1.Banxia Baizhu Tianmatang Regulates NLRP3 Inflammasomes to Ameliorate Cognitive Impairment in Epilepsy

ObjectiveTo explore the mechanism through which Banxia Baizhu Tianmatang ameliorates cognitive impairment in epileptic rats induced by lithium chloride-pilocarpine by regulating the neuroinflammatory reaction mediated by NOD-like receptor protein 3 (NLRP3) inflammasomes. MethodsSixty male SD rats were randomly allocated into blank, model, carbamazepine (0.125 g·kg-1·d-1), Banxia Baizhu Tianmatang (1.04 g·kg-1·d-1), and carbamazepine (0.125 g·kg-1·d-1) + Banxia Baizhu Tianmatang (1.04 g·kg-1·d-1) groups (n=12). After the modeling of epilepsy, rats were administrated with corresponding agents by gavage for 12 weeks. At the 6th and 12th week of the intervention, the rats’ hyper-excited behavior was evaluated by the stylus experiment, and at the 12th week of intervention, the cognitive function was evaluated by Barnes maze. At the same time, the seizure frequency and severity grade (Racine score) were recorded. The serum and hippocampus tissue samples were collected after anesthesia for the following tests. Nissl staining was used to evaluate the degree of neuronal damage in the hippocampal CA1 area. The content of malondialdehyde (MDA) in the hippocampus was determined by the thiobarbituric acid (TBA) method. Serum levels of tumor necrosis factor-α (TNF-α), interleukin-1β (IL-1β), and interleukin-18 (IL-18) were quantified by enzyme-linked immunosorbent assay (ELISA). Immunohistochemical method was adopted to detect the expression of apoptosis-associated speck-like protein containing a card (ASC) in the hippocampus. Western blot was employed to quantitatively analyze the protein levels of NLRP3, cysteinyl aspartate-specific proteinase-1 (Caspase-1), and brain-derived neurotrophic factor (BDNF) in the hippocampus. ResultsThe model group showed increased stylus scores at the 6th and 12th week after modeling, a decreased Barnes maze strategy score at the 12th week, a prolonged incubation period (P<0.05), elevated serum levels of inflammatory factors (P<0.05), decreased neurons with scattered arrangement and large gaps in the hippocampus, increased content of MDA in the hippocampus (P<0.05), an increased positive expression of ASC, and up-regulated protein levels of Caspase-1, NLRP3, and BDNF (P<0.05). Compared with the model group, the intervention with Banxia Baizhu Tianmatang for 12 weeks was accompanied by a decreased stylus score, epileptic seizures with a decreased score, a decreased number, and shortened duration, an increased Barnes maze strategy score, shortened escape latency (P<0.01), declined serum levels of inflammatory factors (P<0.05), regular morphology of hippocampal neurons, reduced MDA content in the hippocampus (P<0.05), a decreased positive expression of ASC, and down-regulated protein levels of Caspase-1, NLRP3, and BDNF (P<0.05, P<0.01). In addition, compared with the carbamazepine group, Banxia Baizhu Tianmatang + carbamazepine showed improved performance in controlling the seizure, improved the cognitive behavior score and morphology of hippocampal neurons, alleviated the oxidative stress products, lowered the levels of inflammatory factors, reduced the positive expression of ASC in the hippocampus, and down-regulated the expression of Caspase-1, NLRP3 and BDNF, with no significant differences. ConclusionBanxia Baizhu Tianmatang may reduce neuroinflammation, control epileptic seizures, and ameliorate cognitive impairment by inhibiting the expression of NLRP3 inflammasomes.
2.Exploring Mechanism of Banxia Baizhu Tianma Decoction in the Treatment of Epilepsy Based on Network Pharmacology and Experimental Verification

Objective To analyze the target,signal pathway and potential mechanism of Banxia Baizhu Tianma Decoction in the treatment of epilepsy based on network pharmacology,and to verify it by molecular docking technology and animal experiments.Methods The active ingredients and drug targets of Banxia Baizhu Tianma Decoction were screened by BATMAN and other databases.The targets of epilepsy-related diseases were obtained by GeneCards and other databases,and the intersection targets were taken.Constructing'drug-ingredient-target-disease'network and PPI network to screen the core targets.The core active ingredients were screened according to GO,KEGG functional enrichment analysis and'pathway-target-active ingredient'network.Molecular docking was used to verify the core targets and core active ingredients.In the animal experiment,the rat model of epilepsy was induced by lithium chloride-pilocarpine,and 40 Wistar rats were divided into normal control group,model control group,carbamazepine group and Banxia Baizhu Tianma Decoction group.The seizures were observed by behavior.Nissl staining was used to observe neuronal damage in hippocampus.Immunohistochemistry was used to detect the expression of BAX,BCL-2 and Caspase-3 protein.Results A total of 1072 targets of Banxia Baizhu Tianma Decoction were screened,1046 disease targets of epilepsy were screened,and 220 intersection targets of Banxia Baizhu Tianma Decoction in the treatment of epilepsy were screened.The core targets AKT1,ALB,ACTB,INS and TNF of PPI network were obtained.KEGG pathway mainly involves TNF signaling pathway,IL-17 signaling pathway,cAMP signaling pathway,pathways of neurodegeneration-multiple diseases,serotonergic synapse and Dopaminergic synapse.The core active ingredients with the highest correlation in the'pathway-target-active component'network were Betulin,Ephedrine,Ergotamine and Thymol.Results of molecular docking indicated that the core target had satisfactory affinity with those active ingredients.Animal experiments showed that Banxia Baizhu Tianma Decoction could effectively reduce epileptic seizures in rats,improve hippocampal neuronal damage in rats,significantly reduce the percentage of neuronal apoptosis,significantly down-regulate the expression of pro-apoptotic proteins BAX and Caspase-3,and up-regulate the expression of anti-apoptotic protein BCL-2.Conclusion Banxia Baizhu Tianma Decoction has the characteristics of multi-component,multi-target and multi-pathway in the treatment of epilepsy.Inhibiting neuronal apoptosis and reducing hippocampal neuronal damage may be one of the important mechanisms for its treatment of epilepsy.

Three-dimensional printed patient-specific instrumentation (3D-PSI) provides a precise and individualized treatment solution for unicompartmental knee arthroplasty (UKA). Currently, this technology is being applied in clinical practice and has demonstrated certain potential. Compared to conventional instrumentation (CI), 3D-PSI offers a broader range of indications, higher-quality preoperative planning, shorter surgical time, a smoother learning curve, more precise osteotomy and prosthesis placement, and better postoperative functional recovery. However, it still has limitations in the rotational alignment of the tibial component. Additionally, the higher cost for patients and increased hospital equipment investment make it less beneficial for surgeons already proficient in CI techniques. Further reliable evidence is needed to compare 3D-PSI with computer navigation and robotic technologies. This review summarizes the advantages and limitations of 3D-PSI assisted UKA and compares 3D-PSI with different auxiliary technologies.

5.Characteristics of psychiatric contact consultation in inpatients with the COVID-19 in Wuhan: a retrospective analysis

Objective:This article summarized the characteristics of inpatients with the COVID-19 by psychiatric contact consultation in Wuhan.Method:This was a retrospective study. A total of 105 patients consulted by the Guangdong psychological medical team from March 1, 2020 to March 7, 2020 were selected. Data were collected by 20 psychiatrists responsible for the contact consultation, including age, gender, the main reasons for the contact consultation and the psychiatric treatment plan. The consultant doctor decided whether to enter the isolation ward according to the patient′s condition introduced by the attending physician, and whom were followed up within a week. Descriptive statistical analysis was used to analyze the data.Results:There were 105 patients from 5 designated hospitals, aged (58.3±18.9) years, 62.9% (66/105) were females, and 102 (97.1%) were consulted face-to-face in the isolated ward. Main reasons for consultation were anxiety (65, 61.9%), depression (27, 25.7%), insomnia (6, 5.7%), agitation (6, 5.7%), and illusion (1, 1%). Seventy-seven of those patients (73.3%) received individualized psychotherapy, 1(1.0%) was prescribed with psychiatric medication, and 27 (25.7%) were treated with both measures. Follow-up showed that psychological symptoms relieved in 102 cases (97.1%).Conclusion:Psychosocial symptoms such as anxiety and depression are common in infected patients, and thereby psychiatric contact and consultation is in great demand in the treatment of COVID-19. Early intervention of mental health professionals in the diagnosis and treatment of patients plays a positive role in the recovery of them.

7.Conditioned Medium Enhances Osteogenic Differentiation of Induced Pluripotent Stem Cell-Derived Mesenchymal Stem Cells

BACKGROUND: Recent studies have shown that induced pluripotent stem cells (iPSCs) could be differentiated into mesenchymal stem cells (MSCs) with notable advantages over iPSCs per se. In order to promote the application of iPSC-MSCs for osteoregenerative medicine, the present study aimed to assess the ability of murine iPSC-MSCs to differentiate into osteoblast phenotype. METHODS: Osteogenic differentiation medium, blending mouse osteoblast-conditioned medium (CM) with basic medium (BM) at ratio 3:7, 5:5 and 7:3, were administered to iPSC-MSCs, respectively. After 14 days, differentiation was evaluated by lineage-specific morphology, histological stain, quantitative reverse transcription-polymerase chain reaction and immunostaining. RESULTS: The osteogenesis-related genes, alp, runx2, col1 and ocn expressions suggest that culture medium consisting of CM:BM at the ratio of 3:7 enhanced the osteogenic differentiation more than other concentrations that were tested. In addition, the alkaline phosphatase activity and osteogenic marker Runx2 expression demonstrate that the combination of CM and BM significantly enhanced the osteogenic differentiation of iPSC-MSCs. CONCLUSION: In summary, this study has shown that osteoblast-derived CM can dramatically enhance osteogenic differentiation of iPSC-MSCs toward osteoblasts. Results from this work will contribute to optimize the osteogenic induction conditions of iPSC-MSCs and will assist in the potential application of iPSC-MSCs for bone tissue engineering.

Objective To investigate the correlation between FA value,ADC value and limb muscle strength score measured by magnetic resonance imaging in patients with ischemic stroke,aims to to analyze the clinical value of magnetic resonance imaging in limb muscle strength.Methods Twenty patients with acute cerebral infarction and treated from June 2015 to Junly 2016 were recruited from This hospital,and the simplified Fugl-Meyer motor function score was observed for all patients within 3 days.Tensor imaging examination was conducted to observe the distribution of nerve fiber bundles,FA value,ADC value changes.Results The FA value and ADC value of the infarct side were significantly different from those of the contralateral side(t=8.70,t=-18.70,P<0.05);There were significant differences in FA value and ADC value between the infarcted ventricle hind limbs and the contralateral side of the infarcted ventricle(t=-5.16,t=-5.08,P<0.05).The FA value of the infarcted ventral hind limbs had positive correlation with the simplified Fugl-Meyer motor function score(R=0.863,P=0.013).Conclusion FA value and ADC value of acute infarct and internal hindlimb are lower than FA value and ADC value of contralateral normal white matter.The FA value of internal capsule hind limbs is closely related to the simplified Fugl-Meyer motor function score.
9.Preliminary Research for the Relationship Between Serum Levels of Low Density Lipoprotein Cholesterol and Achilles Tendon Thickness

Objective: To analyze the relationship between the serum levels of low density lipoprotein cholesterol (LDL-C) and achilles tendon thickness (ATT).
Methods: We studied 154 patients with high serum level of LDL-C (LDL-C≥3.37 mmol/L) from 2014-03 to 2015-03, the patients were at (18-75) years of age. According to《Guidelines on Prevention and Treatment of Blood Lipid Abnormality in Chinese Adults 2007》, the patients were divided into 2 groups:Borderline high LDL-C (3.3-4.12 mmol/L) group n=50 and High LDL-C (≥4.14 mmol/L) group, n=104;in addition, there was a Normal control group (LDL-C<3.37 mmol/L), n=51. ATT values were measured by standard digital radiography and the results were compared among 3 groups;the relationship between serum levels of LDL-C and ATT was studied.
Results: ATT levels in High LDL-C group (9.42 ± 3.63) mm was higher than Borderline high group (8.24±1.73) mm and Normal control group (6.05±0.28)mm, all P<0.05. The liner correlation coefifcient of serum level of LDL-C and the mean ATT was (r=0.346, P<0.001).
Conclusion: Our preliminary research showed that the higher serum level of LDL-C associated with thicker ATT, they had positive correlation. This phenomenon should be further conifrmed by large sample and multi-center investigation.
10.Finite element analysis of elderly femoral neck fracture based on LS-DYNA

BACKGROUND:With the increasing of aging, the incidence and mortality of osteoporotic hip fracture wil rise. It is of great significance to study the pathogenesis and preventing method. At present, finite element analysis can be used to judge fracture, only for the distribution trend of fracture failure in the starting point or section view, but it cannot completely reflect actual situation of fracture. OBJECTIVE:To build the fracture model of the femoral neck fracture caused by fal ing-induced external force based on the finite element analysis LS-DYNA software, and to evaluate the effect of rupture. METHODS:CT image data of one case of elderly femoral neck fracture were col ected. Using Mimics software, region growth of the contralateral area, cavity fil ing, editing, rebuilding the contralateral proximal femur model were conducted. Data were imported in Hypermesh and LS-DYNA software for meshing, and defining material properties. The failure parameters and interfacial properties were set. The load and force boundary constraints simulating the fal ing were simulated. The model of femoral neck fracture was calculated. Rupture effect was evaluated. RESULTS AND CONCLUSION:(1) The validity of contralateral proximal femur three-dimensional model was verified. Based on the finite element analysis software LS-DYNA, the femoral neck fracture model matched the actual fracture line to a degree of close to 83%. (2) Above results confirmed that based on the finite element analysis, LS-DYNA software can wel simulate the femoral neck fracture, which provides experimental basis to the exploration of femoral neck fracture classification mechanism caused by different fal ing-induced external forces.
