Inflammation is the body’s response to damage, infection or other stimuli, but its excessive or continuous development can lead to a variety of diseases. Although modern medical anti-inflammatory therapies were widely used, it is often accompanied by limitations such as more adverse reactions. Based on the “holistic view” and “differential treatment”, traditional Chinese medicine (TCM) regards inflammation as a manifestation of the imbalance of yin and yang in the body and the conflict between good and evil. The application of anti-inflammatory TCM is not only aimed at the pathological state of “inflammation”, but also based on the overall consideration of “syndrome”. According to different types of syndrome, anti-inflammatory TCM can be divided into heat-clearing and detoxifying agents (such as Lonicera japonica and Isatis indigotica ), heat-clearing and drying dampness agents (such as Coptidis Rhizoma), blood-activating and stasis-dissolving agents (such as Salvia miltiorrhiza ) and vital qi-strengthening and pathogenic factor-expelling agents (such as Panax ginseng ). Four types of anti-inflammatory TCM restore the body’s immune balance through the systematic regulation of multi-component, multi-target and multi-pathway, exhibiting a good anti-inflammatory effect. Future research should focus on integrating systematic biology, applying artificial intelligence, carrying out high-quality evidence-based research, and combining traditional Chinese medicine and Western medicine, so as to reveal the overall regulatory law of anti-inf lammatory effects of TCM and promote clinical rational use.

Microglia, the resident immune cells in the central nervous system (CNS), rapidly transition from a resting to an active state in the acute phase of ischemic brain injury. This active state mediates a pro-inflammatory response that can exacerbate the injury. Targeting the pro-inflammatory response of microglia in the semi-dark band during this acute phase may effectively reduce brain injury. Shionone (SH), an active ingredient extracted from the dried roots and rhizomes of the genus Aster (Asteraceae), has been reported to regulate the inflammatory response of macrophages in sepsis-induced acute lung injury. However, its function in post-stroke neuroinflammation, particularly microglia-mediated neuroinflammation, remains uninvestigated. This study found that SH significantly inhibited lipopolysaccharide (LPS)-induced elevation of inflammatory cytokines, including interleukin-1β (IL-1β), tumor necrosis factor-α (TNF-α), and inducible nitric oxide synthase (iNOS), in microglia in vitro. Furthermore, the results demonstrated that SH alleviated infarct volume and improved behavioral performance in middle cerebral artery occlusion (MCAO) mice, which may be attributed to the inhibition of the microglial inflammatory response induced by SH treatment. Mechanistically, SH potently inhibited the phosphorylation of serine-threonine protein kinase B (AKT), mammalian target of rapamycin (mTOR), and signal transducer and activator of transcription 3 (STAT3). These findings suggest that SH may be a potential therapeutic agent for relieving ischemic stroke (IS) by alleviating microglia-associated neuroinflammation.

Aim To explore the mechanism of action of Guizhi Jia Gegen decoction(GGD)in treating pneu-monia-enteritis induced by H1N1 influenza virus infec-tion in a mouse model, using network pharmacology and molecular docking techniques, followed by in vivo verification. Methods A pneumonia-enteritis mouse model was established, and the intervention effects of GGD on the model mice were evaluated using indica-tors such as body weight, rectal temperature, lung in-dex,colon length,H1N1 M gene expression, relative mRNA expression levels of inflammatory cytokines, and pathological sections of the lung and intestine. The targets of the blood-absorbed components of GGD were identified using the Swiss Target Prediction platform, and the disease targets were retrieved from the Gene-Cards platform. The intersecting targets were analyzed through PPI network analysis using the STRING data-base to identify core targets.GO analysis and KEGG pathway enrichment analysis were performed using the Metascape database.RT-qPCR was employed to vali-date the core targets and pathways.Molecular docking was conducted using AutoDock Tools software to verify the interactions between blood-absorbed components and key targets. Results GGD demonstrated signifi-cant therapeutic effects on the pneumonia-enteritis mouse model. The results of network pharmacology in-dicated that the therapeutic effects of GGD were strong-ly associated with targets such as TNF,ALB,PTGS2,MMP9,EGFR,ESR1,SRC,HSP90AA1,PPARG and MMP2.RT-qPCR results indicated that GGD could intervene in pneumonia-enteritis by regulating the targets TNF,ALB,EGFR and the related targets of the NF-κB pathway.Molecular docking results re-vealed that blood-absorbed components such as puerar-in and liquiritin could stably bind to TNF,ALB and EGFR.Conclusion Components such as puerarin and liquiritin in GGD may exert therapeutic effects on pneumonia-enteritis induced by H1N1 influenza virus infection by acting on targets such as TNF,ALB and EGFR.

Objective To identify potential subtypes of demoralization syndrome among stroke patients undergoing neuroendovascular procedures using latent profile analysis, and to examine differences in death anxiety among these subtypes.Method This cross-sectional study used convenience sampling to recruit 202 stroke patients undergoing neuroendovascular procedures at a hospital from November 2024 to March 2025.Participants completed the General Information Questionnaire,Despair Scale(DS),and the Chinese version of the Templer's Death Anxiety Scale(CT-DAS).Using R software,we estimated a series of latent profile models(ranging from 2 to 6 profiles)based on four manifestations of demoralization syndrome:loss of meaning and purpose, dysphoria, helplessness and hopelessness, and sense of failure.Starting from the 2-profile model,we incrementally increased the number of profiles and compared model fit indices to identify the optimal solution.Subsequently, logistic regression analysis was conducted to identify the factors influencing DS scores according to the latent profile classifications. We compared DS scores across subgroups and performed bivariate correlation analysis between DS and CT-DAS subdimensions(Affective,Stress&Pain,Time Awareness, and Cognitive). Results The DS scores classified stroke patients into two distinct subtypes of demoralization syndrome:A Meaninglessness-Dysphoria Group(51.00%,103/202)and a Helplessness-Hopelessness Group(49.00%,99/202).The HHG demonstrated significantly higher proportions(P<0.05)of female patients, those hospitalized for 6~10 days, recipients of arterial stenosis/occlusion surgery, individuals with junior high school or higher education, employed subjects, and rural residents compared to the MDG.Binary logistic regression analysis revealed:Lower probability of HHG membership for urban(OR=0.159,P<0.001)and city(OR=0.224,P=0.007)residents versus rural counterparts;Higher HHG probability in patients hospitalized 6-10 days versus 1~5 days(OR=2.311,P=0.017);Elevated HHG risk among junior high school(OR=4.956,P<0.001)and high school or above graduates(OR=5.102,P=0.001)relative to those with primary education or less;Increased HHG probability in aneurysm embolization(OR=2.419,P=0.040)and arterial stenosis/occlusion surgery(OR=2.733,P=0.014)recipients compared to cerebrovascular angiography patients. The HHG scored significantly lower on the cognitive dimension of death anxiety than the MDG(t=2.421,P=0.016),while showing no differences in affective, stress&pain,or time awareness dimensions(P>0.05).Bivariate Pearson correlations indicated dysphoria(DS)positively correlated with Affective(r=0.192,P=0.006),Time Awareness(r=0.172,P=0.015),and Cognitive(r=0.139,P=0.049)of death anxiety. Conclusions Stroke patients undergoing neuroendovascular procedures exhibit elevated levels of death anxiety, with significant varia-tions observed across different subtypes of demoralization syndrome.Residential location, hospitalization duration, anxiety severity, and surgical type constitute significant determinants of demoralization syndrome.Targeted clinical interventions should be implemented to alleviate demoralization severity and mitigate death anxiety.

Hilar cholangiocarcinoma is insidious in onset and often causes obstructive jaundice due to bile stasis, leading to impaired liver function. For tumors with malignant obstructive jaundice, biliary drainage is often performed before surgery in clinical practice.Currently, the commonly used drainage methods are percutaneous transhepatic biliary drainage(PTBD)and endoscopic retrograde biliary drain-age(ERBD),but there are controversies over the advantages and disadvantages of the two drainage methods.PTBD drainage can often lead to tumor implantation metastasis, but the underlying mechanism remains unclear. We detected tumor cells in PTBD and ERBD bile samples from hilar cholangiocarcino-ma patients, subsequently explored their tumorigenicity and mechanisms through tumorsphere assay in vitro and xenograft tumor models in vivo. The experiments included benign gallstones group(30 cases)as a negative control,PTBD group(14 cases)and ERBD group(13 cases).Tumorsphere formation was i-dentified in 3 cases(23%)among the 13 cases of ERBD group,in 6 cases(42%)among the 14 cases of PTBD group, but there were no tumor cells or formed tumorspheres in the 30 cases of benign gallstone group. The tumor sphere formation ability of cells in the PTBD group was significantly higher than that in ERBD group.Subcutaneous xenograft tumor assays showed that tumor growth in the PTBD group was sig-nificantly higher than that in the ERBD group.Tumor cells in PTBD bile possessed stronger tumorigenici-ty compared with the ERBD group.Mechanically, stem cell transcription factor Nanog mRNA levels were significantly higher in the PTBD group compared to the ERBD group. Both tumorsphere formation and xenograft tumor growth were reduced by Nanog knockdown in three cases of the PTBD group, indicating the important roles of Nanog in tumorigenicity of PTBD group tumor cells. The half-life of Nanog mRNA was longer in PTBD group cells than in ERBD group cells, suggesting potential post-transcriptional regu-lation on Nanog mRNA.The Nanog m6A level was higher in PTBD group tumor cells compared to the ERBD group.Analysis of methyltransferases and demethylases,ALKBH5(α-ketoglutarate dependent dioxygenase alkb family homolog 5)mRNA levels were lower in the PTBD group than in the ERBD group and significantly correlated with the m6A level of Nanog.ALKBH5 knockdown led to an increase in the m6A level of Nanog, while ALKBH5 overexpression decreased the m6A level of Nanog.Dual-luciferase activity assays demonstrated that ALKBH5 knockdown significantly enhanced luciferase activity, whereas ALKBH5 overexpression reduced it.Further studies confirmed that ALKBH5 knockdown upregulated both the mRNA and protein levels of Nanog, whereas overexpressing ALKBH5 downregulated them.ALKBH5 mediated the demethylation modification of Nanog mRNA,and the lower levels of ALKBH5 expression in the PTBD group promoted Nanog's m6A modification.Overexpressing ALKBH5 decreased tumorsphere growth, while ALKBH5 knockdown increased it,which was subsequently reduced by the simultaneous Nanog knockdown again. In sum, tumor cells in PTBD and ERBD drainage bile from hilar cholangiocar-cinoma patients exhibited tumorigenicity.Compared to the ERBD group, tumor cells in PTBD bile with lower ALKBH5 expression levels enhanced Nanog's m6A modification to upregulate Nanog expression levels, resulting in stronger tumorigenicity. These findings are significant for elucidating propensity to tumor implantation metastasis from PTBD drainage.
