1.Biological and physicochemical properties of bioactive ion modified brushite cements

BACKGROUND:As a bone replacement and filling material,calcium phosphate stone bone cement has good biocompatibility,bone conductivity,and other advantages,especially its better biodegradability compared to other calcium phosphate bone cements.It has important application value in bone repair.However,due to its limitations such as insufficient mechanical properties,fast solidification reaction,and poor injection performance,it is currently only suitable for the repair of non weight-bearing bone. OBJECTIVE:To explore the modification of brushite cements with bioactive ions(metal and non metal ions)to expand its application range. METHODS:The author used PubMed,ScienceDirect,CNKI,and WanFang to search the literature published between 2018 and 2023 with the search terms"metal ion,iron,copper,strontium,magnesium,zinc,non-metal ion,modification,bone,brushite cements"in Chinese and"metal ion,iron,Fe,copper,Cu,strontium,Sr,magnesium,Mg,zinc,Zn,non-metal ion,modification,bone,brushite cements"in English.After reading titles and abstracts,the articles were initially screened,and irrelevant and duplicate articles were excluded.Finally,64 articles were included for review. RESULTS AND CONCLUSION:(1)Bioactive ions affect the hydration process of calcium phosphate bone cement.Different ions are substituted by ions and incorporated into the crystal structure of calcium phosphate bone cement,changing the crystal morphology of the cement and causing changes in physical and chemical properties such as setting time,injectability,and compressive strength.(2)Ionic modified calcium phosphate bone cement produces different ion release effects due to different crystal structures.Different types of ions have properties such as promoting angiogenesis/osteogenesis,antibacterial,anti-tumor,etc.In addition,calcium phosphate bone cement has good biodegradability,which has great advantages for the performance of various ions.(3)The physicochemical properties of calcium phosphate bone cement modified with different ions are as follows:iron,copper,strontium,magnesium,zinc,silver,and cobalt can prolong the setting time.Strontium,and magnesium can improve injection performance.Copper,strontium,magnesium,silver and silicon can enhance compressive strength.The ions that can simultaneously improve the three physical and chemical properties of calcium phosphate bone cement include strontium and magnesium.Good physical and chemical properties are a prerequisite for clinical application,so improving the setting time,injectability,compressive strength,and other properties of calcium phosphate bone cement with ions is of great significance for the research and application of bone cement.(4)The biological properties of calcium phosphate bone cement modified with different ions are as follows:copper,strontium,magnesium,zinc,cobalt,lithium,selenium,and silicon have promoting angiogenesis/osteogenic effects.Iron,copper,magnesium,zinc,and silver have antibacterial properties.Magnesium ions have anti-inflammatory properties.Copper and selenium have anti-tumor properties.(5)In summary,magnesium ions can improve the setting time,injectability,and compressive strength of calcium phosphate bone cement,while also promoting neovascularization/osteogenesis,antibacterial properties,and have good application prospects for the treatment of bone defects with concurrent infections.In addition,copper also has anti-tumor properties,so copper ions have great potential in the treatment of bone defects caused by infections and tumors.However,relevant research is still in the basic research stage,and the effects of different ion doping concentrations and synthesis conditions on the physicochemical properties of calcium phosphate bone cement need to be further explored.At the same time,the impact of biological properties also needs to be studied and observed for a longer period of time.
2.Prenatal diagnosis and genetic analysis of three fetuses with duodenal atresia or stenosis.

OBJECTIVE:
To explore the genetic basis for three fetuses with duodenal atresia or stenosis detected by ultrasonography.
METHODS:
Clinical data of three fetuses identified at the Women's Hospital Affiliated to Zhejiang University School of Medicine between January 2021 and August 2022 were collected. Umbilical cord blood and amniotic fluid samples of the fetuses and peripheral blood samples of their parents were collected and subjected to G-banded chromosomal karyotyping and single nucleotide polymorphism array (SNP array) analysis.
RESULTS:
Prenatal ultrasound of the three fetuses revealed duodenal atresia or stenosis. No karyotypic abnormality was detected, whilst SNP array has identified 1.4 ~ 1.9 Mb duplications at 17q12 in all of them, which were all predicted to be pathogenic copy number variations (CNVs).
CONCLUSION
The 17q12 duplications probably underlay the duodenal atresia and stenosis in these fetuses, and chromosomal CNVs should be considered in duodenal atresia and stenosis.

3.Identification and optimization of peptide inhibitors to block VISTA/PSGL-1 interaction for cancer immunotherapy.

Developing new therapeutic agents for cancer immunotherapy is highly demanding due to the low response ratio of PD-1/PD-L1 blockade in cancer patients. Here, we discovered that the novel immune checkpoint VISTA is highly expressed on a variety of tumor-infiltrating immune cells, especially myeloid derived suppressor cells (MDSCs) and CD8+ T cells. Then, peptide C1 with binding affinity to VISTA was developed by phage displayed bio-panning technique, and its mutant peptide VS3 was obtained by molecular docking based mutation. Peptide VS3 could bind VISTA with high affinity and block its interaction with ligand PSGL-1 under acidic condition, and elicit anti-tumor activity in vivo. The peptide DVS3-Pal was further designed by d-amino acid substitution and fatty acid modification, which exhibited strong proteolytic stability and significant anti-tumor activity through enhancing CD8+ T cell function and decreasing MDSCs infiltration. This is the first study to develop peptides to block VISTA/PSGL-1 interaction, which could act as promising candidates for cancer immunotherapy.

Tryptophan 2,3-dioxygnease 2 (TDO2) is specific for metabolizing tryptophan to kynurenine (KYN), which plays a critical role in mediating immune escape of cancer. Although accumulating evidence demonstrates that TDO2 overexpression is implicated in the development and progression of multiple cancers, its tumor-promoting role in esophageal squamous cell carcinoma (ESCC) remains unclear. Here, we observed that TDO2 was overexpressed in ESCC tissues and correlated significantly with lymph node metastasis, advanced clinical stage, and unfavorable prognosis. Functional experiments showed that TDO2 promoted tumor cell proliferation, migration, and colony formation, which could be prevented by inhibition of TDO2 and aryl hydrocarbon receptor (AHR). Further experimentation demonstrated that TDO2 could promote the tumor growth of KYSE150 tumor-bearing model, tumor burden of C57BL/6 mice with ESCC induced by 4-NQO, enhance the expression of phosphorylated AKT, with subsequent phosphorylation of GSK3
5.Exploratory study on the evaluation dimension and index of research hospital

Objective:To explore the evaluation dimensions and indicators of research hospital, provide empirical evidence for the construction of research hospital.Methods:Non-probability sampling was adopted, physicians from 9 hospitals in Shanghai that have a National Clinical Medical Research Center or Shanghai Clinical Medical Research Center were invited as survey participants. The physician participants were divided into senior and junior groups. For senior group, a questionnaire including 5 dimensions and 21 indicators was provided. The survey participants were consulted to determine whether the indicators can be used to measure this dimension, and also invited to propose additional dimensions and indicators for improvement.For junior group, two open questions were proposed to explore their needs of support in both clinical service and research.Results:Based on suggestions from the research participants, the research team, and other expert consultant, this article tried to propose 5 dimensions and 21 indicators for evaluating research hospitals. Among them, 11 were indicators compiled by the research team based on the literature review and agreed by more than 2/3 of senior physicians surveyed, while others were proposed based on the survey results. The research team planned to use the entropy method to determine the weights of different indicators, thus, the participants were not required to make judgments on the weights of dimensions and indicators.Conclusions:The evaluation of research hospitals has not yet formed a systematic consensus. Through the exploration and establishment of evaluation dimensions and indicators, identify hospitals that are closer to the development goals of research hospitals can provide a basis for future next-step practices.
6.Establishment of Fingerprints and Comparative Study on Ginsenoside Content of Panax notoginseng Flower with Different Growing Years

OBJECTIVE:To compare chemical composition types and ginsenoside content of Panax notoginseng flowers with different growing years ,and to explore the effect of growing year on the quality of P. notoginseng flowers. METHODS :Each 10 batches of biennial,triennial and quadrennial P. notoginseng flower were collected and determined by HPLC. The determination was performed on Shim-pack GIST C 18 column with mobile phase consisted of acetonitrile- 0.05% phosphoric acid solution (gradient elution )at the flow rate of 0.5 mL/min. The column temperature was set at 30 ℃,and the detection wavelength was set at 203 nm. The sample size was 20 μL. Similarity Evaluation System of TCM Chromatogram Fingerprint was used to establish the fingerprint of 30 batches of samples ,identify the diagnostic components and analyze the similarity. Cluster analysis was conducted by using SPSS 22.0 software. The contents of ginsenoside Rb 1,Rb2,Rb3 and Rc in 30 batches of P. notoginseng flower with different growing years were determined by above HPLC . The quality control analysis was conducted by using SPSS 22.0 software. RESULTS:Established fingerprint showed good precision ,stability and reproducibility. There were good linear relationship (R2> 0.999),quantitative limit ,precision,stability,repeatability and accuracy of the content determination method . The similarity among samples with different growing ye ars was more than 0.817. Thirty batches of P. notoginseng flower could be grouped into 4 categories,the category Ⅱ was quadrennial samples,the category Ⅲ was triennial samples ,while the categories Ⅰ and Ⅳ were mostly biennial samples and a small number of triennial and quadrennial samples. RSDs of 4 ginsenosides contents and their total contents in biennial samples were 8.90%-21.43% and total saponin contents were 11.65%-17.76%,respectively. RSDs of 4 ginsenosides contents and their total contents in triennial samples were 6.45%-14.23%,and total saponin contents were 15.74%-19.30%. RSDs of 4 ginsenosides contents and their total contents in quadrennial samples were 7.50%-18.86%,and total saponin contents were 15.92%-20.16%. The results of quality control analysis showed that biennial samples mainly distributed in the areas of Ⅱ and Ⅲ ;triennial and quadrennial samples mainly distributed in the areas of Ⅰ and Ⅱ ;the order of ginsenosides content was Ⅰ >Ⅱ >Ⅲ. CONCLUSIONS:Chemical components of P. notoginseng flower with different growing years are generally close in types but there still a re some differences ,among which the content of ginsenosides in biennial samples is lower ,fluctuates more ,and the overall quality is slightly poor ;the content of ginsenosides in triennial and quadrennial samples is higher ,fluctuates less ,and the overall quality is higher and tends to be stable.
7.Epidemiology, clinical and imaging features of pediatric patients with COVID-19: analysis of 14 cases

Epidemiologic, clinical and imaging data were collected from 14 children with confirmed coronavirus disease 2019 (COVID-19) admitted in Beijing Ditan Hospital from January 27, 2020 to February 12, 2020. There were 6 boys and 8 girls with a median age of 3.5 years (6 months-9.4 years). Four patients had a history of travel to Wuhan City or Hubei Province and 2 patients had contacted with people from Wuhan; 13 patients were familial cluster of infection. The incubation period was 4 to 16 days. The clinical manifestations were fever in 8 cases, cough in 5 cases, diarrhea in 1 case; and 2 cases were asymptomatic. Four patients had abnormal peripheral blood routine, including 1 had lymphocytosis, 3 had lymphocytopenia; 3 patients had a slightly elevated CRP, and 3 patients had hepatic dysfunction. Thirteen patients underwent chest CT; and 1 case showed bilateral lung glass exudation, 1 case showed multiple patchy high density shadows of bilateral lung. One patient underwent chest X-ray examination, which was showed no abnormal findings. The pediatric patients with COVID-19 in this series generally have a traceable epidemiological history. The clinical manifestations are fever, cough and diarrhea. Peripheral white blood cell counts were most normal. Chest CT reveals less severe changes than those in adults, most child patients show no manifestation of pneumonia.
9.Clinical Efficacy of Yiqi Huoxue Decoction in the Treatment of Children with Spleen and Kidney Deficiency Type Nephrotic Syndrome

Objective:To study the clinical efficacy ofYiqi Huoxue Decoction in the treatment of children with spleen and kidney deficiency type nephrotic syndrome.Methods:A total of 80 children with spleen and kidney deficiency type nephrotic syndrome in our hospital from June 2014 to October 2016 were enrolled in this study.The subjects were divided into control group (n=40) and treatment group (n=40) according to the random number table method.The control group was treated with prednisone,and the treatment group was treated with prednisone combined with Yiqi Huoxue Decoction,the two groups were treated for 4 months.The clinical efficacy of the two groups were evaluate.The 24h proteinuria,total cholesterol (TC),plasma albumin (Alb),and renal function parameters including blood urea nitrogen (BUN),serum creatinine (Scr),and serum creatinine clearance (Ccr) of the two groups before and after treatment were compared.Results:The total effective rate of the treatment group was 95.00%,which was significantly higher than 67.50% of the control group,the difference was statistically significant (P<0.05).The 24h proteinuria,TC,BUN and Scr of the two groups after treatment were significantly lower than before treatment,the Alb,Ccr of the two groups were significantly higher than before treatment,and above indexes in treatment group were better than the control group,the differences were statistically significant (P<0.05).Conclusion:Yiqi Huoxue Decoction have good clinical efficacy in the treatment of children with spleen and kidney deficiency type nephrotic syndrome,which can obviously improve children renal function,and is worthy of clinical application.
10.Neuroprotection of ligustrazine hydrochloride combined with bone marrow mesenchymal stem cell transplantation in rats with spinal cord injury

BACKGROUND:Ligustrazine hydrochloride which promotes nerve repair can be applied to the treatment of nervous system injury. OBJECTIVE:To investigate the effect of ligustrazine hydrochloride combined with bone marrow mesenchymal stem cels transplantation on electrophysiological property and hindlimb function of rats with spinal cord injury. METHODS:T9 spinal cord transection injury models were made in rats using Alen’s method, and then rat models were randomized into three groups: rats in control group received tail vein injection of culture solution; rats in cel transplantation group underwent bone marrow mesenchymal stem cel transplantationvia the tail vein; rats in combined group were subjected to the tail vein injection of ligustrazine hydrochloride and bone marrow mesenchymal stem cels that lasted for 4 hours. At 1, 2, 4, 6, 8 weeks after modeling, Basso, Beattie and Bresnahan scores and modified Tarlov scores were used to detect the motor function of rats. At 72 hours after modeling, RT-PCR method was used to detect the expression of Bcl-2 and basic fibroblast growth factor around the injured region. At 4 weeks after modeling, somatosensory and motor evoked potentials were measured for evaluation of neurophysiological recovery. At 8 weeks after modeling, horseradish peroxidase tracer was used to assess the regeneration of rat spinal cord nerve fibers; PKH-26 labeling was used to observe the survival and migration of transplanted cels. RESULTS AND CONCLUSION:At 1, 2, 4, 6, 8 weeks after modeling, Basso, Beattie and Bresnahan scores and modified Tarlov scores were significantly higher in the combined group than the cel transplantation folowed by the control group (P < 0.05). At 72 hours after modeling, the expression of Bcl-2 and basic fibroblast growth factor around the injured region was significantly higher in the combined group than the cel transplantation group and control group (P < 0.05). At 4 weeks after modeling, the latencies of somatosensory and motor evoked potentials were ranked as folows: combined group < cel transplantation group < control group (P < 0.05); the amplitudes of somatosensory and motor evoked potentials were ranked as folows: combined group > cel transplantation group > control group (P < 0.05). At 8 weeks after modeling, horseradish peroxidase-labeled pyramidal cels in the cel transplantation group and combined group showed apparent crossing signs; the number of PKH-26-positive cels and horseradish peroxidase-positive cels was the most in the combined group folowed by the cel transplantation group, and was the least in the control group (P < 0.05). These findings indicate that ligustrazine hydrochloride combined with bone marrow mesenchymal stem cels transplantation can facilitate nerve cel regeneration, promote the expression of Bcl-2 and basic fibroblast growth factor, and improve motor function in rats after spinal cord injury.
