1.Expert recommendations on vision friendly built environments for myopia prevention and control in children and adolescents

Abstract
The prevention and control of myopia in Chinese children and adolescents has become a major public health issue. While maintaining increased outdoor activity as a cornerstone intervention, there is an urgent need to explore new complementary approaches that can be effectively implemented in both indoor and outdoor settings. In recent years, environmental spatial frequency has gained increasing attention as one of the key environmental factors influencing the development and progression of myopia. Both animal studies and human research have confirmed that indoor environments lacking mid to high spatial frequency components, often characterized as "visually impoverished", can promote axial elongation and myopia through mechanisms such as disruption of retinal neural signaling, impaired accommodative function, and altered expression of related molecules. Based on the scientific consensus, it is recommended that "enriching of environmental spatial frequency" should be integrated into the myopia prevention and control framework. Following the principles of schoolled organization, family cooperation, community involvement, and student participation, specific measures are put forward in three areas:optimizing school visual settings, improving home spatial environments, and promoting healthy visual behavior. The aim is to create "visually friendly" indoor environments as an important supplement to outdoor activity, thereby providing a novel perspective and strategy for comprehensively advancing myopia prevention and control among children and adolescents.
2.Study on the prediction of Ki-67 expression level in nasopharyngeal carcinoma by multi-parameter MRI

Objective To explore the value of multi-parameter MRI in predicting the Ki-67 expression level in nasopharyngeal carcinoma(NPC).Methods The clinical and MRI data of 63 patients with pathologically confirmed NPC were prospectively collected.All patients underwent routine plain and enhanced nasopharyngeal MRI,diffusion weighted imaging(DWI),and arterial spin labeling(ASL)scans before treatment.Multivariate logistic regression analysis was used to identify the independent risk factors for the Ki-67 expression level.Results The degree of enhancement,the maximum blood flow(BFmax),and the minimum apparent diffusion coefficient(ADCmin)were independent risk factors for the Ki-67 expression level in NPC patients.The area under the curve(AUC)of the prediction model established based on these three factors was 0.920,with a sensitivity of 0.792 and a specificity of 0.897,respectively.Conclusion Multi-parameter MRI based on conventional enhancement,ASL,and DWI can effectively predict the Ki-67 expression level in NPC patients.
3.Association between homocysteine and recurrent pregnancy loss (RPL) and its effects on IVF/ICSI-ET outcomes in RPL patients

Objective:To investigate the association between homocysteine (Hcy) and recurrent pregnancy loss (RPL), as well as its impact on clinical pregnancy outcomes in patients undergoing in vitro fertilization/intracytoplasmic sperm injection and embryo transfer (IVF/ICSI-ET). Methods:This retrospective cohort study collected clinical data from patients undergoing IVF/ICSI-ET at the Reproductive Health Hospital of the Third Affiliated Hospital of Zhengzhou University between December 2020 and March 2024. Patients were divided into no history of pregnancy loss group (named control group, n=1 027) and RPL group ( n=743) based on history of pregnancy loss. Peripheral blood Hcy levels were compared between the two groups. Multivariate logistic regression was performed to adjust for confounding factors and determine whether Hcy is an independent risk factor for RPL. RPL patients were divided into four subgroups based on Hcy quartiles, named Q1 subgroup (Hcy<7.03 μmol/L), Q2 subgroup (7.03 μmol/L≤Hcy<8.63 μmol/L), Q3 subgroup (8.63 μmol/L≤Hcy<10.44 μmol/L), and Q4 subgroup (Hcy≥10.44 μmol/L), to further analyze the impact of Hcy level on pregnancy outcomes after IVF/ICSI-ET in these patients. Results:1) Baseline characteristics between control and RPL groups: statistically significant differences were observed in female age, male age, female body mass index (BMI), duration of infertility, cause of infertility, and peripheral blood Hcy levels (all P<0.05). 2) After adjusting for female age, male age, female BMI, duration of infertility, and cause of infertility via multivariate logistic regression, elevated Hcy levels was identified as an independent risk factor for RPL (a OR=1.366, 95% CI: 1.298-1.438, P<0.001). 3) Baseline characteristics of the four RPL subgroups: antral follicle count (AFC) differed significantly among Q1, Q2, Q3 and Q4 subgroups [17.00 (11.00, 24.00), 15.00 (10.00, 24.00), 14.00 (7.00, 22.25), 15.50 (8.00, 22.00), P=0.043]. No statistically significant differences were observed in other baseline characteristics (all P>0.05). 4) Pregnancy outcomes across the four RPL subgroups: miscarriage rates in the Q1, Q2, Q3 and Q4 subgroups were 18.18% (18/99), 30.61% (30/98), 33.70% (31/92), and 35.96% (32/89), respectively, live birth rates were 44.26% (81/183), 36.17% (68/188), 32.80% (61/186), and 30.65% (57/186), respectively. Intergroup differences in miscarriage rate and live birth rate were statistically significant ( P=0.033, P=0.036). Specifically, miscarriage rate in the Q3 and Q4 subgroups, and live birth rate in the Q4 subgroup were significantly higher than those in the Q1 subgroup (all q<0.05). However, no significant differences were observed in clinical pregnancy rate or early miscarriage rate among the four groups (all P>0.05). After adjusting for confounding factors using multivariate logistic regression, taking the Q1 subgroup as the control, there were no statistically significant differences in the clinical pregnancy rate between the remaining groups and the Q1 subgroup (all P>0.05). The early miscarriage rate in the Q3 subgroup (a OR=2.184, 95% CI: 1.077-4.426, P=0.030) and the early miscarriage rate in the Q4 subgroup (a OR=2.290, 95% CI: 1.116-4.697, P=0.024) were significantly higher than those in the Q1 subgroup; the miscarriage rate in the Q3 subgroup (a OR=2.207, 95% CI: 1.125-4.330, P=0.021) and the miscarriage rate in the Q4 subgroup (a OR=2.377, 95% CI: 1.209-4.674, P=0.012) were significantly higher than those in the Q1 subgroup; the live birth rate in the Q3 subgroup (a OR=0.615, 95% CI: 0.401-0.944, P=0.026) and the live birth rate in the Q4 subgroup (a OR=0.560, 95% CI: 0.364-0.863, P=0.009) were significantly lower than those in the Q1 subgroup. Conclusion:Elevated Hcy is a high-risk factor for RPL in IVF/ICSI-ET patients and may adversely affect pregnancy outcomes.

Creutzfeldt-Jakob disease(CJD)is a rapidly progressive and fatal neurodegenerative disorder caused by prions,with certain infectivity and iatrogenic transmission risks.With the rapid progress and application of new dia-gnostic biomarkers and detection methods,as well as the construction and improvement of surveillance and reporting systems,the detection of CJD in patients domestically and internationally has shown an increasing trend year by year.Due to its long incubation period and heterogeneity of early symptoms,early identification and diagnosis of the disease is difficult,increasing the risk of transmission within medical institutions.Currently,there is a lack of con-sensus on the infection prevention and control of CJD.In order to timely identify and diagnose CJD as well as effec-tively block its transmission in medical institutions,this consensus summarizes 15 clinical concerns and formulates 24 specific recommendations based on the latest domestic and international research findings and clinical evidence,as well as combines with clinical practice,aiming to standardize healthcare-associated infection prevention and control measures for CJD and reduce its transmission risk in medical institutions.

6.Application of bidirectional social support in elderly patients with maintenance hemodialysis

Objective To explore the basis of PERMA on the overall well-being and social support of elderly patients with maintenance hemodialysis(MHD).Methods A total of 60 elderly patients with MHD were selected from the blood purification center of a tertiary A hospital in Zhejiang Province from January to December 2022 as the study subjects.According to the different disease areas of the patients,they were randomly divided into an experimental group or a control group.Finally,30 patients were included in each group.The control group received routine care,and the experimental group received another 4-week bidirectional social support intervention based on the PERMA model.General well-being scale,social support rating scale,sleep duration,activity duration were used to evaluate the effect before intervention,4 weeks after intervention and 1 month after intervention.Results The results of repeated measurement ANOVA showed that there were statistically significant differences in general happiness score,social support score,sleep duration and activity duration in terms of time effect(P<0.05),while there were statistically significant differences in general happiness score,social support score and sleep duration in terms of inter-group effect(P<0.05).Social support score and sleep duration had statistically significant interac-tion effects(P<0.05).Conclusion The bidirectional social support intervention based on well-being PERMA model can enhance the general well-being and social support level of elderly patients with MHD,and improve the sleep and activity status of elderly patients with MHD.

Reform of medical insurance payment methods,make DRG cost accounting an important part of hospi-tal management work.The choice of DRG cost accounting method affects the time of the accounting results to some extent,and also affects the accuracy of the accounting results,which turn affects the hospital's understanding of the surplus of medical insurance settlement and the corresponding control means.Taking a public hospital in Yunnan Province as an example,based on the theory of service unit superposition method,the DRG cost accounting method is improved,and the project cost accounting can be carried out synchron,which can produce the accounting results more simply and quickly,and lay the foundation for the further lean management of the hospital.
8.Research progress of lactylation modification in colorectal cancer

Colorectal cancer(CRC)is a common malignant tumor worldwide,and recurrence and metastasis are the main causes of death.Therefore,it is of great clinical significance to explore the molecular mechanisms that affect the occurrence and development of CRC.Lactylation is a newly recognized post-translational modification of proteins,which can directly stimulate gene transcription of chromatin,enhance protein stability,and promote tumor occurrence and development.Studies have found that lactylation is closely related to the occurrence and development of CRC.This article reviews the research progress of lactylation in CRC,aiming to provide new ideas for the treatment of CRC.
9.Statistical Parametric Mapping Analysis of Drop Jump Variability in Women with Generalized Joint Hypermobility

Objective Based on statistical parametric mapping(SPM),to analyze the variability characteristics of lower limb in females with generalized joint hypermobility(GJH)during drop jump.Methods Fifteen females with GJH(GJH group)were recruited based on the Beighton scores,and 15 healthy females(control group)were matched.Kinematic and kinetic data were synchronously collected using the Qualisys infrared motion capture system and Kistler three-dimensional(3D)force platform.Joint angles and torques were computed using OpenSim.Custom Matlab scripts were used to calculate the standard deviation curves of joint angles,and SPM was applied to analyze differences in movement variability between the two groups during the eccentric,coupling,and concentric phases of drop jumps.The maximum joint flexion angles and torques(mean and standard deviation)were calculated to support the findings,and effect sizes were evaluated using Cohen's d.Results Significant differences were observed between the two groups during various phases of drop jumps.In all statistically significant periods,the GJH group exhibited higher joint angle standard deviations compared to the control group(P<0.05).Differences were primarily concentrated in joint flexion movements.During the coupling phase at different heights,the standard deviation of knee joint flexion angles in GJH group consistently exceeded that of control group(P<0.001).Discrete variables showed significant differences in the standard deviation of the maximum knee flexion angles at different heights:At 30 cm,P=0.001,Cohen's d=2.520;at 40 cm,P=0.014,Cohen's d=1.739;and at 50 cm,P=0.005,Cohen's d=1.768.No significant group differences were found in the mean values or standard deviations of the maximum joint flexion angles and torques.Conclusions Females with GJH exhibit higher movement variability during drop jumps compared to healthy females,particularly in knee flexion movements during the coupling phase.Excessive variability reflects insufficient motor control,reducing their ability to resist external disturbances and leading to high-risk movement patterns(e.g.,excessive flexion or knee valgus).
10.Construction strategy for vascularization of organoids

BACKGROUND:The effective promotion of internal angiogenesis in organoids is a current focal issue in organoid culture.Vascularized organoids,as a newly developed bioculture technology,have significant research and application value in the study of living tissue development,disease formation mechanisms,tissue replacement therapy,and drug screening.OBJECTIVE:To summarize the methods or strategies for vascularization of organoids in recent years,analyze the formation mechanism of vascularization of organoids and the construction strategies,with a view to providing reliable ideas for more in-depth study of the mechanism of organoid genesis and for clinical translation.METHODS:The authors utilized the PubMed and CNKI databases for related formation collection.The keywords were "organoids,vascularization,vascular,vascular development,vessel" in Chinese and English.Finally,77 papers were included for summarization.RESULTS AND CONCLUSION:(1) The mechanism of vascularization formation in organoid organs involves three key factors,namely seed cells,cytokines,and extracellular matrix.Seed cells provide the essential cell source for vascularized organoids;cytokines play an important role in guiding angiogenesis within organoids,and the extracellular matrix provides an external growth environment for vascular cells,promoting the occurrence of vascularized organoids.(2) The construction strategies of vascularized organoids include cell self-reorganization,microvascular fragment infiltration,transplantation into host,and microfluidic chip.In vitro induction of pluripotent stem cells to differentiate into endothelial progenitor cells can integrate with adjacent tissues and have the potential for angiogenesis,so pluripotent stem cells can be used to construct vascularized organoids by self-reorganization.Microvascular fragments retain their cellular complexity,natural structure,and phenotypic plasticity,which is more conducive to simulating natural microvessels and promoting vascularization of organoids.Transplantation into host is currently the best method to achieve complete blood perfusion in organoids,while microfluidic chip provides a solution for achieving extracorporeal blood supply in organoids.(3) Multiple construction strategies of organoid such as co-differentiation of multiple stem cell types,precise regulation of signaling molecules,microvascular infiltration,and in vivo host transplantation,have introduced vascular components into organoids to some extent,making them closer to the corresponding tissues in terms of function and maturity.However,the challenge of achieving perfusion remains,and so far,only in vivo transplantation in hosts has enabled effective perfusion in organoids.Therefore,organoids still face numerous challenges in terms of vascularization.
