1.Association of personality and sleep quality with psychological distress of junior and senior high school stduents

Objective:
To explore the effects of personality and sleep quality with psychological distress of junior and senior high school stduents, so as to provide a reference basis for precise interventions of junior and senior high school students mental health.
Methods:
In October 2023, a convenience sampling method was used to select 9 034 students aged 12-17 from Shiyan City as the study subjects. The Pittsburgh Sleep Quality Index (PSQI) and Kessler Psychological Distress Scale (K10) were used to collect information on sleep quality and psychological distress of junior and senior high school stduents. Between group comparison was conducted by using t-test and Chi-square test. Generalized linear models were employed to analyze the interaction and joint effects of personality and sleep quality on psychological distress.
Results:
The generalized linear model analysis showed that the interaction between personality and sleep quality on psychological distress was statistically significant of junior and senior high school students(effect size=0.80, P <0.01). The general linear model analysis indicated that, after adjusting for variables such as age, gender, screen time, and daily sitting time with the extroverted and good sleep quality group as the reference, the introverted and poor sleep quality group had the largest mean difference in psychological distress scores (difference=0.51, P <0.05). When stratified by sleep quality, psychological distress scores were higher in the introverted and neutral personality groups with both poor and good sleep quality compared to the extroverted group (poor sleep quality: introverted difference=3.71, neutral difference=1.14; good sleep quality: introverted difference=2.23, neutral difference=0.57, all P < 0.05). When stratified by personality, psychological distress scores were higher in the poor sleep quality groups for introverted, neutral, and extroverted individuals compared to their good sleep quality counterparts (differences=8.66, 7.83, 7.34, all P < 0.05 ).
Conclusions
Personality and sleep quality have interactive and joint effects on psychological distress of junior and senior high school stduents. Personalized psychological interventions should be developed based on personality and sleep quality.

Background Swimming is becoming increasingly popular for its combined leisure and fitness benefits. However, polluted swimming pool water may pose various health risks. BACKGROUND:
Valvular heart disease (VHD) has become increasingly common with the aging in China. This study aimed to evaluate regional differences in the clinical features, management strategies, and outcomes of patients with VHD across different regions in China.
METHODS:
Data were collected from the China-VHD Study. From April 2018 to June 2018, 12,347 patients who presented with moderate or severe native VHD with a median of 2 years of follow-up from 46 centers at certified tertiary hospitals across 31 provinces, autonomous regions, and municipalities in Chinese mainland were included in this study. According to the locations of the research centers, patients were divided into five regional groups: eastern, southern, western, northern, and central China. The clinical features of VHD patients were compared among the five geographical regions. The primary outcome was all-cause mortality or rehospitalization for heart failure. Kaplan-Meier survival analysis was used to compare the cumulative incidence rate.
RESULTS:
Among the enrolled patients (mean age, 61.96 years; 6877 [55.70%] male), multiple VHD was the most frequent type (4042, 32.74%), which was mainly found in eastern China, followed by isolated mitral regurgitation (3044, 24.65%), which was mainly found in northern China. The etiology of VHD varied significantly across different regions of China. The overall rate of valve interventions was 32.67% (4008/12,268), with the highest rate in southern China at 48.46% (205/423). In terms of procedure, the proportion of transcatheter valve intervention was relatively low compared to that of surgical treatment. Patients with VHD in western China had the highest incidence of all-cause mortality or rehospitalization for heart failure. Valve intervention significantly improved the outcome of patients with VHD in all five regions (all P <0.05).
CONCLUSIONS:
This study revealed that patients with VHD in China are characterized by significant geographic disparities in clinical features, treatment, and clinical outcomes. Targeted efforts are needed to improve the management and prognosis of patients with VHD in China according to differences in geographical characteristics.

4.Targeted inhibition of macrophage STING signaling alleviates inflammatory injury and ventricular remodeling in acute myocardial infarction.

Mitochondrial DNA (mtDNA) acts as a damage-associated molecular pattern to activate the stimulator of interferon genes (STING) signaling in macrophages, promoting tissue inflammation. However, its role in acute myocardial infarction (AMI) remains unclear. Macrophage-specific Sting1 knockout mice were used to validate STING's pathological role in AMI. Cardiac and liver mtDNA were used to activate macrophages in co-culture systems with cardiomyocytes to assess fibrosis and hypertrophy. Panaxatriol saponin (PTS) was tested for its ability to block mtDNA-driven macrophage activation and subsequent cardiomyocyte damage. STING-PTS binding ability was analyzed. AMI rats received PTS to evaluate its effects on myocardial inflammation and ventricular remodeling. In vivo, macrophage-specific Sting1 knockout reduced myocardial inflammation and injury after AMI. In vitro, mtDNA-activated macrophages induced cardiomyocyte fibrosis and hypertrophy through STING signaling. PTS suppressed mtDNA-driven macrophage activation by directly binding STING, thereby blocking inflammatory cascades. In AMI rats, PTS treatment attenuated acute inflammation and reversed ventricular remodeling. These findings establish the mtDNA-STING axis in macrophages as a critical driver of post-AMI inflammation and identify pharmacological STING inhibition with PTS as a promising therapeutic strategy. The study bridges genetic validation with translational applications, highlighting macrophage STING as a novel target for ischemic heart disease management.
5.Deubiquitinase OTUD6A alleviates acetaminophen-induced liver injury by targeting EZH2 to reduce cell death in hepatocytes.

Acetaminophen (APAP) is the primary cause of drug-induced acute liver failure. Ovarian tumor deubiquitinase 6A (OTUD6A), a recently discovered deubiquitinase of the OTU family, has been primarily studied in tumor contexts. However, its role in APAP-induced liver injury (AILI) remains unclear. Therefore, this study aimed to investigate the involvement of OTUD6A in the pathogenesis of AILI. Our findings demonstrated a substantial upregulation of OTUD6A in both the liver tissue and isolated hepatocytes of mice following APAP stimulation. OTUD6A knockout exacerbated APAP-induced inflammation, hepatocyte necrosis, and liver injury, whereas OTUD6A overexpression alleviated these pathologies. Mechanistically, OTUD6A directly interacted with the enhancer of zeste homolog 2 (EZH2) and selectively removed K48-linked polyubiquitin chains from EZH2, enhancing its stability. This resulted in increased protein levels of EZH2 and H3K27me3, as well as reduced endoplasmic reticulum (ER) stress and cell death in hepatocytes. Collectively, our research uncovers a novel role for OTUD6A in mitigating APAP-induced liver injury by promoting EZH2 stabilization.
6.Effect analysis of trihalomethane reduction in the raw water from Qingcaosha reservoir using various water treatment processes

ObjectiveTo investigate the content of trihalomethanes (THMs) in treated water after different water treatment processes and their correlations with premanganate index, so as to provide data support for the renovation of water production process and optimization of water quality improvement. MethodsFrom 2022 to 2023, seven centralized water supply units using raw water from Qingcaosha reservoir were selected as the testing sites, among which three units with the conventional treatment process, two units with the advanced treatment process, and two units with the advanced treatment process combined CO2 treatment. Monthly water quality testing data were collected, focusing on testing the concentration variations of THMs, trichloromethane, dibromochloromethane, bromodichloromethane, bromoform, and permanganate index. ResultsThe comparison between conventional treatment process and advanced treatment process demonstrated that the conventional treatment process exhibited significantly higher concentrations of trihalomethanes, trichloromethane, bromodichloromethane, and permanganate index in water samples (all P<0.05). When comparing conventional treatment process with advanced treatment process combined with carbon dioxide treatment, the conventional treatment process showed significantly elevated levels of trihalomethanes, dibromochloromethane, bromodichloromethane, and permanganate index (all P<0.05). No statistically significant differences were observed in the comparison of various indicators between advanced treatment process and advanced treatment process combined with carbon dioxide treatment for any of the measured parameters (all P>0.05). Analysis of seasonal variations revealed that finished water during the high-temperature period (May to November) contained significantly higher concentrations of trihalomethanes, trichloromethane, bromodichloromethane, and tribromomethane compared to the low-temperature period (December to April of the following year) (all P<0.05). Significant positive correlations were identified between permanganate index and trihalomethanes (r=0.213, P=0.007), permanganate index and dibromochloromethane (r=0.186, P=0.019), permanganate index and bromodichloromethane (r=0.243, P=0.002), permanganate index and tribromomethane (r=0.193, P=0.014). ConclusionCompared to the conventional water treatment process, advanced treatment process and advanced treatment combined with CO2 injection process can significantly reduce the concentrations of THMs in the treated effluent water. Besides, the generation of THMs is affected by seasonal temperatures, with higher concentrations of THMs, trichloromethane, bromodichloromethane, and bromoform being observed in the high-temperature season. Additionally, the permanganate index shows a significant positive correlation with THMs concentrations, indicating that the content of organic matter in the source of raw water contributes to the generation of THMs in the treated water.
7.Difficulties in the Differentiation and Treatment of Diabetic Kidney Disease and Its Clinical Treatment Model

Diabetic kidney disease (DKD) is one of the main causes of chronic kidney disease. Both traditional Chinese medicine (TCM) and western medicine have their own advantages in the prevention and treatment of DKD, but there are also many difficulties. By analysis of the difficulties faced by TCM and western medicine in the differentiation and treatment of DKD, based on the theory of "miniature masses in the renal collaterals", combined with long-term clinical practice, "internal heat leading to mass" is proposed as the core pathogenesis of DKD. Therefore, a trinity model of "disease-syndrome-symptom" for differentiation and treatment of DKD based on the core pathogenesis has been proposed. This model highlights the status of the core pathogenesis of "internal heat leading to mass" in DKD, and conducts a three-dimensional identification from the perspectives of disease, syndrome and symptom, so as to inspire clinical practice.
8.Value of contrast-enhanced ultrasound liver imaging reporting and data system for differential diagnosis between hepatocellular carcinoma and other hepatic malignancies less than or equal to 3 cm

Objective To investigate the value of contrast-enhanced ultrasound(CEUS)liver imaging reporting and data system version 2017(LI-RADS v2017)in the differential diagnosis between hepatocellular carcinoma(HCC)and other hepatic malignancies(OM)less than or equal to 3 cm.Methods The clinical and imaging data of 163 liver lesions less than or equal to 3 cm in 126 patients with pathologically-confirmed liver malignancy were retrospectively analyzed.The patient underwent CEUS from Jan.2018 to Jun.2022 in our hospital,including 133 with HCC and 30 with OM.Nodules were classified based on the CEUS features according to the CEUS LI-RADS v2017.The evaluation of CEUS characteristics and CEUS LI-RADS v2017 in differential diagnosis of HCC and OM were calculated based on pathological findings.Results There were significant differences in the characteristics of arterial phase enhancement,timing of washout onset and washout degree between the HCC group and the OM group(all P<0.001).The HCC group was mainly LR-3(20 lesions,15.04%),LR-4(25 lesions,18.80%)and LR-5(82 lesions,61.65%),and the OM group was mainly LR-M(28 lesions,93.33%).The specificity and positive predictive value of late washout for HCC were 93.33%(28/30)and 98.06%(101/103),respectively;and the sensitivity of early washout for OM was 93.33%(28/30).With LR-5 as the criterion to diagnose HCC,the sensitivity was 61.65%(82/133),the specificity was 93.33%(28/30),and the positive predictive value was 97.62%(82/84);with LR-4/5 as the criterion to diagnose HCC,the sensitivity was 80.45%(107/133),the specificity was 93.33%(28/30),and the positive predictive value was 98.17%(107/109),with the sensitivity of LR-4/5 being significantly higher than those of LR-5(P=0.001).With LR-M as the criterion to diagnose OM,the sensitivity was 93.33%(28/30)and the specificity was 95.49%(127/133).Conclusion The CEUS LI-RADS v2017 can effectively distinguish HCC from OM less than or equal to 3 cm.LR-5 has a low sensitivity for HCC,while LR-4/5 can improve the sensitivity for HCC.
9.Comparative Study of Clinical Trial Ethics Between China and Foreign Countries Based on CiteSpace

Objective To provide relevant suggestions for ethical considerations in clinical research by searching literature related to ethics in clinical research using the visualization analysis software CiteSpace,and to analyze and compare the current research status of clinical research ethics at home and abroad.Methods The papers on the application of medical ethics in clinical trials from the core collection of CNKI and Web of Science databases were collected and the bibliometric software CiteSpace was used to draw a knowledge map of domestic and foreign literature.The research hotspots and development trends of medical ethics in clinical trials were analyzed.Results In terms of the process of informed consent,risk minimization,fair selection,and protection on the rights and interests of vulnerable groups,the focus of medical ethics research in China were basically consistent with foreign literature.The difference was that foreign research focused more on ethical education,the quality of life of research participants,and ethical counseling.Otherwise,domestic research focused more on the quality management of clinical trials and the specificity of traditional Chinese medicine research,with research on vulnerable groups mostly focused on children.Conclusion Chinese researchers should strengthen multi-dimensional research on medical ethics in China,and gradually expand their view from ethical review points and management research to applied research solved clinical problems and the specificity of research participants,and increasing the number and dimensions of research.
10.Effectiveness of cognitive behavioural therapy in reducing anxiety and promoting rapid postoperative recovery in abortion patients

Objective:To evaluate the effect of cognitive behavioral therapy (CBT) on reducing anxiety and promoting rapid postoperative recovery in patients undergoing induced abortion surgery.Methods:Non-randomized controlled trials were adopted, a total of 480 patients scheduled for induced abortion between January 2024 and January 2025 were consecutively enrolled and allocated to the intervention group ( n=240) or control group ( n=240). The control group received conventional education, whereas the intervention group implemented cognitive behavioral therapy on the basis of control group. The Hamilton anxiety scale (HAMA), the self-rating anxiety scale (SAS) and the 36-item short-form health survey (SF-36) were the main observed outcome measures. The anxiety, pain degree 24 h after the intervention, postoperative recovery, postoperative complications and changes in quality of life after the intervention were compared between the two groups of patients. Taking the baseline characteristics of the patients as independent variables and the scores of HAMA, SAS and SF-36 as dependent variables, multiple linear regression analysis was conducted. Results:After the intervention, the scores of HAMA and SAS in the intervention group were 5.76±1.13 and 40.98±3.76 respectively, which were lower than those in control group (7.08±1.42; 46.42±4.14), and the differences were statistically significant (all P<0.001). The score of the visual analogue scale 24 h after the operation was lower than that in control group (1.64±0.48 vs. 2.83±0.76), and the difference was statistically significant ( P<0.001). The duration of vaginal bleeding, the time of menstrual resumption and the recovery time of daily activities in the intervention group were all shorter than those in control group [(8.23±2.36) d vs. (10.87±2.16) d; (28.34±3.65) d vs. (33.47±4.18) d; (18.98±5.24) d vs. (25.42±4.16) d], and the differences were statistically significant (all P<0.001). Six weeks after the operation, the total incidence of complications in the intervention group was 5.0% (12/240), which was lower than that in control group [12.9% (31/240)], and the difference was statistically significant ( P=0.002). After the intervention, the total score of each dimension of SF-36 was higher than that in control group (334.68±5.79 vs. 305.46±5.93), and the difference was statistically significant ( P<0.001). Multiple linear regression analysis showed that higher educational attainment (bachelor's degree and above compared with junior high school and below HAMA β=-0.52, 95% CI: -0.85--0.19, P=0.002; SAS β=-0.75, 95% CI: -1.31--0.19, P=0.008) and higher household income (HAMA β=-0.15, 95% CI: -0.22--0.08, P<0.001; SAS β=-0.18, 95% CI: -0.28--0.08, P<0.001) were protective factors for reducing anxiety levels. Married was the protection factor in SAS scores ( β=-0.68, 95% CI: -1.22--0.14, P=0.013), and higher number of pregnancies was a risk factor for SAS scores ( β=0.19, 95% CI: 0.07-0.31, P=0.002). Age increase was a protective factor for HAMA score ( β=-0.03, 95% CI: -0.05--0.01, P=0.001). Higher educational attainment (junior college: β=3.25, 95% CI: 0.60-5.90, P=0.016; bachelor's degree and above: β=5.02, 95% CI: 2.33-7.71, P<0.001), higher household income ( β=1.25, 95% CI: 0.55-1.95, P<0.001), and married status ( β=2.81, 95% CI:0.28-5.34, P=0.030) were all significant positive predictors of quality of life. Conclusion:Cognitive-behavioral therapy effectively alleviates anxiety, reduces post-operative complications, accelerates recovery, and improves the quality of life in women undergoing induced abortion.
