1.The residual physical symptoms in Chinese patients with major depressive disorder after acute treatment phase: a cross-sectional survey among eleven mental health centers

Objective To investigate the prevalence and characters of residual physical symptoms in Chinese depressive patients after acute treatment and the influence on the quality of life and social function.Methods In the multi-centers cross-sectional study,1 503 adult depressive outpatients,from 11 hospitals,who had been treated by antidepressants for 8-12 weeks with significant improvement of at least 50% by Self-rating Visual Analog Scale were involved.The group with residual physical symptoms could be differentiated by Brief 16-Item Quick Inventory of Depressive Symptomatology Self-Report (QIDS-SR16) >5,while the QIDS-SR16 of remission group was ≤5.The QIDS-SR16,Patient Health Questionnaire-15 (PHQ-15),Quality of Life Enjoyment and Satisfaction Questionnaire-Short Form (Q-LES-Q-SF) and Sheehan Disability Scale (SDS) were used to assess the depressive symptoms,physical symptoms and social function.Results 49.8% (748/1 503) patients had residual physical symptoms (PHQ-15>4).The most common physical symptoms (PHQ-15≥1) were feeling tired or having low energy (56.3%,846/1 503),trouble sleeping (50.6%,760/1 503),headache(43.6%,655 / 1 503),constipation,loose stools or diarrhea(41.3%,620/1 503),feeling heart pound or race (37.6%,565/1 503),dizziness (34.0%,511/ 1 503),nausea,gas or indigestion (33.6%,505/1 503),etc.The 75.7% (555/733) of the physical symptoms (PHQ-15>4) in residual group was significantly higher than the 25.1% (193/770;x2=385.38,P<0.01) in remission group.The differences on social function (t=2.77-10.47) and life quality (t=3.04-11.41) between two groups were significant (P<0.01).Generalized linear regression analysis showed that gender (female),present history of first episode were related to better social function,and the factors related to both social function and quality of life included residual depressive symptoms,residual physical symptoms of different severity,comorbid medical conditions,age,duration of antidepressant treatment for this episode.Conclusion The rate of residual physical symptoms is higher in patients with major depression who responded to acute phase treatment.The more severe the residual physical symptoms,the worse the quality of life and social function.It suggests that physical symptoms be focused in order to achieve clinical recovery.

5.Effects of early application of pediatric amino acids on extrauterine growth and development of preterm and small for gestational age infants

Objective To explore the effect of early application of pediauic amino acids on extrauterine growth and development of preterm and small for gestational age infants.Methods A total of 106preterm and small for gestational age infants was selected in our NICU from June 2011 to May 2013,and randomly divided into two groups:treatment group (group T) and control group (group C).Next,each group was divided into two subgroups according to gestational age and birth weight.Such as ≥34W group (group T1,group C1),< 34W group (group T2,group C2),< 1.5 kg group (group T3,group C3) and ≥ 1.5 kg group (group T4,group C4).They were observed the effect of extrauterine growth and correlative serum biochemical indicators with application of pediatric amino acid in postnatal 6 hours and 12 hours.Resuits After 2-week treatment,the head circumference and weight growth of group T were higher than that of group C (P < 0.05,or P < 0.01),and the time of birth weight recovery was significantly shortened(P < 0.05,or P < 0.01).The incidence rate of extrauterine growth retardation(EUGR) in the group T was lower than that in group C,there is significantly statistical differences(P < 0.01).The concentration of blood urea nitrogen (BUN) in group T was obviously increased than that in the group C(P <0.01).The levels of ALT,aspartate aminotransferase (AST),total bilirubin (TBIL),cholesterol (CH) and triglycerides (TG) were not different between the group T and group C.The comparison of alanine aminotransferase (ALT) levels were statistically significant between group C4 and group T4.In the different gestational age groups,the concentration of BUN in group T was obviously higher than that in the group C after 2-week treatment(P <0.05),the levels of AST,TBIL,CH and TG were not different between the group T and group C (P > 0.05).The incidence rate of EUGR in the 4th group C was increased significantly than that in the 4th group T(P <0.05).Conclusions The preterm and small for gestational age infants were safe after the pediatric amino acid was used at the 6 h after birth.Amino acid can promote growth of head circumference and weight,shorten the birth weight recovery time and reduce the incidence of EUGR.
6.The role of Hedgehog pathway in hepatic fibrosis and hepatic stellate cell activation

Objective To investigate the role of Hedgehog pathway in hepatic fibrosis and its association with activation of hepatic stellate cells. Methods Twenty male Spragur-Dawley rats were divided into control and model groups with 10 each. The animal models were induced by injection with CCl4 and fed with fat-rich diet. The rats in both groups were sacrified at the 8 week with 5 each and the liver tissues were removed for HSC-T6 culture. The deposition of collagen fiber in liver was detected with HE and Masson staining. RT-PCR was used to detect the expressions of Sonic hedgehog (Shh), smoothened (Smo), patched (Ptc), Gli-1 and α-smooth muscle action (α-SMA) mRNA in HSC-T6 and liver tissues. The influence of cyclopamine (Cyc) and lipopolysaccharide (LPS) on HSC-T6 proliferation were assayed by MTT. The expressions of Shh, Smo, Ptc, Gli-1 and α-SMA mRNA after intervention with Cyc (100μmol/L) and LPS were measured by real-time PCR. Results A lot of lipo and collagen deposited in liver of model rats. The Shh,Smo,Gli-1 and α-SMA mRNA were highly expressed in model rats than those in control group (2-△△Ct were 20.45±3.31 vs. 1, 12.78 ± 0. 53 vs. 1, 10.88 ± 2.41 vs. 1, 4.91 ± 2. 59 vs. 1, respectively, all P value <0. 05). In vitro Cyc inhibited HSC-T6 proliferation in dose dependant manner (F=636.81, P<0.01). Compared to the control group, the mRNA expressions of Smo, Ptc, Gli-1,α-SMA in HSC-T6 were significantly reduced after Cyc intervention (2△△Ct, were 0. 20±0. 11, 0. 21 ± 0. 08, 0. 28 ± 0. 05,0. 27±0.10,respectively, all P values<0.01). Conclusion The expression of members of Hedgehog pathway are increased in the progress of hepatic fibrosis, which may accelerate the hepatiee fibrosis by activating HSC.
7.Correlation of height change of intervertebral space of fused segment with axial synptoms after anterior cervical fusion

[Objective] To investigate the height change of intervertebral space after anterior cervical fusion and its relation to neck axial symptoms.[Method]Totally 122 patients receiving anterior cervical fusion were followed up for 2 to 5 years,average 3.9 years.The height change of intervertebral space of the fused segment on X-rays were evaluated before,3 months and 2 years after operation.All patients were classified into 3 groups by means of the postoperative height change of intervertebral space.The axial symptoms after operation were observed and analyzed during follow-up.[Result]The incidence of neck axial symptoms was 54.55%,25.84% and 45.45%,respectively.There were significant differences in statistical analysis in group A group B,group B to group C(x2=7.18,P0.0125).[Conclusion]There is significant relation between height change of intervertebral space and neck axial symptoms after anterior cervical fusion.
8.THE FLEXION REFLEX POTENTIAL AND DORSAL ROOT POTENTIAL INDUCED BY STIMULATION OF THE CUTANEOUS NERVE IN THE RAT

On the intact and awake rat, the flexion reflex potential (FRP)and dorsal root potential (DRP) induced by stimulation of the cutaneous nerve with graded intensity stimulations to the sural nerve (a pure cutaneous nerve) were observed, there appeared A-FRP and C-FRP one after the other on the hamstring nerve (a group of nerves innervating the hindlimb flexors), and A-DRP and C-DRP on the L_5 dorsal root at the same time. The threshold and the latencices of A-FRP and A-DRP had no differences, and had significant positive correlations. The amplitudes of A-FRP and A-DRP increased simultaneously with the increase of the stimulating intensity in certain range. The threshold and latency of C-FRP are larger than those of C-DRP. The threshold and the latency of the C-FRP and C-DRP showed significant positive correlation. The amplitudes of C-FRP and C-DRP increased simultaniously with the increase of the stimulating intensity in certain range. The train stimuli (three pulses, 5ms interval, 0. 5ms duration) induced larger FRP and DRP than those induced by single stimulus. Thus, FRP and DRP both have the property of temporal summation.
9.THE EFFECT OF REMOTE SEGMENTAL ELECTRO-ACUPUNCTURE ON THE FLEXION REFLEX POTENTIAL AND THE DORSAL ROOT POTENTIAL

Deseription was given of the effect of electric acupuncture of Neiguan point on the flexion reflex potential (FRP) of the hindlimbs and the dorsal root potential (DRP). On the intact and awake rats, the graded train stimulation (three pulses, 5ms interval, 0.5ms duration) was given to the sural nerve (a pure cutancous n(?), the C-FRP and C-DRP with their proper amplitudes (70~80% maximum) were obtained on the hamstring nerve (a group of nerves innervating the hindlimb flexors) and L5 dorsal root respectively. Then the electric acupuncture with low freqency and low intensity (4Hz, 1~3V)and low freqency and high intensity ( 4Hz, 20~40V), lasting 5 minutes, was given to the ipsilateral Neiguan point. After acupuncture, the amplitudes of both C-FRP and C-DRP decreased without change of their latebcues. The former one declined heavier and recovered slower than the latter one did. The A-FRP and A-DRP were not affected. Te result of the experiment suggests that the ipsilatcral Neiguan point cletric acupuncture can induce hindlimb's analgesia, the presynaptic inhibition might be involved in the mechanism of such analgesia.
