1.Design and application effect of continuing education case library combined with case-based learning for rehabilitation therapists

ObjectiveTo investigate the demand and the application outcomes of case-based learning (CBL) combined with teaching case library in continuing education courses for rehabilitation therapists. MethodsA convergent mixed-methods research design was adopted, involving 51 rehabilitation therapists and 31 instructors who participated in the advanced training program at the Department of Rehabilitation Medicine, Peking University Third Hospital between October, 2022 and October, 2024. Self-developed questionnaires were used to collect data on the perceived needs of teachers and students regarding CBL and teaching case library. Differences between CBL + teaching case library and traditional lecturing in student evaluations, classroom participation and interaction were compared using Student Evaluation of Teaching in Medical Lectures, Classroom Participation Scale and Flanders Interaction Analysis System. Semi-structured interviews were conducted to obtain evaluations and attitudes towards this method from both instructors and students' perspectives. ResultsThe survey showed that 91.4% of participating teachers and students supported the use of CBL in the courses, and 82.7% advocated that the teaching case library should include typical cases. Significant differences were observed in teaching preference between teachers and students (χ² = 17.597, P < 0.01). Application effects demonstrated that CBL+teaching library significantly outperformed traditional teaching methods in student previewing behaviors, classroom interaction and learning outcomes (|Z| ≥ 2.646, P < 0.01). Flanders Interaction Analysis indicated that CBL+teaching library was superior to traditional teaching in terms of students' motivation to speak and autonomous learning. Qualitative Research generated four positive themes including cultivating clinical reasoning, being close to clinical practice, deepening knowledge understanding and improving teaching quality; and three negative themes including increasing teaching burden, high software and hardware requirements and posing great challenges to students were generated. ConclusionCompared with traditional teaching methods, CBL combined with teaching case library is closely linked to clinical practice, facilitating students' clinical reasoning, enhancing teaching effectiveness and satisfaction, and therefore aligning with the goals and needs of continuing education for rehabilitation therapists, which is highly recognized by both instructors and students.

3.A retrospective cohort analysis on the association between blood donation intervals and adverse reactions to blood donation in Shenzhen, China

Objective: To statistically analyze the association between blood donation intervals and the incidence of adverse reactions to blood donation based on blood donor data from Shenzhen. Methods: Basic data and records of adverse reactions to blood donation among voluntary whole blood donors in Shenzhen from January 2017 to June 2025 were extracted. A total of 795 404 whole blood donations were recorded, including 502 743 from males and 292 661 from females, with 1 088 and 751 cases of adverse reactions, respectively. Analyses were performed using R software, including restricted cubic spline (RCS), binary logistic regression, and generalized estimating equations (GEE) to evaluate the correlation between donation intervals and adverse reactions. Results: A total of 1 839 cases of adverse reactions were recorded, accounting for 0.23% of the total donations. Both binary logistic regression analysis and the generalized estimating equations (GEE) model showed that there was no significant difference in the incidence of adverse reactions between male and female donors with an interval of ≥6 months compared to those with an interval of ≥3 months but<6 months. Furthermore, using the 3-5 month interval group as the reference, the analysis indicated that the incidence of adverse reactions significantly increased in males with a 9-12 month interval, while no statistically significant differences were observed across any of the female subgroups. Conclusion: There was no significant correlation between adverse reactions to blood donation and the donation interval. Compared with a donation interval of ≥6 months, an interval of<6 months does not lead to an increase in the incidence of adverse reactions among male or female donors. The findings of this study may help enhance the willingness of blood donors to participate.

Using the <23 years age group as a reference, the incidence of adverse reactions significantly decreased in both male and female donors across the 23-30, 31-40, 41-50, and 51-60 age groups, with the differences being statistically significant. Conclusion: There is a negative correlation between the rate of adverse donation reactions and age, with the incidence of such reactions among whole blood donors over 55 years old being no higher than that among donors aged 55 years or younger. These findings carry positive significance for safeguarding the donation rights of older donors and meeting clinical blood demand.

6.Determination of multi-component content and analysis of differential components in Aurantii Fructus Immaturus from different origins

OBJECTIVE To determine the contents of 15 components in Aurantii Fructus Immaturus from different origins (Citrus aurantium, C. junos, C. aurantium Linn., C. sinensis Osb., C. sinensis), screen differential components, and provide references for the quality evaluation of Aurantii Fructus Immaturus. METHODS HPLC method was adopted to determine the contents of synephrine, N-methyltyramine, 5,7-dihydroxychromone-7-neohesperidoside, neoponcirin, narirutin, naringin, hesperidin, neohesperidin, naringenin, hesperetin, sinensetin, nobiletin, tangeretin, 5-demethylnobiletin, and auraptene in 46 batches of Aurantii Fructus Immaturus from different origins. The determination was performed on Waters Symmetry C18 column with mobile phase consisted of acetonitrile-0.1% formic acid (gradient elution) at the flow rate of 1.0 mL/min; column temperature was set at 40 ℃ , detection wavelength was 284 nm, and sample injection volume was 5 μL. The differences between different origins of Aurantii Fructus Immaturus were analyzed by cluster analysis, principal component analysis (PCA) and orthogonal partial least squares-discriminant analysis (OPLS-DA), and differential components were screened. RESULTS The linear relationships of the aforementioned 15 components were all good within the tested mass concentration ranges (all r>0.999 0). The RSDs for precision, stability (24 h), and repeatability tests were all less than 2.00%. The average recovery rate ranged from 91.1% to 103.9% (all RSDs<3.10%). Cluster analysis, PCA, and OPLS-DA revealed that C. sinensis Osb. and C. sinensis were clustered into one category, while C. aurantium,C. junos and C. aurantium Linn. were clustered into another category. The variable importance projection values for neohesperidin, auraptene, naringin, neoponcirin, tangeretin, hesperidin, sinensetin, and 5,7-dihydroxychromone-7-neohesperidoside were all greater than 1. CONCLUSIONS In this study, the contents of 15 components in Aurantii Fructus Immaturus from different origins are determined, and 8 differential components, including neohesperidin, auraptene, naringin, and neoponcirin, are screened out.
7.Construction of an Active Monitoring Model for Adverse Events of Pediatric Antimicrobial Drugs Based on the China Hospital Pharmacovigilance System

Objective To develop an active monitoring model for adverse drug events(ADEs)related to antimicrobial use in children based on the China hospital pharmacovigilance system(CHPS).Methods Trigger items for the active monitoring model were initially drafted through a review of relevant literature,adverse reaction databases,and drug label warnings,and subsequently refined using the Delphi method.A retrospective analysis was performed on pediatric inpatients who received antibiotics at Anning First People's Hospital between January 1 and December 31,2024.The detection rate and positive predictive value(PPV)of the active monitoring model were calculated and compared with spontaneous ADE reports from the same period.Risk factors for ADEs were further analyzed using logistic regression.Results 25 trigger items were established for the active monitoring model.Among 1,784 cases,233 ADEs were identified,yielding a detection rate of 13.06%(233/1,784).The spontaneous reporting rate of adverse events during the same period was 1.85%(33/1,784).The difference between the two was statistically significant(P<0.001).There were 727 positive trigger events,with 299 cases of ADE detected,resulting in an overall PPV of 41.13%(299/727).Logistic regression revealed that antibiotic use exceeding 3 days(OR=1.454,95%CI:1.012-2.088)was significantly associated with ADE occurrence.Conclusion Compared with conventional spontaneous reporting,the active monitoring model can significantly improve the detection rate of ADEs of pediatric antimicrobial drugs and achieve active and real-time monitoring of drug adverse events.
8.Molecular characteristics and genetic evolution analysis of CRISPR loci in Listeria monocytogenes

Objective To detect clustered regularly interspaced short palindromic repeats (CRISPR) in Listeria monocytogenes, and analyze the structure and homology of CRISPR loci. Methods Totally 34 strains of Listeria monocytogenes isolated in our laboratory were identified, PCR amplified and sequenced. The repeat sequence structure and spacer sequence homology in CRISPR loci were analyzed by bioinformatics software. Results A total of 7 CRISPR loci were detected in 34 strains. The mutation rate of the first 2 and last 2 bases of the Repeat sequence of CRISPR loci was higher, while the mutation rate of the middle part was lower. Seven CRISPR sites form eight CRISPR structural types, among which the Repeat sequences of CRISPR1 and CRISPR2 are relatively conserved, while the Repeat sequences of CRISPR1 and CRISPR5 can form dumbbell shaped secondary structures. The number of Spacer sequences contained in each CRISPR site ranges from 2 to 15, with an average of 2.43. The 136 Spacer sequences detected were not only homologous to Listeria plasmids and bacteriophages, but also homologous to uncultured virus sequences, staphylococcal bacteriophages, and Listeria innocua. The same CRISPR genotype did not show large-scale clustering, but some strains in the same year were in the same evolutionary cluster with close genetic relationships. Conclusion The CRISPR structure of Listeria monocytogenes in this study exhibits high specificity, and its homology with bacteriophages provides a theoretical basis for the application of bacteriophages in the control and prevention of Listeria monocytogenes.
9.Comparison of value of Logistic regression model and XGBoost model in predicting upper limb lymphedema after radical mastectomy

Objective:To study the value of Logistic regression model and XGBoost model in predicting upper limb lymphedema after radical mastectomy.Methods:A total of 150 patients with breast cancer admitted to the Department of Breast Surgery of the Affiliated Hospital of Jining Medical College from Jan. 2023 to Jan. 2024 were collected as the study objects, all of whom underwent surgical treatment. The subjects were divided into the occurrence group (30 cases) and the non-occurrence group (120 cases) according to whether upper limb lymphedema occurred after operation. The clinical data of the patients in the study were collected, and the risk prediction models of upper limb lymphedema after radical mastectomy were constructed based on Logistic regression analysis and XGBoost method, respectively, and the predictive value of the two models was compared.Results:Among the 150 patients, 30 patients developed upper limb lymphedema after surgery, with an incidence of 20.00% (30/150) . The age, body mass index (BMI) level, clinical stage (stage III) , number of excised lymph nodes ≥ 10, postoperative non functional exercise, postoperative adjuvant radiotherapy, and proportion of hypertension in the incidence group were higher than those in the non incidence group ( t=3.21, 5.21, χ 2=30.68, 16.24, 12.40, 11.11, 8.58, P<0.05) . Logistic regression analysis showed that BMI, clinical stage (stage III) , number of lymph nodes removed ≥10, postoperative non-functional exercise, postoperative adjuvant radiotherapy and hypertension were all risk factors for upper limb lymphedema after radical mastectomy ( P<0.05) . ROC curve showed that the area under the curve (AUC) of multivariate Logistic regression and XGBoost model predicting upper limb lymphedema after radical mastectomy was 0.886 (95% CI: 0.843-0.941) and 0.874 (95% CI: 0.829-0.935) , respectively. There was no significant difference in AUC between the two models ( Z=1.34, P=0.462) . The Calibration curves of multivariate Logistic regression and XGBoost model were tested by Hosmer-Lemeshow, but there was no significant difference ( χ 2=0.83, P=0.516; χ 2=0.71, P=0.637) . Both models fit well. Conclusions:Multiple Logistic regression and XGBoost model were equally effective in predicting upper limb lymphedema after radical mastectomy. BMI, clinical stage (stage III) , number of lymph nodes removed ≥10, postoperative non-functional exercise, postoperative adjuvant radiotherapy, and hypertension are all important factors affecting upper limb lymphedema after radical mastectomy.
10.Construction of continuity of care plan for adolescent fixed orthodontic patients based on timing theory

Objective:To construct a continuity of care plan for adolescent fixed orthodontic patients based on the theory of timing, and provide systematic and standardized nursing interventions for adolescent fixed orthodontic patients.Methods:Guided by the theory of timing, preliminary plan items were formulated through literature review and questionnaire survey. Using the Delphi method, 15 experts were consulted for two rounds to determine the continuity of care plan for adolescent fixed orthodontic patients from October to December, 2023.Results:The effective response rates of the two rounds of questionnaires were 15/15, respectively. The expert authority levels were 0.93 and 0.94, and the Kendall harmony coefficients were 0.28 and 0.38, respectively. The differences were statistically significant (both P<0.001). The final established plan included 5 primary indicators, 23 secondary indicators, and 47 tertiary indicators. Conclusions:The continuity of care plan for adolescent fixed orthodontic patients based on the timing theory is reliable and scientific, and can meet the continuity of care needs of patients at different stages, providing reliable clinical basis for carrying out continuity of care.
