1.Changes and predictive value of serum IL-1,IL-12 and IFN-α levels in early pregnancy in hepatitis B pregnant women with abnormal liver function

Objective To explore the changes and predictive value of serum interleukin-1(IL-1),interleu-kin-12(IL-12)and interferon α(IFN-α)levels in early pregnancy in hepatitis B pregnant women with abnor-mal liver function.Methods A total of 100 hepatitis B pregnant women with normal liver function in early pregnancy in Shijiazhuang Maternal and Child Health Hospital from January 2022 to May 2022 were selected,and were divided into normal liver function group(52 cases)and abnormal liver function group(48 cases)ac-cording to whether there was abnormal liver function in middle and late pregnancy,and another 50 healthy pregnant women of the same gestational week in the same period in Shijiazhuang Maternal and Child Health Hospital were selected as the control group.The general information,liver function indicators[alanine amin-otransferase(ALT),aspartate aminotransferase(AST),glutamine transferase(GGT)],serum interleukin(IL)-1,IL-12,and interferon α(IFN-α)levels among the three groups were compared.The serum levels of IL-1,IL-12,IFN-α in pregnant women with different degrees of liver dysfunction in the liver dysfunction group were compared,and the relationship between the serum levels of IL-1,IL-12,IFN-α and the degree of liver dysfunction was analyzed,as well as the value of predicting liver dysfunction in pregnant women with hepatitis B in the middle and late pregnancy.Results The serum levels of ALT,AST,GGT,IL-1 and IL-12 in early pregnancy from high to low in three groups were liver dysfunction group,normal liver function group,and control group(P<0.05),while the level of IFN-α in early pregnancy from low to high in three groups were liver dysfunction group,normal liver function group,and control group(P<0.05).In the abnormal liver func-tion group,there were 12 cases with abnormal concentration values of 3 enzymes,16 cases with abnormal con-centration values of 2 enzymes,and 20 cases with abnormal concentration values of 1 enzyme.The levels of IL-1 and IL-12 from high to low were pregnant women with 3 abnormal values of enzyme,2 abnormal values of enzyme,and 1 abnormal value of enzyme(P<0.05).The level of IFN-α from low to high were pregnant women with 3 abnormal values of enzyme,2 abnormal values of enzyme,and 1 abnormal value of enzyme(P<0.05).The levels of serum IL-1 and IL-12 in the abnormal liver function group were positively correlated with the degree of liver function abnormality(r=0.771,0.793,P<0.05),while the levels of IFN-α were negatively correlated with the degree of liver function abnormality(r=-0.755,P<0.05).The area under the curve of serum IL-1,IL-12 and IFN-α in the early pregnancy for predicting abnormal liver function of hep-atitis B pregnant women in the middle and late pregnancy was 0.936(95%CI:0.869-0.976,P<0.05).Conclusion The levels of serum IL-1,IL-12 and IFN-α in early pregnancy of hepatitis B pregnant women are closely related to the degree of liver dysfunction in the middle and late pregnancy,and the combined detection of the three serum indicators has a high predictive value for predicting the liver dysfunction in the middle and late pregnancy of hepatitis B pregnant women.
2.Study on the efficacy and safety profile of tenofovir alafenamide fumarate in the treatment of patients with decompensated hepatitis B cirrhosis

Objective:To explore the efficacy and safety profile of tenofovir alafenamide fumarate (TAF) in the treatment of patients with decompensated hepatitis B cirrhosis.Methods:A two-way cohort study method was used to enroll patients with decompensated hepatitis B cirrhosis who visited four medical centers, including Xinjiang Uygur Autonomous Region Hospital of Traditional Chinese Medicine, from April 2021 to April 2024 and were treated with TAF and followed up for 48 weeks. The primary efficacy indicator was hepatitis B virus (HBV) DNA seronegative conversion rate at 48-weeks, and the secondary efficacy indicator was alanine aminotransferase (ALT) return to normal rate at 48-weeks. Relevant safety indicators, adverse drug reactions (ADRs), and clinical adverse outcomes were collected.Results:A total of 74 cases were included. Of these, 52 were males with an average age of (53.14 ± 9.15) years. Twenty-five and thirty-three cases completed 24 and 48 weeks of follow-up, respectively. The HBV DNA negative conversion rate was 96.97% (32/33), which was higher than the baseline of 58.1% (43/74) following 48 weeks of TAF treatment. The ALT return to normal rate was 72.73% (24/33), which was higher than the baseline of 47.30% (35/74); however, the renal function and blood lipid levels did not change significantly compared with the baseline level after completing 48 weeks of treatment (P>0.05). During the follow-up period, one case developed hepatocellular carcinoma, and no other adverse clinical outcomes, such as liver transplantation or death, were reported.Conclusion:TAF has a good efficacy and safety profile in the treatment of patients with decompensated hepatitis B cirrhosis.

4.Clinical phenotype and gene mutation analysis of neurodevelopmental disorders caused by CTNNB1 gene mutation

Objective:To investigate the clinical phenotype and gene mutation in a child with developmental disorders caused by CTNNB1 gene mutation. Methods:Clinical data of a child with CTNNB1 gene mutation who was admitted to Xiamen Hospital of Fudan University Affiliated Pediatric Hospital in May 2017 were collected, whole exome sequencing technology was applied to verify the family lineage of the child, and the pathogenicity of mutation site was analyzed. Results:The patient was a 6 years and 1 month old male, with a clinical phenotype including mental retardation, motor developmental disorders, speech disorders, visual disorders (internal strabismus), microcephaly, and behavioral problems (social withdrawal, overdependence, etc.), as well as panic syndrome (i.e., sudden shrieking in response to auditory and visual stimuli, extensional rigidity of the body, etc., followed by short periods of general extensional rigidity). The whole exome sequencing results showed the presence of a de novo mutation c.283(exon4)C>T in the CTNNB1 gene, and the c.283(exon4)C>T mutation was interpreted as pathogenic (PVS1+PS2+PS1+PM2+PM) according to the American College of Medical Genetics and Genomics variant classification criteria and guidelines. No relevant genetic variants were found in the parental family verification. Conclusion:CTNNB1 gene mutation c.283(exon4)C>T can cause neurodevelopmental disorders, including mental retardation, motor developmental disorders, speech disorders, visual disorders, microcephaly and behavioral abnormalities.
5.Synthesis and antidiabetic activity evaluation of benzopyrimidine derivatives

Thioredoxin-interacting protein (TXNIP), which mainly regulates glucose homeostasis in pancreatic β cells, is a novel target in the treatment of diabetes.In this study, 4-hydroxybenzopyrimidine was used as the raw material, four nitrogen-containing rings (imidazole, methylpiperazine, pyrazole, morpholine) were introduced, benzopyrimidine skeleton with nitrogen-containing rings derivatives targeting TXNIP was designed and synthesized, and the protective effect of compounds on palmitic acid-stimulated islet β cells was investigated.A total of 20 benzopyrimidine derivatives were designed and synthesized, and the structures were confirmed by 1H NMR and ESI-MS.Pharmacological studies showed that most of the compounds exhibited protective effects on islet β cells, with better axtivity for compounds C-1, C-2, C-4 and D-2 (cell survival rate > 70%) compared with PA model group (38.3%), Among the four compounds, D-2 had the highest activity of 87.2%, so it could become a potential new anti-diabetic chemical entity.
6.Comparative analysis between air-free trans-axillary laparoscopic thyroidectomy and open thyroidectomy

Objective To compare the clinical efficacy between air-free trans-axillary laparoscopic thyroidectomy and open thyroidectomy. Methods A retrospective analysis was performed on the data of both 96 patients who underwent air-free trans-axillary laparoscopic thyroidectomy (laparoscopic group) and 232 patients who underwent open thyroidectomy (open group) from August 2015 to July 2020. Comparative analysis were performed on operative data, the postoperative data of recovery and complications between two groups. Results The operative time in laparoscopic group was longer significantly than that in open group [(168±23) min vs. (95±18) min, P=0.022]. There was no significantly difference in the rate of recognition of recurrent laryngeal nerve between two groups (100.0% vs. 99.1%, P=0.138). The rate of recognition of both superior (98.0% vs. 97.0%, P=0.003) and inferior parathyroid glands (95.8% vs. 99.1%, P=0.028) in laparoscopic group was higher significantly than those in open group. The amount of drain of the first day after operation in laparoscopic group was more significantly than that in open group[(42±22) mL vs. (25±10) mL, P=0.006]. The postoperative length of hospital stay in laparoscopic group was longer significantly [(2.8±0.6) d vs. (1.8±1.2) d, P=0.016]. The cost of hospital stay was (13 186±1 015) yuan vs. (12 632±1 456) yuan (P=0.365) and the rate of postoperative complication was 10.4% vs. 9.1%(P=0.126), which all showed no significant difference between two groups. Conclusions Air-free trans-axillary laparoscopic thyroidectomy could be feasible and safe, and have better appearance. However, both operative time and hospital stay were longer.
7.Synthesis of ferulic acid derivatives and their inhibition of cholinesterase inhibitory activity

Ferulic acid (FA), a natural product of phenylpropanoids containing phenolic hydroxyl groups, has a wide range of pharmacological activities and some therapeutic effect on Alzheimer's disease (AD).Using FA as the raw material, the ferulic acid carbamate aniline derivatives were first synthesized by 4-step esterification reaction, splicing carbamate active functional groups, hydrolysis reaction and amide condensation.These FA derivatives were evaluated for in vitro cholinesterase inhibition activity by the Ellman method.A total of 15 novel FA derivatives were designed and synthesized, and their structures were confirmed by 1H NMR, 13C NMR and ESI-MS.Cholinesterase activity tests showed that compounds 5c, 5f, 5j, 5g, 5m possessed good acetylcholinesterase inhibition activity.Except for 5l, 5m, almost all compounds have inhibition activity on butyrylcholinesterase, which is much higher than that on acetylcholinesterase.In conclusion, compounds 5c, 5f, 5j and 5g can be used as potential anti-AD inhibitors targeting cholinesterase..

9.Synthesis and biological evaluation of moscatilin analogs as anti-inflammatory agents

Using syringaldehyde as raw material, the phosphine ylide intermediate was efficiently synthesized through acetylated hydroxyl protection, aldehyde group reduction, chlorination and reaction with triphenylphosphine. On this basis, moscatilin (MST) and its 12 analogs (MST-1-MST-12) were synthesized by wittig reaction, deacetylation and double bond reduction. All the structures were confirmed by 1H NMR, 13C NMR and ESI-MS. Bacterial lipopolysaccharide-induced mouse macrophage RAW264.7 inflammation model was used to conduct preliminary anti-inflammatory activity tests in vitro for the target compounds. Results showed that all compounds could inhibit the production of inflammatory factor NO, and that MST-5 exhibited the strongest anti-inflammatory activity (IC50= 0.428 μmol/L).Further exploration is expected for the study of the anti-inflammatory mechanism of MST-5.
10.Laparoscopic treatment of abdominal and retroperitoneal cystic masses in 11 children

Objective:To explore the feasibility and efficacy of laparoscopic resection of abdominal and retroperitoneal cystic masses in children.Methods:A retrospective analysis of 11 cases of abdominal and retroperitoneal cystic masses in Department of Pediatric Surgery, Quanzhou First Hospital Affiliated to Fujian Medical University from June 2015 to January 2019 was performed, and all the patients underwent laparoscopic resection or laparoscopic-assisted resection, with 6 cases of boys and 5 cases of girls, aged 8 months to 10 years (with the average of 59 months). Meanwhile, 9 cases were from the abdominal cavity and 2 cases were from the retroperitoneum.Results:All patients underwent laparoscopic or laparoscopic-assisted resection without switching to laparotomy.The operation time was 60-210 minutes, with the average of 120 minutes.The intraoperative blood loss was 5-30 mL, with the average of 10 mL.There was no blood transfusion.All patients were discharged 3-8 days after surgery, with the average of 5 days.The postoperative pathological results included 5 cases of mature teratoma, 1 case of paraneoplastic cyst, 2 cases of intestinal duplication, 2 cases of lymphangioma, and 1 case of hepatic cyst.Totally, 11 cases were followed up for 7-51 months, with the average of 20.9 months.No recurrence occurred.Conclusions:Laparoscopic or laparoscopic-assisted resection of abdominal and retroperitoneal cystic masses has advantages of minimal invasion, rapid recovery in children, and it is safe and effective.
