1.Lower plasma selenoprotein P levels in regularly exercising young adults

Objective:
Physical exercise can provide many health benefits in humans. Exercise-induced reactive oxygen species (ROS) formation and its downstream signaling cascades are reported to induce mitochondrial biogenesis in exercising tissues. Selenoprotein P (SELENOP) is the antioxidant hepatokine whose hypersecretion is associated with various metabolic diseases. It was reported to impair exercise-induced reactive oxygen species signaling and inhibit subsequent mitochondrial biogenesis in mice. However, the relationship between selenoprotein P and mitochondrial dynamics in humans has not yet been reported. While reduction of plasma selenoprotein P becomes an attractive therapeutic target for metabolic diseases, the role of regular exercise in this regard is still unknown. This study aimed to analyze the influence of regular habitual exercise on plasma selenoprotein P levels and its association with leucocyte mitochondrial DNA copy number in healthy young adults.
Methodology:
Plasma selenoprotein P levels and leucocyte mitochondrial DNA copy numbers were compared in 44 regularly exercising subjects and 44 non-exercising controls, and the correlation between the two parameters was
analyzed. Plasma selenoprotein P levels were measured by Enzyme-linked Immunosorbent Assay, and leucocyte
mitochondrial DNA copy numbers were measured using the qPCR method.
Results:
The regular-exercise group had lower plasma selenoprotein P levels with higher leucocyte mitochondrial DNA copy numbers than the non-exercise group. There was a tendency of negative correlation between the two variables in our studied population.
Conclusion
Regular habitual exercise has a beneficial effect on reducing plasma selenoprotein P levels while raising mitochondrial DNA copy numbers.

2.Effects of maternal exposure to arsenic on social behavior and related gene expression in F2 male mice.

BACKGROUND:
Arsenic is a developmental neurotoxicant. It means that its neurotoxic effect could occur in offspring by maternal arsenic exposure. Our previous study showed that developmental arsenic exposure impaired social behavior and serotonergic system in C3H adult male mice. These effects might affect the next generation with no direct exposure to arsenic. This study aimed to detect the social behavior and related gene expression changes in F2 male mice born to gestationally arsenite-exposed F1 mice.
METHODS:
Pregnant C3H/HeN mice (F0) were given free access to tap water (control mice) or tap water containing 85 ppm sodium arsenite from days 8 to 18 of gestation. Arsenite was not given to F1 or F2 mice. The F2 mice were generated by mating among control F1 males and females, and arsenite-F1 males and females at the age of 10 weeks. At 41 weeks and 74 weeks of age respectively, F2 males were used for the assessment of social behavior by a three-chamber social behavior apparatus. Histological features of the prefrontal cortex were studied by ordinary light microscope. Social behavior-related gene expressions were determined in the prefrontal cortex by real time RT-PCR method.
RESULTS:
The arsenite-F2 male mice showed significantly poor sociability and social novelty preference in both 41-week-old group and 74-week-old group. There was no significant histological difference between the control mice and the arsenite-F2 mice. Regarding gene expression, serotonin receptor 5B (5-HT 5B) mRNA expression was significantly decreased (p < 0.05) in the arsenite-F2 male mice compared to the control F2 male mice in both groups. Brain-derived neurotrophic factor (BDNF) and dopamine receptor D1a (Drd1a) gene expressions were significantly decreased (p < 0.05) only in the arsenite-F2 male mice of the 74-week-old group. Heme oxygenase-1 (HO-1) gene expression was significantly increased (p < 0.001) in the arsenite-F2 male mice of both groups, but plasma 8-hydroxy-2'-deoxyguanosine (8-OHdG) and cyclooxygenase-2 (COX-2) gene expression were not significantly different. Interleukin-1β (IL-1β) mRNA expression was significantly increased only in 41-week-old arsenite-F2 mice.
CONCLUSIONS
These findings suggest that maternal arsenic exposure affects social behavior in F2 male mice via serotonergic system in the prefrontal cortex. In this study, COX-2 were not increased although oxidative stress marker (HO-1) was increased significantly in arsnite-F2 male mice.

3.IS HIGH-FIDELITY PATIENT SIMULATION-BASED TEACHING SUPERIOR TO VIDEO-ASSISTED LECTUREBASED TEACHING IN ENHANCING KNOWLEDGE AND SKILLS AMONG UNDERGRADUATE MEDICAL STUDENTS?

Introduction:
Medical simulation is a technique that allows interactive and immersive activity by recreating all or part of a clinical experience without exposing the patients to the antecedent risks. High-fidelity patient simulation-based teaching is an innovative and efficient method to address increasing student enrolment, faculty shortages and restricted clinical sites.
Objectives:
To assess the effectiveness of high-fidelity patient simulation (HFPS) as compared to video-assisted lecture-based teaching method (VALB) among undergraduate medical students.
Methods:
he study was a Randomized Controlled Trial which involved 56 final year undergraduate medical students. The effectiveness of teaching based on HFPS (intervention group) and VALB (control group), on acquisition of knowledge, was assessed by multiple choice questions (MCQs) in the first and fourth week. Similarly, the skills competency was assessed by objective structured clinical examination (OSCE) in the second and fourth week. Mean and standard deviation (SD) for total score of knowledge and skills assessments were used as outcome measures. P value < 0.05 was considered to be statistically significant.
Results:
In both groups, students had significant higher mean MCQ scores at Post-tests. The intervention group had higher mean change score of MCQ marks than the control group but the difference was not statistically significant. In both the first and second skills assessments, mean OSCE scores for intervention group were higher than control group but this difference was not statistically significant.
Conclusion
There was significant gain in knowledge in both methods of teaching but did not reach statistical difference in terms of skills enhancement in the intervention group as compared to the control group.

4.The impact of stressors on the learning outcome of highfidelity patient simulation in undergraduate medical students

Introduction: Simulation of the clinical setting incorporates
an educational approach connecting a learner to a particular
environment of learning. Undergraduate students in the
health sector experience anxiety during simulation that
influences their performance which ultimately affects their
learning outcome. This study attempts to correlate the
impact of stressors on learning outcome of high-fidelity
patient simulation (HFPS) in undergraduate medical
education.
Objective: This research is to analyze the impact of
stressors and its relevance on the learning outcome of HFPS
as a teaching-learning tool for the management of
emergency surgical conditions including trauma.
Materials and Methods: This study is a Quasi-experimental
time series design. A total number of 347 final-year
undergraduate (MBBS) students of Melaka-Manipal Medical
College, Malaysia. They were grouped and assessed
individually by pre-test and post-tests on their knowledge,
performance and associated stressor scores. The one-way
repeated measure of Analysis of Variance (ANOVA) was
used to determine the statistically significant differences in
total score at pre-test simulation and post-test-simulation
sessions. Friedman test was used for assessment of
individual components of stressors. Pre-test and post-tests
scores were compared to note progress in confidence and
stress reduction. P value <0.001 was considered statistically
significant.
Results: ANOVA with Bonferroni post hoc analysis showed
a statistically significant (p <0.001) difference in stressor
score over time. The drop-in stress was significant initially
but flattened out later.
Conclusion: Stress significantly decreased as the students
were exposed to more sessions of HFPS which ultimately
translated into better learning outcome.
5.The pattern and burden of neurological disorders: A systemic review of Neurology Department, Yangon General Hospital, Myanmar

Objective: To understand the pattern and burden of neurological disorders at Department of Neurology,
Yangon General Hospital (YGH), Myanmar. Method: All patients admitted to Department of Neurology
had been prospectively registered and then analyses were performed on one year data from 1st January
to 31st December 2019. Results: Among study population of total 17606 cases, 65.5% attended
to neurology out-patient clinics, 20.1% were seen as referral cases and 14.4% were inpatients at
neurology ward. When analyzed the inpatients, 58% were male. Mean age was 47.2 ±19.1 years and
41-65 year age group was most commonly affected. The most common inpatient neurological diseases
were cerebrovascular diseases (CVD)(40.8%), central nervous system (CNS) infections (15.2%) and
peripheral nerve lesions (9.4%). Autoimmune diseases and headache disorders were more common
in women. Mortality rate in inpatients was 4.6%, among which stroke occupied for 55.1% and CNS
infection was 23.7%. Three most common neurological diseases in out-patient clinic were seizure
disorders (21.9%), CVD (20.7%) and headache disorders (11.2%). Among the referral cases, 43.7%
were CVD, 17.8% were CNS infections and 11% had seizures disorders. The most referred patients
(93.3%) came from general medical wards.
Conclusions: Entire range of neurological disorders was present in YGH. CVD and CNS infections
were the most common cause of incidence as well as the mortality. Knowing the burden can raise the
awareness about it and point out the prioritizing provision of essential health services. As similar in
controlling the communicable diseases, the importance of health education, preventive measures on
non-communicable disorders should be considered as national concern.
6.Peripheral neuropathy induced by drinking water contaminated with low-dose arsenic in Myanmar.

BACKGROUND:
More than 140 million people drink arsenic-contaminated groundwater. It is unknown how much arsenic exposure is necessary to cause neurological impairment. Here, we evaluate the relationship between neurological impairments and the arsenic concentration in drinking water (ACDW).
PARTICIPANTS AND METHODS:
A cross-sectional study design was employed. We performed medical examinations of 1867 residents in seven villages in the Thabaung township in Myanmar. Medical examinations consisted of interviews regarding subjective neurological symptoms and objective neurological examinations of sensory disturbances. For subjective neurological symptoms, we ascertained the presence or absence of defects in smell, vision, taste, and hearing; the feeling of weakness; and chronic numbness or pain. For objective sensory disturbances, we examined defects in pain sensation, vibration sensation, and two-point discrimination. We analyzed the relationship between the subjective symptoms, objective sensory disturbances, and ACDW.
RESULTS:
Residents with ACDW ≥ 10 parts per billion (ppb) had experienced a "feeling of weakness" and "chronic numbness or pain" significantly more often than those with ACDW < 10 ppb. Residents with ACDW ≥ 50 ppb had three types of sensory disturbances significantly more often than those with ACDW < 50 ppb. In children, there was no significant association between symptoms or signs and ACDW.
CONCLUSION
Subjective symptoms, probably due to peripheral neuropathy, occurred at very low ACDW (around 10 ppb). Objective peripheral nerve disturbances of both small and large fibers occurred at low ACDW (> 50 ppb). These data suggest a threshold for the occurrence of peripheral neuropathy due to arsenic exposure, and indicate that the arsenic concentration in drinking water should be less than 10 ppb to ensure human health.

8.Noise-Induced Hearing Loss in the Police Force.

BACKGROUND: Noise-induced hearing loss (NIHL) is a major preventable occupational health problem with 250 million people worldwide known to have disabling impairment of moderate to greater severity. The aims of the study are to estimate the prevalence of NIHL in the police force; and study its association with age, sex, duration of service (years), smoking and alcohol habits, use of hearing protective devices, as well as preexisting chronic diseases. METHODS: A cross-sectional study was conducted on 543 police personnel who had undergone periodic medical examination over a 12-month period. The diagnostic criteria for NIHL were (1) history of occupational noise exposure, (2) bilateral hearing loss, (3) hearing loss of > or = 25 dBA at 4,000 Hz in two consecutive audiograms, and (4) no significant medical history affecting hearing. Severity of NIHL was based on the World Health Organization grading. RESULTS: Males (74.8%) made up the majority of the police force. The mean age for police personnel was 35.55 +/- 9.57 years, and the mean duration of service was 14.75 +/- 9.39 years. Compliance with the usage of hearing protective devices was seen in 64.4%. The prevalence of NIHL in this study population was 34.2%, with a higher prevalence in males (37.7%) than in females (23.9%). The study also showed strong associations between NIHL and male sex (odds ratio, 1.9; P < 0.05), and hypertension (odds ratio, 3.3; P < 0.001). Overall, 93% were found to have mild NIHL, 3.5% had moderate NIHL, and 3.5% had severe NIHL. No police personnel were found to have profound hearing loss. CONCLUSION: The prevalence of NIHL in this study is high compared to other similar studies among police personnel. This study shows that increasing age, male, presence of hypertension, diabetes, and longer duration of service are significant associated factors for NIHL. Preventative strategies include health surveillance, implementation of a hearing conservation program, and legislation.

There has been significant magnitude of problems of diabetes in Myanmar, according to the estimates of
International Diabetes Federation (IDF) and the recent National Survey on the prevalence of diabetes. There has
been a wide gap of equity between the urban and rural healthcare delivery for diabetes. Myanmar Diabetes Care
Model (MMDCM) aims to deliver equitable diabetes care throughout the country, to stem the tide of rising burden of
diabetes and also to facilitate to achieve the targets of the Global Action Plan for the Prevention and Control of
NCDs (2013-2020). It is aimed to deliver standard of care for diabetes through the health system strengthening at all
level. MMDCM was developed based on the available health system, resources and the country's need.
Implementation for the model was also discussed.
10.Rates and predictors for influenza vaccine prescriptions among HIV-infected clinic patients in Singapore.

INTRODUCTIONAlthough Singapore national guidelines recommend influenza vaccination for individuals with comorbidities, the vaccine uptake remains relatively low. This study examines the rates of influenza vaccine prescriptions in a clinic population, and patient, doctor and clinic factors that could affect the vaccine prescribing rates.
MATERIALS AND METHODSThis retrospective review utilised electronic medical records from HIV-infected patients seen in an infectious disease (ID) specialist clinic. Data from 40 randomly selected patients per physician were analysed for the outcome of influenza vaccine prescriptions from 1 January to 31 December 2007. All 7 consultants and the 6 ID registrars who had spent at least 4 months in the Department during 2007 were included. Data analysed included patient, physician, and clinic characteristics, and clinically relevant outcomes of admission within a year, and the length of hospital stay.
RESULTSOf the 461 HIV-infected patients analysed for this study, only 107 (23%) were prescribed influenza vaccine in 2007. Vaccine prescribing rates by individual physicians ranged from 0% to 77%. The outcome of vaccine prescribing was analysed by patient demographics (age >40 years, gender, race), physician characteristics (doctor grade, gender and training), and clinic volumes (number of patients per clinic session). Multivariate analysis demonstrated that patients with female doctors (OR 1.8, 95% CI, 1.1 to 3.0, P = 0.017), and doctors with overseas medical training (OR 11.6, 95% CI, 6.0 to 22.2, P <0.001) were significantly more likely to have influenza vaccine prescribed. On univariate analysis, patients were more likely to be admitted if they were male (OR 2.1, 95% CI, 1.0 to 5.1, P = 0.041), and over 40 years old (OR 2.1, 95% CI, 1.1 to 4.5, P = 0.024). Patients prescribed influenza vaccine showed a non-significant trend for protection against admission (OR 0.7, P = 0.288), and shorter length of stay (median 5 vs 9 days, P = 0.344).
CONCLUSIONInfluenza vaccine prescribing for HIV-infected outpatients in 2007 was only 23%, even in an ID specialist clinic. There was substantial variability in prescribing rates by individual physicians. Neither patient demographics nor patient volumes per clinic session had an impact on the prescribing rates, but significant predictors included physician gender and medical school training. Patients prescribed influenza vaccine had fewer admissions and shorter hospital lengths of stay, although these trends were non-significant.
