1.Follow up study of the association between bedroom light at night exposure and body mass index in children

Objective:
To explore the association between bedroom light at night (LAN) exposure and body mass index (BMI) in children at 1 year follow up, so as to provide new strategies for obesity prevention.
Methods:
From December 2021 to May 2022, cluster random sampling was conducted, involving 648 children from two primary schools in Tianchang, Chuzhou City, Anhui Province, China, to assess bedroom LAN exposure of children during sleep. A questionnaire survey and physical examination were carried out in May 2022. Multivariate linear regression was performed to analyze the correlation between bedroom LAN exposure and BMI variable quantity at 1 year follow up (May, 2023).
Results:
The median intensity of bedroom LAN exposure during the sleep episode was [1.11(0.35,3.24)lx] in children. The proportion of the sample exposed to an average light intensity of ≥3 lx was 27.5%, while 19.0% was exposed to a LAN intensity of ≥5 lx during the sleep episode. In the multivariable linear regression, after adjusting for covariates, including sex, baseline age, sleep duration, family monthly income, and maternal education level, exposure to a 1 h-average post bedtime LAN intensity of ≥3 lx ( β=0.25, 95%CI =0.05-0.44) and LAN≥5 lx ( β=0.34, 95% CI = 0.12-0.55) was associated with a gain of 0.25 and 0.34 kg/m 2, respectively, in the children s BMI at the 1 year follow up ( P < 0.05).
Conclusions
A positive correlation was found between bedroom LAN exposure and BMI variable quantity at 1 year follow up in children. Thus, reduced bedroom LAN exposure might be useful for interventions aimed at obesity prevention.
2.Xuandi Ziyin Mixture (玄地滋阴合剂) for Central Precocious Puberty in Girls with Syndrome of Yin Deficiency and Fire Exuberance: A Prospective Cohort Study

ObjectiveTo observe the clinical effectiveness and safety of Xuandi Ziyin Mixture (玄地滋阴合剂) for central precocious puberty (CPP) in girls with syndrome of yin deficiency and fire exuberance, and to analyse the effect of body mass index (BMI) on the effectiveness. MethodsA total of 236 girls with CPP of yin deficiency and fire exuberance syndrome were included, and all of them were given Xuandi Ziyin Mixture, 30 ml each time, twice a day, for a total treatment period of 6 months. Before and after treatment, children's weight, height and bone age were measured, BMI and BMI Z-score (BMI Z) and the difference between bone age and actual age were calculated; ultrasound was used to detect uterine and ovarian sizes, and to calculate uterine volume (Vuterus), bilateral ovarian volume (Vleft ovary, Vright ovary), and bilateral maximal follicle diameters (rleft follicle and rright follicle); and serum sex hormones were measured, including follicle-stimulating hormone (FSH), luteinising hormone (LH), prolactin (PRL), estradiol (E2), and testosterone (T), and were scored for traditional Chinese medicine (TCM) syndrome. Multiple linear regression was used to analyse the influence factors of the difference between bone age and actual age, and changes in uterine volume. The children were divided into the normal weight group and the overweight/obesity group according to baseline BMI, and the bone age, the difference between bone age and actual age, Vuterus and BMI Z scores before and after treatment were compared between the two groups. ResultsFinally, 199 children entered the statistical analysis. Compared with pre-treatment, the bone age, BMI and BMI Z scores of the children increased after treatment, and the difference between bone age and actual age, TCM syndrome scores, Vuterus, Vleft ovary, Vright ovary, rleft follicle and rright follicle decreased; and the levels of serum FSH, LH, E2, and T significantly decreased (P<0.05 or P<0.01). The difference between bone age and actual age was negatively correlated with LH and Vuterus (P<0.05), and changes in uterine volume were positively correlated with LH (P<0.01). Comparing between the groups before and after treatment, the bone age, difference between bone age and actual age, and BMI Z scores of children in the normal weight group (100 cases) were significantly smaller than those in the overweight/obesity group (99 cases) (P<0.01). 3.Development of Questionnaire for Parental Acceptability of School based Depression Symptom Screening

Objective:
The study aimed to develop Questionnaire for Parental Acceptability of School based Depression Symptom Screening by Delphi method, so as to provide a reference and suggestions for promoting screening adolescents depression.
Methods:
The study was conducted through the literature review and two rounds of the Delphi expert consultation. The initial construction of scale dimension and item was conducted through the literature search. A survey of 12 experts from 6 different provinces, cities and regions in China was conducted, and the Delphi technique was used to collect and analyze expert opinions data about the importance and feasibility of questionnaire content. The dimensions and specific items of the questionnaire were finally determinded.
Results:
The response rates were 100% in both rounds of Delphi consultation. Kendall coordination degree W =0.22 ( χ 2=63.72, P <0.01), which showed the agreement of expert opinions. The initial scale consisted of 4 dimensions and 17 items, including parental cognition of school based depression screening, parental cognition of school depression screening results feedback and management, parental attitude of school based depression screening, parental utilization of mental health services. After two rounds of expert consulattion, the final scale consisted of 3 first grade dimensions including parental cognition of depression symptom, parental attitude of school based depression symptom screening and parental mental health promotion behaviour, and also cinsisted of 4 second grade dimensions including parental knowledge of depression symptom, parental attitude of school based depression symptom screening, parental choice preference of school based depression symptom screening, parental utilization of mental health services, which had a total of 25 items.
Conclusion
The questionnaire is developed for assessing parental acceptability of school based depression screening and associated factors. The finding will facilitate mental health services providers in the screening implementation, but further empirical research is still needed.
4.An interview survey on the development of family doctor work mode in Shanghai Xuhui district

Objective:To survey the status quo of family doctor work mode in Shanghai Xuhui district.Methods:Semi-structured and structured in-depth interviews were conducted in Shanghai Xuhui district from April to June 2021, 11 directors, 12 deputy directors in charge and 30 family doctors from 12 community health service centers participated in the survey and completed two stages and four sessions of interviews on the development of the integrated and high-quality family doctor work mode and the ways to realize and the challenges to face. The records of interviews were transcribed, sorted and analyzed using the Colaizzi 7-step analysis method.Results:The survey showed that the family doctor contracting was carried out in a large team mode in the whole district, mainly for the elderly, and most of contracted residents were not included in health management and follow-up services. The team was composed of family doctors and assistants, and the routine outpatient service was the main work pattern, and the regular services also included the chronic disease follow-up and health check-up for elderly. On the issue of how to output high-quality integrated services, the majority of doctors (12/13) believed that the contracted individual should be taken as the unit of fine service, carrying out overall health assessment, optimizing medication plan, lifestyle guidance, one-stop service in hospital, etc.; only one doctor suggested that the family should be the management unit. For upgrading the working mode and service quality, insufficient time and energy were the main obstacle. Public health work occupied a lot of working time, but it seemed not be transformed into favorable resources and conveniences in health management and services. The professional assistants should carry out some responsibility to save family doctor′s time. The survey suggests that informatization, service space, and sufficient drug supply are the keys for ensuring high-quality and high-efficiency integrated services.Conclusion:The organizational structure of the family doctor team in Shanghai Xuhui district is relatively mature, but the integrated and high-quality service output has not yet reached. It is necessary to make regional overall planning and increase efforts to achieve the integration of medical treatment and prevention, so as to gain time for family doctors to carry out high-quality services. At the same time, it is also necessary to cultivate effective family doctor assistants, provide an information work platform that matches the work attributes and goals of family doctors, open up an integrated health management service space, and ensure the full range supply of drugs.
5.Design of wearable auxiliary device based on upper limb lifting workers and ergonomics simulation analysis

In order to solve the problem of high incidence of work-related musculoskeletal disorders (WMSDs) in upper limb lifting workers, a wearable auxiliary device which could be used in upper limb lifting operation was designed. The auxiliary device could be used in upper limb lifting through torsion spring device to reduce the tension of the arm muscles. The back bracket could be adjusted to adapt to the wearers of different height. The 3D model of the auxiliary device was constructed by using SoildWorks software. And the relative position and constraint between the auxiliary device and the digital simulation human were adjusted by using Jack as the main simulation tool. And then the virtual model of upper limb lifting were simulated and analyzed. Through the comparison of Rapid Upper Limb Assessment (RULA) scale, Krist comfort score and simulation results of two-handed reachable domain before and after wearing the auxiliary device, which proved that the wearable auxiliary device could reduce the risk of WMSDs in upper limb lifting workers.

8.Effects of oxycodone,sufentanil and fentanyl on immune function in patients with colon cancer under postoperative analgesia

Objective To compare the effect of oxycodone,sufentanil and fentanyl on immune function in patients with colon cancer under postoperative analgesia,provide guidance and basis for clinical medication.Methods Ninety patients 49 males and 41 females,aged <65 years,ASA physi-cal status Ⅰ or Ⅱ,were enrolled and randomly divided into 3 groups using a random number table (n=30):oxycodone group (group O),sufentanil group (group SF)and fentanyl group (group F).All patients underwent the same way to maintain anesthesia.The PCIA pump recipe were as follows:group O,oxycodone 2 mg/kg+granisetron 3 mg,added normal saline to 100 ml;group SF,sufen-tanil 0.002 mg/kg+granisetron 3 mg,added normal saline to 100 ml;group F,fentanyl 0.02 mg/kg+granisetron 3 mg,added normal saline to 100 ml;The background volume of PCIA electronic pump was 2 ml/h,the amount of each press was 2 ml,the lock time was 1 5 min,the pump time lasted for 24 h and maintained for 2 days after operation.The percentage of CD4+,CD8+and NK cells in venous blood were recorded at the end of surgery,6 h,12 h,24 h and 48 h after surgery.Results In CD4+,in the time points of 6 h,12 h,24 h after surgery,it was significantly higher in group O than that in groups SF and F (P<0.05);CD8+in group O was higher than that of group F,and CD8+in group F was higher than group SF at the time of 6 h after surgery.The group O was significantly higher than that of the groups SF and F at the times of 12 h,24 h after surgery (P<0.05);In NK cells,at the time points of 12 h,24 h and 48 h after surgery,it was significantly higher in group O than in groups SF and F (P<0.05).Conclusion Three analgesic drugs caused decline in immune function.Sufentanil and fentanyl have the similar effects on immunity,but oxycodone have a relatively small effect on immunity.Oxycodone is a more appropriate analgesic drug for colon cancer under postoperative analgesia.
9. Analysis of the influence of iron overload in glucose metabolism in thalassemia major patients

Objective:
This study aimed at determining the characteristics of the glucose homeostasis and its relationship with iron overload of the patients with β-thalassemia major (β-TM).
Method:
From Sun Yat-sen Memorial Hospital between January 2014 and December 2015, a total of 57 transfusion-dependent β-TM patients with 5-18 years old were enrolled in this study and fasting blood glucose(FBG) and insulin level, serum ferritin (SF), serum iron, transferrin, total iron binding capacity, unsaturated iron binding capacity were determined.Insulin resistance index (IRI), insulin sensitivity index and β-cell function index (BFI) were also estimated. Besides, in 36 patients cardiac T2* and liver T2* were estimated.
Result:
(1) Four patients(7%) with β-TM were diagnosed diabetes mellitus, and 14(24%) had impaired fasting glucose. (2) The incidence of abnormal glucose metabolism was significantly different according to levels of SF and degrees of the cardiac iron overload(χ2=9.737,
10.Influencing factors of indwelling catheter time in the patients with prostate hyperplasia and malignant tumor after prostatic surgery

Objective To investigate the influencing factors of the indwelling catheter time in patients with prostate hyperplasia and malignant tumor after prostate surgery.Methods A total of 447 prostate surgery patients with prostate hyperplasia and malignant tumor were selected for medical record review using convenience sampling method. The demographic data,diagnose,complications,surgical method,medical advice and nursing care items were collected. The indwelling catheter time in or outward of the hospital and its influence factors were analyzed.ResultsA number of 300(44.74%) patients had urination disorders before operation.A number of 258(57.72%) patients had complications. Little information about rehabilitated exercise of urination function was recorded.There were statistically significant differences in indwelling catheter time in the hospital among benign prostatic hyperplasia patients with different education levels, health payments and complications (t/F=5.048, -2.433, 2.086;P<0.05). Significant differences in indwelling catheter time outward of the hospital were also observed in patents with different preoperative urination disorders (t=0.551, P<0.05). Prostate malignancy patients with different resident places and bad habits had statistically significant differences in indwelling catheter time outward of the hospital (t=2.062, -1.974;P<0.05). For patients with benign prostatic hyperplasia, education levels, health payment and complications could explain 60.8% of changes in indwelling catheter time in the hospital. For patients with prostate malignancy, resident place and bad habits could explain 68.4% of changes in indwelling catheter time outward of the hospital. According to medical records, only 42.5% patients had urinary flow rate, urodynamic and residual urine volume examinations. Only 17.4% of medical records had urinary function related admission, which were found in 26.4% of medical records when patients were discharged.ConclusionsMedical workers are lack of attentions to the rehabilitated exercise of urination function for patients with prostate hyperplasia and malignant tumor after prostate surgery. It is urgent to build a scheme of rehabilitated exercise of urination function for postoperative patients of prostate, which based on the fast-track surgery theory and used the time as axis.
