1.Correlation between poor position of lumbar pedicle screws and postoperative neurological symptoms

Objective To analyze the correlation between the poor position of pedicle screw after lumbar internal fixation and the occurrence of postoperative neurological symptoms.Methods The clinical data of patients who underwent lumbar pedicle screw surgery in our hospital from January 2017 to December 2023 were analyzed retrospectively.Record the patient's sex,age,diagnosis,surgical segment,the presence of postoperative neurological complications,the types of neurological complications.According to the postoperative CT,the screw penetration zone and grade were evaluated,and the distribution of different pedicle screw penetration types was recorded.The correlation between different types of pedicle screw penetration and postoperative neurological symptoms was analyzed.Results A total of 184 patients were included,including 80 males and 104 females.Age ranged from 18 to 82 years,with an average of 60.1±14.2 years.35 patients developed new neurological symptoms after operation.The incidence of postoperative neurological symptoms caused by pedicle medial quadrant screw penetration and lower quadrant screw penetration was higher than that of pedicle superior quadrant and lateral quadrant screw penetration.From L1 to L5,the risk of postoperative neurological symptoms caused by screw penetration in the medial quadrant of the pedicle increases gradually.When the penetration distance of pedicle medial quadrant screws in L4 and L5 segments is≥2mm,the risk of postoperative neurological symptoms is high.When the penetration distance of pedicle medial quadrant screws in L1,L2 and L3 segments is greater than 4mm,the risk of postoperative neurological symptoms is high.The distance of pedicle screw penetrating the medial quadrant of pedicle was positively correlated with the incidence of postoperative neurological symptoms(P<0.05).Conclusion The lateral and superior quadrants of the pedicle may be regarded as safety zones for lumbar pedicle screw placement.For the L1~L3 segments,screw penetration depth should not exceed 4 mm;whereas for the L4~L5 segments,the safety threshold must be strictly controlled within 2 mm.

To investigate the prevalence of influenza D virus(IDV)in cattle and swine populations in Jilin Province,China,277 nasopharyngeal swabs were collected from livestock exhibiting influ-enza-like symptoms for IDV detection.Virus isolation was performed using swine testicular(ST)cells for PCR-positive samples,followed by comprehensive analyses including whole-genome se-quencing,phylogenetic analysis,electron microscopic observation of viral morphology,and glycosy-lation site prediction.Two IDV strains were successfully isolated from bovine samples,designated as D/bovine/China/JL22/2024(JL22)and D/bovine/China/JL34/2024(JL34).These strains were demonstrated to have specific hemagglutination activity against turkey red blood cells,while no he-magglutination to chicken,rabbit,or guinea pig erythrocytes.Virus-inoculated ST cells exhibited distinct cytopathic effects(CPE)within 48 h,with a hemagglutination titer of 4 log2 in the culture supernatant.Phylogenetic analysis of the hemagglutinin-esterase-fusion(HEF)gene indicated that these strains were most closely related to the Japanese isolate D/Yamagata2019,belonging to the YAMA2019 lineage.Genomic sequence analysis showed the absence of genetic reassortment in these isolates.In this study,two IDV strains were successfully isolated and characterized,which provides preliminary insights into their genomic sequences and biological properties.The findings confirm the presence of IDV in bovine populations in Jilin Province and provide the fundamental data for future epidemiological surveillance and control strategies of IDV.

4.A cohort study on the preventive effect of preserving the urethral ridge in transurethral Holmium laser enucleation of the prostate on retrograde ejaculation

Objective:To investigate the efficacy of preserving the urethral ridge during Holmium laser enucleation of the prostate(HoLEP)in preventing postoperative retrograde ejaculation and to evaluate its impact on sexual function.Methods:This prospective cohort study enrolled patients with benign prostatic hyperplasia(BPH)who underwent HoLEP at Xiamen Haicang Hospital(Haicang Hospital Affiliated to Xiamen Medical College)from November 2022 to June 2024. Inclusion criteria were as follows:diagnosis of BPH confirmed by color Doppler ultrasound;International Prostate Symptom Score(IPSS)> 7;maximum urinary flow rate(Q max)< 15 ml/s;prostate-specific antigen(PSA)< 4 ng/ml;and an active sexual life with intact antegrade ejaculation. Exclusion criteria included neurogenic bladder,active urinary tract infection(UTI),and other relevant conditions. Patients were grouped based on the operating surgeon's comprehensive judgment during surgery,considering the degree of prostatic median lobe hyperplasia(preserved if hyperplasia was mild,not preserved if severe). The EP-HoLEP group underwent “tunnel technique” enucleation of the middle lobe hyperplasia with preservation of the urethral ridge,while the HoLEP group underwent conventional prostate enucleation. Primary outcomes included postoperative retrograde ejaculation rate,International Index of Erectile Function(IIEF),Ejaculation Projection Score(EPS),IPSS,Quality of Life Score(QOL),Q max,post-void residual urine volume(PVR),operative time,and postoperative complications. Univariate analysis was used to screen potential influencing factors,followed by multivariate logistic regression to identify independent predictors. Results:Seventy patients with BPH were enrolled,with 35 in each group. Baseline characteristics,including age[(69.97 ± 5.14)years vs.(72.34 ± 5.08)years],body mass index(BMI)[(22.99 ± 1.41)kg/m2 vs.(23.16 ± 1.38)kg/m2],prostate volume[47.4(31.9,59.4)ml vs. 44.2(34.9,61.7)ml],PSA[4.0(1.9,8.2)ng/ml vs. 3.1(2.6,5.0)ng/ml],hemoglobin[(130.09 ± 12.92)g/L vs.(125.69 ± 17.26)g/L],IPSS[17(10,22)vs. 17(10,27)],QOL[5(4,5)vs. 4(4,5)],Q max[7.5(6.3,9.1)ml/s vs. 7.0(5.9,8.9)ml/s]and PVR[65(22,167)ml vs. 60(16,150)ml]showed no statistically significant differences between the two groups( P > 0.05). Operative time[65(55,76)min vs. 63(55,73)min],postoperative 2-hour hemoglobin[(124.17 ± 14.89)g/L vs.(120.11 ± 15.44)g/L],and postoperative hospital stay[(3.94 ± 1.89)days vs.(3.66 ± 1.53)days]were also comparable between the two groups( P > 0.05). No significant difference was observed in the decrease in IIEF score[1(0,2)vs. 2(0,6), P = 0.203]. EPS at 3 months[2(1,3)vs. 1(0,2), P < 0.001]and at 6 months[2(1,2)vs. 1(0,2), P < 0.001]postoperatively were significantly higher in the EP-HoLEP group. The incidence of postoperative UTI did not differ significantly[5.7%(2/35)vs. 2.9%(1/35), P = 1.00]. Two cases of urinary retention occurred after catheter removal in the EP-HoLEP group,while none occurred in the HoLEP group. No blood transfusions or urethral strictures were reported in either group. The incidence of retrograde ejaculation was significantly lower in the EP-HoLEP group[28.6%(10/35)vs. 68.6%(24/35), P <0.001]. Multivariable logistic regression analysis showed that urethral ridge preservation was an independent protective factor for retrograde ejaculation after HoLEP( OR = 0.159,95% CI 0.053 ? 0.476, P = 0.001). Conclusions:Urethral ridge preservation during HoLEP is safe and feasible,significantly reduces retrograde ejaculation,and preserves ejaculatory function,though it offers limited erectile function preservation. This approach is suitable for middle-aged,young,or younger elderly patients who prioritize ejaculatory quality,and provides clinical evidence for surgical optimization.
5.Research progress on Noggin′s regulation of osteoporosis in signaling pathways

Noggin is a protein secreted by human cells and belongs to the bone morphogenetic protein (BMP) family, which can inhibit the formation and proliferation of bone tissue. The initial research found that Noggin is mainly involved in the development of the embryonic skeletal and nervous systems in vertebrate embryos. Subsequent studies have found that Noggin also plays an important role in bone metabolism and bone mineral density regulation in adults, mainly by regulating the activities of signaling pathways such as Wnt/β-catenin, notch and hedgehog. Meanwhile, there is also a certain mutual influence among various channels. This article mainly reviews the relevant research on the regulation of osteoporosis by Noggin in signaling pathways, clarifies the related mechanism of Noggin in various signaling pathways, and analyzes the current research status and prospects of Noggin.

9.The correlation between FLAIR sequence high signal vascular sign and magnetic resonance perfusion weighted imaging in patients with middle cerebral artery stenosis cerebral infarction

Objective:To evaluate the correlation between vascular hyperintensity of magnetic resonance fluid-attenuated inversion recovery (FLAIR) sequence(FVH) and related parameters of magnetic resonance perfusion weighted imaging (MR-PWI) in patients with middle cerebral artery stenosis cerebral infarction, and to explore the hemodynamic factors related to FVH and the effect of FVH on the short-term clinical prognosis of patients.Methods:A total of 116 patients with middle cerebral artery stenosis cerebral infarction in the Department of Neurology, Qingpu Branch of Zhongshan Hospital Affiliated to Fudan University from January 2020 to December 2022 were collected.According to the diagnostic criteria of FVH, they were divided into FVH (+ ) group (78 cases) and FVH(-) group (38 cases). All patients underwent magnetic resonance(MR) and MR-PWI scans.Demographic and cerebrovascular risk factors were collected, clinical neurological function of patients was assessed by national institate of health stroke scale(NIHSS) upon admission and discharge, and cognitive function of patients was assessed by mini-mental state examination (MMSE). Short-term clinical outcome was assessed using modified Rankin scale(mRS) at the 90th day after discharge.The degree of middle cerebral artery stenosis, positive or negative FVH, FVH score, hypoperfusion volume and MR-PWI related parameters, including peak time (Tmax), mean transit time (MTT), cerebral blood volume (CBV) and cerebral blood flow (CBF), were evaluated in relation to clinical symptoms.SPSS 22.0 statistical software was used for t test, Chi-square test and Pearson correlation analysis. Results:There were significant differences in hypoperfusion volume, Tmax, MTT and CBF between FVH (+ ) group and FVH(-) group( t=1.989, 3.830, 5.223, 3.911, all P<0.05). In terms of short-term clinical outcome, the improvement rate of neurological function ((8.25±6.39)%, (12.22±6.08)%) and MMSE score(25.48±1.59), (26.31±1.26) in FVH (+ ) group were significantly lower than those in FVH(-) group, and the number of patients with progressive stroke during hospitalization in FVH(+ ) group was more than that of FVH(-) group(22(28.21%), 4(10.53%)) (all P<0.05). Pearson correlation analysis showed that FVH score was positively correlated with hypoperfusion volume ( r=0.786, P<0.01) and MTT ( r=0.692, P<0.01), and negatively correlated with CBF ( r=-0.568, P<0.01), but no significant correlation with the degree of arterial stenosis ( r=0.363, P>0.05). Conclusion:FVH is closely related to the Tmax, MTT and CBF values shown in MR-PWI, and the incidence of stroke in progression and short-term adverse prognosis are more likely in FVH(+ ) group, suggesting that FVH can be used as a convenient imaging indicator to reflect the hypoperfusion status of patients with middle cerebral artery stenosis cerebral infarction, and can provide an objective basis for further individualized treatment.
10.Experience of Fire Needling Combined with Black Cloth Ointment in the Treatment of Keloid based on the Principle of "Phlegm-Stasis-Tumor"

This paper summarized the experience of fire needling combined with black cloth ointment in the treatment of keloid. In terms of the pathogenesis of keloid, it is believed that "phlegm causes stasis, and then stasis accumulates into tumors" is the key; the internal obstruction of phlegm and stasis caused by insufficient endowment and invasion of external pathogens is the root of the disease, while the endogenous generation of phlegm and dampness is the branch. It is therefore proposed to explore the etiology and pathogenesis of keloid from the perspective of "phlegm-stasis-tumor", and the treatment principle is mainly to eliminate phlegm, dispel stasis and dissolve tumors, supplemented with heat-clearing and toxin-resolving. Treatment methods include oral administration of Chinese herbal medicinals and external treatment. Self-made formula for keloid is taken orally, with the main functions of dissolving phlegm, dispelling stasis and clearing heat, to finally dissolve the tumors. For external treatment, fire needling is used to pierce the skin and to open the striae and interstices, and force out the toxin pathogens such as phlegm-heat and stasis-heat with fire power, thereby clearing heat, dissolving phlegm, dispelling stasis and resolving toxins. Black cloth ointment can be used to the affected area to unblock meridians and collaterals, clear heat-toxin and dissolve phlegm-stasis, thereby softening hardness and dissipating masses.
