1.Effect of Qingyi Decoction on toll-like receptor 4/nuclear factor-κB signaling pathway in patients with acute pancreatitis

Objective To investigate the effect of Qingyi Decoction enema on the toll-like recep-tor(TLR)4/nuclear factor(NF)-κB signaling pathway in patients with acute pancreatitis(AP).Methods A total of 100 patients with mild AP were selected as the study objects,and randomly di-vided into control group(n=50)and observation group(n=50).The control group received con-ventional western medicine therapy combined with ulinastatin,while the observation group was treated with Qingyi Decoction retention enema on the basis of control group.Clinical efficacy,symptom re-lief,traditional Chinese medicine syndrome scores,pancreatic function indicators,inflammatory mark-ers as well as TLR4 mRNA and NF-κB mRNA expression levels were compared between the two groups.Results The clinical effective rate in the observation group was 92.00%which was significantly higher than 76.00%in the control group(P<0.05).The observation group had significantly shorter times for abdominal distension pain relief,first bowel movement,recovery of bowel sounds and normalization of amylase(AMS)levels compared to the control group(P<0.05).After 10 days of treatment,both groups showed significantly lower scores for abdominal pain,tenderness,fullness,dryness,nausea,vomiting and short red urine syndrome compared to before treatment,with the observation group showing significantly lower scores than the control group(P<0.05).After 10 days of treat-ment,serum AMS and lipase(LPS)levels were significantly lower in both groups compared to be-fore treatment,with the observation group showing significantly lower levels than the control group(P<0.05).After 10 days of treatment,serum tumor necrosis factor(TNF)-α,triggering receptor expressed on myeloid cells(TREM)-1,microtubule-associated protein 1 light chain 3(MAP1-LC3)and intercellular adhesion molecule(ICAM)-1 levels were significantly lower in both groups com-pared to before treatment,with the observation group showing significantly lower levels than the con-trol group(P<0.05).After 10 days of treatment,TLR4 mRNA and NF-κB mRNA expression lev-els were significantly lower in both groups compared to before treatment,with the observation group showing significantly lower levels than the control group(P<0.05).Conclusion Qingyi Decoc-tion can effectively improve the clinical symptoms of AP patients such as abdominal pain andabdomi-nal distension,inhibit inflammatory response,and promote the recovery of pancreatic function,which may be related to the inhibition of TLR4/NF-κB signaling pathway.
2.Epidemiological investigation and analysis of occupational injuries of workers in metallurgical and shipbuilding and repairing industry

Objective:To understand the occupational injury situation of front-line workers in metallurgical and shipbuilding and repairing industry, and explore the risk factors of occupational injury.Methods:From September 2023 to March 2024, using cluster sampling method, front-line workers from 2 metallurgical enterprises in Shaoguan and Jinan City and 2 shipbuilding and repairing enterprises in Jiangmen and Shenzhen City were selected as the investigation objects. 6248 questionnaires were distributed and collected, and 6178 were effective questionnaires, with a effective recovery rate of 98.88%. The basic information, living habits, working system, protection and occupational injury of workers were investigated, and the data of occupational injury in factories was collected. The types, jobs and main causes of occupational injuries in different industries were analyzed, and the influencing factors of occupational injuries were analyzed by univariate and multi-factor logistic regression.Results:The incidence of occupational injury was 3.13% (128/4086) in metallurgical industry and 4.02% (84/2092) in shipbuilding and repairing industry. The top three occupational injuries in the metallurgical industry were furnace worker (17.19%, 22/128), steel rolling worker (14.84%, 19/128), maintenance worker (10.16%, 13/128), and the top three injury types were mechanical injury (24.22%, 31/128), height fall (20.31%, 26/128) and object strikes (17.97%, 23/128). The top three occupational injuries in shipbuilding and repairing industry were welder (20.24%, 17/84), riveter (9.52%, 8/84) and crane (8.33%, 7/84). The top three injury types were hit by objects (34.52%, 29/84), hit by falling objects (22.62%, 19/84), and lifting injury (20.24%, 17/84). The injuries of workers in metallurgical industry and shipbuilding and repairing industry were mainly fractures, accounting for 32.03% (41/128) and 60.71% (51/84), respectively. The incidence of occupational injury was higher in males, with sleep disorder, high temperature exposure and chemical toxicity exposure ( P<0.05). There were significant differences in age, smoking degree, working age and emotional state between workers with occupational injury and those without occupational injury ( P<0.05). Multivariate analysis showed that male, age above 50 years old, moderate smoking, working years of 5-9 years, mild anxiety, poor health status and high temperature exposure were risk factors for occupational injury ( OR=25.57, 3.72, 14.27, 2.09, 1.50, 4.36, 0.66, P<0.05) . Conclusion:The incidence of occupational injury is higher in shipbuilding and repairing industry, and fracture is the main type of occupational injury. The occurrence of occupational injury is affected by gender, age, smoking, working age, emotional state, health status and high temperature exposure.

4.Study on the risk and countermeasure of prescription drugs sold online by drug retail enterprises

OBJECTIVE To analyze the risks of prescription drugs sold online by drug retail enterprises, and to provide countermeasures and suggestions for risk prevention and control of prescription drugs sold online. METHODS The risk hierarchy structure model of prescription drugs sold online by drug retail enterprises was constructed by using analytic hierarchy process. Multiple rounds of risk research and judgment were carried out on 123 pairs of evaluation indicators by using Delphi expert survey method. The normalized weight calculation and consistency test of risk judgment matrix were carried out to perform fuzzy quantitative research. RESULTS The risk of prescription dispensing and review (6.48%), the risk of drug first and prescription later (5.48%), the risk of rational drug use guidance (4.99%), the risk of buying drugs by abnormal channel (4.97%), the risk of “first diagnosis, non-chronic disease and non-common disease” (4.43%), and the quality and safety risk of returned drugs (4.34%) and the application risk of regulatory technology (4.06%) were high risks; the overall risk of drug retail enterprises (chain) selling prescription drugs online was 38.67%, and the overall risk of drug retail enterprises (individual) selling prescription drugs online was 61.33%, with a difference of 22.66% between them. CONCLUSIONS There were 7 high-risk indicators for prescription drugs sold online by drug retail enterprises. Among them, the risk of prescription dispensing and review, the risk of drug first and prescription later, and the risk of rational drug use guidance are the top three high-risk points. The risk of prescription drugs sold online by drug retail enterprises (individual) is higher than that of drug retail enterprises (chain). It is recommended that regulatory authorities focus on and regulate the prescription drugs sold online by drug retail enterprises (individual), and encourage drug retail enterprises (chain) to establish a systematic online sales process for prescription drugs; for high-risk points of prescription drugs sold online, it is recommended that regulatory authorities and drug retail enterprises focus on it and take effective risk prevention and control measures to ensure the safe use of prescription drugs by the general public.
5.Analysis of occupational noise exposure and hearing loss in workers in petrochemical enterprises

{L-End}Objective To study the situation of occupational noise exposure and hearing loss in workers in petrochemical enterprises. {L-End}Methods A total of 2 052 workers from four large-scale petrochemical enterprises in China were selected as the research subjects using convenience sampling method. The noise-exposed level of job position was detected by individual noise dosimeters, and pure-tone audiometry of the workers was conducted. {L-End}Results The noise-exposed levels at various job positions in the workplace of the enterprises ranged from 83.2 to 86.7 dB(A), and 61.1% of the job positions had a cumulative noise exposure (CNE) > 95.0 dB(A) per year. The detection rate of high-frequency hearing loss was 12.6%(259/2 052) among the research subjects. The detection rate of speech frequency hearing loss was 5.5%(112/2 052) among the research subjects. The result of CNE stratified analysis showed that the detection rate of hearing loss at frequencies of 2.0, 3.0, 4.0, and 6.0 kHz, as well as high frequencies, gradually increased with increasing CNE (all P<0.05). {L-End}Conclusion The noise hazard in petrochemical enterprises is severe, and long-term occupational noise exposure can increase the risk of hearing loss among workers. Efforts should be made to strengthen noise control and hearing protection measures.
6.Estimation of the number of workers with occupational noise-induced hearing loss from manufacturing industry in China, 2020

{L-End}Objective To estimate the number of workers with occupational noise-induced hearing loss (ONHL) in the manufacturing industry of China in 2020. {L-End}Methods Multiple data sources were integrated to collect information on workers in the manufacturing industry from the Occupational Diseases and Hazards Monitoring Information System under China Disease Prevention and Control Information System. The total number of workers was split based on age from the 2018 Fourth National Economic Census and the 2020 National Survey of Occupational Disease Hazards. Additionally, data on the prevalence of hearing loss in the general population was adjusted based on the age composition of Jilin Province, and noise prevalence was standardized based on the age composition of the employed population in the Seventh National Population Censu. Attribution fractions (AF) was estimated. The prevalence ratio (PR) was calculated by the prevalence of hearing loss in the occupational noise-exposed workers and general population. The proportion and attributable cases (AC) of ONHL cases in all hearing loss cases were estimated for occupational noise-exposed workers. The number of ONHL was estimated based on AF and the total number of workers with hearing loss. {L-End}Results In 2020, a total of 30 059 525 workers were exposed to occupational noise in the manufacturing industry in China, with noise-exposed prevalence and standardized noise-exposed prevalence of 28.94% and 28.52%, respectively. The prevalence of hearing loss among occupational noise-exposed workers increased with age, and a total of 8 054 789 workers suffered from hearing loss. Most of the cases were at the age between 45-<55 years old. The total PR and 95% uncertainty interval (UI) was 2.83 (2.58-3.06) and the total AF and 95%UI was 64.63% (61.22%-67.30%), and both peaks were in the age of 30-<45 years. The AC and 95%UI was 5 205 980 (4 930 620-5 420 345) persons, and most of the cases were at the age between 40-<55 years. {L-End}Conclusion Occupational noise poses a serious threat to the hearing health of workers in the manufacturing industry of China, especially among middle-aged and young adults.

Under the regulation of various cytokines and different concentrations of cytokines, primary CD4+ T cells can differentiate into different Th subgroups. T helper cells 17 (Th17) and regulatory T cells (Treg cells) are research hotspots in recent years. They play diverse immunomodulatory roles by secreting various target cytokines. Th17 and Treg cells are different from each other but connect with each other, their regulatory mechanism is complex, and they play important roles in non-small cell lung cancer (NSCLC). This paper reviews the progress of the differentiation, the development and the immunological function of Th17 and Treg cells and their immunomodulatory effect on the NSCLC.

Objective To establish a set of operational status assessment indicators to meet the needs of informationized hospital management.Methods Assessment indicators were selected and weights were set respectively through literature review,field interview,and questionnaire survey.Six target dimensions were key performance indicators medical business,operational performance,cost control,medical insurance,balance and risk management,and development capability.Thus a set of operational status evaluation indicators was established in IT means,and based on the informationization level of a tertiary A general hospital in Zhejiang province.Results In the principle of public welfare,objectivity,effectiveness and prospectiveness,we analyzed and sorted out relevant data in the current hospital informationization,identifying six quantitative indicators,15 level-1 indicators,and 86 level-2 indicators.Conclusions It is feasible to build a set of assessment indicators for hospital operation and management in view of both technology and methodology.
9. Effects of occupational noise exposure on asymmetric hearing loss in automobile manufacturing workers

Objective:
To investigate the characteristics of asymmetric hearing loss in automobile manufacturing workers and the effect of occupational noise exposure on asymmetric hearing loss.
Methods:
A cross-sectional approach was used in this study. From March 2017 to February 2018, Subjects (7066) from four complete vehicle factories were given a pure tone audiometry (Hearing thresholds were measured at frequencies of 0.5, 1, 2, 3, 4 and 6 kHz in each ear) and were required to complete a health-related information questionnaire. According to the inclusion criteria, a total of 6339 workers were selected. The mean hearing thresholds for the left and right ears at overall frequencies were compared using the repeated means analysis of variance (ANOVA) test. The threshold differences at each frequency were compared using paired t tests.
