Cemental tear is a rare and indetectable condition unless obvious clinical signs present with the involvement of surrounding periodontal and periapical tissues. Due to its clinical manifestations similar to common dental issues, such as vertical root fracture, primary endodontic diseases, and periodontal diseases, as well as the low awareness of cemental tear for clinicians, misdiagnosis often occurs. The critical principle for cemental tear treatment is to remove torn fragments, and overlooking fragments leads to futile therapy, which could deteriorate the conditions of the affected teeth. Therefore, accurate diagnosis and subsequent appropriate interventions are vital for managing cemental tear. Novel diagnostic tools, including cone-beam computed tomography (CBCT), microscopes, and enamel matrix derivatives, have improved early detection and management, enhancing tooth retention. The implementation of standardized diagnostic criteria and treatment protocols, combined with improved clinical awareness among dental professionals, serves to mitigate risks of diagnostic errors and suboptimal therapeutic interventions. This expert consensus reviewed the epidemiology, pathogenesis, potential predisposing factors, clinical manifestations, diagnosis, differential diagnosis, treatment, and prognosis of cemental tear, aiming to provide a clinical guideline and facilitate clinicians to have a better understanding of cemental tear.

2.Efficacy of liposomal bupivacaine for sciatic nerve block in mice

Objective:To evaluate the efficacy of liposomal bupivacaine (LB) for sciatic nerve block in mice.Methods:Twenty-four healthy adult male C57BL/6 mice, aged 6-8 weeks, weighing 25-30 g, were divided into 4 groups ( n=6 each) using a random number table method: 1.33% LB 80 μl group (group A), 1.33% LB 40 μl group (group B), 0.66% LB 80 μl group (group C), and 0.66% LB 40 μl group (group D). The sciatic nerve block was performed using the corresponding concentration of LB or the equal volume of LB in each group. The mechanical paw withdrawal threshold (MWT) was measured using Von Frey filaments immediately after sciatic nerve block, at 5 min of block, and every 1 h until MWT recovered to the baseline level. The sciatic nerve block time-MWT curve was plotted to calculate the area under the curve (AUC). Results:The time-MWT curves exhibited similar bimodal characteristics in each group. From the time point immediately after the blockade to the first MWT peak after the blockade, there were no statistically significant differences in the AUC among the four groups ( P>0.05). From the first MWT peak to the first trough, the AUC was significantly greater in A group than in B, C and D groups and in B and C groups than in D group ( P<0.05). From the first trough to the second MWT peak, there was no significant difference in the AUC between A group and D group ( P>0.05), and the AUC was significantly greater in B and C groups than in A and D groups ( P<0.05). From the second MWT peak to the baseline level, the AUC was significantly greater in A group than in B, C and D groups ( P<0.05), and there was no significant difference in the AUC among B, C and D groups ( P>0.05). In the total duration of the sciatic nerve block, the AUC was significantly greater in A group than in B, C and D groups and in B and C groups than in D group ( P<0.05).There was no significant difference in the AUC between B group and C group ( P>0.05). Conclusions:LB exhibits characteristic bimodal changes when used for sciatic nerve block; higher concentrations and volume of LB result in stronger and longer-lasting block effects in mice.
3.Potential value of HPV integration testing in a triage management for HPV-positive women

Objective:To investigate the dynamic characteristics of human papillomavirus (HPV) genomic integration during cervical lesion progression and the clinical value of HPV integration detection in stratify HPV-positive women, and to explore its molecular mechanisms in cervical carcinogenesis.Methods:A prospective cohort study was designed to enroll high-risk HPV (HR-HPV) positive women who underwent cervical cancer screening in Drum Tower Hospital Affiliated to Nanjing University Medical School and Nanjing Maternity and Child Health Care Hospital from July 2022 to July 2024. Cervical exfoliated cells samples were collected, and HPV whole genome targeted capture and high-throughput sequencing technology were used. The HPV integration patterns, host gene functional region distribution and pathway enrichment characteristics of 157 samples with different cervical lesions grades were analyzed, including 31 cases of normal cervix, 40 cases of cervical intraepithelial neoplasia (CIN) Ⅰ, 32 cases of CIN Ⅱ, 42 cases of CIN Ⅲ, and 12 cases of cervical cancer.Results:HR-HPV integration was detected in 80.2% (126/157) of the 157 HR-HPV positive samples. The incidence of HR-HPV integration in cervical cancer patients was 12/12, which was higher than that in normal women (77%, 24/31). The incidence of HPV16 integration was significantly higher in high-grade lesions, and the incidence of HPV16 integration was 43% (18/42) in CIN Ⅲ patients and 8/12 in cervical cancer patients ( P<0.001). A total of 14 438 integration events were detected in 126 samples with HPV integration. The integration sites were mainly distributed in the host intergenic region (51.0%, 7 359/14 438) and intronic region (38.1%, 5 494/14 438), and the integration frequency of viral L1 gene was the highest (28.4%, 4 498/16 781). Functional enrichment analysis showed that HPV integration-related host genes were significantly enriched in transport of small molecules,cyclic guanosine monophosphate (cGMP)-protein kinase G (PKG) signaling pathway, and purine ribonucleotide biosynthetic process, which synergistically drove carcinogenesis through multiple mechanisms. Conclusions:HPV integration events are significantly associated with the progression of cervical lesions. HPV integrated detection based on cervical exfoliated cells is expected to optimize the current screening strategy, reduce excessive intervention of HPV positive women and facilitate their accurate triage management.

8.Establishment of evaluation criterion of high-quality nursing services in comprehensive hospitals of Grade Three in Guangdong Province

Objective To establish an evaluation criterion for high-quality nursing services in comprehensive hospitals of Grade Three in Guangdong Province and provide references for promoting the evaluation of high quality nursing services and standardising high quality nursing services.Methods Based on literature review,brainstorming and references of relevant domestic systems,standards and policies,a pool of evaluation index items and a preliminary questionnaire were constructed.Delphi method was used for two rounds of expert consultations on evaluation indicators.Results The positive coefficients of the two rounds of expert consultations were of 80.00%and 91.67%,respectively,with the authority coefficients both at 0.93.The Kendall coordination coefficients for hospital part were 0.128 and 0.116 respectively,for ward part were 0.193 and 0.107 with statistical differences(both P<0.001).The evaluation criteria for high-quality nursing services in general hospitals above the third level in Guangdong Province involved in nursing services of hospitals and wards.There were 8 primary indicators,22 secondary indicators and 65 tertiary indicators in the nursing services at hospital part,and 9 primary indicators,23 secondary indicators and 50 tertiary indicators in the nursing services at ward part.Conclusions The evaluation criteria for high-quality nursing services in comprehensive hospitals above the third level in Guangdong Province,established in this study,are scientifically valid and reliable.They allow improvement of management system for the high-quality nursing services as well as guide the healthy development of high-quality nursing services.
9.Research progress in inhibitors of structural protein Gag-Pol of human immunodeficiency virus and its mechanism

Gag-Pol protein is one of the important structural proteins of human immunodeficiency virus(HIV),comprising the basic scaffold proteins and functional enzymes required during the lifecycle of HIV.Currently,the inhibitors targeting different functional domains on Gag-Pol include capsid(CA)inhibitors,protease inhibitors,reverse transcriptase inhibitors,and integrase inhibitors.The CA inhibitors inhibit the maturation of CA or disrupt the assembly of CA,so as to affect the replication of HIV.The primary mechanism of protease inhibitors is to inhibit the protease from cleaving at the cleavage site CA-spacer peptide 1(SP1).The reverse transcriptase inhibitors block the reverse transcription process of HIV by mimicking the reverse transcription substrates.The integrase inhibitors impact the activity of integrase by targeting the zinc-finger structure at the active center of integrase.This article summarizes the inhibitors targeting HIV Gag-Pol protein and their mechanisms,and reviews the approved dosages and usages of approved patent drugs,so as to provide the references for the future clinical combination therapies and the development of new inhibitors targeting HIV Gag-Pol protein.
10.Duration and risk factors of invasive mechanical ventilation after neonatal abdominal surgery under general anesthesia

Objective:To study the duration of invasive mechanical ventilation (MV) and its influencing factors after neonatal abdominal surgery under general anesthesia in neonatal intensive care unit (NICU).Methods:From January 2018 to December 2020, neonates received abdominal surgery under general anesthesia and needed endotracheal intubation and MV after surgery in NICU of our hospital were retrospectively studied. According to MV duration, the neonates were assigned into <72 h group and ≥72 h group. Multivariate logistic regression was used to analyze the risk factors of postoperative MV duration.Results:A total of 113 neonates were enrolled, including 57 male (50.4%) and 56 female (49.6%). The gestational age was (35.7±3.6) weeks, the birth weight was (2 497±933) g, the average operation age was 9.9(3.6, 22.2) d and the average hospital stay was 22.0(12.0,37.0) d. Congenital intestinal obstruction (37/113, 32.7%) was the most common diagnoses on discharge, followed by neonatal necrotizing enterocolitis(28/113,24.8%) and gastrointestinal perforation (18/113,15.0%). The duration of operation was 80.0 (55.8,117.3) min. All neonates needed MV with endotracheal intubation. The duration of postoperative respiratory support was 30.0(7.0,84.5) h. 48 neonates (42.5%) had endotracheal intubation removed within 24 h after surgery. Multivariate logistic regression analysis showed that preoperative respiratory support ( P=0.004), congenital heart disease( P=0.013) and intravenous midazolam ( P=0.032) were independent risk factors for prolonged postoperative MV. Conclusions:The need of preoperative respiratory support, congential heart disease and intravenous midazolam were independent risk factors for the duration of postoperative MV after neonatal abdominal surgery under general anesthesia.
