1.Bullous hemorrhagic dermatosis in a 65-year-old Filipino woman secondary to enoxaparin: A case report.

Bullous hemorrhagic dermatosis (BHD) is a rare cutaneous manifestation characterized by tense hemorrhagic bullae that appear at sites distant from low molecular weight heparin (LMWH) injections, typically within seven days of exposure. As of March 2022, only 94 cases have been reported. It most commonly affects elderly males with predisposing factors for thromboembolism, such as carcinoma, and usually involves the extremities.
This case highlights the importance of maintaining a high index of suspicion for bullous hemorrhagic dermatosis (BHD) in patients receiving low molecular weight heparin, even beyond the typical 7-day window and in demographics not commonly affected. Early recognition and prompt discontinuation of the offending agent, as demonstrated in this atypical presentation involving a Filipino elderly woman with multiple comorbidities and no malignancy, can lead to favorable outcomes. Clinicians should be aware of this rare but reversible complication to avoid misdiagnosis and ensure appropriate management.

2.Beyond Body Weight: Body Composition and Cellular Health in a Workplace Obesity Intervention Programme

Introduction:
Obesity is a chronic disease characterized by complex
alterations in body composition and metabolic health. Traditional reliance on body weight may overlook important
physiological adaptations during lifestyle intervention.
Methodology:
A cohort of 99 participants at programme entry underwent
body composition assessment using bioelectrical impedance analysis. Parameters included percent body fat (PBF),
visceral fat area (VFA), skeletal muscle index (SMI), and
phase angle (PhA). Correlation analyses were performed to
explore relationships between adiposity, muscle mass, and
cellular health.
Results:
Participants demonstrated elevated adiposity (PBF 38.9 ±
8.9%; VFA 142.9 ± 56.1 cm²). PhA was inversely associated
with VFA (r ≈ −0.23, p <0.05) and positively associated with
SMI (r ≈ 0.49, p <0.001). Notably, a subset of overweight
individuals exhibited relatively preserved PhA, indicating
heterogeneity in metabolic health despite similar adiposity
levels.
Conclusion
Body composition and PhA provide complementary
insights into metabolic health beyond body weight. These
findings support the role of bioimpedance-derived parameters in identifying metabolic heterogeneity and guiding
personalized obesity management.

3.Effectiveness of GLP-1 Receptor Agonists on Weight Loss in Malaysian Patients with Type 2 Diabetes

Introduction:
Glucagon-like peptide-1 receptor agonists (GLP-1 RAs)
induce clinically meaningful weight reduction in patients
with type 2 diabetes mellitus (T2DM), but real-world data
in Southeast Asian populations are limited. This study
evaluated the effectiveness of GLP-1 RAs in achieving
clinically significant weight loss in Malaysian patients with
obesity and T2DM.
Methodology:
A retrospective cohort study was conducted among adults
with T2DM and obesity attending the Endocrinology Clinic
at Hospital Sultan Abdul Aziz Shah between January
2023 and December 2024. Patients receiving GLP-1 RAs
(semaglutide or liraglutide) were compared with those on
standard care. Anthropometric outcomes were assessed
over 6–12 months, with weight loss thresholds of ≥3, ≥5,
and ≥10%. Between-group comparisons used Fisher’s exact
test, and odds ratios (OR) were calculated.
Results:
Eighty-five patients were included (GLP-1, n = 47; control,
n = 38). The GLP-1 group achieved significantly higher rates
of any weight loss (70.0% vs 41.2%; OR = 3.33, p = 0.019)
and ≥3% weight loss (42.5% vs 14.7%; OR = 4.29, p = 0.011).
Number needed to treat was 3–4 patients. Proportions
achieving ≥5 and ≥10% weight loss were higher in the
GLP-1 group but did not reach statistical significance.
Conclusion
GLP-1 RA therapy significantly improves the likelihood
of clinically meaningful weight loss in Malaysian patients
with obesity and T2DM. These findings support the
integration of GLP-1 RAs into routine obesity management
strategies in Southeast Asia.

4.Baseline Physical Activity Enhances GLP-1 Receptor Agonist Weight Loss in Obese Malaysian Patients with T2DM

Introduction:
While Glucagon-like peptide-1 receptor agonists (GLP-1

mellitus (T2DM), interindividual variability exists.
Lifestyle factors, particularly baseline physical activity,
may modify weight loss outcomes. This study examined
whether baseline activity influences GLP-1 RA efficacy in
Malaysian patients.
Methodology:
A retrospective cohort of adults with obesity and T2DM
receiving GLP-1 RAs (semaglutide or liraglutide) from
January 2023 to December 2024 was analyzed. Baseline
physical activity was classified as active or inactive.
Primary outcomes were achievement of ≥3 and ≥5% weight
loss over 6–12 months. Associations were assessed using
Fisher’s exact test and logistic regression.
Results:
Among GLP-1-treated patients, those reporting baseline
physical activity were more likely to achieve ≥3% weight loss
and demonstrated trends toward higher, ≥5%, weight loss.
Logistic regression suggested baseline activity increased
the odds of clinically meaningful weight reduction, though
statistical significance was limited by sample size.
Conclusion
Baseline physical activity may enhance GLP-1 receptor
agonist-mediated weight loss in obese patients with T2DM.
Integrating lifestyle interventions with pharmacotherapy
may optimize treatment outcomes. Larger prospective
studies are warranted to confirm these findings.

5.Maternal mid-upper arm circumference as a predictor of low birth weight outcome among newborn deliveries of adolescents in a tertiary level hospital

BACKGROUND
Maternal malnutrition is a major cause of low birth weight (LBW) newborn outcome especially among adolescent mothers. It is one of the key drivers of child stunting and initiates the vicious cycle of intergenerational malnutrition. The body mass index prior to pregnancy or at the initial trimester is currently being used to establish the desired weight gain throughout pregnancy. However, Filipino adolescents often delay their first antenatal visit at a later stage of pregnancy. Without a baseline weight, the establishment of appropriate weight gain and nutritional status is often challenging. Mid-upper arm circumference (MUAC) was proven to be a good proxy measure of acute malnutrition, however, there was no global consensus on what MUAC cut-off point to use to identify pregnant adolescents at risk for delivering LBW babies. Finding the optimal cut-off could facilitate early identification and intervention of pregnant adolescents who are nutritionally at risk and could eventually break the cycle of intergenerational malnutrition.
OBJECTIVESThe study aims to determine the association of maternal MUAC and the birth weight outcomes among newborn deliveries of adolescents in a tertiary hospital for a period of six months and to identify the optimal maternal MUAC cut-off point that can be used to predict low birth weight outcome among newborn deliveries of adolescents in a tertiary hospital.
METHODSA cross-sectional study was conducted among adolescents ages 10 to 19 years who delivered babies in a tertiary hospital in the Philippines for a period of six months. Maternal MUAC and LBW outcome were documented, and their association was determined using a logistic regression analysis. To measure diagnostic accuracy, the sensitivity, specificity, and the area under the curve were taken for each MUAC point. A receiver operating characteristic (ROC) curve was used to aid the MUAC cut-off determination.
RESULTSOut of 237 newborn deliveries, 35% were noted with low birth weight while 65% had normal birth weight. Most of the mothers were in their late adolescence at 78%. The crude association for the MUAC cut-offs ≤23.00 cm, ≤23.50 cm, and ≤24.00 cm and LBW showed a significant value of 2.19, 2.25, and 2.39 at 95% CI, respectively. However, it is only the cut-off ≤24.00 cm that showed significant results for adjusted association by the logistic regression analysis. The MUAC cut-off ≤24.00 cm also showed a better trade-off value between the sensitivity and specificity. Furthermore, the optimal maternal MUAC measurement that predicts LBW newborn outcome points to ≤24.00 cm cut-off based on the ROC curve.
CONCLUSIONThis study shows that the maternal MUAC is predictive of LBW outcome among adolescent deliveries.A MUAC cut-off of ≤24.00 cm was superior to lower cut-offs studied. The pregnant adolescents might need a higher MUAC cut-off than adults to allow timely intervention and prevention of poor neonatal outcomes. By doing this simple screening test, suspected pregnant adolescents can be easily identified and referred for further confirmatory testing.

6.Treatment of large bone defects in load-bearing bone: traditional and novel bone grafts.

Large bone defects in load-bearing bone can result from tumor resection, osteomyelitis, trauma, and other factors. Although bone has the intrinsic potential to self-repair and regenerate, the repair of large bone defects which exceed a certain critical size remains a substantial clinical challenge. Traditionally, repair methods involve using autologous or allogeneic bone tissue to replace the lost bone tissue at defect sites, and autogenous bone grafting remains the "gold standard" treatment. However, the application of traditional bone grafts is limited by drawbacks such as the quantity of extractable bone, donor-site morbidities, and the risk of rejection. In recent years, the clinical demand for alternatives to traditional bone grafts has promoted the development of novel bone-grafting substitutes. In addition to osteoconductivity and osteoinductivity, optimal mechanical properties have recently been the focus of efforts to improve the treatment success of novel bone-grafting alternatives in load-bearing bone defects, but most biomaterial synthetic scaffolds cannot provide sufficient mechanical strength. A fundamental challenge is to find an appropriate balance between mechanical and tissue-regeneration requirements. In this review, the use of traditional bone grafts in load-bearing bone defects, as well as their advantages and disadvantages, is summarized and reviewed. Furthermore, we highlight recent development strategies for novel bone grafts appropriate for load-bearing bone defects based on substance, structural, and functional bionics to provide ideas and directions for future research.

7.Effects of Compositional Isotemporal Substitutions of 24 Hours Activity Behaviors on Novel Obesity Indicators in College Students.

Objective To explore the effects of time reallocation among moderate-to-vigorous physical activity(MVPA),light physical activity(LPA),sedentary behavior(SB),and sleep on a body shape index(ABSI),body roundness index(BRI),conicity index(CI),and relative fat mass(RFM)of college students by the compositional isotemporal substitution method,thus providing measures for alleviating the obesity problem of college students. Methods Two hundred and ten college students(111 males and 99 females)aged 18-22 years old were recruited from Tianjin University of Science and Technology from April to June in 2023.Three-dimensional acceleration sensors were used to collect data of MVPA,LPA,SB,and sleep of college students.The body height,body weight,and waist circumference were measured,and four novel obesity indicators(ABSI,BRI,CI,and RFM)were calculated.The effects of substituting each activity behavior for 15 min on the obesity indicators were predicted,and the dose-effect relationship was explored at intervals of 5 min from -30 to 30 min.Results MVPA was negatively correlated with ABSI(β=-0.03,P=0.001),BRI(β=-0.27,P=0.049),CI(β=-0.10,P=0.001),and RFM(β=-9.95,P=0.004).LPA was negatively correlated with CI(β=-0.05,P=0.011)and RFM(β=-8.74,P=0.007).Neither SB nor sleep had correlations with ABSI,BRI,CI,and RFM.The results of 15 min isotemporal substitutions showed that increasing the MVPA time decreased the ABSI,BRI,CI,and RFM by 0.006-0.008,0.306-0.393,0.162-0.205,and 2.468-2.897,respectively.Decreasing the MVPA time increased the ABSI,BRI,CI,and RFM by 0.012-0.014,0.548-0.632,0.286-0.328,and 4.358-4.748,respectively.In the dose-effect relationship from -30 min to 30 min,MVPA was irreplaceable,and the negative benefits from substituting MVPA for other activity behaviors were much greater than the positive benefits from substituting MVPA for other activity behaviors.Conclusions Future research should take 24 hours activity behaviors as a whole.Increasing the time spent on MVPA and LPA and decreasing the time spent on SB is one of the effective ways to alleviate the obesity problem among college students.

8.Impacts of Internalized Weight Bias and Weight Control Beliefs on Eating Behaviors in Women Losing Weight.

Objective To investigate the internalized weight bias and weight control beliefs regarding the current status,influencing factors,and impacts on eating behaviors of women losing weight. Methods A convenient sampling method was used to select the females who underwent physical examination in the Peking Union Medical College Hospital from August to December 2023.The general information questionnaire,Weight Bias Internalization Scale,Weight Control Belief Questionnaire,and Chinese version of Three-Factor Eating Questionnaire were utilized for investigation.Latent class analysis was conducted to explore the potential categories based on the characteristics of internalized weight bias and weight control beliefs.Univariate analysis and multiple-factor linear regression analysis were performed to explore influencing factors of potential categories and their effects on eating behaviors. Results A total of 518 subjects were included in this study.The internalized weight bias and weight control beliefs could be classified into three potential categories: low weight bias tolerance type(53.7%),medium weight bias persistence type(29.5%),and high weight bias overcritical type(16.8%).Logistic regression analysis showed that body mass index,unmarriage,and divorce were the influencing factors of the high weight bias overcritical type.The scores of non-control eating and emotional eating in medium weight bias persistence type and high weight bias overcritical type were higher than those in low weight bias tolerance type(all P<0.001). Conclusions The attitudes of Chinese women towards body weight can be categorized into low weight bias tolerance type,medium weight bias persistence type,and high weight bias overcritical type.The high weight bias overcritical type is characterized by high weight bias and high personal responsibility attribution,and it is closely associated with negative eating behaviors.A high body mass index,unmarriage,and divorce are high-risk factors for developing the high weight bias overcritical type.

9.Location specificity of the acupoints selected by the syndrome differentiation of acupuncture and moxibustion in weight loss: the possibility of "local weight loss" and "general regulation" based on complex network analysis.

OBJECTIVE:
To explore the characteristics and rules of the locations of acupoints selected in weight loss after syndrome differentiation of acupuncture and moxibustion, and provide quantitative evidence for the location specificity of acupoint selection in weight loss.
METHODS:
Clinical research articles on acupoint selection based on syndrome differentiation of acupuncture and moxibustion in weight loss were retrieved in China National Knowledge Infrastructure (CNKI), from the inception to September 20th, 2024, and the data about acupoints and differentiated syndromes were extracted. Based on graph theory, the acupoint-syndrome network was established and its topological parameters such as node degree, value of betweenness centrality, description length and number of community were calculated.
RESULTS:
①The description length of the limbs was 4 255.592, and that of the trunk was 3 274.312. The information contained in the acupoint-syndrome network for the acupoints on the limbs was greater than that for those on the trunk. ②The value of betweenness centrality and node degree showed a nonlinear relationship, with R²of 0.812 1 for the trunk and 0.321 8 for the limbs. The values of betweenness centrality for the acupoints on the trunk were uniformly distributed, and the difference among these values was much smaller than that for the acupoints on the limbs. It suggested that the distance from each trunk acupoint to network center was similar, and the importance among these acupoints to network was similar, while the importance among acupoints located on the limbs was different significantly. ③The frequency proportion of acupoints on the trunk showed uniform distribution among different syndromes, while that of some acupoints located on the limbs such as Taichong (LR3), Neiting (ST44) and Taixi (KI3) presented a correlation with the syndromes.
CONCLUSION
In weight loss with acupuncture and moxibustion, the correlation between the limb acupoints and syndromes is more diverse and specific than that between the trunk acupoints and syndromes. The differences in acupoint selection for simple obesity treated with acupuncture and moxibustion are mostly reflected in the acupoints on the four limbs rather than those on the trunk. It provides an approach to acupoint selection for "local weight loss" and "general regulation" in treatment with acupuncture and moxibustion.
