1.Serum levels of CGN and SDC-1 in patients with HBGH and their relationship with disease and disease outcome

Objective To explore the relationship between the expression levels of serum cingulate protein(CGN)and polyligand glycan 1(SDC-1)and the disease condition and outcome of hypertensive basal ganglia hemorrhage(HBGH).Methods A total of 123 patients with HBGH admitted to the Second People's Hospi-tal of Lianyungang from February 2019 to February 2022 were selected as the study objects,and 120 healthy volunteers who underwent physical examination in the hospital during the same period were selected as the health group.Serum CGN and SDC-1 expression levels were detected in the two groups.According to the dis-ease outcome,the patients were divided into the improved group(92 cases)and the deteriorated group(31 ca-ses).Receiver operating characteristic(ROC)curve and the area under the curve(AUC)were used to analyze the predictive value of serum CGN and SDC-1 expression levels on the disease outcome of patients with HB-GH.Results Serum CGN and SDC-1 expression levels in the severe group were higher than those in the mod-erate group and the mild group,and serum CGN and SDC-1 levels in the moderate group were higher than those in the mild group,and the differences were statistically significant(P<0.05).Serum CGN and SDC-1 expression levels in HBGH patients in three groups were higher than those in health group,and the differences were statistically significant(P<0.05).Serum CGN and SDC-1 expression levels in the deteriorated group were higher than those in the improved group,and the differences were statistically significant(P<0.05).The AUC of serum CGN and SDC-1 for predicting the disease outcome of HBGH patients was 0.742(95%CI:0.792-0.697)and 0.861(95%CI:0.906-0.910),respectively,and the AUC of the combination of the two was 0.917(95%CI:0.962-0.870).The amount of blood loss and ventricular rupture in the deteriorated group were higher than those in the improved group,and the Glasgow Coma Scale(GCS)score on admission was lower than that in the improved group,and the differences were statistically significant(P<0.05).Multi-variate Logistic regression analysis showed that serum CGN≥51.63 pg/mL(OR=3.815),serum SDC-1≥450.67 μg/L(OR=4.230)and GCS score ≤8(OR=5.333)were the influencing factors for disease outcome of HBGH patients(P<0.05).Conclusion The increased expression levels of serum CGN and SDC-1 are closely related to the disease aggravation and the deterioration of the disease outcome in patients with HBGH,and they have certain predictive value for the disease outcome in patients with HBGH.
2.Prevalence of dentinal hypersensitivity and related factors in Chinese urban adults

Objective:To assess the prevalence of dentinal hypersensitivity (DH) and related factors in urban adults in China.Methods:The study was designed as an observational, cross-sectional epidemiological study carried out in adults aged 18-69 years old in seven cities (Beijing, Shanghai, Wuhan, Chengdu, Xi′an, Guangzhou, and Harbin) of China. The study was conducted from March 2021 to May 2023. Patients were required to complete a questionnaire regarding the subjects′ socio-economic factors, dietary behavior, oral health behavior and personal antecedent factors. DH was clinically diagnosed by judging whether the tooth cold air stimulation provoked DH or not, and recorded by investigator pain rating Schiff score. Compare the findings of six cities (Harbin excluded) with a similar study conducted in 2008.Results:In total, 11 622 subjects from seven cities in China participated the study. Fifty two point two percent (6 072/11 622) of subjects reported DH in questionnaire, 36.7% (4 266/11 622) of subjects reported experiencing DH in response to cold air stimulation for at least one study tooth. Risk factors including age, sex, city, toothbrush method and acid reflux showed marked associations with DH ( P<0.05). The prevalence of DH of urban residents in six cities (Harbin excluded) was 33.7% (3 335/9 882), higher than that in 2008 [29.7%(2 354/7 939)]. Conclusions:Overall, DH was common among urban adults in China and the prevalence increased in recent years. Better understanding of DH and its associated factors should be considered in its prevention and management by dental professionals.

Objective To explore the value of 3D-ASL parameters for predicting glioma grade before operation and the relationship between the expression of Ki-67 and microvessel density(MVD).Methods 3DASL images of 50 patients with pathologically proven gliomas (high grade:n=20,1ow grade:n 30) were analyzed retrospectively.The mean maximum relative cerebral blood flow was obtained by the hot spot method.The Ki-67 labeling index and MVD of surgical specimen were measured.The results were analyzed statistically.Results The mean rCBF,Ki-67 and MVD of low grade gliomas (n=30) were 1.69±1.27,3.73%±3.01%,25.43±13.03,while the high grade gliomas (n=20) were 3.18± 1.46,49.25% ±20.15%,58.69±20.39 respectively.The difference between the two groups was statistically significant (P<0.01),which was positively correlated with the grade(r=0.620,0.871,0.698,P<0.01).rCBF was positively correlated with Ki-67 and MVD (r =0.535,0.723,P <0.01).Ki 67 labeling index was positively correlated with MVD (r=0.629,P<0.01).Conclusion The rCBF value is useful for predicting glioma grade before operation and related to tumor MVD and cell proliferation.The biologic behavior of glioma can be predicted to a certain extent.
5.Therapeutic effects of sodium nitroprusside combined verapamil for no-reflow during percutaneous coronary intervention

Objective:To explore therapeutic effects of sodium nitroprusside (SNP) combined verapamil on no-reflow during percutaneous coronary intervention (PCI).Methods: A total of 106 patients, who suffered from no-reflow during PCI in our department from Jan 2011 to Dec 2013, were selected.According to random number table method, patients were divided into SNP group (n=55, received SNP based on routine treatment) and combined treatment group (n=51, received verapamil based on SNP group).Cardiac troponin I (cTnI) level before and 16h~18h after PCI, cardiac function indexes after 12-month follow-up, incidence of major adverse cardiovascular events (MACE) were measured and compared between two groups.Results: Compared with before PCI, there were significant rise in cTnI level in both groups on 16~18h after PCI, P=0.001 both;compared with SNP group, there were significant reductions in cTnI level [(1.31±0.44)μg/L vs.(0.11±0.02)μg/L] and percentage of cTnI>0.10μg/L (94.5% vs.54.9%) in combined treatment group, P=0.001 both.Compared with SNP group after 12 months, there was significant rise in left ventricular ejection fraction [(62.29±3.06)% vs.(65.65±3.94)%], and significant reductions in left ventricular end-diastolic dimension[(50.24±3.73)mm vs.(47.60±4.72)mm] and left ventricular end-systolic dimension [(33.29±2.11)mm vs.(31.00±4.33)mm] in combined treatment group, P<0.05 all.There were no significant adverse reactions during hospitalization and follow-up in both groups.Conclusion: When no-reflow occurs during PCI, intracoronary injection of SNP combined verapamil can improve cardiac function, and its safety is good, which is worth extending.
6.Effect of Probucol on Enzyme and Receptors of High Density Lipoprotein During Reverse Cholesterol Transportation in Experimental Rabbits With Atherosclerosis

Objective: To explore the effect of probucol on enzyme and receptors of high density lipoprotein (HDL) during reverse cholesterol transportation in experimental rabbits with atherosclerosis. Methods: A total of 24 New Zealand rabbits were randomly divided into 3 groups: Control group, the rabbits were fed by normal diet, High cholesterol group, the rabbits were fed by high cholesterol diet and Probucol group, the rabbits were fed by high cholesterol diet with probucol.n=8 in each group, all animals were treated for 12 weeks. Blood levels of lipids were examined by colorimetric method, serum lecithin cholesterol acyltransferase (LCAT) and cholesterol ester transfer protein (CETP) were detected by ELISA, expressions of ATP binding cassette transporter A1 (ABCA1) and scavenger receptor class B type I (SR-BI) in aortic plaque were measured by immunohistochemistry; the above indexes were compared among different groups.
Results: ① for blood lipids by mmol/L, the following indexes in Probucol group were lower than High cholesterol group: TC (15.95±1.51 vs 21.95±3.71), LDL-C (13.01±2.28 vs 17.90±3.51), HDL-C (0.56±0.10 vs 1.13±0.12), all P<0.01.② the following perimeters in High cholesterol group were lower than Control group: for CETP by μg/ml (1.24±0.54 vs 2.07±0.64), for LCAT by μg/ml (15.02±3.81 vs 27.01±8.26), all P<0.05; compared with High cholesterol group, Probucol group had increased CETP (3.43±1.01) and LCAT (38.10±7.96), all P<0.05. ③ positive expressions of ABCA1 and SR-BI in aortic plaque in Probucol group were higher than High cholesterol group: for ABCA1 by % (46.81±10.01 vs 24.10±8.48), for SR-BI by % (48.04±10.90 vs 18.61±6.77), all P<0.01.
Conclusion: Probucol may increase blood levels of LCAT, CETP via up-regulating the expressions of ABCA1, SR-B1 and elevating the reverse cholesterol transportation of HDL, therefore improve HDL function in experimental rabbits with atherosclerosis.
7.Occurrence and prognosis of coronary slow flow in emergency percutaneous coronary intervention: correlations with homocysteine.

OBJECTIVETo investigate the correlation of the occurrence and prognosis of coronary slow flow phenomenon (CSF) with blood homocysteine (Hcy) levels in patients receiving emergency percutaneous coronary intervention therapy (PCI).
METHODSFrom January, 2010 to December, 2011, 138 patients with ST-elevation myocardial infarction received emergency angioplasty, among whom 46 patients developed CSF and 92 did not (control group). Blood Hcy levels were determined in these patients. The patients with CSF were classified into two groups with mild and moderate Hcy elevations (32 and 14 cases, respectively), and the left ventricular ejection fraction (LVEF) during hospitalization and at 3 months of follow-up as well as major adverse cardiac events (MACE) were compared between the two groups and analyzed for their association with Hcy level.
RESULTSThe patients with CSF showed significantly higher blood Hcy levels than the control patients (P=0.001). At 3 months of follow-up, the patients with CSF and moderate Hcy elevation had significantly lower LVEF (P=0.031) and higher incidence of MACE (P=0.019) than those with mild Hcy elevation. Hcy levels were negatively correlated with LVEF (r=-0.310, P=0.036) and positively with MACE (r=0.342, P=0.02).
CONCLUSIONA high blood Hcy level is closely correlated with the occurrence of CSF in emergency PCI, affects the recovery of LVEF and increases the incidence of MACE.

8.Effects of biatrial infusion on pulmonary artery pressure after cardiopulmonary bypass in patients undergoing mitral valve replacement

Objective To investigate the effects of biatrial infusion on pulmonary artery pressure (PAP)after cardiopulmonary bypass (CPB) in patients undergoing mitral valve replacement.Methods Twenty NYHA Ⅱ or Ⅲ patients aged 22-53 yr weighing 34-57 kg undergoing mitral valve replacement complicated by pulmonary hypertension (mean pulmonary artery pressure (MPAP) > 50 mm Hg) were randomly divided into 2 groups ( n = 10 each): infusion via right atrium group (group R) and infusion via both atria group (group B). After induction of anesthesia, a three cavity floating Swan-Ganz catheter was placed via right internal jugular vein to monitor CVP,PAP, pulmonary capillary wedge pressure (PCWP) and CO. The patients received infusion of prostaglandin E1 30-150 ng· kg- 1 · min - 1 and phenylephrine 0.2-0.6 μg· kg- 1 · min- 1 via central veins in group R and infusion of prostaglandin E1 30-150 ng·kg-1 ·min-1 via central veins and phenylephrine 0.2-0.6 μg·kg-1 ·min-1 via left atrium in group B. MAP, HR, MPAP, PCWP, CVP and CO were recorded 5 min before administration (T0), and 5,10, 30 and 60 min after administration (T1-4). Pulmonary vascular resistance index (PVRI), systemic vascular resistance index (SVRI) and CI were also calculated. Results D:\1111111111\MDB\zhmzxzz98201008.mdbCompared with the value at T0, MAP, MPAP, PCWP and PVRI were significantly decreased, while CI was increased at T1-4 in group R, and MAP, CI and SVRI were significantly increased, while HR, MPAP, PCWP, CVP and PVRI decreased at T1-4 in group B ( P < 0.05).MAP, CI and SVRI were significantly higher, while HR, MPAP, PCWP, PVRI and CVP lower in group B than in group R ( P < 0.05). Conclusion Biatrial infusion can effectively reduce PAP and pulmonary vascular resistance after CPB in patients undergoing mitral valve replacement.
9.Comparative analysis between ischemic colitis and ulcerative colitis

Objective To study the differences of clinical and histopathologic features between ischemic colitis (IC) and ulcerative colitis (UC), which can make for the differential diagnosis and treatments. Methods Comparative analysis were focused on the clinical and histopathologic data of IC patients (20 cases) and UC patients (30 eases). Results The onset duration of IC [(5±7) d] was significantly shorter than that of UC [(953±1354) d]. IC patients tended to have history of cadiovascular diseases or abdominal surgery (65%). The typical clinical manifestations of IC were sudden onset of abdominal pain (85%), hematochezia (60%)and diarrhea (50%), but UC usually presented with abdominal pain (83%), diarrhea (63%), bloody mucopurulent stool (57%) and tenesmus (20%). Colonoscopy showed longitudinal ulceration (60%)on the mesenteric side, the segmental lesions usually involved only 1/4-1/2 of the perimeter of the lumen (80%), and the boundary of lesions was clear (75%) in IC patients. On the contrary, the ulcer of UC was usually map-like (43%), the lesions usually involved the entire lumen (90%), and the pseudopolyp (37%) was another colonoscopic feature of UC. The histopathologic study revealed that dilation and hyperaemia of vessels (90%) were common in IC. Severe edema of mucosa (95%) was usually presented in IC, and the thickened vascular wall (50%) was the histopathologic characteristic of IC.Crypt abscesses (47%) usually appeared in UC. Conclusions The histopathologic characteristics and differences of colonoscopic findings and clinical features provide strong bases for the differentialdiagnosis between IC and UC.
10.Significance of VEGF-C and VEGF-D in lymphatic metastasis of pancreatic cancer

Objective To analyze the intratumoral and peritumoral microvessel density (MVD) and microlymphatic vessel density (MLVD) in pancreatic cancer and record the expression of vascular endothelial growth factor(VEGF)-C and VEGF-D. And to explore the significance of VEGF-C and VEGF-D during the lymphatic metastasis and development of pancreatic cancer. Methods The expression of VEGF-C and VEGF-D, VEGF-R3, CD34 were assayed by immunohistochemical staining in 30 cases of pancreatic cancer tissues. Results The positive rates of VEGF-C and VEGF-D protein in the central portion of tumors (30% and 16.7%) were significantly lower than those in the marginal portion (73.3% and 56.7%), P <0.01. The group with high expression of VEGF-C and VEGF-D in the marginal portion had significantly higher incidences of lymph node metastasis, lymphatic invasion and venous invasion( P <0. 01 ). MLVD in both of the VEGF-C and VEGF-D positive groups was higher than that in the negative groups( P <0. 01 ), and the lymph node me-tastasis increased. MVD in VEGF-C positive group was significantly higher than that in the negative group. MVD had no significant difference between VEGF-D positive and negative group ( P =0. 07). Conclusions The expression of VEGF-C and VEGF-D in the marginal portion of tumor is significantly correlated with lym-phatic metastasis in pancreatic cancer patients, and may induce lymphangiogenesis. VEGF-C may play an im-portant role in the regulation of angiogenesis and lymphangiogenesis in pancreatic cancer, and VEGF- D maybe only participate in the regulation of lymphangiogenesis.
