1.Predictive value of changes in body mass index and blood glucose indicators in early pregnancy for gestational diabetes mellitus

Objective:To investigate the predictive value of changes in body mass index (BMI) and blood glucose indicators in early pregnancy for gestational diabetes mellitus (GDM).Methods:A total of 126 pregnant women with GDM who were admitted to the Department of Obstetrics at Jiaxing Maternity and Child Health Care Hospital from January 2022 to December 2023 were retrospectively analyzed and included in the GDM group. Additionally, 140 healthy pregnant women were included in the non-GDM group. Changes in BMI and levels of blood glucose indicators [fasting blood glucose (FBG), 1-hour postprandial blood glucose, 2-hour postprandial blood glucose (2hPG), and glycated hemoglobin during early pregnancy were compared between the two groups. Influential factors for GDM were analyzed using a multivariate logistic regression model. The predictive value of changes in BMI and blood glucose indicators during early pregnancy for GDM was assessed using receiver operating characteristic curves.Results:The BMI at 10 [(24.60 ± 0.82) kg/m2] and 14 weeks of pregnancy [(23.50 ± 0.85) kg/m2] in the GDM group was significantly higher compared with the non-GDM group (both P < 0.05). The levels of FBG [(5.98 ± 0.36) mmol/L], 1-hour postprandial blood glucose [(10.95 ± 0.83) mmol/L], and 2hPG [(8.60 ± 0.82) mmol/L] in the GDM group were also significantly higher than those in the non-GDM group ( t = 12.25, 23.90, 5.98, all P < 0.001). The levels of total cholesterol [(6.35 ± 1.10) mmol/L] and triglycerides [(3.23 ± 0.60) mmol/L] in the GDM group were significantly higher compared with the non-GDM group ( t = 6.45, 7.45, both P < 0.001). The number of cesarean sections ( n = 69 cases) and the incidence of adverse delivery outcomes [26.98% (34/126)] in the GDM group were significantly higher than those in the non-GDM group ( χ2 = 16.72, 12.71, both P < 0.001). Multivariate logistic regression analysis indicated that BMI at 10 weeks of pregnancy, BMI at 14 weeks of pregnancy, FBG, and 2hPG were independent risk factors for GDM ( OR = 1.292, 6.514, 2.601, 1.784, all P < 0.05). According to Receiver Operating Characteristic curve analysis, the area under the curve (AUC) (95% CI) for BMI at 10 and 14 weeks of pregnancy were 0.717 (0.630-0.795) and 0.719 (0.631-0.796), respectively. The AUC (95% CI) for FBG and 2hPG levels were 0.731 (0.635-0.813) and 0.802 (0.712-0.873), respectively. The AUC (95% CI) for the combined evaluation of these indicators was 0.911 (0.839-0.958), indicating that the combined evaluation had a higher predictive value for GDM than the use of a single indicator ( P < 0.05). Conclusions:The changes in BMI and blood glucose indicators during early pregnancy are closely related to the occurrence of GDM. The combination of these measurements has a high predictive value for GDM and is of significant importance for its prevention.
2.The value of IVIM in the differential diagnosis of benign and malignant breast nodules

Objective To explore the application value of intravoxel incoherent motion(IVIM)imaging parameters in the differential diagnosis of benign and malignant breast nodules and their correlation with the expression of Ki-67 receptor in breast cancer.Methods The data of 41 female patients who completed 3.0 T breast magnetic resonance imaging(MRI)with complete surgical pathology results in the Second Hospital of Jiaxing City from October 2022 to January 2025 were analysed and evaluated.Conventional images and diffusion-weighted images with 11 b values were collected.The IVIM imaging parameters of real diffusion coefficient(D),perfusion related diffusion coefficient(D*)and perfusion fraction(f)were measured and calculated in the region of interest of each lesion.The receiver operating characteristic curve were plotted to quantify the differential diagnostic efficacy of each parameter of IVIM imaging in benign and malignant breast nodules.The differences of parameters between benign and malignant breast nodules and between the groups with different expression levels of Ki-67 receptor in breast cancer were analysed,and the correlation between each parameter and the expression level of Ki-67 in breast cancer was counted.Results The D value of benign breast nodules group(benign groups)was significantly higher than that of malignant breast nodules group(malignant groups),and the D*value was significantly lower than that of malignant group,and the differences between benign group and malignant group were statistically significant(t=-4.773,t=2.063,P<0.05);Thefvalue of benign group was slightly lower than that of malignant group,and the differences between two groups were not statistically significant(t=0.035,P>0.05).Among the parameters of IVIM imaging,D value had the best differential diagnostic efficacy for benign and malignant breast nodules,with area under the curve(AUC)of 0.870(95%CI:0.755-0.985)and a specificity of 75.0%;D*value had the second best differential diagnostic efficacy after D value,with an AUC of 0.789(95%CI:0.658-0.920),but it had the highest sensitivity of 88.2%;And the differential diagnosis efficiency off value was the worst,much less than D and D*values.The D value in the high Ki-67 expression group of breast cancer was lower than that in the low expression group,while the D* and f values in the high expression group were higher than that in the low expression group,and the differences of each imaging parameter of IVIM were not statistically significant between two groups(t=-2.617,t=2.169,t=0.647,P>0.05).There was a significant negative correlation between the expression of the Ki-67 receptor and the D value(r=-0.615,P<0.05),and no significant correlation was seen with either D *or f value(r=0.223,r-0.031,P>0.05).Conclusion The D and D*values of IVIM imaging parameters have great clinical value in the differential diagnosis of benign and malignant breast nodules.

Objective:To summarize the clinical characteristics of elderly patients with stage Ⅰ diffuse large B-cell lymphoma (DLBCL) and analyze the factors associated with prognosis.Methods:A case series study was conducted by retrospectively collecting clinical data from patients aged over 60 years with newly diagnosed stage Ⅰ DLBCL across 20 medical centers in Jiangsu Province, China, between June 2010 and April 2023. The involved site, classification and treatment plan were summarized. The primary endpoints were progression-free survival (PFS) and overall survival (OS). Statistical analyses were performed using the Kaplan-Meier method, and Cox regression model.Results:The study included 255 patients with a median age of 69 years, of whom 130 (51.0%) were male, 66 (25.9%) were aged ≥75 years and 26 (10.1%) had a high Charlson Comorbidity Index (CCI) score of ≥2. Extranodal involvement was observed in 163 (63.9%) patients, with the stomach (37.4%, 61/163), intestine (19.0%, 31/163), testes (11.0%, 18/163), and breast (7.4%, 12/163) being the most frequently affected sites. The non-germinal center B-cell (non-GCB) subtype was prevalent in 63.7% of patients (142/223), with no significant difference between the nodal and extranodal groups ( P=0.681). Furthermore, 73.9% (184/249) and 11.7% (29/249) of patients received the R-CHOP (rituximab, cyclophosphamide, doxorubicin, vincristine, prednisone) and R-miniCHOP regimen, respectively. The overall 3-year PFS rate was 81.5%, and the 3-year OS rate was 85.6%. Patients aged ≥75 years ( HR=2.910, 95% CI 1.565-5.408, P=0.001) and/or with a CCI score ≥2 ( HR=2.324, 95% CI 1.141-4.732, P=0.020) had a significantly poorer PFS. Incorporating age ≥75 years and CCI score ≥2 into the stage-modified international prognostic index (sm-IPI) can better stratify the prognosis of elderly patients with stage Ⅰ DLBCL. The 3-year PFS rate was 48.7% in the high-risk group versus 85.7% in the low-risk group ( P<0.001). Conclusions:Our findings show that the elderly patients with stage Ⅰ DLBCL were predominantly characterized by extranodal involvement (particularly in the stomach and intestinal tract) and non-GCB subtype. Age ≥75 years and CCI ≥2 were identified as independent prognostic factors. The newly established sm-IPI-75-CCI incorporating these factors demonstrated superior prognostic discrimination compared to conventional risk assessment systems.
4.Effect of liver-specific NMNAT1 knockout on NAD + and overall metabolism in obese mice

Objective:To investigate the effect of nicotinamide nucleotide adenylyltransferase 1(NMNAT1) liver-specific knockout on nicotinamide adenine dinucleotide(NAD + ) and overall metabolism in obese mice. Methods:NMNAT1 fl/fl mice were injected with AAV8-TBG-CRE through the tail vein to produce NMNAT1 liver-specific knockout mice, which were fed with either chow diet or high-fat diet, and the body weight of the mice was measured weekly, followed by glucose tolerance test, insulin tolerance test, body composition analysis, metabolic cage test and related assays, and finally the mouse liver was taken for liquid chromatography-mass spectrometry to detect the changes of metabolites. The related metabolic pathways were analyzed by metabolic database enrichment. Results:There was no difference in body weight between NMNAT1 LKO mice and control mice, and the body composition measurement of mice by EchoMRI also showed no difference in fat mass and lean mass. Metabolic cage experiments showed that NMNAT1 LKO had no effect on oxygen consumption, carbon dioxide production, respiration exchange ratio, energy expenditure and other indexes in mice. The glucose tolerance test and insulin tolerance test showed that NMNAT1 LKO did not affect glucose tolerance and insulin sensitivity in mice. NMNAT1 LKO does not affect the content of triglycerides in the liver of obese mice. NAD + detection confirmed that NMNAT1 LKO reduced hepatic NAD + content of obese mice by approximately 50%, resulting in the increase and accumulation of the NMNAT1 substrates, i. e. nicotinamide mononucleotide(NMN) and nicotinic acid mononucleotide(NaMN). Conclusion:Liver-specific knockout of NMNAT1 significantly reduced the NAD + content and changed the NAD + metabolism in the liver of obese mice, but without altering the glucose, lipid or energy metabolism.
5.Thrombotic microangiopathy with renal involvement complicated by hematopoietic stem cell transplantation: a case report and literature review

This article reports a rare case of thrombotic microangiopathy (TMA) with renal involvement complicated by allogeneic hematopoietic stem cell transplantation (allo-HSCT). The patient appeared increased serum creatinine 20 d after allo-HSCT, and gradually appeared hypertension, oliguria and edema. Despite discontinuing suspected medications, serum creatinine level did not decrease. Treatment with basiliximab and mycophenolate mofetil was initiated to prevent rejection, leading to gradual normalization of urine output and serum creatinine level. However, after stopping mycophenolate mofetil, the patient experienced recurrent increased blood pressure and decreased pulse oximetry, responding well to prednisone but recurring upon cessation, with gradually increased serum creatinine level. Renal pathology indicated that chronic TMA after allo-HSCT caused renal injury, primarily affecting the glomeruli. The renal function achieved long-term stability through low-dose prednisone and symptomatic treatment. By reviewing relevant literature, we discussed the clinical manifestations, laboratory tests, pathological features and treatment strategies of TMA with renal involvement complicated by allo-HSCT.
