1.Association of school bullying and insomnia with depression-anxiety-stress emotions among primary and secondary school students

Objective:
To explore the interaction between school bullying and insomnia in relation to depression-anxiety-stress emotions among primary and secondary school students,so as to provide a basis for preventing negative emotional states in adolescents.
Methods:
In October 2024, a stratified cluster sampling method was used to select 3 058 students in grade 5-6 of primary, junior and senior high school in Sheyang County of Jiangsu Province. The Delaware Bullying Victimization Scale, Insomnia Severity Index, Depression-Anxiety-Stress Scale-21, and Study Condition Questionnaire were employed to investigate school bullying, insomnia, depression-anxiety-stress emotions, and academic performance. The χ 2 test and Logistic regression were used to analyze the association between school bullying and insomnia interactions and depression-anxiety-stress emotions among primary and secondary school students, multiplicative interaction analysis was conducted, and additive interaction analysis was performed using R software.
Results:
The detection rates of depression-anxiety-stress emotions among primary and secondary school students were 21.6%, 28.4% and 10.8%, respectively. The detection rates of physical bullying, relationship bullying, verbal bullying and cyberbullying in school bullying were 10.6%, 14.0%, 22.3%, and 6.2%, respectively. The detection rate for insomnia was 23.1%. Results from Logistic regression analysis showed that, after adjusting for relevant factors, physical, relational, verbal, and cyberbullying and insomnia were positively correlated with the detection rates of depression ( OR = 5.72- 10.93), anxiety ( OR =6.35-12.17), and stress emotions ( OR =5.97-14.52) among primary and secondary school students (all P <0.01). The multiplicative interaction between physical, relational, verbal, and cyberbullying and insomnia was positively correlated with the detection rates of depression ( OR =8.00-18.01), anxiety ( OR =11.35-17.76), and stress emotions ( OR =7.64-9.12) in primary and secondary school students (all P <0.01). Additive interactions were observed between physical, relational, verbal, and cyberbullying and insomnia in relation to the detection rates of depression, anxiety, and stress emotions among primary and secondary school students (both RERI and AP >0 and the credible interval excluded 0, SI >1 and the credible interval excluded 1).
Conclusion
School bullying and insomnia are associated with depression, anxiety, and stress emotions among primary and secondary school students, and they exhibit both multiplicative and additive interactions.
2.α-ketoglutarate ameliorated arsenic-induced hepatic lipid deposition in offspring via PI3K/AKT signaling pathway

ObjectiveTo investigate the protective effect of α-ketoglutarate (α-KG) on hepatic lipid deposition in offspring caused by arsenic exposure during pregnancy. Methods8-week-old institute of cancer research (ICR) mice were mated in a ratio of 2∶1 between females and males, and the detection of vaginal plugs confirmed pregnant. A total of 32 pregnant mice were randomly divided into four groups: control group, arsenic group, α-KG group, arsenic+α-KG group. On gestational day 0-16 (GD0-GD16), the arsenic and arsenic+α-KG groups were exposed to sodium arsenite (NaAsO2 ,15 mg/L) in drinking water everyday, and the α-KG and arsenic+α-KG groups were gavaged with α-KG (2 g/kg) everyday. On GD16, pregnant mice were euthanized to collect fetal liver, and fetal body weight and crown-rump length were measured. Gene expression differences between the control group and the arsenic group were analyzed by transcriptome. The total triglycerides (TGs) and subtypes in fetal liver were detected by liquid chromatography tandem mass spectrometry (LC-MS/MS). Oil red O staining was used to observe the histopathological changes in the liver. Quantitative polymerase chain reaction (qPCR) was used to detect the expression level of genes related to lipid synthesis, transport, and degradation, and phosphatidylinositol 3' -kinase/ protein kinase B (PI3K/AKT) in the liver of fetus. ResultsTranscriptomics analysis showed that 2 144 genes were downregulated and 1 675 genes were upregulated in the arsenic exposed fetal liver; body weight and crown-rump length were reduced (PTuKey<0.05); the level of hepatic TGs was elevated in arsenic group (PTuKey<0.05); oil-red O staining showed a significant increase in lipid droplets in arsenic group (PTuKey<0.01); the expression of lipid synthesis-related genes were significantly upregulated (PTuKey<0.05); the expression of β-oxidation-related genes and lipid degradation-related genes were downregulated (PTuKey<0.05); the expression of PI3K, AKT decreased(PTuKey<0.05). Compared with the arsenic group, the body weight and crown-rump length of fetus increased in the arsenic+α-KG group (PTuKey<0.05); the level of hepatic TGs decreased in the arsenic+α-KG group (PTuKey<0.05); oil red O staining showed lipid droplets significantly decreased (PTuKey<0.01); the expression of lipid synthesis-related genes were downregulated (PTuKey<0.05), the expression of β-oxidation-related genes and lipid degradation-related genes were upregulated (PTuKey<0.05); the expression levels of PI3K and AKT increased (PTuKey<0.05). Conclusionα-KG alleviated hepatic lipid deposition in offspring exposed to arsenic during pregnancy through activating PI3K/AKT signaling pathway.
3.Longitudinal association between family types and developmental trajectories of depressive symptoms among children and adolescents

Objective:
To explore the developmental trajectories of depression in children and adolescents and their association with family types, and to analyze the role of being an only child in the context, so as to provide a basis for early identification of mental health issues in children and adolescents.
Methods:
The study was a secondary analysis based on the existing database of the Jiangsu Provincial Student Common Diseases and Health Influencing Factors Monitoring and Intervention Project. A total of 11 502 students who had completed at least two measurements using the Chinese version of the Center for Epidemiologic Studies Depression Scale (CES-D) between 2022 and 2024, and had complete information on family type, gender, and age, were selected as the study subjects. Latent class trajectory modeling was used to identify depression developmental trajectories. Multinomial Logistic regression was applied to analyze the association between family type and depression developmental trajectories, and the interaction effect of family type and being an only child was tested.
Results:
Three types of depression developmental trajectories were identified among children and adolescents: low stable type (91.3%, 10 504 students), moderate rising type (4.3%, 500 students), and high declining type (4.3%, 498 students). Significant differences were observed among the different trajectory groups in terms of gender, age, parental education level, family type, baseline CES-D score, and baseline school type ( χ 2/H=17.48, 139.97, 19.72 , 30.77, 1 081.35, 220.81, all P <0.05). Multinomial Logistic regression analysis showed that, using the low stable type as the reference trajectory group and the nuclear family as the reference family type, after adjusting for confounding factors such as gender, age, and parental education level, single parent families ( OR=1.87, 95%CI= 1.16-3.03) and grandparent headed families ( OR=1.83, 95%CI =1.04-3.21) were significantly associated with the high declining type trajectory (both P <0.05). No significant association was found between family type and the moderate rising type trajectory (all P >0.05). The interaction effect between family type and being an only child was not statistically significant ( LRT=7.71, df=8, P =0.46).
Conclusions
Depressive symptoms in children and adolescents show heterogeneous developmental patterns during school age. Children and adolescents from single parent and intergenerational families are more likely to follow the high decreasing trajectory.
4.Research on the current status and risk prediction model of oral frailty among the elderly in Anhui Prov-ince

Objective:To investigate the occurrence and influencing factors of oral frailty among the elderly in China.Methods:General information questionnaire,Oral Frailty Scale,Sarcopenia Screening Questionnaire(SARC-F),Social Network Scale-6(LSNS-6)and Subjective Cognitive Decline Questionnaire(SCD-Q9)were used to conduct a survey in Anhui Province.A survey was conducted among 3 063 elderly people to analyze their current status and influencing factors related to oral frailty.Results:The incidence of oral frailty among the elderly in Anhui Province was 46.82%(1434/3063).Binary logistic regression analysis showed sarcopenia(OR=8.742,95%CI:7.156-10.679),social isolation(OR=1.601,95%CI:1.313-1.953),and subjective cogni-tive decline(OR=2.424,95%CI:1.905-3.085),90 years old and above(OR=2.261,95%CI:1.304-3.922)and having disability(OR=1.341,95%CI:1.040~1.729)are risk factors for oral frailty in the elderly in Anhui Province.Conclusion:The incidence of oral frailty is high among the elderly in Anhui Province.Risk factors for oral frailty include sarcopenia,social isolation,subjective cognitive decline,advanced age,and disability.
5.Regulatory mechanism of exercise promoting mitochondrial biogenesis in skeletal muscle

BACKGROUND:Mitochond rial biogenesis in skeletal muscle and its regulatory mechanisms during exercise have become focal points of research.Pathways such as AMP-activated protein kinase,peroxisome proliferator-activated receptory coactivator 1α,mitogen-activated protein kinase,calcium-regulated signaling play profound roles in exercise-induced mitochondrial biogenesis,impacting muscle metabolic optimization,enhanced athletic performance,and the prevention of metabolic diseases.However,the interactions among these pathways,their regulatory mechanisms,and their comprehensive effects on exercise-induced mitochondrial biogenesis in skeletal muscle remain unclear.OBJECTIVE:To explore the signaling pathways related to mitochondrial biogenesis in skeletal muscle,precisely analyze the induction and regulatory details of exercise within these pathways,and clearly elucidate the principles by which exercise-activated signaling pathways promote mitochondrial generation and functional enhancement.This will establish a theoretical foundation for improving muscle metabolism,enhancing exercise efficiency,and preventing metabolic diseases.METHODS:An extensive literature search was conducted using China National Knowledge Infrastructure(CNKI),WanFang,VIP,PubMed,and Web of Science.The latest publications related to mitochondrial biogenesis in skeletal muscle and its regulatory mechanisms were collected from inception to August 2024.By integrating findings from multiple signaling pathways,the regulatory mechanisms of exercise on mitochondrial biogenesis were systematically reviewed,with a focus on the interactions and synergistic mechanisms of AMP-activated protein kinase,peroxisome proliferator-activated receptor y coactivator 1α,protein kinase A,mitogen-activated protein kinase,calcium-regulated signaling pathways.RESULTS AND CONCLUSION:(1)Mitochondrial biogenesis in skeletal muscle is a complex biological process involving the coordinated regulation of multiple signaling pathways.This process aims to optimize the metabolic capacity,fatigue resistance,and overall athletic performance of skeletal muscle in response to changes in energy demand and external stress.The core mechanisms include the interactions and regulation of key factors such as AMP-activated protein kinase,peroxisome proliferator-activated receptor gamma coactivator 1α,and mitogen-activated protein kinase.(2)AMP-activated protein kinase senses the cellular energy status and activates peroxisome proliferator-activated receptor gamma coactivator 1α,thereby promoting mitochondrial biogenesis.Peroxisome proliferator-activated receptor gamma coactivator 1α,as the main regulator of mitochondrial biogenesis in skeletal muscle,can modulate the synthesis of mitochondrial proteins and DNA,enhance the antioxidant stress response,and improve mitochondrial function.(3)The mitogen-activated protein kinase signaling pathway,particularly p38 mitogen-activated protein kinase,further promotes mitochondrial generation by activating peroxisome proliferator-activated receptor gamma coactivator 1α during stress responses.(4)Additionally,calcium signaling and protein kinase A pathways play significant roles in the metabolic regulation of skeletal muscle.(5)Exercise can significantly enhance mitochondrial biogenesis capacity in skeletal muscle by activating these multiple signaling pathways,optimizing cellular metabolic efficiency,increasing muscle endurance,and improving athletic performance.(6)Future research should focus on in-depth exploration of the interaction mechanisms among AMP-activated protein kinase,peroxisome proliferator-activated receptor gamma coactivator1α,mitogen-activated protein kinases,and calcium signaling under different exercise intensities and modalities;strengthen studies across diverse age groups,genders,and health conditions;validate the universality and population-specificity of research findings;investigate the intricate mechanisms of emerging regulatory factors such as FNIP1 and PERM1 and their potential in exercise interventions;and promote the translation of exercise health research outcomes into clinical applications.
6.Biomechanical Analysis of Maxillary Molar Intrusion by Clear Aligners

Objective This study investigates the force distributions and movement patterns of the maxillary dentition during molar intrusion with clear aligners,aiming to provide a theoretical basis for optimizing clinical orthodontic treatment strategies.Methods A three-dimensional(3D)finite element model of the periodontal ligament-teeth-clear aligners complex was established to simulate different intrusion modes,including bilateral first molar intrusion,bilateral second molar intrusion,and simultaneous intrusion of bilateral first and second molars.The von Mises stress distribution characteristics and displacement patterns of each tooth under different intrusion conditions were systematically analyzed.Results Compared with simultaneous molar intrusion,the individual intrusion design resulted in greater intrusive movement(4.260-10.500 μm)accompanied by distal-lingual crown inclination(distal displacement:-7.690--5.100 μm;buccal displacement:-20.500--6.750 μm).Anchorage teeth displayed a displacement trend opposite to that of the intruded molars.The anterior teeth demonstrated minimal displacement and low stress levels.During maxillary molar intrusion with clear aligners,the maximum equivalent stress in the periodontal ligaments occurred at the anchorage teeth mesial to the intruded molars,primarily concentrated in the apical region and the mesial aspect of the buccal cervical area.Conclusions A sequential intrusion strategy enhances vertical control efficiency compared to simultaneous intrusion.Unanticipated mesiodistal and buccolingual displacements in the posterior region necessitate the implementation of counteracting mechanisms in aligner design.In clinical practice,priority should be given to monitoring the risks of root resorption and bone remodeling effects in stress-concentrated zones(apical and buccal cervical regions)of anchorage teeth.
7.Effect of different doses of 3,3'-iminodipropionitrile in Tourette syndrome mice

Objective To investigate the effects of different doses of 3,3'-iminodipropionitrile(IDPN)in mice with Tourette syndrome(TS)to optimize the dosage and establish a stable TS model.Methods Thirty-two male C57BL/6J mice were divided randomly into a control group and a model group.The model group was further subdivided and administered low-dose(300 mg/kg),medium-dose(350 mg/kg),and high-dose(400 mg/kg)IDPN,respectively,while the control group received an equal volume of saline by intraperitoneal injection for 7 days.Modeling effectiveness was assessed on days 0 and 7 using stereotypy scoring,the number of head and body twitches,and open-field testing.Dopamine and tumor necrosis factor(TNF-α)levels in serum and brain tissue were measured by enzyme-linked immunosorbent assay.The morphology of striatum and hippocampus tissues were observed by hematoxylin/eosin(HE)staining.Results The stereotypy scores indicated successful modeling of TS in the medium-and high-dose groups.Significant behavioral changes in the open-field test were only detected in the high-dose IDPN group(P<0.05).Serum dopamine levels were significantly increased(P<0.05)in the model group,and TNF-α levels were significantly elevated in the medium-and high-dose groups(P<0.05),but there was no significant difference in brain-tissue levels(P>0.05).HE staining showed that the neurons and glial cells in the striatum and hippocampus were morphologically normal in the control group,but there were some neurodegenerative changes and a few swollen neuronal cell bodies in the striatum and hippocampus in the model group,and obvious lymphocyte infiltration in the striatum and hippocampus in the high-dose group.Conclusions Through systematic comparison of varying IDPN dosages in establishing a TS model,this study identified 400 mg/kg as the optimal dosage for effective model induction.These findings provide data to support dose optimization in the TS model and offer valuable references for ensuring the smooth progress of early-stage experiments,which could aid the evaluation of the therapeutic effects of subsequent drug interventions.
8.The association between unhealthy lifestyle and depressive symptoms, anxiety symptoms, and stress among secondary school students in Jiangsu Province

Objective:To analyze the association between unhealthy lifestyles and depressive symptoms, anxiety symptoms and stress among secondary school students in Jiangsu Province.Methods:From September to November 2023, a multistage stratified cluster random sampling method was used to select secondary school students from 13 districts and cities in Jiangsu Province. A questionnaire survey was conducted on their unhealthy lifestyles (low physical activity, smoking, drinking, internet addiction, poor sleep quality, and unhealthy diet), as well as their depressive symptoms, anxiety symptoms and stress. The multivariate logistic regression model and mixed graph model were used to construct a network and analyze the association between unhealthy lifestyles and depressive symptoms, anxiety symptoms and stress.Results:A total of 81 414 secondary school students were finally included in this study, including 39 725 (48.79%) female students and 41 689 (51.21%) male students. The prevalence of depressive symptoms, anxiety symptoms and stress were 18.55%, 32.09% and 12.91%, respectively. The multivariate logistic regression model showed that after adjusting for age, gender, urban-rural status, residential status, and family type, compared with students without unhealthy lifestyles, students with low physical activity, smoking, alcohol consumption, internet addiction, poor sleep quality, and unhealthy diet had a significantly increased risk of depressive symptoms ( OR=1.12, 95% CI:1.07-1.17; OR=1.60, 95% CI: 1.49-1.72; OR=1.79, 95% CI: 1.71-1.88; OR=3.05, 95% CI: 2.77-3.36; OR=6.66, 95% CI: 6.40-6.93; OR=1.29, 95% CI: 1.24-1.34) and a significantly increased risk of anxiety symptoms ( OR=1.09, 95% CI: 1.05-1.13; OR=1.42, 95% CI: 1.33-1.52; OR=1.76, 95% CI: 1.69-1.83; OR=2.40, 95% CI: 2.17-2.65; OR=5.79, 95% CI: 5.59-6.00; OR=1.16, 95% CI: 1.12-1.21). Students who smoked, drank alcohol, had internet addiction, and had poor sleep quality had a significantly increased risk of stress ( OR=1.49, 95% CI: 1.38-1.61; OR=1.79, 95% CI: 1.70-1.89; OR=2.25, 95% CI: 2.04-2.48; OR=6.74, 95% CI: 6.43-7.06). The node with poor sleep quality (bridge strength=0.48) in the network constructed by the mixed graph model had the strongest centrality of the bridge connecting unhealthy lifestyles with depressive symptoms, anxiety symptoms and stress. Conclusion:Low physical activity, smoking, alcohol consumption, internet addiction, poor sleep quality, and unhealthy diet increase the risk of depressive symptoms and anxiety symptoms among Jiangsu Province secondary school students. Smoking, alcohol consumption, internet addiction, and poor sleep quality increase the risk of stress among Jiangsu Province secondary school students. Sleep quality is an important intervention target for Jiangsu Province secondary school students to alleviate their negative emotions.
9.Analysis of the effects of the incentive-coordination-supervision mechanism applied to training manage-ment of medical staff under modern hospital management system

Objective This study aims to investigate and analyze the effects of the incentive-coordination-supervision mechanism applied to training management of medical staff under the modern hospital management system.Methods A total of 84 medical staff members working at the hospital from February 2022 to February 2024 were selected for the study.They were di-vided into a reference group(n=42,February 2022 to February 2023)and a study group(n=42,March 2023 to February 2024)based on the time period.The reference group underwent routine management,while the study group underwent manage-ment based on the incentive-coordination-supervision mechanism.The quality of management,occurrence of adverse events,management satisfaction,and core competencies were compared between the two groups.Results The study group had higher scores in cultural construction,reward and punishment mechanism,reporting system,and communication and coordination mech-anism than the reference group(all P<0.05).The occurrence rate of adverse events was lower in the study group than in the reference group(7.14%vs 23.81%)(P<0.05).The study group had higher satisfaction rates in management methods,man-agement content,management forms,and communication skills(95.24%,92.86%,97.62%,and 95.24%,respectively)compared to the reference group(78.57%,73.81%,76.19%,and 73.81%,respectively)(all P<0.05).The study group had higher scores in all core competencies compared to the reference group(all P<0.05).Conclusion The application of the incentive-coordination-supervision mechanism in training management of medical staff shows favorable effects,improving the qual-ity of management,management satisfaction,and core competencies,and reducing the occurrence of adverse events.
10.Causes and management strategies of anesthetic complications during percutaneous spinal endoscopic surgery under local anesthesia

Objective:To investigate the causes and management strategies of anesthetic complications during percutaneous spinal endoscopic surgery under local anesthesia.Methods:A total of 16 800 patients (8 625 males and 8 175 females) who underwent percutaneous spinal endoscopic surgery under local anesthesia (including intravenous basic anesthesia) in Tianjin Hospital, Shandong Public Health Clinical Center and Hebei General Hospital from February 2012 to February 2023 were retrospectively analyzed. The average age was 45.3±21.6 years (range, 12-84 years). There were 220 cases of posterior cervical keyhole endoscopic surgery, 50 cases of thoracic transforaminal endoscopic surgery, 70 cases of thoracic posterior interlaminar endoscopic surgery, 11 670 cases of lumbar transforaminal endoscopic surgery, and 4 790 cases of lumbar posterior interlaminar endoscopic surgery. The occurrence time, clinical manifestations, management of intraoperative anesthesia complications were recorded, as well as surgical segments, puncture sites, complication symptoms, signs, outcome and prognosis.Results:All patients received percutaneous water-mediated uniaxial spinal endoscopic surgery under local anesthesia. There were 9 patients experienced anesthesia complications, including 6 cases of epidural diffusion of anesthetics and 3 cases of anesthetics mistakenly entering the subarachnoid space. There were 4 males and 5 females, aged 48.4±18.2 years (range, 28-84 years). There were 1 case of T 12L 1 disc herniation, 1 case of C 5-6 disc herniation, 3 cases of L 4-5 disc herniation and 4 cases of L 5S 1 disc herniation. Surgical segments and procedures: 1 case of C 5-6 posterior Keyhole endoscopic surgery, 1 case of T 12L 1 transforaminal endoscopic surgery, 2 cases of L 4-5 transforaminal endoscopic surgery, 1 case of L 4-5 interlaminar endoscopic surgery, and 4 cases of L 5S 1 interlaminar endoscopic surgery. Anesthesia complications all appeared 5-10 min after injection of local anesthetics, with symptoms of decreased oxygen saturation, decreased blood pressure, altered consciousness, and sensory and motor dysfunction of limbs. 6 patients with epidural diffusion of anesthetics recovered completely after symptomatic treatment in 5 cases, and 1 case was left with foot drop. Three patients with anesthetics mistakenly entering the subarachnoid space were immediately converted to the supine position, of which one recovered by mask oxygenation; 1 patient improved after emergency tracheal intubation, rehydration, and application of vasoconstrictive medications; and 1 patient developed multiple complications such as multiorgan failure, rhabdomyolysis, and sepsis after tracheal intubation, and recovered at 3 months after surgery with symptomatic treatment. Conclusions:Epidural diffusion and entering into subarachnoid space of anesthetics are serious complications of local anesthesia in percutaneous spinal endoscopic surgery. In addition to sensory and motor dysfunction of the limbs, the functions of the respiratory and circulatory systems can also be affected. It is necessary to be alert to the occurrence of anesthesia-related complications during operation and early identification and treatment.
