1.Association of copy number variation in X chromosome-linked PNPLA4 with heterotaxy and congenital heart disease

Background::Heterotaxy (HTX) is a thoracoabdominal organ anomaly syndrome and commonly accompanied by congenital heart disease (CHD). The aim of this study was to analyze rare copy number variations (CNVs) in a HTX/CHD cohort and to examine the potential mechanisms contributing to HTX/CHD.Methods::Chromosome microarray analysis was used to identify rare CNVs in a cohort of 120 unrelated HTX/CHD patients, and available samples from parents were used to confirm the inheritance pattern. Potential candidate genes in CNVs region were prioritized via the DECIPHER database, and PNPLA4 was identified as the leading candidate gene. To validate, we generated PNPLA4-overexpressing human induced pluripotent stem cell lines as well as pnpla4-overexpressing zebrafish model, followed by a series of transcriptomic, biochemical and cellular analyses. Results::Seventeen rare CNVs were identified in 15 of the 120 HTX/CHD patients (12.5%). Xp22.31 duplication was one of the inherited CNVs identified in this HTX/CHD cohort, and PNPLA4 in the Xp22.31 was a candidate gene associated with HTX/CHD. PNPLA4 is expressed in the lateral plate mesoderm, which is known to be critical for left/right embryonic patterning as well as cardiomyocyte differentiation, and in the neural crest cell lineage. Through a series of in vivo and in vitro analyses at the molecular and cellular levels, we revealed that the biological function of PNPLA4 is importantly involved in the primary cilia formation and function via its regulation of energy metabolism and mitochondria-mediated ATP production. Conclusions::Our findings demonstrated a significant association between CNVs and HTX/CHD. Our data strongly suggested that an increased genetic dose of PNPLA4 due to Xp22.31 duplication is a disease-causing risk factor for HTX/CHD.
2.Exploration of the Mechanism of Baihe Dihuang Decoction in Treating Insomnia with Intestinal Flora Disturbance in Mice Based on TLR4/NF-κB/MLCK Pathway

Objective Based on TLR4/NF-κB/MLCK pathway,the therapeutic effect and mechanism of Baihe Dihuang Decoction on insomnia with intestinal flora disturbance in mice induced by p-chlorophenlalanine(PCPA)and multi-factor stimulation were studied.The aim is to provide theoretical basis for clinical use.Methods Eighty-four KM mice were randomly divided into normal group,model group,positive group(Diazepam,1.38 mg·kg-1),Baihe group(2.25 g·kg-1),Dihuang group(2.25 g·kg-1)and Baihe Dihuang Decoction group(4.5 g·kg-1).Insomnia mouse model was established by intraperitoneal injection of PCPA for 2 days combined with 4 weeks of multi-factor stimulation,including stimulating the tail with forceps clip for 2 minutes,reversing day and night for 24 hours,wetting the padding for 24 hours,tilting the cage at 45° for 24 hours,alternating the cages for 24 hours,fasting food for 24 hours,and getting cold bath for 3 minutes,etc.After successfully modeling,corresponding drug treatment was given.The anxiety-like behavior of mice was observed by elevated cross maze system.The latency and duration of sleep were observed by righting reflex experiment.16sRNA sequencing was used to analyze the composition and structure of intestinal flora in mice.The concentration changes of γ-aminobutyric acid(GABA),glutamic acid(Glu),tryptophan(Trp),5-hydroxytryptophan(5-HTP)and 5-hydroxytryptamine(5-HT)in brain and colon were detected by liquid chromatography-mass spectrometry(LC-MS).Immunohistochemical method was used to detect the expressions of zona atresia protein 1(ZO-1)and Occludin in colon.Real-time fluorescence quantitative PCR(qRT-PCR)was used to detect the genes including interleukin-1β(IL-1β),interleukin-6(IL-6),tumor necrosis factor-α(TNF-α),ZO-1,Occludin,Toll-like receptor 4(TLR4),nuclear factor κB(NF-κB)and myosin light chain kinase(MLCK)in colonic tissue.Western Blot was used to detect the expressions of TLR4,NF-κB,phosphorylated nuclear factor κB(p-NF-κB),MLCK,myosin light chain(MLC)and phosphorylated myosin light chain(p-MLC)in colonic epithelial tissue.Results Compared with the normal group,the distance of entering the open arm and the duration of stay in the open arm of model group in the elevated cross maze were significantly shortened(P<0.05).The sleep latency was significantly prolonged,and the sleep duration was significantly shortened(P<0.05).The richness and uniformity of intestinal flora were decreased(P<0.05,P<0.01).The concentration of neurotransmitters GABA,Trp,5-HTP and 5-HT in the brain decreased significantly(P<0.01),while the concentration of Glu increased significantly(P<0.01).The concentration of GABA and Glu in colon decreased significantly(P<0.01),while the concentration of Trp,5-HTP and 5-HT increased significantly(P<0.01).The expression levels of inflammatory factors IL-1β,IL-6 and TNF-α in colonic tissue were significantly increased(P<0.01),and the expression levels of ZO-1 and Occludin genes and proteins were significantly decreased(P<0.01).The gene expression levels of TLR4,NF-κB and MLCK were significantly increased(P<0.01),and the protein expression levels of TLR4,p-NF-κB,MLCK and p-MLC were significantly increased(P<0.01).Compared with the model group,Baihe Dihuang Decoction could significantly prolong the distance of entering the open arm and the duration of stay in the open arm of insomnia mice in the elevated cross maze(P<0.05).The sleep latency was significantly shortened,and the sleep duration was significantly increased(P<0.05).The richness and uniformity of intestinal flora were increased(P<0.05,P<0.01).The concentration of neurotransmitters GABA,Trp,5-HTP and 5-HT in brain was increased(P<0.05,P<0.01),and the concentration of Glu was decreased(P<0.01).The concentration of GABA and Glu in colon was increased(P<0.01),while the concentration of Trp,5-HTP and 5-HT was decreased(P<0.05,P<0.01).The expression levels of IL-1β,IL-6 and TNF-α genes were down-regulated(P<0.01),the expression levels of ZO-1 and Occludin genes and proteins were up-regulated(P<0.01),TLR4,NF-κB,MLCK gene expression levels and TLR4,p-NF-κB,MLCK,p-MLC protein expression levels were down-regulated in the pathway(P<0.01).Conclusion Baihe Dihuang Decoction can effectively treat insomnia with intestinal flora disorders.Its mechanism of action may be related to the regulation of brain and intestinal neurotransmitter disorders,down-regulating TLR4/NF-κB/MLCK signaling pathway,and up-regulating tight junction proteins expression,reducing inflammatory responses,and then repairing the mechanical barrier of intestinal mucosa.
3.Analysis of mask fit testing based on two-dimensional photographic measurement of facial shape

Objective To investigate the pass rates of fit tests for various brands of medical protective masks and to explore methods for quickly matching these masks based on their head and face dimensions.Methods A total of 202 medical staff from designated hospitals in Tianjin were selected as subjects.Quantitative fit tests were conducted on 5 brands of masks(A,B,C,D,and E)using an aerosol condensation nucleus counter.Two-dimensional photographic measurement was used to obtain the face length and width of the subjects,categorizing them into face types#1 to#10.The pass rates of masks across different face zones,brands,and face types were compared.Results A total of 202 testers participated in this study.According to the guidelines,face type#1 was the most common[43.6%(88/202)],followed by face type#3[18.2%(37/202)].The majority of subjects were categorized as face types#1,#2,#3,and#4,totaling 176 subjects(87.1%).A total of 914 tests were conducted,with 678 passes,resulting in an overall mask pass rate of 74.18%.The pass rates of masks A,B,and C were significantly higher than those of masks D and E[87.03%(161/185),85.57%,(166/194),82.02%(146/178)vs.62.98%(114/181),51.70%(91/176),all P<0.05].The pass rate of adjustable head-mounted masks was significantly higher than that of non-adjustable masks[79.54%(587/738)vs.51.70%(91/176),P<0.05].The fit factor(FF)for mask B in face types#1 to#5 was significantly higher than that in face types#6 to#10[200(163,200)vs.132(86,200),P<0.05].Conclusions Two-dimensional photographic measurement can quickly obtain facial information of the subjects and match the corresponding masks.Hospitals can match masks with higher test pass rates according to the proportion of face types among medical staff.When selecting masks,preference should be given to adjustable head-mounted masks.
4.Xuandi Ziyin Mixture (玄地滋阴合剂) for Central Precocious Puberty in Girls with Syndrome of Yin Deficiency and Fire Exuberance: A Prospective Cohort Study

ObjectiveTo observe the clinical effectiveness and safety of Xuandi Ziyin Mixture (玄地滋阴合剂) for central precocious puberty (CPP) in girls with syndrome of yin deficiency and fire exuberance, and to analyse the effect of body mass index (BMI) on the effectiveness. MethodsA total of 236 girls with CPP of yin deficiency and fire exuberance syndrome were included, and all of them were given Xuandi Ziyin Mixture, 30 ml each time, twice a day, for a total treatment period of 6 months. Before and after treatment, children's weight, height and bone age were measured, BMI and BMI Z-score (BMI Z) and the difference between bone age and actual age were calculated; ultrasound was used to detect uterine and ovarian sizes, and to calculate uterine volume (Vuterus), bilateral ovarian volume (Vleft ovary, Vright ovary), and bilateral maximal follicle diameters (rleft follicle and rright follicle); and serum sex hormones were measured, including follicle-stimulating hormone (FSH), luteinising hormone (LH), prolactin (PRL), estradiol (E2), and testosterone (T), and were scored for traditional Chinese medicine (TCM) syndrome. Multiple linear regression was used to analyse the influence factors of the difference between bone age and actual age, and changes in uterine volume. The children were divided into the normal weight group and the overweight/obesity group according to baseline BMI, and the bone age, the difference between bone age and actual age, Vuterus and BMI Z scores before and after treatment were compared between the two groups. ResultsFinally, 199 children entered the statistical analysis. Compared with pre-treatment, the bone age, BMI and BMI Z scores of the children increased after treatment, and the difference between bone age and actual age, TCM syndrome scores, Vuterus, Vleft ovary, Vright ovary, rleft follicle and rright follicle decreased; and the levels of serum FSH, LH, E2, and T significantly decreased (P<0.05 or P<0.01). The difference between bone age and actual age was negatively correlated with LH and Vuterus (P<0.05), and changes in uterine volume were positively correlated with LH (P<0.01). Comparing between the groups before and after treatment, the bone age, difference between bone age and actual age, and BMI Z scores of children in the normal weight group (100 cases) were significantly smaller than those in the overweight/obesity group (99 cases) (P<0.01). Compared with pre-treatment, the bone age of the children in both groups increased, but the difference between bone age and actual age and Vuterus were significantly smaller (P<0.01). The COVID-19 epidemic has spread to the whole world for three years and has had a serious impact on human life, health and economic activities. China's epidemic prevention and control has gone through the following stages: emergency unconventional stage, emergency normalization stage, and the transitional stage from the emergency normalization to the "Category B infectious disease treated as Category B" normalization, and achieved a major and decisive victory. The designated hospitals for prevention and control of COVID-19 epidemic in Tianjin has successfully completed its tasks in all stages of epidemic prevention and control, and has accumulated valuable experience. This article summarizes the experience of constructing a hospital infection prevention and control system during the "Category B infectious disease treated as Category A" period in designated hospital. The experience is summarized as the "Cluster" hospital infection prevention and control system, namely "three rings" outside, middle and inside, "three districts" of green, orange and red, "three things" before, during and after the event, "two-day pre-purification" and "two-director system", and "one zone" management. In emergency situations, we adopt a simplified version of the cluster hospital infection prevention and control system. In emergency situations, a simplified version of the "Cluster" hospital infection prevention and control system can be adopted. This system has the following characteristics: firstly, the system emphasizes the characteristics of "cluster" and the overall management of key measures to avoid any shortcomings. The second, it emphasizes the transformation of infection control concepts to maximize the safety of medical services through infection control. The third, it emphasizes the optimization of the process. The prevention and control measures should be comprehensive and focused, while also preventing excessive use. The measures emphasize the use of the least resources to achieve the best infection control effect. The fourth, it emphasizes the quality control work of infection control, pays attention to the importance of the process, and advocates the concept of "system slimming, process fattening". Fifthly, it emphasizes that the future development depends on artificial intelligence, in order to improve the quality and efficiency of prevention and control to the greatest extent. Sixth, hospitals need to strengthen continuous training and retraining. We utilize diverse training methods, including artificial intelligence, to ensure that infection control policies and procedures are simple. We have established an evaluation and feedback mechanism to ensure that medical personnel are in an emergency state at all times.
