1.Mechanism of Yangxin Dingji Capsules in Preventing Ventricular Arrhythmia Based on TAK1/MKK3/p38 MAPK Pathway

ObjectiveTo explore the protective effect and mechanism of Yangxin Dingji capsules on isoproterenol (ISO)-induced ventricular arrhythmia in SD rat cardiomyocytes based on the transforming growth factor-β activated kinase 1 (TAK1)-mitogen-activated protein kinase kinase 3 (MKK3)-p38 mitogen-activated protein kinase (p38 MAPK) signaling pathway. MethodFifty male SPF-grade SD rats were randomly divided into a normal group, a model group, a propranolol group, a low-dose Chinese medicine group, and a high-dose Chinese medicine group. The ventricular arrhythmia model was constructed using the ISO "6+1" method. The propranolol group received propranolol at 0.015 g·kg-1·d-1. The Chinese medicine groups received Yangxin Dingji capsules at doses of 0.5、 2 g·kg-1·d-1, respectively. The normal and model groups were given an equal volume of 0.9% NaCl solution. Electrocardiogram (ECG) changes in SD rats were recorded using the BL-420F biological function experimental system. Hematoxylin-eosin (HE) staining was used to observe the pathological changes in the heart. Serum levels of cardiac troponin I (cTnI), creatine kinase-MB (CK-MB), interleukin-1β (IL-1β), tumor necrosis factor-α (TNF-α), and transforming growth factor-β1 (TGF-β1) were measured using enzyme-linked immunosorbent assay (ELISA). The mRNA expression of IL-1β and TNF-α was assessed using real-time fluorescence quantitative polymerase chain reaction (Real-time PCR). Reactive oxygen species (ROS) expression was detected using immunofluorescence. Protein expression levels of TAK1, phosphorylated TAK1 (p-TAK1), MKK3, phosphorylated MKK3 (p-MKK3), p38 MAPK, phosphorylated p38 MAPK (p-p38 MAPK), nuclear factor-κB (NF-κB), phosphorylated NF-κB (p-NF-κB), IL-1β, and TNF-α were measured using Western blot or immunohistochemistry. ResultCompared with normal group, the model group showed significant ventricular arrhythmia in ECG, with an increased arrhythmia score (P<0.01). Pathological damage to myocardial tissue was evident, and serum levels of cTnI, CK-MB, IL-1β, TNF-α, and TGF-β1 were elevated (P<0.01). The mRNA and protein expression of IL-1β and TNF-α in myocardial tissue was also increased (P<0.01). ROS level and protein expression of p-TAK1, p-MKK3, p-p38 MAPK, and p-NF-κB were elevated in myocardial tissue (P<0.01). In the propranolol and Chinese medicine groups, the incidence of sustained ventricular tachycardia (SVT) and arrhythmia scores were significantly reduced compared to model group (P<0.05, P<0.01). Pathological damage to cardiomyocytes was alleviated, and levels of myocardial injury markers and inflammatory factors in serum and myocardial tissue were decreased. The ROS level in myocardial tissue was also reduced (P<0.01), with a noticeable reduction in related molecules in the p38 MAPK pathway (P<0.05, P<0.01). ConclusionThe expression of p38 MAPK pathway molecules was up-regulated in myocardial tissue of ISO-induced ventricular arrhythmia rats. Yangxin Dingji capsules may inhibit cardiac inflammation damage by regulating the expression of related molecules in the p38 MAPK pathway, thereby exerting a protective effect on myocardial cells, with TAK1 being a potential target.
2.Analysis of urinary iodine level in Hashimoto thyroiditis patients

Objective :
To analyze the difference of urinary iodine level in Hashimoto thyroiditis ( HT) patients, and to explore the possible relationship between urinary iodine level and HT under different iodine nutritional sta- tus,so as to provide some references for reasonable iodine intake in HT patients.
Methods :
A total of 101 hospi- talized HT patients were selected as HT group and divided into 3 groups according to thyroid function : HT group with hyperthyroidism (41 cases) .There were 25 cases in HT group with normal thyroid function.There were 35 cases in HT combined with hypothyroidism group.In addition,30 healthy subjects were selected as control group. Serum levels of thyroid stimulating hormone(TSH) ,triiodothyronine(T3 ) ,thyroxine (T4 ) ,thyroid peroxidase an- tibody (TPOAb) and thyroglobulin antibody (ATG) were detected by chemiluminescence assay.The size and mor- phological structure of thyroid organs were examined by ultrasonography.Urinary iodine was determined by catalytic spectrophotometry with arsenic and cerium.The nutritional status of iodine was classified into iodine deficiency ( < 100 μg/ L) ,iodine adequacy( 100 -199 μg/ L) ,iodine adequacy (200 -299 μg/ L) and iodine excess ( ≥ 300 μg/ L) .Non-parametric test was used to compare urinary iodine level between HT group and control group,one- way ANOVA and t test were used to compare urinary iodine level between HT group and control group ,and Spearman correlation analysis was used to compare the correlation between urinary iodine level and T3 ,T4 ,TSH, ATG and TPOAb under different iodine nutrition status.
Results :
Compared with control group,ATG and TPOAb levels in HT group increased (P<0. 001) ,and urinary iodine levels increased (P<0. 05) ,with statistical signifi- cance.Compared with the control group in different thyroid function states,only the HT group with hypothyroidism increased the urinary iodine level (P<0. 01) ,and the difference was statistically significant.Spearman correlation analysis showed that urine iodine level was positively correlated with ATG and TPOAb levels in iodine excess condi- tion (P<0. 05) ,and urine iodine level was positively correlated with TSH level in iodine sufficient condition and iodine excess condition in HT patients (P<0. 05) .
Conclusion
The urinary iodine level of HT patients was high- er than that of normal people.When the urinary iodine level of residents is ≥ 300 μg/ L,iodine intake is prone to HT.When the urinary iodine level of HT patients is ≥ 200 μg/ L,iodine consumption is prone to hypothyroidism, and iodine intake should be limited.
3.Effects of long non-coding RNA OTUD6B-AS1 on proliferation,invasion and migration of lung adenocarcinoma A549 cells

Objective The aim of this study was to investigate the effects of LncRNA OTUD6B-AS1 on the proliferation,mi-gration and invasion of lung adenocarcinoma A549 cells.Methods Lung adenocarcinoma A549 cell line was cultured in vitro,and transient transfection of OTUD6B-AS1 and empty plasmid group were used as the control group.Overexpression and control cell mod-els were constructed,and divided into OTUD6B-AS1 overexpression group and empty plasmid group(NC group).The cell model was divided into the empty plasmid group(NC group)and OTUD6B-AS1 overexpression group.The transfection efficiency of OTUD6B-AS1 mRNA was verified through qRT-PCR.The CCK-8 experiment was used to detect the effect of OTUD6B-AS1 on the prolifera-tion activity of lung adenocarcinoma cells,and the Transwell assay was used to detect the effect of OTUD6B-AS1 on the migration and invasion ability of lung adenocarcinoma cells.Results Compared to the NC group,the overexpression OTUD6B-AS1 group had a sig-nificant increase in the expression of OTUD6B-AS1(P<0.05).The CCK-8 assay results showed that the proliferation activity of A549 cells in the OTUD6B-AS1 overexpression group was significantly reduced compared to the NC group(P<0.05).The results of the Transwell assay showed that the OTUD6B-AS1 overexpression group had significantly lower cell migration and invasion abilities than the NC group(P<0.05).Conclusion Overexpression lncRNA OTUD6B-AS1 in lung adenocarcinoma A549 cells can signifi-cantly inhibit the proliferation,migration,and invasion ability of A549 cells.
4.Effect of silicon dioxide exposure on airway surface microenvironment and NEK7/NLPR3 inflammasome in rats

Objective:
To examine the effect of SiO2 exposure on the airway surface microenvironment and NIMA-related kinase 7 (NEK7)/nucleotide-binding oligomerization domain-like receptor containing pyrin domain 3 (NLRP3) inflammasome in rats.
Methods:
Twenty-four specific pathogen-free male rats of the SD strain were randomly divided into the control group and the model group, of 12 rats in each group. Rats in the model group were given SiO2 suspensions through disposable tracheal intubation perfusion to model silicosis in rats, while rats in the control group was perfused with the same amount of physiological saline. The pH value and glucose level were measured in the rat bronchoalveolar lavage fluid (BALF) 14 and 28 days after modeling. Lung tissues were stained with HE and Masson and the distribution of inflammatory cells and the deposition of pulmonary interstitial collagens were observed in lung tissues under a light microscope. The expression of transforming growth factor β1 (TGF-β1), collagen type Ⅰ(ColⅠ), collagen type Ⅲ (Col Ⅲ), interleukin-1β (IL-1β), NLRP3, N-terminal domain of Gasdermin D (GSDMD-NT), caspase-1, and NEK7 was quantified in lung specimens using immunohistochemistry.
Results:
Lower pH values were measured in rat BALF in the model group than in the control group 14 [(6.38±0.05) vs. (6.68±0.08), P<0.05] and 28 days after modeling [(6.63±0.14) vs. (6.86±0.05), P<0.05], while higher glucose levels were seen in the model group than in the control group 14 [(0.39±0.06) vs. (0.31±0.04) mg/dL, P<0.05] and 28 days after modeling [(0.39±0.08) vs. (0.31±0.06) mg/dL, P<0.05]. HE and Masson staining showed mild to moderate alveolitis and pulmonary fibrosis in rats 14 days post-exposure to SiO2, and showed moderate to severe alveolitis and pulmonary fibrosis 28 days post-exposure. Immunohistochemistry detected higher TGF-β1, ColⅠ, Col Ⅲ, IL-1β, NLRP3, GSDMD-NT, caspase-1 and NEK7 expression in rat lung tissues in the model group than in the control group (all P<0.05).
Conclusions
SiO2 exposure may cause changes in rat airway surface microenvironment, including BALF acidification and elevated glucose. Pyroptosis induced by activation of NEK7-associated NLRP3 inflammasome may be an important mechanism of pulmonary fibrosis caused by silicosis.
5.Application of virtual surgical planning combined with intraoperative navigation in the accurate resection of maxillary tumor and simultaneous reconstruction

Objective:To explore the application of of virtual surgical planning combined with intraoperative navigation in the accurate resection of maxillary tumor and simultaneous reconstruction.Methods:From October 2015 to December 2016, the patients with maxillary tumor treated at the First Affiliated Hospital of Zhengzhou University were included in this study. Before the surgery, the virtual surgical planning was used to complete tumor identification, virtual tumor resection, fibula reconstruction, and then stereomodel was printed for the fabrication of fibular osteotomy guide plate and prebent of personalized titanium mesh. During the operation, navigation technology was used to determine the position of the designed osteotomy line for the accurate resection of maxillary tumor. The bone defect reconstruction was performed using fibular flap guided by osteotomy plate and prebent personalized titanium mesh. Histopathological examination was conducted to evaluate the safety of the surgical boundary. The postoperative color gradient map, self evaluation of facial appearance satisfaction, mouth opening degree and jaw vertical distance were used to evaluate the reconstruction of maxillary defects.Results:A total of 5 patients were enrolled in this study, 3 males and 2 females, with a median age of 39 years old (ranged 27 to 50). Among these cases, 2 were benign tumors and 3 were malignant. The operation of 5 cases was successful and all the fibular flaps were survived. There were 27 frozen pathological examinations, 25 were negative. One patient was treated with both adjuvant biological therapy and radiotherapy. Two patients were treated with only radiotherapy. The color gradient map showed that the position of fibular flap was highly consistent with the pre-operative surgical design. Four patients reported their facial appearance were excellent and one was good postoperatively. The mouth opening degree of the patients was 2.8-3.2 cm. The vertical jaw distance between mandible and fibula repair area was 6.5-10.0 mm. After 14-28 months follow-up, one patient with positive surgical boundary had local recurrence.Conclusions:Virtual surgical planning combined with simultaneous intraoperative navigation can improve the accuracy of maxillary tumor resection and reconstruction, and so as to improve the the quality of life of patients after surgery.

7.Clinical and imaging characteristics of patients with different types of infectious sacroiliitis

To investigate the clinical manifestations and imaging characteristics of patients with different types of infectious sacroiliitis. Clinical data of 40 patients diagnosed with infectious sacroiliitis were retrospectively analyzed. Among the 40 patients, 16 patients were diagnosed as non-brucellar and non-tuberculous infectious sacroiliitis (ISI), 13 with tuberculous infectious sacroiliitis (TSI), and 11 with brucellar sacroiliitis (BSI). In the ISI and TSI group, female patients accounted for 11/16, 12/13, while the proportion of unilateral involvement was 15/16 and 12/13, respectively. Compared with ISI and TSI group, BSI patients were mainly male (8/11) and presented more bilateral involvement (6/11) ( P<0.05). Bone erosion was more common in ISI and TSI groups than in BSI group (6/15, 7/11 and 2/10), as well as abscess formation (3/15, 4/11 and 1/10, respectively). Symptoms in all patients relieved 1-2 weeks after administration of antibiotics or anti-tuberculosis treatment, but the resolution of the magnetic resonance imaging findings delayed about 6 (3-9) months. ISI and TSI patients with infectious sacroiliitis should be differentiated from spondyloarthritis, with a characteristic of more female patients, unilateral sacroiliitis, bone erosion, soft tissue involvement and abscess formation. However, BSI patients are mainly male, more bilateral involvement and less bone destruction and abscess formation. Antibiotic therapy demonstrates significant therapeutic effects, but resolution of the magnetic resonance imaging findings responses late.
8.Repair of tongue defect with submental artery island flap and free anterolateral thigh flap after resection of tongue cancer: A comparative analysis

Objective:To compare and analyse the clinical outcome, advantages and disadvantages of submental artery island flap (SAIF) and free anterolateral thigh flap (ALTF) in the repair and reconstruction of tongue after radical surgery of tongue cancer.Methods:From January, 2016 to December, 2018, a total of 40 patients received tongue repair and reconstruction with either SAIF or ALTF after radical resection of tongue cancer. There were 28 males and 12 females, with an average age of 51 years old. Eighteen patients received tongue repair and reconstruction with SAIF and 22 with ALTF. Postoperative follow-up were carried out and the clinical data were collected. Swallowing, speech and softness of the tongue between the 2 repair methods were compared and statistically analysed. P<0.05 indicated a significant statistical difference between 2 groups. Results:All flaps survived. One ALTF had a venous vascular crisis. The flap survived after the removal of thrombus at the anastomotic site. Functional recovery of tongue was analysed after the follow-up of 12-48 months. It was found that there was no significant difference in speech function between the 2 groups (SAIF vs ALTF: 13 vs 15, P=0.206). The swallowing dysfunction in SAIF group was significantly higher than that of the ALTF group (SAIF vs ALTF: 15 vs 7, P=0.014). Moreover, the average time of surgery in SAIF group (3.5 h) was significantly less than that of the ALTF group (6.8 h), which had statistically significance ( P<0.05). Conclusion:The SAIF and ALTF are ideal flaps for repairing the tongue defect caused by tongue cancer surgery. SAIF features a simple surgical procedure and a short time for flap taking. ALTF provides sufficient amount of tissue to cover the scars left by the surgery, reduce donor site complications, and benefit the recovery of swallowing and speech functions.

The Singapore Health Services cluster (SingHealth) radiology film archives are a valuable repository of local radiological cases dating back to the 1950s. Some of the cases in the archives are of historical medical interest, i.e. cerebral angiography in the workup of patients with hemiplegia. Other cases are of historical social interest, being conditions seen during earlier stages of Singapore's development, i.e. bound feet. The archives form a unique portal into the development of local radiology as well as the national development of Singapore. A selection from the archives is published in commemoration of the International Day of Radiology in 2020, as well as the 200th anniversary of the Singapore General Hospital in 2021. This pictorial essay comprises gastroenterology, musculoskeletal and obstetrics and gynaecology cases from the archives.
10.Percutaneous transluminal stenting versus directional atherectomy for lower limb artery TASC Ⅱ Class A and B superficial femoral artery lesions

Objective To compare the effect and safety of stenting versus directional atherectomy (DA) in the treatment of TASCⅡ A and B superficial femoral artery lesions.Methods 100 patients with TASC Ⅱ A and B lesions were divided into percutaneous transluminal stenting(PTS) group (n =50) and DA group (n =50).Patients were compared in terms of technical success rate,treatment success rate,first operation cost,postoperative ankle brachial index (ABI),limb salvage rate,survival,and patency.Results The technical success rate in both PTS and DA group was 100%.The treatment success rate was 98% vs.86%,P>0.05.Postoperative ABI:0.82 ±0.19 vs.0.80 ±0.27,P>0.05.First operation cost:(34 820 ± 1 051) yuan vs.(45 635 ± 1 358) yuan,P <0.001;All patients were followed-up for up to 2-year,the cumulative patency rate was 81.6% vs.72.9% (P>0.05).Limb salvage rate was 97.9% vs.93.8 %,P > 0.05.Conclusion There were no significant differences in the effect and safety of PTS versus DA in the treatment of TASCⅡ A and B superficial femoral artery lesions.
