1.Consistency of MSCT 3D processing technique and QCT in measuring BMD for lumbar vertebra

Objective:To study the consistency between post-processing bone mineral density(BMD)values of multi-slice spiral computed tomography(MSCT)scan and the BMD value of quantitative computed tomography(QCT)for lumbar vertebra,so as to explore the feasibility of utilizing MSCT scan-based post-processing BMD values for lumbar vertebra in clinical practice.Methods:The MSCT equipment and QCT equipment were respectively adopted to conduct imaging scan for the L2-L4 of lumber vertebra of QRM-ESP145 European Spine Phantom(ESP),and L2-L4 of lumbar vertebra of adult sheep,and L2-L4 of lumbar vertebra of adult volunteer.The L2-L4 of ESP lumber vertebra and L2-L4 of lumbar vertebra of adult sheep were scanned respectively MSCT and QCT for three times,so as to measure BMD values.The L2-L4 of lumbar vertebrae of volunteers were scanned respectively by the two methods for one time according to the standard of clinical examination,which were reconstructed by three times so as to obtain mean of them.The BMD values of QCT scan were set as control group,and the BMD values of MSCT scan were set as experiment group.The experiment group was further divided into experiment 1 group[two dimension(2D)regional volumetric BMD values of the lumbar vertebra]and experiment 2 group[three dimension(3D)global volumetric BMD post-processing of the lumbar vertebra]according to the reliability of experiment.Then,the consistency between the MSCT 3D post-processing BMD values of three groups and QCT-measured BMD values was compared and analyzed.Results:The MSCT 3D post-processing BMD values of L2-L4 of ESP lumbar vertebra of three groups were respectively(120.83±0.97),(199.57±0.54)and(119.19±1.04)mg/cm3,and that of L2-L4 of lumbar vertebra of adult sheep of three groups were respectively(414.89±1.72),(410.50±0.77)and(420.25±2.71)mg/cm3,and that of L2-L4 of lumbar vertebra of volunteer were respectively(141.22±0.09),(137.38±0.37)and(152.03±1.03)mg/cm3.There were not statistically significant differences in BMD values between MSCT examination and QCT examination(P>0.05).Conclusion:MSCT 3D post-processing BMD values on lumbar vertebra has high consistency with that of QCT measurements,which post-processing technique can replace QCT to conduct BMD examination,and reduce unnecessary radiation exposure and examination costs for patients.

3.A study on the impact of different scanning modes of MSCT on image quality and radiation dose in coronary CTA examinations

Objective:To investigate the relationship between image quality and radiation dose in axial scan and spiral scan of coronary computed tomography angiography (CCTA). Methods:A total of 60 patients who underwent CCTA in Beijing Friendship Hospital of Capital Medical University from July 2021 to January 2022 were selected,and they were divided into an axial scan group and a spiral scan group,with 30 cases in each group. The image quality,radiation dose,average heart rate during the examination,diagnostic value for moderate and severe coronary stenosis (degree≥50%) and patients' satisfaction of two kinds of modes were analyzed. Results:The excellent rates of image quality of axial scan group and spiral scan group were respectively 96.67% (29/30) and 93.33% (28/30),without significant differences (P>0.05). The dose-length product (DLP),computed tomography dose index of volume (CTDIvol) and effective dose (ED) in the axial scan group were significantly lower than those in the spiral scan group (t=180.637,30.945,368.414,P<0.05),respectively. There were no statistically significant differences in the average heart rate,DLP and ED under different heart rate between the two groups during the examination process (P>0.05). The examination of digital subtraction angiography was used as "gold standard",and the diagnostic sensitivity,specificity,accuracy,positively predictive value and negatively predictive value in the axial scan group were significantly higher than those in the spiral scan group (x2=11.140,12.830,13.340,9.350,14.450,P<0.05),respectively. The satisfaction degree of service effect of patients in axial scan group was 93.33% (28/30) for examination mode of CCTA imaging,which was significantly higher than that of spiral scan group,and the difference was significant(x2=5.455,P<0.05). Conclusion:The objective assessment,diagnostic value and patient's satisfaction of image quality of axial scan of CCTA are higher than those of spiral scan mode,which radiation dose is lower than that of spiral scan mode. CCTA should choose reasonable scan mode according to heart rate and degree of coronary artery stenosis,and other,actual condition of patients.

5.Comparison of conservative and surgical treatment of testicular appendage torsion in children

Objective:To investigate the clinical efficacy of conservative and surgical treatment of testicular appendage torsion in children and to summarize the experience.Methods:The clinical data of 106 children with testicular appendage torsion admitted in Tengzhou Central People's Hospital Affiliated to Xuzhou Medical University from January 2012 to December 2018 were retrospectively analyzed. According to different therapeutic methods, all the children were divided into conservative group (25 cases) and surgical group (81 cases). The clinical efficacy was compared between the two groups.Results:106 children with testicular appendage torsion were cured. The hospitalization time [(5.61±1.04)d vs (11.32±1.89)d], pain relief time [(9.11±2.56)d vs (68.5±19.87)d], disappearance time of scrotal edema [(5.11±1.08)d vs (10.56±1.50)d] and the utility time of antibiotic [(4.89±1.32)d vs (7.06±1.26)d] in the surgical group were shorter than those in the conservative group, with statistically significant difference(all P<0.05). After 6 months of follow-up, there were no cases of recurrence and discomfort in the two groups. Conclusions:Children with testicular appendage torsion should be actively treated with surgery, which can not only significantly shorten the course of disease, but also reduce the incidence of complications and missed diagnosis. For children who with testicular appendage torsion, surgeries should be performed actively. It can significantly shorten the duration of symptoms, but also reduce the occurrence of complication and the occurrence probability of misdiagnosis.
6.Rotating magnetic fields and granisetron treatment for preventing nausea and vomiting induced by chemo-therapy

Objective To observe the efficacy of a rotating magnetic field and granisetron hydrochloride in preventing nausea and vomiting caused by a eisplatin regimen, and any side effects. Methods Sixty-eight patients receiving cisplatin regimen chemotherapy were randomly assigned to two groups: a magnetic treatment group and a drug treatment group. The patients in the two groups were exposed to a rotating magnetic field or received granisetron hydrochloride, respectively. The effects of the treatments were observed. Results Both treatments could effectively prevent and treat the vomiting caused by chemotherapy. The rate of response to the rotating magnetic field was 88.2% and to the drug 91.2%. However, tardive vomiting was significantly better controlled in the rotating magnetic field group. The incidence of side effects in the magnetic field group was 20.6% , and in the drug treatment group it was 45.6%. Conclusion The efficacy of a rotating magnetic field and granisetron in treating acute vomiting were simi- lar. The rotating magnetic field was more effective in preventing tardive vomiting and had fewer side effects. Magnetic therapy should be more generally applied in clinical practice.
7.Tumor antigen-pulsed dendritic cell vaccine for treatment of patients with advanced malignant tumor:a clinical observation

Objective: To observe the safety and clinical efficacy of tumor antigen-pulsed dendritic cell(DC) vaccine in treatment of advanced malignant tumor.Methods: Ninety-one patients with non-small cell lung cancer,colon and rectal cancer,melanoma,renal carcinoma,breast cancer and other malignant tumors were enrolled in this study.All patients met the selecting standard and signed informed consent.Human dendritic cells were obtained from peripheral blood monocytes by culturing them with granulocyte macrophage-colony stimulating factor and interleukin-4.DC vaccine was prepared from tumor antigen pulsed immature dendritic cells in vitro.Patients received the vaccine therapy once every week and one cycle was defined as once every week for 3 weeks.Results: All the patients received 96 cycles of DC vaccine treatment.Symptoms of toxicity included fever,shivering,aching pain of muscle,asthenia,itching,stifle and transient fatigue;most of the symptoms automatically recovered.Clinical efficacy of the treatment was evaluated in 76 patients.Thirty-one of the 76 patients were stable after treatment and 45 were in progressive situation,with the clinical benefiting rate being 40.8%.Eighty-five patients were followed up.The median time for progression was 2.6 months;the overall survival time was 0.9-30.6 months;and the median survival period was 4.5 months,with the one year survival rate being 9.2%.Conclusion: The results suggest that the DC vaccine therapy is well tolerated in treating patients with advanced malignant tumors and has satisfactory clinical benefit;the clinical value of DC vaccine therapy needs to be further observed.
8.Relationship of Genomic Characteristics,Virulence of Sporothrix Sche nckii and Clinical Patterns of Sporotrichosis

Objective To explore the relationship of genomic characte ristics,virulence of S.schenckii and clinical patterns of sporotrichosis.Met hods Isolates of S.schenckii were collected from different clinical patterns of sporotrichosis.Genomic DNAs of S.schenckii isolates were amplified by rando mly amplified polymorphic DNA (RAPD).Suspensions of different isolates of S.sc henckii were inoculated into BALB/c mice in order to observe their pathogenicity.Histopathology of the skin and viscera of the mice was studied to determine th e numbers and distribution of spores.Results The electrophoretic profiles of PCR products differed in the isolates from different clinical patterns of sporo trichosis:1 800 bp,850 bp,500 bp,and 180 bp in disseminated sporotrichosis;1 400 bp,800 bp,700bp,and 500 bp in the regional lymphangitic sporotrichosis,2 500 bp,1400 bp,1000 bp,and 700 bp in fixed cutaneous sporotrichosis.The earl ier onset of illness,wider distribution of lesions and higher mortality rate we re observed in the BALB/c mice inoculated with isolates from disseminated sporot richosis,followed by those from regional lymphangitic sporotrichosis and those from fixed cutaneous sporotrichosis.Histopathologically more spores were found in the mice inoculated by isolates from disseminated sporotrichosis,followed by those from regional lymphangitic sporotrichosis and those from fixed cutaneous sporotrichosis.Conclusions There is a strong correlation among genomic charac teristics,virulence of S.schenckii and clinical patterns of sporotrichosis.
9.Molecular Determination of Dermato phyte Fungi Using Arbitrarily Prime d Polymerase Chain Reaction

Objective To study the significance of AP-PCR i n identification and subtyping of der-matophytes.Methods Using a pair of random primers,OPAA11(5' -ACCCGACCTG -3' ),and OPD18(5' -GAGAGCCAAC -3' )the DNAs of 64isolates of dermatophytes(9species of 3genera),Sporothrix schenckii and Candida albicans were amplified by AP-PCR and analyse d by electrophoresis.Results Distinct DNA band patterns were observed in diffe rent dermatophyte species.Common major DNA bands were observed in Trichophyton rubrum isolated from different areas with s train difference.Conclusion Using OPAA11and OPD18as primers,AP-PCR may be applied in the identification and subtypi ng of dermatophytes.
