Objective To observe the impact of low-dose oxycodone-nalbuphine on pain mediator release in patient-controlled intravenous analgesia(PCIA)following laparoscopic radical resection of rectal cancer.Methods 120 patients who underwent laparoscopic radical surgery for colorectal cancer in our hospital from October 2022 to September 2023 were selected and randomly divided into the control group and the experimental group(60 cases in each).PCIA was administered within 48 h postoperatively,formulated as 10 mg hydromorphone and 40 mg nalbuphine diluted in saline to 100 ml.No background dose was administered in the control group,and a background dose of 1 ml/h was administered in the test group.Compare the number of effective PCIA compressions,the rate of remedial analgesia,the rate of remedial antiemesis,the dosage of analgesic pump,Visual Analogue Scale(VAS),Ramsay sedation score,the release of serum pain mediators and the occurrence of postoperative adverse reactions in patients within ≤24 hours and 24-48 hours.Results VAS Scores:The VAS scores during activity in the experimental group at 24 h and 48 h after surgery(2.21±0.25,1.78±0.44)were lower than those in the control group(2.58±0.23,2.12±0.27)(both P＜0.05).The VAS scores at rest in the experimental group at 12 h,24 h,and 48 h after surgery(2.52±0.28,2.14±0.26,1.75±0.32)were lower than those in the control group(3.17±0.39,2.68±0.33,2.04±0.21)(P＜0.05).Pump Usage:The amounts of analgesic pump used in the experimental group at 24 h and 48 h after surgery[(27.64±0.28)ml,(49.11±0.25)ml]were higher than those in the control group[(12.06±0.33)ml,(20.15±0.36)ml)](both P＜0.001).PCIA Pressing Times:The effective pressing times of PCIA in the experimental group within ≤24 h and 24-48 h(5.89±0.31,4.73±0.28)were lower than those in the control group(7.28±0.38,6.21±0.37)(both P＜0.001).Rescue Analgesia and Antiemesis Rates:The rescue analgesia rate(11.67％vs 28.33％,P=0.023)and rescue antiemesis rate(5.00％vs 18.33％,P=0.023)in the experimental group were both lower than those in the control group.Ramsay Sedation Scores:The Ramsay sedation scores in the experimental group at 12 h,24 h,and 48 h after surgery(2.18±0.31,1.74±0.42,1.46±0.14)were lower than those in the control group(2.43±0.48,2.12±0.32,2.03±0.38)(P＜0.001).In the experimental group,substance P[(35.26±5.27)pg/ml,(26.37±4.17)pg/ml],neuropeptide Y[(147.28±27.43)pg/ml,(127.26±18.49)pg/ml],and prostaglandin E2[(56.48±3.69)at 24 hours and 48 hours after the operation 9)The levels of pg/ml,(42.47±5.35)pg/ml and norepinephrine[(3.12±0.48)ng/ml,(1.57±0.19)ng/ml]were all lower than those of the control group[(41.27±6.48)pg/ml,(34.37±4.86)pg/ml,(35.26±5.27 pg/ml,(26.37±4.17)pg/ml,(62.47±4.26)pg/ml,(53.95±4.73)pg/ml,(3.64±0.64)ng/ml,(2.16±0.26)ng/ml,(P＜0.001)].Conclusion Low-dose oxycodone-nalbuphine in patient-controlled intravenous analgesia following laparoscopic radical resection of rectal cancer can improve analgesic effects and reduce serum pain mediator release.

Objective To observe the value of diffusion weighted imaging intravoxel incoherent motion(IVIM)for quantitative evaluating chronic allograft dysfunction(CAD).Methods Totally 104 CAD patients were prospectively enrolled and were assigned into CAD 1,2 and 3 groups(n=11,61,32)based on impairment severity of estimated renal function,and 36 healthy volunteers were enrolled as control group.The true diffusion coefficient(D value),microcirculation perfusion diffusion coefficient(D*value)and perfusion score(f value)of renal cortex and medulla IVIM parameters were compared among groups and within groups to assess the value of IVIM parameters for diagnosing CAD.Results The D value of transplanted renal cortex in all CAD groups were lower than that in control group(all P＜0.05),which decreased among CAD 1,2 and 3 groups(all P＜0.05).The D value of transplanted kidney medulla in CAD 2 and 3 groups were lower than that in control group(both P＜0.05).The D*values of transplanted renal cortex in all CAD groups were lower than that in control group,while of renal medulla in CAD 2 and 3 groups were lower than that in control group(both P＜0.05).The f values of cortex and medulla in CAD 2 and 3 groups were lower than those in control group(all P＜0.05),while of cortex in CAD 3 group was lower than that in CAD 1 and 2 groups(both P＜0.05).The area under the curve(AUC)of cortical IVIM combined model for diagnosing CAD was 0.96,better than the D*value and f value(AUC=0.74,0.83,P＜0.05)but not significantly different with that of the D value(AUC=0.94,P=0.32).AUC of medullary IVIM combined model for diagnosing CAD was 0.91,better than that of D,D*and f value(AUC=0.80,0.67 and 0.80,all P＜0.05).Conclusion IVIM parameters could be used to quantitatively evaluate CAD.

AIM: To explore the antibacterial activity and underlying mechanism of ursolic acid combined with fusidic acid against Staphylococcus aureus (SA) ATCC29213 and Methicillin-Resistant Staphylococcus aureus (MRSA) ATCC43300 in vitro. METHODS: The minimum inhibitory concentration (MIC) of the combined use of ursolic acid and fusidic acid on SA, MRSA was determined by the micro broth dilution method and the micro checkerboard method, and the partial inhibitory concentration index (FICI) was calculated to determine the combined effect. And the bactericidal effect of fusidic acid combined with ursolic acid was studied by the time-killing curves. The agar double dilution method was used to determine the anti-drug resistance mutation concentration (MPC) and anti-drug resistance mutation selection window (MSW) of fusidic acid alone and in combination with ursolic acid. The viable count of biofilm carrier were determined by serial dilution method and the semi-quantitative biofilm by crystal violet staining method. RESULTS: The combined use of ursolic acid and fusidic acid for SA and MRSA FICI were 0.312 5 and 0.375, respectively. The time-killing curve showed that 1MIC ursolic acid combined with 1MIC fusidic acid has a synergistic bactericidal effect on SA and MRSA. The MPC of fusidic acid to MRSA was 256 μg/mL and the MSW was 256. After fusidic acid combined with ursolic acid, the MPC decreased to 8 μg/mL. The combined group was significantly reduced compared to the fusidic acid group. The semi-quantitative and biofilm bacterial counts of combined group were markedly decreased compared to the fusidic acid group after the biofilm cultivate for 48 h and 72 h.CONCLUSION: The combined use of UA and FA has a synergistic effect on SA and MRSA.

Objective To investigate the effect of SP600125 on the proliferation, cell cycle, apoptosis and invasion of human cervical cancer HeLa cells. Methods CCK-8 method was used to detect the proliferation of HeLa cells treated with different concentrations of SP600125 at different time points. The 20 μmol/L of SP600125 was determined for subsequent experiments. Cell proliferation ability was detected using plate clone formation assay; nuclear morphology was observed by DAPI staining; cell cycle and apoptosis were measured by flow cytometry; cell migration and invasion were detected by cell scratch and Transwell methods; the mRNA and protein levels of p53, Mad2L1 and CDC20 were measured by qRT-PCR and Western blot after SP600125 treatment at different time points. Results Compared with control group (0.1%DMSO), cells proliferative activity were reduced by 10, 20, 30, 40 and 50 μmol/L SP600125 treatment for 24h. Compared with control group, the rate of apoptosis was significantly increased in SP600125 treatment groups, and the cell proportion in G₂/M phase increased (P < 0.001), while the cell proportion in G₀/G₁ phases cells was reduced after SP600125 treatment for 24h and 48h (P < 0.001), and the clonal number, migration and invasion ability of HeLa cells also decreased significantly (P < 0.001). qRT-PCR and Western blot results showed a significant decrease in Mad2L1 mRNA and protein expression (P < 0.05) and a significant increase in p53 and CDC20 mRNA and protein expression (P < 0.01). Conclusion SP600125 can induce cell cycle arrest of cervical cancer HeLa cells in G₂/M phase by upregulating p53 and CDC20 and downregulating Mad2L1 expression, and promote cell apoptosis to inhibit cell proliferation, migration and invasion.

Stroke is considered a leading cause of mortality and neurological disability, which puts a huge burden on individuals and the community. To date, effective therapy for stroke has been limited by its complex pathological mechanisms. Autophagy refers to an intracellular degrading process with the involvement of lysosomes. Autophagy plays a critical role in maintaining the homeostasis and survival of cells by eliminating damaged or non-essential cellular constituents. Increasing evidence support that autophagy protects neuronal cells from ischemic injury. However, under certain circumstances, autophagy activation induces cell death and aggravates ischemic brain injury. Diverse naturally derived compounds have been found to modulate autophagy and exert neuroprotection against stroke. In the present work, we have reviewed recent advances in naturally derived compounds that regulate autophagy and discussed their potential application in stroke treatment.

OBJECTIVE: To observe the effect of the interaction teaching mode integrated with Visible Body virtual anatomy platform in teaching . METHODS: A total of 60 students in the class of 2017 in the discipline of acupuncture-moxibustion and , Xiangnan University were recruited and randomized into an observation group and a control group, 30 students in each one. In the control group, the traditional practical teaching mode was used. In the observation group, the interaction teaching mode integrated with virtual anatomy platform was adopted. The teaching duration was 10 class hours in both groups. After accomplishing the teaching schedule, the practical examination was conducted in the localization of commonly-used acupoints, very useful acupoints and the dangerous acupoints as well as acupuncture manipulation techniques. Moreover, the degree of satisfaction was investigated among the students in the two groups and the self-learning ability was evaluated in 3-month follow-up visit. RESULTS: In the observation group, the scores for the localization and acupuncture manipulation of commonly-used acupoints, very useful acupoints and the dangerous acupoints, as well as the degree of satisfaction of the 3 items, i.e. interesting, interaction and leaning-assistance were all higher than those in the control group (<0.05). The degree of satisfaction in the acceptance and leaning-participation, as well as the scores of self-learning ability in 3-month follow-up visit were not different statistically between the two groups (>0.05). CONCLUSION The interaction teaching mode integrated with virtual anatomy platform improves the effect on teaching and achieves the high student satisfaction.

AIM: To observe the anti-inflammatory effect of CQMUH-011 and to explore its mechanism.METHODS: Three kinds of animal models, mouse ear swelling induced by xylene, rat granuloma induced by cotton ball and rat rheumatoid arthritis induced by Freund's complete adjuvant, were established to study the anti-inflammatory effect of CQMUH-011.The ear swelling degree, dry weight of cotton ball granuloma, arthritis index, paw swelling and ankle joint pathological changes were measured to reflect the severity of inflammation.The anti-inflammatory mechanisms of CQMUH-011 were investigated by detecting the serum levels of tumor necrosis factor-α (TNF-α) and interleukin-6 (IL-6) by ELISA.Malondialdehyde (MDA), myeloperoxidase (MPO) and nitric oxide (NO) were determined by corresponding kits.RESULTS: Treatment with CQMUH-011 significantly decreased TNF-α, IL-6 and NO concentrations, MDA contents and MPO activity in the serum.Meanwhile, Ear swelling degree, dry weight of cotton ball granuloma, arthritis index, paw swelling and ankle joint pathological damage were attenuated.CONCLUSION: CQMUH-011 has an anti-inflammatory effect, which may be related to inhibiting the production of inflammatory factors and attenuating lipid peroxidation.

Objective To study the molecular epidemiology of C.albicans isolates in infectious disease patients and to explore biofilm phenotypic characterization responsible for biofilm formation in clinical strains.Methods A total of 104 hospital-acquired C.alibcans clinical isolates collected from sterile sites and mucosal lesions of 92 infectious disease patients ( viral hepatitis, tuberculosis and AIDS) in Shanghai Public Health Clinical Center were analyzed.MLST analysis was performed to identify their phylogenetic status.The capability of biofilm formation was measured by [2,3-bis-(2-methoxy-4-nitro-5-sulphenyl)-2H-tetrazolium-5-carboxanilide] XTT assay.The results were compared using Kruskal-Wallis test.Results MLST analysis identified 63 DSTs with a decentralized phylogeny among 104 C.albicans isolates, of which 41 DSTs (65.1%) had not been reported in the online MLST database.The Single Locus Sequence Query from the C.albicans database identified new alleles.MEGA6 analysis of the MLST data assigned the 104 isolates within 14 of the 18 known clades; among them the clade 1 contained the greatest proportion of isolates (26.9%).Of the 43 novel DSTs isolates, 37 ( 86.0%) clustered within 11 of the 18 known clades.16 high biofilm formers were found from a total of 104 clinical isolates.The biofilm formation capabilities differed in strains isolated from different anatomical sites (H =18.23,P=0.0326).Biofilm formation by blood-originated isolates was lower than that of catheter-originated isolates ( Z=-72.20,P<0.001).Genotypes also affected the biofilm formation capability of the C.albicans isolates (H=10.01,P=0.0185).Conclusions A high level of diversity within C.albicans isolates.Microevlution clearly influences C.albicans genetic alterations upon environmental selection.The site of isolation and genotype associates with the biofilm formation capability.
