1.Application of extended antigen matching in transfusion for patients with autoimmune hemolytic anemia

Objective: To evaluate the clinical value of extended red blood cell (RBC) antigen matching in transfusion for patients with autoimmune hemolytic anemia (AIHA). Methods: Clinical transfusion procedures involving extended RBC antigen matching for cross-compatibility testing were analyzed in two AIHA cases. Results: Following the implementation of extended RBC antigen matching (four-tier matching), the major cross-match reaction results shifted from strongly positive to weakly positive. The hemoglobin (Hb) levels increased significantly after transfusion, with no observed hemolytic transfusion reactions (HTRs). Conclusion: Extended RBC antigen matching may provide a safe and effective transfusion strategy for AIHA patients.

POEMS syndrome is a rare condition associated with plasma cell disorders, and it often involves multiple systems and has diverse clinical manifestations. This article reports two cases of POEMS syndrome with hepatosplenomegaly as the initial manifestation. During the course of the disease, the patients presented with lower limb weakness, hepatosplenomegaly, lymph node enlargement, ascites, hypothyroidism, positive M protein, and skin hyperpigmentation, and 18F-FDG PET-CT imaging revealed bone lesions mainly characterized by osteolytic changes and plasma cell tumors. There was an increase in the serum level of vascular endothelial growth factor. The patients were finally diagnosed with POEMS syndrome, and the symptoms were relieved after immunomodulatory treatment.
3.Ginkgo biloba extract inhibits the proliferation and migration of human glioma cell line U87

Objective To find potential effect of Ginkgo biloba extract on proliferation and migration of human gli-oma cell line.Methods Glioma cell line U87 was cultured and incubated with Ginkgo biloba extract at doses of 0,10,50,and 100 μg/mL,respectively.The proliferation activity of the cells in each group was detected by 5-ethynyl-2'-deoxyuridine(EDU)experiment,the migration activity of the cells in each group was examined by scratch experiment,the invasion activity of the cells in each group was detected by Transwell experiment and the expression of epithelial-mesenchymal transition(EMT)-related proteins,phosphatidylinositol-3-kinase(PI3K)/protein kinase B(AKT)signaling pathway proteins and E2F transcription factor 1(E2F1)protein in each group of the cells were detected by Western blot.Reply experiment was added with PI3K/AKT pathway activator 740 Y-P.Results Ginkgo biloba extract at concentrations of 10,50,and 100 μg/mL significantly inhibited the proliferation,migration,and invasion of U87 cells(P<0.05).The protein level of vimentin,N-cadherin,p-PI3K,p-AKT and E2F1 was significantly decreased(P<0.05),while the protein level of E-cadherin and ZO-1 was significantly increased(P<0.05).After addition of 740 Y-P into the cultural system,the inhibitory effect of Ginkgo biloba extract on the proliferation and metastasis of U87 cells was inhibited and the protein level of vimentin,N-cadherin,p-PI3K,p-AKT,and E2F1 increased(P<0.05),while the protein level of E-cadherin and ZO-1 was decreased(P<0.05).Conclusions Ginkgo biloba extract may inhibit proliferation,migration and EMT of U87 cells,which is potentially related to the PI3K/AKT/E2F1 pathway.
4.Application of ASSR with different stimuli in hearing impaired children's hearing threshold assessment

OBJECTIVE To investigate the application value of narrow-band CE-Chirp ASSR(NB CE-Chirp ASSR)and modulated acoustic ASSR in the assessment of hearing threshold in children with hearing impairment.METHODS Forty-eight children with sensorineural hearing loss were tested by pure tone audiometry(PTA),NB CE-Chirp ASSR and modulated acoustic ASSR.According to the results of pure tone audiometry,they were divided into mild to moderate group(48 ears)and severe to profound group(48 ears).The difference and correlation between pure tone hearing threshold and ASSR response threshold of different stimuli were compared between 500-4 000 Hz.RESULTS The absolute differences between the NB CE-Chirp ASSR response threshold and the PTA threshold in 48 children were all smaller than the differences between the modulated acoustic ASSR response threshold and the PTA threshold,and the differences were statistically significant(P<0.001).Under the same stimulus sound,the absolute difference between ASSR response threshold and pure tone hearing threshold in the mild to moderate group was higher than that in the severe to very severe group.The correlation coefficients between NB CE-Chirp ASSR threshold and pure tone hearing threshold are higher than those between modulated sound ASSR threshold and pure tone hearing threshold at 500-4 000 Hz.The test time of NB CE-Chirp ASSR[(20.92±9.33)min]was significantly shorter than that of modulated acoustic ASSR[(33.68±10.97)min](P=0.004).CONCLUSION NB CE-Chirp ASSR can more accurately assess the hearing threshold of children with different degrees of hearing loss than modulated acoustic ASSR.
5.Osteolipoma of thyroid gland:a clinicopathological study of 7 cases

Purpose To investigate the clinicopathologic features and imaging characteristics of osteolipoma in thyroid gland,and to improve the understanding and diagnosis of the disease.Methods The clinical,pathological,and imaging data of 7 cases of osteolipoma in thyroid gland were retrospectively analyzed,and the histological morphol-ogy of thyroid osteolipoma was observed using hematoxylin-eosin(HE)staining,the pathological changes and imaging manifestations of the disease were analyzed,and relevant literature was reviewed.Results The study included 2 male and 5 female patients,with a male-to-female ratio of 1∶2.5.The patients were ranged in age from 30 to 77 years,with a mean age of 54.3 years.The clinical symptoms were mainly anterior cervical tumor.Laboratory examination showed normal thyroid function 5 cases(5/7),slightly decreased thyroid stimulating hormone(TSH)in 1 case(1/7),in-creased free triiodothyronine(FT3)and decreased TSH in 1 case(1/7).Preoperative ultrasound examination(7/7)showed solid nodules in the thyroid gland,with smooth edges,hypoechoic solid components,and coarse calcifications in the nodules;X-ray computed tomography(CT)examination(2/7)showed thyroid mixed density nodules with clear boundaries.Histological morphology showed that the tumor was composed of mature adipose tissue and bone tissue with different proportions,and the cells exhibited no atypia or necrosis,and mitotic figures were rare.Two cases were com-plicated with papillary thyroid carcinoma and 3 cases with nodular goiter.All cases received surgical treatment,and were followed up from 11 to 42 months,without tumor recurrence.Conclusion Osteolipoma of thyroid gland is rare in clinic and has relatively specific pathological characteristics and imaging changes,aiming to improve the understanding of the pathological and imaging manifestations of osteolipoma.

7.Application of hypotension prediction index in intraoperative hemodynamic management of robot-assisted laparoscopic cystectomy:A case report and literature review

Objective:To analyze the intraoperative hemodynamic management by hypotension prediction index(HPI)in one patient underwent robot-assisted laparoscopic cystectomy,and to provide the reference for anesthesia monitoring and hemodynamic management in the similar major surgery.Methods:The clinical data,intraoperative hemodynamic data,usage and dosage of vasoactive drugs,and clinical outcomes of one patient underwent robot-assisted laparoscopic cystectomy with HPI-guided intraoperative hemodynamic management were retrospectively analyzed,and the relevant literatures were reviewed.Results:The patient,a 72-year-old female,was admitted due to macroscopic hematuria for 5 months accompanied by dysuria for 3 months.The cystoscope results showed a 7 cm× 7 cm× 5 cm mass on the right side of the bladder trigone and a 4 cm × 3 cm × 3 cm mass near the bladder neck.The positron emission tomography/computed tomography(PET/CT)results showed thickening of the right posterior bladder wall with high metabolism,and the preliminary diagnosis was bladder malignancy.After preoperative anesthesia evaluation,the robot-assisted laparoscopic cystectomy was planned.After entering the operating room,the routine monitoring was conducted,and the monitor equipped with HPI software was used to guide intraoperative hemodynamic management.After routine anesthesia induction,the tracheal intubation was performed by video laryngoscope.The patient experienced intraoperative hypotension(IOH)for six times,the cumulative time of mean arterial pressure(MAP)<65 mmHg was 13.7 min,accounting for 4.40%of the anesthesia duration,and the time-weighted average of MAP<65 mmHg was 0.28 mmHg.The time range with HPI≥85 roughly overlapped with and included the period of MAP<65 mmHg.At 146 time points with HPI≥85,the MAP remained greater than 65 mmHg at 68.5%(100/146)of the points.At 47 time points with MAP<65 mmHg,HPI≥85 occurred at 97.9%(46/47)of the points.On the first postoperative day,the patient's hypersensitive cardiac troponin I was<0.01 μg·L-1,and no perioperative adverse events occurred.The patient was discharged on the eighth day.Conclusion:HPI can promptly and accurately predict the occurrence of IOH in the patients undergoing robot-assisted laparoscopic cystectomy.The use of HPI-based hypotension correction strategies during surgery can maintain the time-weighted average of MAP<65 mmHg at a lower level.
8.Study on the predictive value of the CT pulmonary angiography parameters from the 2022 ESC/ERS guidelines for chronic thromboembolic pulmonary hypertension

Objective To assess the predictive value of computed tomography pulmonary angiography(CTPA)cardiovascular parame-ters for chronic thromboembolic pulmonary hypertension(CTEPH)under the 2022 European Society of Cardiology/European Respiratory Society(ESC/ERS)guidelines,and to compare with the 2021 Chinese guidelines.Methods A total of 201 suspected CTEPH patients were retrospectively selected.All patients underwent right heart catheterization(RHC)and CTPA evaluation.According to the Euro-pean guidelines,they were divided into three groups:mean pulmonary artery pressure(mPAP)≤20 mmHg control group(63 cases)(1 mmHg=0.133 kPa),mPAP>20 mmHg CTEPH group(138 cases),and mPAP≥25 mmHg CTEPH group(123 cases).Inter-group comparison of CTPA cardiovascular parameters was performed,and receiver operating characteristic(ROC)curve analysis was performed for each parameter.Results Under the 2022 European guidelines,the diagnostic efficacy of the diameter of the main pulmonary artery trunk(MPAd)was the highest[area under the curne(AUC)was 0.933].The binary logistic regression analysis revealed that the MP Ad,right ventricular free wall thickness(RVWT),and interventricular septal angle(IVSA)were inde-pendent risk factors for the diagnosis of CTEPH(P<0.05).Under both the Chinese and European guidelines,the MPAd,the transverse diameter and area of the right atrium,the transverse diameter and area of the bi-ventricle,the RVWT,and the IVSA showed significant statistical differences in CTEPH and control groups(P<0.05).Conclusion Under both the Chinese and European guidelines,the MP Ad measured by CTPA has the highest diagnostic efficacy for CTEPH,while the IVSA has the strongest correlation with clinical prognostic indicators.The right atrium structure also has evaluation value.
9.Research progresses in correlations of TIPS related hepatic encephalopathy and gut microbiota in patients with cirrhotic portal hypertension

Cirrhotic portal hypertension could complicate impaired intestinal barrier function,small intestinal bacterial overgrowth and dysbiosis of intestinal microbiota,further exacerbate the severity of cirrhosis.TIPS might lead to hepatic encephalopathy(HE).The research progresses in correlations of cirrhotic portal hypertension,TIPS related HE and gut microbiota were reviewed in this article.
10.Effect of RBM20 and MURC digenic heterozygosity variation on the structure and biological characteristics of myocardial cells

Objective To investigate the impact of variations in RNA-binding motif protein 20(RBM20)and muscle-restricted coiled-coil(MURC)digenic heterozygosity variation on the structural and biological characteristics of human cardiomyocyte AC 16(an adult left ventricular myocardial cell line).Methods Cardiomyocyte AC 16 cell lines were constructed with control,negative scramble,wild-type,MURC single-gene mutant,RBM20 single-gene mutant,and RBM20 and MURC digenic mutant groups.The localization of RBM20 and MURC in cardiomyocytes,cell area,cytoskeletal arrangement,cytoskeleton-related proteins,cell polarity,and intracellular calcium concentration were observed using Western blotting,immunofluorescence staining,and reverse transcription polymerase chain reaction.Myocardial apoptosis was detected using flow cytometry.Ki-67 staining and wound healing assays were performed to detect cardiomyo-cyte proliferation and migration,respectively.Results Digenic mutations had a more pronounced impact than single-gene mutations in RBM20 or MURC on the structural and biological characteristics of cardiomyocytes,manifested by increased cell area,upregulated mRNA expression of hypertrophy-related genes,such as myosin heavy chain 7 and alpha-actin,increased cytoskeleton disturbance,decreased flu-orescence intensity of cytoskeletal proteins β-tubulin and Vinculin(all P<0.01);increased fluorescence intensity of the polarity protein Part 6(P<0.05);and significantly elevated cardiomyocyte apoptosis rate,decreased proliferative activity,and elevated migration rate and intracellular calcium ion concentration(all P<0.01).Conclusion The digenic heterozygous variation in RBM20 and MURC may induce changes in the morphological structure and biological characteristics of myocardial cells,including increased cell area,cytoskeleton dis-turbance,cell polarity,increased apoptosis rate and mobility,decreased cell proliferation activity,and calcium processing ability.
