The prevention and treatment of chronic obstructive pulmonary disease (COPD) pose a major global challenge, with difficulties in effectively integrating holistic regulation and precise micro-intervention. The traditional Chinese medicine (TCM) theory of state-target pattern differentiation and treatment advocates the combination of state regulation and target intervention, offering a new paradigm for integrated TCM and Western medicine. This article systematically reviews the theoretical connotations of this theory and constructs a dynamic correspondence framework linking TCM pattern states, signaling pathways, and targeted interventions across different stages of COPD. At the acute exacerbation stage, the phlegm-heat obstructing the lung state is characterized by hyperactivation of the nuclear factor (NF)-κB/NOD-like receptor protein 3 (NLRP3) inflammation pathway, and formulas for clearing heat and resolving phlegm suppress the cytokine storm through multi-component synergy. At the stable stage, the lung-spleen Qi deficiency state is characterized by functional decompensation of the nuclear factor erythroid 2-related factor 2 (Nrf2)/Kelch-like ECH-associated protein 1 (Keap1) antioxidative stress pathway, and therapies for tonifying the lung and spleen repair the essence of Qi deficiency through antioxidative defense and energy metabolism. At the severe/very severe stable stage, the lung-kidney deficiency state is pathologically based on dysregulation of the transforming growth factor (TGF)-β/Wnt repair network and depletion of stem/progenitor cells, and therapies for tonifying the kidney and reinforcing Qi absorption achieve simultaneous regulation of deficiency and stasis by inhibiting aberrant fibrosis and guiding ordered regeneration. These three pathways are not isolated but exhibit dynamic shifts in dominance and cross-talk, constituting the molecular network basis of deficiency causing excess and intermingled deficiency and excess. Furthermore, this article points out the current shortcomings in quantitative evidence, objective syndrome standardization, and coverage of syndrome states, and proposes future directions including building multi-omics databases, conducting biomarker-stratified randomized controlled trials, and developing disease-pattern combined animal models, aiming to provide a theoretical perspective that integrates holism and micro-targeting for precision prevention and treatment of COPD.

Objective To analyze the death rate and life loss of injuries in Jinshan District of Shanghai from 1990 to 2023, and to provide a scientific basis for the formulation of injury prevention and control measures. Methods Excel 2010 was used to establish a database for injury death reports in Jinshan District from 1990 to 2023, and the crude mortality rate, standardized mortality rate, age-specific mortality rate, potential years of life lost (PYLL), rate of potential years of life lost (PYLLR) and standardized rate of potential years of life lost (SPYLLR) were calculated. Results From 1990 to 2023, the crude mortality rate due to injuries in Jinshan District was 52.25/100 000, while the standardized mortality rate was 36.95/100 000. Based on the standardized mortality rate, the top four causes of injury-related deaths were traffic accidents, falls, drowning, and suicide. Traffic accidents ranked first as the leading cause of death for both males (19.30/100 000) and females (8.41/100 000). The top cause of death for the 0-14 years old group was drowning, the 15-64 years old group was traffic accidents, and the 65 years old and above was falls. Over the 34-year period, according to SPYLLR, the top four causes of death for both genders were consistently traffic accidents, drowning, suicide, and falls. Conclusion From 1990 to 2023, traffic accidents, falls, drowning, and suicide have been the primary causes of injury-related deaths in Jinshan District, Shanghai. It is essential to develop targeted prevention and control strategies based on the specific characteristics of these injury-related fatalities.

Objective: To establish a database of CD36 antigen-negative donors through large-scale screening of apheresis platelet donors in Shenzhen for CD36 deficiency subtypes and blood group distribution, and to assess clinical demand and blood supply capacity through a retrospective analysis of the apheresis platelet donation volumes from 2019 to 2023. Methods: Flow cytometry with fluorescent CD36 monoclonal antibodies was employed to screen platelet/monocyte CD36 deficiency (Type I and Ⅱ), and statistical analyses were conducted using SPSS software (version 27.0). Results: Among 11 603 apheresis platelet donors, 248 (2.14%) exhibited CD36 deficiency, comprising 51 type Ⅰ (0.43%, 51/11, 603) and 197 type Ⅱ (1.70%, 197/11, 603) cases, with significant difference (P<0.001). CD36 deficient platelets were mainly distributed in blood group B (2.28%, 902.3/39 602.1) and AB (2.14, 269/12 544.5), significantly exceeding those in blood group A (1.43%, 667/46 508.4) and O (1.64%, 1 000/60 965.6) (P<0.001). The proportion of donors with 10-100 U from CD36 deficient donors was the highest (51%, 1 446.4/2 838.3). Conclusion: Sustained screening for CD36-deficient donors is recommended to meet the clinical transfusion needs for immunized patients and those requiring antigen-negative products. Regional resource-sharing mechanisms should be optimized to maximize utilization of CD36-deficient platelet inventories.

Failure in embryo implantation is a common issue in assisted reproduction technology, and establishment of endometrial receptivity plays a critical role in the process of embryo implantation. Although it was extensively studied in the endometrial physiology, the molecular mechanism underlying regulation of endometrial receptivity remains elusive. Ample clinical evidence suggests that endometriosis is closely associated with female infertility in reproductive age, it is still unclear how endometriosis adversely affects endometrial receptivity that leads to failure in embryo implantation. This literature review is mainly focusing on recent progress on adverse effect of the endometriosis on endometrial receptivity during early pregnancy.
