1.Relationship between key molecules of IL-6/STAT3 signaling pathway and breast cancer susceptibility

Objective:To investigate the relationship between key molecules of interleukin-6/signal transducer and activator of transcription 3 (IL-6/STAT3) signaling pathway and breast cancer susceptibility.Methods:A case-control study design was adopted. 136 patients with breast cancer admitted to Department of Breast Surgery, Yantai Yantaishan Hospital from Mar. 2021 to Mar. 2023 were selected as the case group, and 136 healthy subjects of the same age were matched as the control group in a 1:1 ratio. The clinical data and key molecules of IL-6/STAT3 signaling pathway were compared between the two groups. The risk factors of breast cancer susceptibility were evaluated by conditional Logistic regression and addition analysis, and the diagnostic efficiency of receiver operating characteristic curve (ROC) and area under curve (AUC) were plotted.Results:The body mass index (BMI) (22.21±0.68 vs. 21.30±0.70 kg/m 2), age at menarche (13.50±1.24 years vs. 14.83±1.42 years), IL-6 (3.50±1.05 vs. 2.41±0.74), Janus protein kinase 1 (JAK1) mRNA (1.23±0.36 vs. 0.88±0.26), STAT3 mRNA (1.68±0.50 vs. 1.17±0.35), and menopause (30.15% vs. 55.88%), breastfeeding (82.35% vs. 92.65%), physical exercise (44.12% vs. 58.09%) were significantly different between the case group and the control group ( t=10.87, 8.23, 9.89, 9.19, 9.75, χ2=18.37, 6.59, 5.31, P<0.05). Breastfeeding [odd ratio ( OR) : 0.550], interleukin-6 (IL-6) ( OR: 4.409), janus protein kinase 1 (JAK1) ( OR: 5.370) and signal transducer and activator of transcription 3 (STAT3) ( OR: 4.386) mRNAs were risk factors for breast cancer susceptibility ( P<0.05). ROC curve showed that the combined diagnostic efficacy of IL-6, JAK1, STAT3 mRNA in breast cancer was significantly better than that of a single diagnostic efficacy. The incidence rate of breast cancer in those who were exposed to IL-6 mRNA and breast feeding was 3.508 times higher than that in those who were not exposed. The incidence rate of breast cancer in those who were exposed to JAK1 mRNA and breast feeding at the same time was 4.136 times higher than that in those who were not exposed. The incidence rate of breast cancer in those who were exposed to STAT3 mRNA and breast feeding was 3.537 times higher than that in those who were not exposed. Conclusion:The key molecules of IL-6/STAT3 signaling pathway and breast-feeding are predisposing factors of breast cancer, and they have additive interaction, thus increasing the susceptibility of breast cancer, which has significant practical significance for the differential diagnosis and treatment of this disease.

4.White Matter Deficits Underlying the Impaired Consciousness Level in Patients with Disorders of Consciousness.

In this study, we aimed to (1) identify white matter (WM) deficits underlying the consciousness level in patients with disorders of consciousness (DOCs) using diffusion tensor imaging (DTI), and (2) evaluate the relationship between DTI metrics and clinical measures of the consciousness level in DOC patients. With a cohort of 8 comatose, 8 unresponsive wakefulness syndrome/vegetative state, and 14 minimally conscious state patients and 25 patient controls, we performed group comparisons of the DTI metrics in 48 core WM regions of interest (ROIs), and examined the clinical relevance using correlation analysis. We identified multiple abnormal WM ROIs in DOC patients compared with normal controls, and the DTI metrics in these ROIs were significantly correlated with clinical measures of the consciousness level. Therefore, our findings suggested that multiple WM tracts are involved in the impaired consciousness levels in DOC patients and demonstrated the clinical relevance of DTI for DOC patients.

5.Advance in Rehabilitation for Parkinson's Disease (review)

The core symptoms of Parkinson's disease include motor symptoms and non-motor symptoms. For the motor symptoms, the rehabilitation includes the basic training of joints, gait training, the training of core muscle group and posture balance, the training of oral, facial, neck muscle and pharyngeal reflex, the training of lip movement, vocalization, sound volume, rhythm and speed, and the training of deep breathing exercises and abdominal pressure. For the non-motor symptoms, the rehabilitation includes the training of memory, attention, problem solving ability, activities of daily living and others, psychotherapy, family support, social participation, and other comprehensive treatments combined with medicine.
6.Progress of Dysphagia in Parkinson's Disease (review)

Dysphagia commonly occurs in Parkinson's disease (PD) at any stage, which may result in serious complications. The mechanism is uncertain. Swallowing Disturbance Questionnaire (SDQ) and Munich Dysphagia Test-Parkinson's Disease (MDT-PD) are often used for screening dysphagia in PD patients. But Non-motor Symptoms Questionnaire (NMSQ) and Unified Parkinson's Disease Rating Scale (UPDRS) underestimate the severity of dysphagia in PD patients. Videofluoroscopic Swallowing Examination (VFSE) and Videoendoscoptic Swallowing Study (VESS) are the gold standards for the dysphagia. There is no systemic management yet. Interventions includes medication, deep brain stimulation, etc. Early diagnosis and management facilitate to decrease the complications.
7.Application of Virtual Reality in Rehabilitation for Freezing of Gait in Parkinson's Disease (review)

Virtual reality is an integrated technology which simulates the real world via the computer, with characteristics of immersion, interaction and imagination. It is widely used in neurological rehabilitation. Freezing of gait (FOG) is a destructive symptom which appears in late stage of Parkinson's disease, which can be divided into subtypes of moving forward with very small steps, leg trembling in place and total akinesia, and often results in fall and injury of the patients. Virtual reality can simulate the environment of FOG to provide a new idea for the assessment. Virtual reality combined with treadmill training can provide challenging functional training in a complex environment to improve the gait. How it works is remained unclear.
8.Study of oxaliplatin combined with capecitabine in the preoperative gemcitabine concurrent chemotherapy for improving the local control rate of colorectal cancer and anal sphincter preservation rate

Objective To investigate the effect of oxaliplatin combined with capecitabine on the rate of local control and anal sphincter preservation of rectal cancer.Methods Eighty patients with stage Ⅱ/Ⅲ colorectal adenocarcinoma were randomly divided into control group and observation group,each group in 40cases.The patients in the observation group were treated with oxaliplatin and capecitabine.The control group was treated with oxaliplatin combined with leucovorin and 5-fluorouracil.The patients were followed up for 2~4 weeks.The local curative effect, perioperative biochemical index,R0 (excision margin without cancer cell) resection rate and sphincter preserving rate were compared between the two groups.Results In the control group, the short-term curative effect was 47.50%, which in the observation group was 60.00%, the difference was statistically significant (x2=4.22,P<0.05).There were no statistically significant differences in the biochemical indicators between the two groups (all P>0.05).The R0 resection rates of the control group and the observation group were 62.50%,57.50% (x2=0.67,P>0.05).The anal sphincter preservation rates of the control group and observation group were 32.50%,37.50% respectively (x2=0.67,P>0.05).The local control rates of the control group and observation group were 90.00%,92.50%,which of the observation group was higher than the control group, but the difference was not statistically significant(x2 =0.32,P>0.05).Conclusion The combination of oxaliplatin and capecitabine can improve the local curative effect in patients with rectal cancer without affecting the biochemical indicators of perioperative patients, but the effect of R0 resection rate and preserving anus rate in patients with rectal cancer compared with the chemotherapy regimen of FOLFOX4 has no significant difference, it is worthy of clinical promotion.
9.Exploration and practice of comprehensive experiment teaching in undergraduate preventive medicine

This study started with the comprehensive experimental research and practice of pre-ventive medicine. Taking the school of public health of China Medical University as an example, this paper introduced the ideas, principles, and experience of reform and implementation of comprehensive experimen-tal teaching. The methods of comprehensive experiments construction and implementation were introduced, such as subjects, application, basic training, implementation, writing, replying and score. In addition, the comprehensive experimental topics (such as classic experiment, new technology experiment, interesting ex-periment and fund project experiment et al), and basic training (such as literature searching, epidemiology and statistical analysis, laboratory safety and quality of experimental data, network sharing et al) were pre-sented in detail. It gave the evaluation that the comprehensive experimental teaching played an important role in the process of improving the quality of teaching and formed a unique preventive medicine experi-mental teaching system and talent training mode.
10.Application of Neural Prosthesis in Rehabilitation of Cervical Spinal Cord Injury (review)

Neural prosthesis control system is based on brain-computer interface and functional electrical stimulation technology, by an-alyzing the electroencephalograph control commands directly into the muscle system or an external device, which compensated efferent pathway from the brain-spinal cord, and recovered motor function of patients with cervical spinal cord injury. This paper described the basic structure, working principle and key technology of neural prosthetic system, summarized the application, problems and prospects of neural prosthetic technology in the rehabilitation of cervical spinal cord injury.
