1.Evaluation of different detection methods for decline pattern of syphilis antibody in non-congenital syphilis children

Objective To explore the application values of different detection methods in monitoring the decline pattern of syphilis-spe-cific antibody in the non-congenital syphilis children.Methods A total of 80 non-congenital syphilis children were included in the study.The serum specimens were collected after birth,and the syphilis-specific antibodies were detected using electrochemilumines-cence immunoassay(ECLIA),western blotting(WB),treponema pallidum particle agglutination assay(TPPA),enzyme-linked im-munosorbent assay(ELISA),and toluidine red unheated serum test(TRUST).Follow-up was conducted every three months until the positive results of ELISA and TRUST turned to negative.Results The results of ECLIA showed that the syphilis-specific antibody lev-els in the non-congenital syphilis children declined to 25%of the level at birth within 2 to 3 months,and the rate of decline was inde-pendent of the initial concentration.WB analysis indicated that the specific IgG bands in non-congenital syphilis children at birth were consistent with those of their mother,and the sequence of specific antibodies decline was as follows:TPN47,TPN15,TPN45,and TPN17.Due to methodological limitations,the absorbance values of ELISA showed no significant change during the first three months after birth when high concentrations of antibodies were present in the samples,but it showed high sensitivity in the detection for the samples with low-concentration of syphilis antibodies.The detection rates of ECLIA,TPPA,and WB were compared by using ELISA as the reference method.At birth,the detection rates of syphilis antibodies were 100%,100%,and 90%,respectively.In 3 months after birth,the detection rates were 100%,100%,and 75%.In 6 months after birth,,they were 100%,46%,and 15%.In 9 months after birth,they were 83%,33%,and 0%.The positive rate of TRUST was 17.5%at birth.and turned to negative in 3 month of follow-up.Conclusion Syphilis specific IgG antibodies may fully transferred to the fetus and decline in a predictable pattern after birth.The comprehensive analysis for the results of the four methods suggested that dynamic detection using ECLIA method could be used to pre-dict the risk of non-congenital syphilis or terminate the follow-up at 3 months,while the seroconversion detected by WB was earlier than that by TPPA,while ELISA required the longest follow-up period.
2.The correlation between sarcopenia and anastomotic leakage after rectal cancer surgery

Objective To quantitatively assess the correlation between the skeletal muscle index(SMI)of patients and the occur-rence of anastomotic leakage(AL)in rectal cancer patients after surgery,and to analyze the risk factors for AL in rectal cancer patients and the influencing factors of sarcopenia.Methods The clinical,pathological,and related imaging data of 362 patients who under-went radical surgery for rectal cancer were retrospectively analyzed.All patients underwent pelvic MRI and abdominal CT scans(plain/enhanced)within one month before surgery,and the third lumbar vertebra skeletal muscle area(L3-SMA)was measured from the images.All patients were divided into AL group(56 cases)and control group(306 cases)based on the presence or absence of postoperative complications.The differences in clinical characteristics and imaging parameters between the two groups were analyzed.A logistic risk prediction model was established.Results Significant differences were observed between the two groups in sarcopenia,type of surgery,surgical approach,serum albumin level,operation duration,stoma type,and extramural vascular invasion(EMVI)(P<0.05).These factors were incorporated in a multivariate logistic regression analysis model,the area under the curve(AUC)of receiver operating characteristic(ROC)curve of the model was 0.810[95%confidence interval(CI)0.743-0.876,P<0.001],with a sensitivity of 0.865 and specificity of 0.669.Conclusion Sar-copenia is a significant risk factor for AL after rectal cancer surgery.It enhances the predictive efficacy for postoperative AL and serves as a basis for identifying high-risk populations for AL in clinical practice.

5.Peripheral blood biomarkers in children and adolescents with depressive disorders

Depressive disorder is a highly prevalent mood disorder among children and adolescents, which can exert profoundly negative impacts on psychosocial development. However, in the current medical system there still lacks objective indicators for the risk assessment, diagnosis and treatment selection of depression in this population. Biomarkers, which serve as objective indicators of physiological states, can offer crucial insights into pathological mechanisms of depressive disorders. This paper provides a comprehensive review of existing research on peripheral blood biomarkers associated with depression in children and adolescents, highlighting the feasibility of their clinical application. Studies indicate that substances in immune, neuroendocrine, neurotrophic, and multi-level omics system in peripheral blood can be significantly linked to the diagnosis and severity of depression in this population. Nevertheless, the predictive role of specific biomarkers necessitates integration with other multimodal data, and replication through studies with large sample size is needed.
6.Epidemiological investigation of iron deficiency among preschool children in 10 provinces, autonomous regions, or municipalities in China

Objective:To understand the current status of anemia, iron deficiency, and iron-deficiency anemia among preschool children in China.Methods:A cross-sectional study was conducted with a multi-stage stratified sampling method to select 150 streets or townships from 10 Chinese provinces, autonomous regions, or municipalities (East: Jiangsu, Zhejiang, Shandong, and Hainan; Central: Henan; West: Chongqing, Shaanxi, Guizhou, and Xinjiang; Northeast: Liaoning). From May 2022 to April 2023, a total of 21 470 children, including community-based children aged 0.5 to<3.0 years receiving child health care and kindergarten-based children aged 3.0 to<7.0 years, were surveyed. They were divided into 3 age groups: infants (0.5 to<1.0 year), toddlers (1.0 to<3.0 years), and preschoolers (3.0 to<7.0 years). Basic information such as sex and date of birth of the children was collected, and peripheral blood samples were obtained for routine blood tests and serum ferritin measurement. The prevalence rates of anemia, iron deficiency, and iron-deficiency anemia were analyzed, and the prevalence rate differences were compared among different ages, sex, urban and rural areas, and regions using the chi-square test.Results:A total of 21 460 valid responses were collected, including 10 780 boys (50.2%). The number of infants, toddlers, and preschoolers were 2 645 (12.3%), 6 244 (29.1%), and 12 571 (58.6%), respectively. The hemoglobin level was (126.7±14.8) g/L, and the serum ferritin level was 32.3 (18.5, 50.1) μg/L. The overall rates of anemia, iron deficiency, and iron-deficiency anemia were 10.4% (2 230/21 460), 28.3% (6 070/21 460), and 3.9% (845/21 460), respectively. The prevalence rate of anemia was higher for boys than for girls (10.9% (1 173/10 780) vs. 9.9% (1 057/10 680), χ2=5.58, P=0.018), with statistically significant differences in the rates for infants, toddlers and preschoolers (18.0% (475/2 645), 10.6% (662/6 244), and 8.7% (1 093/12 571), respectively, χ2=201.81, P<0.01), and the rate was significantly higher for children in rural than that in urban area (11.8% (1 516/12 883) vs. 8.3% (714/8 577), χ2=65.54, P<0.01), with statistically significant differences in the rates by region ( χ2=126.60, P<0.01), with the highest rate of 15.8% (343/2 173) for children in Central region, and the lowest rate of 5.3% (108/2 053) in Northeastern region. The prevalence rates of iron deficiency were 33.8% (895/2 645), 32.2% (2 011/6 244), and 25.2% (3 164/12 571) in infants, toddlers, and preschoolers, respectively, and 30.0% (3 229/10 780) in boys vs. 26.6% (2 841/10 680) in girls, 21.7% (1 913/8 821), 40.0% (870/2 173), 27.1% (2 283/8 413), 48.9% (1 004/2 053) in Eastern, Central, Western, and Northeastern regions, respectively, and each between-group showed a significant statistical difference ( χ2=147.71, 29.73, 773.02, all P<0.01). The prevalence rate of iron-deficiency anemia showed a significant statistical difference between urban and rural areas, 2.9% (251/8 577) vs. 4.6% (594/12 883) ( χ2=38.62, P<0.01), while the difference in iron deficiency prevalence was not significant ( χ2=0.51, P=0.476). Conclusions:There has been a notable improvement in iron deficiency and iron-deficiency anemia among preschool children in China, but the situation remains concerning. Particular attention should be paid to the prevention and control of iron deficiency and iron-deficiency anemia, especially among infants and children in the Central, Western, and Northeastern regions of China.

The 2019 Chinese practice guideline for the prevention and treatment of mother-to-child transmission of hepatitis B virus developed by Chinese Society of Infectious Diseases,Chinese Medical Association,has shown a good guiding effect in standardizing the process for blocking the mother-to-child transmission of hepatitis B virus in China.Clinical practice guidelines and consensus statements require timely updates based on new research evidence,in order to better guide clinical practice and research.Chinese Physician Association for Infectious Diseases,Chinese Medical Doctor Association,and Chinese Society of Infectious Diseases,Chinese Medical Association,cooperated with multidisciplinary experts and performed updates and supplementations to the above guideline,in order to provide guidance and a reference for clinical physicians and medical staff engaged in maternal and child health care.
8.Research progress on ethical issues in the process of fertility preservation in tumor patients

Fertility preservation technology provides an effective method for protecting the reproductive resources of patients with malignant tumors.At the same time,it is inevitable to face great ethical dilemmas in the process of technological implementation.From an ethical perspective,this paper sorted out the research on fertility preservation in tumor patients abroad,as well as analyzed the ethical issues faced by tumor patients in the communication decision-making and implementation stages of fertility protection.Specifically,these included informing and discussing the reproductive information and preservation choices of tumor patients,fertility decision-making of adolescent tumor patients,psychological support for fertility in tumor patients,ownership and use rights of reproductive materials and their disposal methods,fairness in the allocation of social health resources and other ethical issues.Combining with the actual situation in China,this paper further discussed how to put forward corresponding countermeasures based on following ethical principles such as favorable without harm,informed consent,cautious application,and ethical supervision,with a view to promoting the further development and application of fertility preservation technology in clinical practice.
9.Types analysis and coping suggestion of medical disputes arising from medical remarks

The language expression level of doctors directly affects the effectiveness of information transmission and communication. In the past 20 years, a large number of studies have conducted questionnaire surveys related to “doctor-patient communication” among medical staff and patients in the form of scales, but there were few studies on the practical application of doctor-patient communication and case analysis of doctor-patient disputes. Using the Complaint Register Book of a tertiary A hospital as research material, this paper summarized five causes of medical disputes caused by medical remarks, namely, stiff tone and improper body language of the medical staff, unclear communication of treatment plans, unable to answer patients’ questions, different doctors’ statements are contrary each other, and a large gap between disease prediction and disease development. This paper aimed to analyze the handling points of such disputes, with a view to applying them in doctor-patient communication courses for medical students or language skills training for doctors in the future, optimizing doctor-patient communication, and promoting the development of doctor-patient relationships.
10.Epidemiological analysis of notifiable infectious diseases in a tertiary general hospital in tianjin from 2019 to 2023

Objective Through the analysis of notifiable infectious diseases in a tertiary general hospital from 2019 to 2023,the epidemic characteristics of notifiable infectious diseases were understood,providing theoretical basis for the prevention and control of infectious diseases in hospitals.Methods The data of notifiable infectious diseases from 2019 to 2023 were col-lected from the Information system of Chinese Center for Disease Control and Prevention and analyzed according to the characteris-tics of time,population and route of transmission by descriptive epidemiological method.The change trend of major infectious dis-eases around 2023 was analyzed.SPSS 22.0 was used for statistical analysis.Results A total of 25 kinds,17 620 cases of in-fectious diseases were reported from 2019 to 2023,including 0 cases of Category A,13 kinds of Category B,3 986 cases and 6 kinds Category C,13 021 cases.The major infectious diseases were influenza and other infectious diarrhea.Among the patients,the male had more cases than female,mainly students.According to the routes of transmission,respiratory infectious diseases have been reported mainly,with a total of 10 970 cases(62.26%),followed by digestive tract infectious diseases,with a total of 4 661 cases(26.45%).Comparing the changing trends of major infectious diseases before and after 2023,it was found that in-fluenza showed a significant upward trend,while other major infectious diseases showed a downward trend.Conclusion The epidemic characteristics of infectious diseases in the hospital have changed in 2023,and relevant prevention and control strategies should be formulated according to the changing trend of infectious diseases in different periods.
