments found that compared to vertical upward movement,vertical downward movement is estimated to be faster(i.e.,keystroke time is advanced),showing the existence of the gravity internal model effect.In the microgravity environment of spaceflight,there are no significant differences in average deviation rate and absolute value of average deviation rate among three stages(pre-flight,in-flight,post-flight)and seven tests,indicating that no obvious changes in astronauts'speed perception ability were found at the existing test time points and paradigms.However,the gravity internal model effect(difference between vertical downward and vertical upward)showed a trend of fading in the early stage of astronauts entering orbit.Conclusion Based on the computer screen TTC estimation paradigm,no significant changes in human speed perception ability were found during long-term spaceflight,but microgravity may weaken the human brain's gravity internal model.
4.Clinical application of bidirectional dualloop technology in retrieving Aegisy inferior vena cava filters beyond the time window

Objective To discuss the application of bidirectional dual loop technique in retrieving Aegisy inferior vena cava filters.Methods The clinical data of 12 patients with deep vein thrombosis in the lower limbs complicated by pulmonary embolism,who received mechanical thrombectomy and Aegisy inferior vena cava(IVC)filter implantation at the Zhoukou Municipal Central Hospital of China from January 2016 to November 2023,were retrospectively analyzed.The retrieval of Aegisy IVC filters beyond the time window was tried by standard methods,guide wire stripping,balloon dilatation,unidirectional single loop technique,unidirectional dual loop technique,etc.However,because of the difficulty in retrieval of filter,bidirectional dual loop technique had to be used.The indwelling time of the IVC filters was 30-365 days,with an average time of 105 days.Results The IVC filter was completely removed in 11 patients.Angiography showed that IVC was unobstructed,no contrast extravasation was observed,and the average operation time was 126 minutes.The retrieval of filter failed in one patient,and the patient had to receive anticoagulation therapy lifelong.No surgery-related complications occurred.Conclusion For retrieving Aegisy IVC filters beyond the time window,bidirectional dual loop technique is clinically safe and effective,and this technology is worthy of clinical promotion and application.
5.Pollution status and distribution characteristics of indoor air bacteria in subway stations and compartments in a city of Central South China

Background Bacteria are the most diverse and widely sourced microorganisms in the indoor air of subway stations, where pathogenic bacteria can spread through the air, leading to increased health risks. Objective To understand the status and distribution characteristics of indoor air bacterial pollution in subway stations and compartments in a city of Central South China, and to provide a scientific basis for formulating intervention measures to address indoor air bacteria pollution in subways. Methods Three subway stations and the compartments of trains parking there in a city in Central South China were selected according to passenger flow for synchronous air sampling and monitoring. Temperature, humidity, wind speed, carbon dioxide (CO2), fine particulate matter (PM2.5), and inhalable particulate matter (PM10) were measured by direct reading method. In accordance with the requirements of Examination methods for public places-Part 3: Airborne microorganisms (GB/T 18204.3-2013), air samples were collected at a flow rate of 28.3 L·min−1, and total bacterial count was estimated. Bacterial microbial species were identified with a mass spectrometer and pathogenic bacteria were distinguished from non-pathogenic bacteria according to the Catalogue of pathogenic microorganisms transmitted to human beings issued by National Health Commission. Kruskal-Wallis H test was used to compare the subway hygiene indicators in different regions and time periods, and Bonferroni test was used for pairwise comparison. Spearman correlation test was used to evaluate the correlation between CO2 concentration and total bacterial count. Results The pass rates were 100.0% for airborne total bacteria count, PM2.5, and PM10 in the subway stations and train compartments, 94.4% for temperature and wind speed, 98.6% for CO2, but 0% for humidity. The overall median (P25, P75) total bacteria count was 177 (138,262) CFU·m−3. Specifically, the total bacteria count was higher in station halls than in platforms, and higher during morning peak hours than during evening peak hours (P<0.05). A total of 874 strains and 82 species were identified by automatic microbial mass spectrometry. The results of identification were all over 9 points, and the predominant bacteria in the air were Micrococcus luteus (52.2%) and Staphylococcus hominis (9.8%). Three pathogens, Acinetobacter baumannii (0.3%), Corynebacterium striatum (0.1%), and Staphylococcus epidermidis bacilli (2.2%) were detected in 23 samples (2.6%), and the associated locations were mainly distributed in train compartments during evening rush hours. Conclusion The total bacteria count in indoor air varies by monitoring sites of subway stations and time periods, and there is a risk of opportunistic bacterial infection. Attention should be paid to cleaning and disinfection during peak passenger flow hours in all areas.
6.Effect of Jiuxin Pill (救心丸)on Exercise Tolerance and Quality of Life in Patients of Stable Angina Pectoris:A Randomized,Double-Blind,Placebo-Controlled,Multi-Center Clinical Trial

ObjectiveTo evaluate the effect and safety of Jiuxin Pill (救心丸) on exercise tolerance and quality of life in patients with stable angina pectoris (SAP). MethodsA randomised, double-blind, placebo-controlled, multicentre study design was used to enroll 170 patients of SAP from nine centres, which were divided into 85 patients each in the trial group and control group with 1∶1 ratio. Both groups maintained the original western medicine treatment plan, and added Jiuxin Pill or placebo respectively, 2 pills (0.05 g) each time twicely for 28 days. The main outcomes were total exercise time (TED) in the exercise treadmill test and Seattle Angina Questionnaire (SAQ) scores including physical limitation (PL), angina stability (AS), angina frequency (AF), treatment satisfaction (TS), and disease perception (DP). The secondary outcomes were exercise treadmill test indicators including heart rate recovery in 1 min (HRR1), metabolic equivalents (METs), maximum magnitude of ST-segment depression, and the Borg rating of perceived exertion scale, the average number of angina attacks per week, withdrawal and reduction rate of nitroglycerin, traditional Chinese medicine syndrome scores, incidence of major adverse cardiovascular events. Safety indicators were evaluated and the occurrence of adverse events during the trial was recorded. Data was collected before treatment, day 28±2 in treatment period, and follow-up at day 56 which is 28±2 days after treatment period finished. ResultsEighty-four and eighty-five patients respectively from trial group and control group were included to the full analysis set (FAS) and safety analysis set (SS). Compared with the group before treatment and with the control group after treatment, the trial group had higher TED, HRR1, and METs, and lower maximum magnitude of ST-segment depression and Borg rating of perceived exertion scores after treatment (P<0.01). Compared with the group before treatment and with the control group after treatment and at follow-up, the total SAQ score and scores of AS, AF, TS and DP of the trial group after treatment and at follow-up elevated, while the average number of angina attacks per week and traditional Chinese medicine syndrome scores reduced (P<0.01). There was no statistically significant difference in the withdrawal and reduction rate of nitroglycerin between groups (P>0.05). Major adverse cardiovascular events occurred in 1 case (1/84, 1.19%) in the trial group and 1 case (1/85, 1.18%) in the control group, and the difference between groups was not statistically significant (P>0.05). A total of 3 cases of adverse events occurred in the trial group (3/84, 3.57%), and a total of 6 cases of adverse events occurred in the control group (6/85, 7.06%), and there was no statistically significant difference in the incidence of adverse events between groups (P>0.05). ConclusionIn the treatment of SAP, Jiuxin Pill combined with conventional western medicine can further enhance exercise tolerance, improve quality of life, and demonstrate great safety.
7.Effect of Saikosaponins and Paeoniflorin on Neurotransmitters in the Hippocampal Brain Region of Rats with Premenstrual Dysphoric Disorder Model of Liver Qi Depression Using UHPLC-MS/MS Target Metabolomics

OBJECTIVE
To study the therapeutic effect of saikosaponins and paeoniflorin on rats with premenstrual dysphoric disorder(PMDD) model of liver Qi depression and the effect on neurotransmitters in hippocampal brain area.
METHODS
The PMDD rat model of liver Qi depression was prepared by the time-chosen chronic restraint stress method, and the depressive symptoms of rats were evaluated by the intervention of saikosaponins and paeoniflorin, the open field experiment and forced swimming experiment, and the changes of neurotransmitter levels in the hippocampal brain region of rats were detected by UHPLC-MS/MS target metabolomics.
RESULTS
The levels of 5-HT, Glu, GABA and NE in the hippocampal brain area of PMDD rats were significantly increased, and the levels of 5-HT, Glu, GABA, NE, DA and E were not statistically significant from those of the normal group after the intervention of saikosaponins and paeoniflorin.
CONCLUSION
Saikosaponins and paeoniflorin alleviates depression in rats with PMDD by regulating neurotransmitter levels, and it may be the pathway for the clinical treatment of PMDD with liver qi depression in the formulations of Bupleuri Radix and Paeoniae Radix Alba.
8.The Effect of Yangyin Xifeng Tongluo Formula (养阴熄风通络方) and Its Core Herb-Pairs on Prevention of Aconitine-Induced Ventricular Precontraction in Rats

ObjectiveTo observe the effect of Yangyin Xifeng Tongluo Formula (养阴熄风通络方) and its core herbs combination on prevention of ventricular precontraction (PVC). MethodsExperiment 1: Forty SD rats were randomly divided into model group, formula group, core herb-pairs medium-dose group and amiodarone group, with 10 rats in each group. Rats in the model group were given 10 ml/(kg·d) of pure water by gavage, rats in the formula group were given 1.125 g/(kg·d) of Yangyin Xifeng Tongluo Granules (养阴熄风通络方颗粒) by gavage, rats in the core herb-pairs medium-dose group were given 0.585 g/(kg·d) of granules of core herb-pairs by gavage, and rats in the amiodarone group were given 18 mg/(kg·d) of amiodarone hydrochloride tablets by gavage. The rats in each group were gavaged once a day for 14 consecutive days, and then a PVC model was established using rat tail vein injection of aconitine 25 μg/kg to compare the mortality and incidence of PVC, ventricular tachycardia (VT) and ventricular fibrillation (VF), the time of the appearance of PVC, and the duration of PVC in the rats in each group. Experiment 2: Sixty SD rats were randomly divided into model group, the amiodarone group, and the core herb-pairs of low, medium and high dose groups, 12 rats in each group. Rats in the model group were gavaged with 10 ml/(kg·d) of purified water, rats in the low, medium, and high dose groups were gavaged with 0.2925, 0.585, and 1.17 g/(kg·d) of the core herb-pairs granules respectively, and rats in the amiodarone group were gavaged with 18 mg/(kg·d) of amiodarone hydrochloride tablets. All of them were gavaged once a day. After 14 days, rats in each group were injected intravenously into the tail vein with aconitine 25 μg/kg, and then the rats in each group were observed to show the mortality and incidence of PVC, VT and VF, and the time of appearance and duration of PVC. ResultsExperiment 1: Compared with the model group, The difference in the incidence of PVC in rats of all groups was not statistically signi-ficant (P>0.05), and the mortality and incidence of VT and VF in rats in the formula group, the core herb-pairs medium-dose group, and the amiodarone group significantly reduced, with delayed time of the occurrence of PVC and shorter duration of the PVC (P<0.05 or P<0.01) as compared with the model group, whereas the difference between each group with medication intervention was not statistically significant (P>0.05). Experiment 2: There was no statistically significant difference in the incidence of PVC in all groups (P>0.05), and the mortality and incidence of VT and VF reduced in the core herb-pairs of low-, medium-, and high-dose groups and in the amiodarone group, and the appearance of PVC delayed and its duration shortened (P<0.05 or P<0.01) as compared with the control group, whereas the differences were not statistically significant in the comparison among groups with medication (P>0.05). ConclusionBoth Yangyin Xifeng Tongluo Formula and its core herb-pairs could delay the time of occurrence and shorten the duration of PVC.
9.Clinical application of magnetic resonance imaging-guided radiation therapy in the treatment of pancreatic cancer

At present, pancreatic cancer is a solid tumor with the worst prognosis. Compared with surgery and chemotherapy, radiotherapy plays an auxiliary role in the treatment of pancreatic cancer. In recent years, significant advances have been achieved in radiotherapy technology, which have been gradually applied in the diagnosis and treatment of pancreatic cancer. In this article, the progress in radiation therapy in the treatment of pancreatic cancer was reviewed, especially the clinical application of magnetic resonance imaging-guided radiation therapy in the treatment of pancreatic cancer, aiming to deepen the understand of the progress in radiation therapy for pancreatic cancer, and providing reference for improving the survival rate of pancreatic cancer patients.
10.Evaluation of the feasibility of 4-channel functional electrical stimulation for treating sequelae of cerebral ischemic stroke using magnetic resonance diffusion tensor imaging

Objective To evaluate the feasibility of 4-channel functional electrical stimulation(FES)in the treatment of cerebral ischemic stroke sequelae using 3.0T magnetic resonance diffusion tensor imaging(DTI).Methods Patients with the sequelae of cerebral ischemic stroke who were admitted to Shandong Provincial Third Hospital from February 2020 to November 2022(n=100)were enrolled and divided into two groups at a ratio of 1:1 according to random numbers generated by Excel.Besides routine rehabilitation training,observation group was given 4-channel FES to stimulate the quadriceps muscle,tibialis anterior muscle,gastrocnemius muscle and hamstring muscle of the affected lower extremity by simulating the time series of muscle contraction of normal subjects when walking;while control group was treated with dual-channel FES,with electrodes placed on the muscle points of the tibialis anterior muscle and peroneus longus and brevis muscles of the affected lower extremity for stimulation.All patients received 3.0T MRI before and after treatment,and the relevant DTI parameters were recorded and compared.Meanwhile,Berg balance scale(BBS)and Fugl-Meyer assessment scale for lower extremity(FMA-LE)were used to evaluate the therapeutic efficacies in two groups of patients with the sequelae of cerebral ischemic stroke.The correlation of BBS/FMA-LE score with DTI parameters in observation group was also discussed.Results After treatment,both FMA-LE and BBS scores of control group and observation group showed increasing trends,and the increases in observation group were more obvious(P<0.05).After 2 weeks of treatment,compared with control group,observation group had higher FMA-LE score,BBS score,fractional anisotropy(FA),average diffusion coefficient(DCavg)and volume ratio anisotropy(VRA),but lower exponential attenuation(Exat)(all P<0.05).In observation group,FMA-LE score was positively correlated with FA,DCavg and VRA,but negatively correlated with Exat;while BBS score was positively correlated with FA and VRA,but negatively correlated with Exat,and had no significant correlation with DCavg.Conclusion DTI can be used to evaluate the therapeutic efficacy of 4-channel FES on the sequelae of cerebral ischemic stroke,and its parameters are significantly correlated with BBS score and FMA-LE score,suggesting that MRI can assist doctors in understanding the correlation between neurological impairments and neuropathological changes,and provide reference for accurate assessment of prognosis and formulation of individualized rehabilitation program.
