Objective: To establish an accurate and rapid typing method for Rh typing of samples from patients who have received recent blood transfusions by utilizing the difference in osmotic fragility between fresh and old red blood cells. Methods: A lysing solution suitable for destroying old RBCs was prepared. Sixty-one samples collected in our hospital in 2024 with Rh typing of double groups were treated with the lysing solution to remove the old allogeneic red blood cells while preserving the patient's own fresh red blood cells, followed by repeat Rh typing tests. Results: For 61 samples with Rh typing in double groups, 41 were accurately detected identified through the red blood cell lysis method, yielding an identification rate of 67.21%. No significant difference was observed compared to the detection rate of the commonly used capillary centrifugation modified method (χ
=0.103, P>0.05). Conclusion: The red blood cell lysis method provides a novel and rapid experimental approach for clinical use in processing Rh-typed samples that are of double groups, thereby offering a basis for Rh compatibility blood transfusion.

ObjectiveTo investigate the clinical efficacy and safety of Yanling Yishou prescription combined with aerobic exercise in the intervention of heart and spleen deficiency syndrome in middle-aged white-collar workers with "stress-induced aging". MethodsA randomized, controlled, and open-label trial was conducted involving 156 patients with stress-induced aging and heart and spleen deficiency syndrome who were recruited from the Affiliated Hospital of Nanjing University of Chinese Medicine. Participants were randomly assigned via a central randomization system into an intervention group and a control group, with 78 cases in each group. The intervention group received Yanling Yishou prescription combined with a standard aerobic exercise protocol, while the control group received the standard aerobic exercise protocol only. The composition of Yanling Yishou prescription was derived from Qinggong Medical Records: 10 g Ziziphi Spinosae Semen, 10 g Codonopsis Radix, 10 g Astragali Radix, 10 g Paeoniae Radix Alba, 10 g Angelicae Sinensis Radix, 10 g Rehmanniae Radix, 10 g Atractylodis Macrocephalae Rhizoma, 10 g Poria, and 3 g Glycyrrhizae Radix et Rhizoma, decocted in water and administered 2 hours after breakfast and dinner, respectively. The aerobic exercise protocol was prescriptionted according to the 2017 guidelines of the American College of Sports Medicine, consisting of a 5-minute warm-up, a progressively increasing exercise stage (starting at 5 minutes and increasing by 1-2 minutes daily up to a maximum of 20 minutes), and a 10-minute cool-down. Both groups were treated for 28 days and followed up for 35 days. Outcome measures included Checklist Individual Strength (CIS), sleep quality indicators ( total sleep time, light sleep, and deep sleep), traditional Chinese medicine (TCM) symptom scores for heart and spleen deficiency syndrome, Hospital Anxiety and Depression Scale (HADS), physical function score from the SF-36 scale (SF-36 PF), Pain Visual Analog Scale (Pain VAS), and vital signs. ResultsCompared with baseline, the intervention group showed a significantly greater reduction in CIS scores and improvement in sleep quality (P<0.05, P<0.01), while the control group showed no significant changes. Post-treatment comparisons between groups revealed that the intervention group achieved significantly better outcomes in fatigue reduction and sleep improvement (P<0.05, P<0.01). In terms of TCM syndrome scores, the intervention group showed significant improvements in fatigue, frequent awakening, vivid dreaming, poor appetite, palpitations, and loose stools (P<0.05, P<0.01), while the control group showed no significant improvement in poor appetite, palpitations, or loose stools. After treatment, the intervention group showed significantly better improvements in fatigue, frequent awakening, vivid dreaming, and palpitations compared to the control group (P<0.05, P<0.01). No significant differences were observed between the two groups in SF-36 PF, HADS, and Pain VAS scores before and after treatments. No serious adverse events occurred in either group during the study period. ConclusionYanling Yishou prescription combined with aerobic exercise can alleviate symptoms such as fatigue, frequent awakening, vivid dreaming, and palpitations, improve sleep quality, and alleviate heart and spleen deficiency symptoms, thereby improving the quality of life in middle-aged white-collar workers with stress-induced aging.

OBJECTIVE:
To explore the mechanism of acupuncture on improving ovarian hypofunction in aged rats from two perspectives: the overall regulation of the hypothalamic-pituitary-ovarian (HPO) axis and the local ovarian follicle stimulating hormone (FSH)/cyclic adenosine monophosphate (cAMP) signaling pathway.
METHODS:
Six 3-month-old female SPF-grade Sprague-Dawley (SD) rats were selected as the blank group. Another twelve 9-month-old female SD rats were randomly divided into a model group and an acupuncture group, with six rats in each. The acupuncture group received acupuncture at "Baihui" (GV20), "Guanyuan" (CV4), and bilateral "Ciliao" (BL32) for 20 min per session, once every other day, for a total of 10 sessions. Vaginal smear tests were performed daily to observe the estrous cycle of the rats. Ovarian morphology was observed using HE staining, and follicles at various stages were counted. ELISA was used to detect levels of serum FSH, luteinizing hormone (LH), estradiol (E2), anti-müllerian hormone (AMH), hypothalamic gonadotropin-releasing hormone (GnRH), pituitary FSH and LH, and ovarian cAMP. Immunohistochemistry and Western blot were used to detect the protein expression of ovarian cAMP protein kinase catalytic subunit, FSH receptor (FSHR), and P450. Real-time quantitative PCR was used to measure mRNA expression levels of FSHR and P450 in ovarian tissue.
RESULTS:
Compared with the blank group, the model group showed an increased rate of estrous cycle disorder (P<0.01), reduced granulosa cell layers with blurred boundaries and disordered arrangement, decreased numbers of developing follicles at all stages, and increased numbers of atretic follicles (P<0.01); the serum levels of FSH and LH were increased (P<0.01), while E2 and AMH levels were decreased (P<0.01); the hypothalamic GnRH and pituitary FSH and LH levels were elevated (P<0.01), and ovarian cAMP level was decreased (P<0.01); the positive expression and protein expression of ovarian P450, cAMP protein kinase catalytic subunit, and FSHR were reduced (P<0.01), and ovarian FSHR and P450 mRNA expression was decreased (P<0.01). Compared with the model group, the acupuncture group showed a reduced rate of estrous cycle disorder (P<0.01), clear granulosa cell margins, increased numbers of primordial and secondary follicles, and decreased numbers of atretic follicles (P<0.01); the serum FSH and LH levels were decreased (P<0.01, P<0.05), while E2 and AMH levels were increased (P<0.05, P<0.01); the hypothalamic GnRH and pituitary FSH and LH levels were decreased (P<0.01, P<0.05), and ovarian cAMP level was increased (P<0.01); the positive expression and protein expression of ovarian P450, cAMP protein kinase catalytic subunit, and FSHR were elevated (P<0.01), and ovarian FSHR and P450 mRNA expression was increased (P<0.01).
CONCLUSION
Acupuncture could delay ovarian hypofunction in aged rats, possibly through regulating the HPO axis and the FSH/cAMP signaling pathway.

Objective:To investigate the current situation of the use of transjugular intrahepatic portosystemic shunt (TIPS) for portal hypertension, which should aid the development of TIPS in China. Methods:The China Portal Hypertension Alliance (CHESS) initiated this study that comprehensively investigated the basic situation of TIPS for portal hypertension in China through network research. The survey included the following: the number of surgical cases, main indications, the development of Early-TIPS, TIPS for portal vein cavernous transformation, collateral circulation embolization, intraoperative portal pressure gradient measurement, commonly used stent types, conventional anticoagulation and time, postoperative follow-up, obstacles, and the application of domestic instruments. Results:According to the survey, a total of 13 527 TIPS operations were carried out in 545 hospitals participating in the survey in 2021, and 94.1% of the hospital had the habit of routine follow-up after TIPS. Most hospitals believed that the main indications of TIPS were the control of acute bleeding (42.6%) and the prevention of rebleeding (40.7%). 48.1% of the teams carried out early or priority TIPS, 53.0% of the teams carried out TIPS for the cavernous transformation of the portal vein, and 81.0% chose routine embolization of collateral circulation during operation. Most of them used coils and biological glue as embolic materials, and 78.5% of the team routinely performed intraoperative portal pressure gradient measurements. In selecting TIPS stents, 57.1% of the hospitals woulel choose Viator-specific stents, 57.2% woulel choose conventional anticoagulation after TIPS, and the duration of anticoagulation was between 3-6 months (55.4%). The limitation of TIPS surgery was mainly due to cost (72.3%) and insufficient understanding of doctors in related departments (77.4%). Most teams accepted the domestic instruments used in TIPS (92.7%).Conclusions:This survey shows that TIPS treatment is an essential part of treating portal hypertension in China. The total number of TIPS cases is far from that of patients with portal hypertension. In the future, it is still necessary to popularize TIPS technology and further standardize surgical indications, routine operations, and instrument application.

BACKGROUND:For teeth with normal dental crown height, pulp cavity retention crown restoration with different depths of the pulp cavity and different repair materials affects the stress and flexural strength of tooth tissue. For short crown molar defects, the research on pulp cavity repair mainly focuses on clinical observation and in vitro flexural strength experiments. OBJECTIVE:To establish a three-dimensional finite element model for short crown molar restored by the endocrown after root canal treatment to analyze the effects of different pulp cavity retention depths and different repair materials on the distribution and size of dentin equivalent stress. METHODS:Based on establishing the complete model of the short crown mandible first molar,a three-dimensional finite element model was established for repairing the distal adjacent defect of the short crown molar with different pulp cavity retention depths(h=2,3,4 mm)and different repair materials(zirconia, lithium disilicate).Under the oblique loading, the equivalent stress distribution was observed. The peak value of dentin equivalent stress and the mean value of equivalent stress near the bottom of the mesial pulp cavity wall were calculated. RESULTS AND CONCLUSION:(1)Equivalent stress concentration areas:The stress of complete short crown molar and restored models mainly concentrated in the mesial root mesial neck and mesial root lingual neck. The stress concentration area was found in the mesial pulp cavity wall corresponding to the bottom layer of restored models, and the stress concentration was obvious in the 4 mm retention depth group.(2)Under the same repair material, the peak value of dentin equivalent stress was the lowest at 3 mm for all models after repair. The average value of equivalent stress near the bottom of the mesial pulp cavity wall was lowest at 3 mm.(3)Under the same retention depth, there was no significant difference between the two materials in the dentin equivalent stress peak and the mean value near the bottom of the mesial pulp cavity.(4)The results showed that under the conditions of this experiment, the endocrown was used to repair the defect of the short crown molar and the retention depth was 3 mm,which was more beneficial to protect the remaining dental tissue. The selection of zirconia or lithium disilicate as the repair material had little effect on the dentin stress.

TGF-β1 is considered a key mediator in the formation of hepatic fibrosis and mainly acts by activating the downstream Smad signaling pathway.Smad2 and Smad3 are two major downstream regulators that promote TGF-β1-mediated tissue fibrosis, while Smad7 is a negative-feedback regulator of the TGF-β1/Smad pathway and inhibits TGF-β1-mediated hepatic fibrosis.A growing number of studies are showing that natural products can delay the progression of hepatic fibrosis by regulating the TGF-β1/Smad pathway, inhibiting HSC activation, and reducing ECM deposition. This article reviews the molecular mechanism of the TGF-β1/Smad signaling pathway in hepatic fibrosis, and summarizes the natural products that target the regulation of this pathway, providing a reference for research into the treatment of hepatic fibrosis.

In order to understand the difference and expression of genes in CEK cells before and af-ter baicalin interferes with IBV,further reveal and analyze the mechanism of baicalin interfering IBV replication in CEK cells in vitro. After IBV infected CEK cells for 2 h,9.75 mg/L baicalin liq-uid was added to interfere with CEK cells, which was recorded as the baicalin(H-IBV)group,and three replicate wells were set in the control group and the IBV(IBV)infection group. After 36 h culture, cell samples were collected and subject to transcriptome for sequencing. The results showed that there were 102 differentially expressed genes in H-IBV group compared with IBV in-fection group, among which 48 genes weresignificantly up-regulated and 54 genes were significantly down-regulated.Through functional annotation in GO and KEGG databases,it was found that dif-ferentially expressed genes were mainly annotated in biological processes such as cellular proces-ses,biological regulation, metabolism, and secondary pathways such as viral infectious diseases, signal transduction and interaction.Retinol metabolism pathway, phospholipid transfer to mem-brane,IL-27 mediated signaling pathway,MDA5/RIG-I and Toll-like receptor signaling pathway were significantly enriched in CEK cells, and the production of type Ⅰ interferon and interferon al-pha and the process of antiviral infection were also positively regulated. There were more differen-tial genes enriched in nucleic acid catalysis, immune system, and reaction, and interbiological reac-tion.Through the STRING network interaction map,it was found that most immune-related genes could form a 36-node interaction network centered on IRF7,TLR3 and STAT1.Therefore,com-pared with IBV group, the differentially expressed genes after baicalin treatment were mainly an-notated and enriched in the biological process, and the immune system and response were en-hanced,mainly through the positive regulation of IRF7 in the MDA5/RIG-I receptor signaling pathway and the inhibition of TLR3 signal transduction in the Toll-like receptor signaling path-way.Positive regulation of IL-27 mediated pathway and regulation of JAK-STAT signaling path-way were supplemented by activation of the expression of IRF7,TLR3,STAT1 and other related genes, and interaction with corresponding downstream proteins to promote the expression of IFN-α and regulatory cytokines, coupled with negative regulation of viral(defense)response and viral processes. Thus, baicalin interferes with IBV replication in CEK cells.
