Objective:To explore the relationship between gallbladder polyps and colorectal polyps, providing insights into whether the presence of gallbladder polyps can serve as an indicator for colonoscopy screening.Methods:Clinical data from 2 542 patients who underwent colonoscopy and abdominal ultrasound at the First Affiliated Hospital of Shihezi University between January and December 2022 were retrospectively analyzed. Patients were divided into colorectal polyp group ( n=1 266) and non-colorectal polyp group ( n=1 276) based on colonoscopy findings. Univariate and multivariate Logistic regression models were used to analyze the relationship between gallbladder polyps and colorectal polyps. Results:The prevalence rates of gallbladder polyp in colorectal polyp group and non-colorectal polyp group were 16.67% (211/1 266) and 11.21% (143/1 276). Multivariate Logistic regression analysis showed a lower risk of colorectal polyps in women ( P<0.001, OR=0.523, 95% CI: 0.440-0.622). Age ( P<0.001, OR=1.059, 95% CI: 1.050-1.068), and hypertriglyceridemia ( P=0.013, OR=1.350, 95% CI: 1.066-1.709), low level of high-density lipoprotein ( P<0.001, OR=1.588, 95% CI: 1.280-1.969), and gallbladder polyp ( P<0.001, OR=1.712, 95% CI: 1.344-2.180) were independent risk factors for colorectal polyp. There was no significant difference in hypercholesterolemia, elevated low-density lipoprotein, hyperuricemia, or cholecystectomy between colorectal polyp group and non-colorectal polyp group ( P>0.05). Conclusion:The identification of gallbladder polyps via abdominal ultrasound may indicate a higher likelihood of colorectal polyps in patients, underscoring the need for further colonoscopy screening in individuals with gallbladder polyps.

ObjectiveTo study the pharmacological substances and mechanisms through which sesquiterpene ZH-13 from Aquilariae Lignum Resinatum improves neuroinflammation. MethodsBV-2 microglial cells were stimulated with lipopolysaccharide （LPS） to induce neuroinflammation. The cells were divided into the normal group， the model group， and the ZH-13 low- and high-dose treatment groups （10， 20 μmol·L^-1）. The model group was treated with 1 μmol·L^-1 LPS. Cell viability was assessed using the cell proliferation and activity assay （CCK-8 kit）. Nitric oxide （NO） release in the cell supernatant was measured using a nitric oxide kit （Griess method）. The mRNA expression levels of interleukin-1β （IL-1β）， tumor necrosis factor-α （TNF-α）， inducible nitric oxide synthase （iNOS）， and interleukin-6 （IL-6） were detected by real-time fluorescence quantitative polymerase chain reaction （Real-time PCR）. The phosphorylation of mitogen-activated protein kinase （MAPK） pathway proteins was assessed by Western blot. ResultsCompared with the model group， ZH-13 dose-dependently reduced NO release from BV-2 cells under LPS stimulation （P<0.05， P<0.01）. In the 20 μmol·L^-1 ZH-13 treatment group， the mRNA expression levels of IL-1β， TNF-α， iNOS， and IL-6 were significantly reduced compared to the model group （P<0.05， P<0.01）. In both the low- and high-dose ZH-13 groups， the expression of the inflammatory factor TNF-α and the phosphorylation of c-Jun N-terminal kinase （JNK） in the upstream MAPK pathway were significantly reduced （P<0.05）. After stimulation with the JNK agonist anisomycin （Ani）， both low- and high-dose ZH-13 treatment groups showed reduced phosphorylation of JNK proteins compared to the Ani-treated group （P<0.01）. ConclusionThe sesquiterpene compound ZH-13 from Aquilariae Lignum Resinatum significantly ameliorates LPS-induced neuroinflammatory responses in BV-2 cells by inhibiting excessive JNK phosphorylation and reducing TNF-α expression. These findings elucidate the pharmacological substances and mechanisms underlying the sedative and calming effects of Aquilariae Lignum Resinatum.

Duodenum-preserving pancreatic head resection， also known as Beger surgery， has a high incidence rate of bile duct injury after surgery， while the treatment modality for bile duct injury depends on the severity of the injury， and endoscopic therapy is often challenging in case of severe bile duct injury. Recently a patient with biliary fistula after Beger surgery was admitted to Affiliated Hangzhou First People’s Hospital， Westlake University， and successful diagnosis and treatment were achieved through oral choledochoscopy-assisted percutaneous-endoscopic rendezvous technique.

Objective : To investigate the effect of long non⁃coding RNA (LncRNA) anoctamin 1 antisense RNA⁃1 (ANO1⁃AS1) on the proliferation and apoptosis of esophageal squamous cell carcinoma (ESCC) cells and its possible mechanisms. Methods : Silenced ANO1⁃AS1 lentivirus was transfected in ESCC cells TE⁃1 and EC109. Subsequently, the expression levels of ANO1⁃AS1 and calcium⁃activated chloride channel protein 1 (ANO1) in the cells were detected by qRT⁃PCR. CCK⁃8 and colony formation assays were used to detect the proliferation of TE⁃1 and EC109 cells. ANO1 positively related expressed genes were obtained from the LinkedOmics database and then the gene set was enriched for pathways and possible pathways were validated. The expression levels of proliferating cell nuclear antigen(PCNA), P53 protein, apoptosis⁃related protein ( Bax and Bcl⁃2), ANO1 protein and phosphatidylinositol⁃3⁃kinase/protein kinase B (PI3K/Akt) pathway⁃related protein were assessed by Western blot. Results: After transfection of lentivirus with silent expression function, the expression level of ANO1⁃AS1 was significantly reduced in TE⁃1 and EC109 cells (P < 0. 05); After down⁃regulation of ANO1⁃AS1, compared with the negative control group, the proliferation ability of ESCC cells was reduced (P < 0. 05) and the rate of clone formation decreased (P < 0. 05); Western blot results showed that, compared with negative controls, the expression of PCNA decreased, the expression of oncogene P53 protein increased ( P < 0. 05 ), the expression of proteins ( Bax) increased, Bcl⁃2 decreased and the levels of phosphorylation of the pathway proteins PI3K and Akt decreased (P < 0. 05) . Conclusion Knockdown of ANO1⁃AS1 can decrease proliferation and promote apoptosis in ESCC, which may be achieved by affecting PI3K/Akt pathway activation.

Objective:To explore the consistency of peripheral whole blood and venous serum procalcitonin (PCT) levels, and the value of peripheral whole blood PCT in evaluating pediatric bacterial infection.Methods:This multicenter cross-sectional parallel control study was conducted in 11 children′s hospital. All the 1 898 patients older than 28 days admitted to these hospitals from March 2018 to February 2019 had their peripheral whole blood and venous serum PCT detected simultaneously with unified equipment, reagent and method. According to the venous serum PCT level, the patients were stratified to subgroups. Analysis of variance and chi-square test were used to compare the demographic characteristics among groups. And the correlation between the peripheral blood and venous serum PCT level was investigated by quantitative Pearson correlation analysis.The PCT resultes were also converted into ranked data to further test the consistency between the two sampling methods by Spearman′s rank correlation test. Furthermore, the ranked data were converted into binary data to evaluate the consistency and investigate the best cut-off of peripheral blood PCT level in predicting bacterial infection.Results:A total of 1 898 valid samples were included (1 098 males, 800 females),age 27.4(12.2,56.7) months. There was a good correlation between PCT values of peripheral whole blood and venous serum ( r=0.97 , P<0.01). The linear regression equation was PCT?venous serum=0.135+0.929×PCT peripheral whole blood. However, when stratified to 5 levels, PCT results showed diverse and unsatisfied consistency between the two sampling methods ( r=0.51-0.92, all P<0.01). But after PCT was converted to ordinal categorical variables, the stratified analysis showed that the coincidence rate of the measured values by the two sampling methods in each boundary area was 84.9%-97.1%. The dichotomous variables also showed a good consistency (coincidence rate 96.8%-99.3%, Youden index 0.82-0.89). According to the severity of disease, the serum PCT value was classified into 4 intervals（<0.5、0.5-<2.0、2.0-<10.0、≥10.0 μg/L）, and the peripheral blood PCT value also showed a good predictive value (AUC value was 0.991 2-0.997 9). The optimal cut points of peripheral whole blood PCT value 0.5、1.0、2.0、10.0 μg/L corresponding to venous serum PCT values were 0.395, 0.595, 1.175 and 3.545 μg/L, respectively. Conclusions:There is a good correlation between peripheral whole blood PCT value and the venous serum PCT value, which means that the peripheral whole blood PCT could facilitate the identification of infection and clinical severity. Besides, the sampling of peripheral whole blood is simple and easy to repeat.

Objective:To compare the outcomes of low-dose-rate prostate brachytherapy (BT) and radical prostatectomy (RP) in patients with T 1c-T 3a prostate cancer. Methods:A group of 745 patients with T 1c-T 3a prostate cancer between January 2010 and August 2017 at Peking Union Medical College Hospital were identified. The records of these patients, who were followed up for a minimum of 2 years, were reviewed. 384 cases received BT. Their characters included age(72.1±6.6), tPSA (12.4±6.1) ng/ml, prostate volume (33.6±13.8) ml, Gleason grade group (2.0±1.2). In this group, T 1c-T 2a stage was diagnosed in 189 cases, T 2b-T 2c stage in 182 cases and T 3a stage in 13 cases.361 cases received RP. Their characters included age(65.7±6.2), tPSA(12.6±6.4) ng/ml, prostate volume (37.2±17.8) ml, Gleason grade group (1.9±1.2). In this group, T 1c-T 2a stage was diagnosed in 177 cases, T 2b-T 2c stage in 170 cases and T 3a stage in 14 cases.The log-rank test compared survival rates between the two modalities, and Cox regression identified factors associated with bRFS. Results:Median follow-up was 60 months. Kaplan-Meier analysis did not show any statistically significant differences in terms of cRFS( P=0.321), cancer specific survival (CSS, P=0.643) and overall survival (OS, P=0.565) rate between the two groups. BT was associated with improved bRFS compared to RP( P=0.018). Risk of biochemical recurrence was significantly lower with BT compared with RP in the patients with a biopsy Gleason grade group 2 and 3 ( P=0.008), or prostate volume ≤35 ml ( P=0.027), or tPSA ≤10 ng/ml ( P=0.013), or the clinical T stages of T 2b and T 2C( P=0.031), or in the intermediate-risk group according to NCCN risk classification ( P=0.003). On multivariate analysis of all 745 patients, age≤ 70 and T stage≥T 2b was associated with significantly shorter bRFS. Conclusions:BT produced equivalent cRFS, CSS and OS compared to RP, while it was associated with improved bRFS. BT On multivariate analysis of all 745 patients, age≤ 70 and T stage≥T 2b was associated with significantly lower bRFS.

Objective To observe the effects of long-chain non-coding RNA XLOC009038 on the proliferation, apoptosis, migration and invasion of esophageal squamous cell carcinoma EC 109 and EC9706 cells and explore its mechanism. Methods XLOC009038 interfering plasmid was constructed and transfected into EC 109 and EC9706 cells to down-regulate the expression of XLOC009038 gene. MTT colorimetry and clonogenic assay were used to observe the changes of cell proliferation and cloning ability before and after gene down-regulation.Flow cytometry was used to detect apoptosis. Transwell was used to measure the changes of cell migration and invasion ability before and after transfection. Western blot was used to detect the expression of procaspase 3 protein in cells before and after transfection. Results The expression of XLOC009038 gene in the two cells was significantly lower than that in the control group (P < 0.001). After down-regulation of XLOC009038 gene expression, the cloning and proliferation ability of EC 109 and EC9706 cells decreased significantly (P< 0.05). Compared with the control group, the migration and invasion ability of EC 109 and EC9706 cells decreased significantly (P < 0.001).Flow cytometry showed that the apoptosis rate of EC 109 and EC9706 cells increased after down-regulation of XLOC009038 (P <0.001). The expression of procaspase 3 increased in the experimental group after interfering with XLOC009038 (P = 0.013; P < 0.001). Conclusions Over-expression of XLOC009038 might be closely related to occurrence and development of the esophageal cancer. Over-expression of XLOC009038 can enhance the proliferation, apoptosis, migration and invasion of esophageal cancer cells in vitro through the procaspase3 pathway.

Objective:To evaluate effect of telephone follow-up combined with written instruction on compliance and Helicobacterpylori (H.pylori) eradication in patients with H.pylori infection.Methods:A total of 160 H.pylori positive patients were randomly divided into an experimental group and a control group (n=80 in each group).Ml the patients got the guide instruction named the guidance of clinical medication for H.pylori infection patients before the treatment.The patients in the experimental group were added individualized follow-up with telephone.The compliance,eradication ofH.pylori,adverse events,and satisfaction were compared between the 2 groups.Results:The eradication rate of H.pylori in the perprotocol analysis for the experimental group and control group were 64.4％ (47/73) and 56.5％(35/62),respectively (P=0.380),while in the intention-to-treat analysis,the rates were 58.8％ (47/80) and 43.8％ (35/80,P=0.082),respectively.The compliance rate in the experimental group was significantly higher than that in the control group (91.3％ vs 77.5％,P＜0.05).There was significant difference in patients' satisfaction in good ones (75.3％ vs 51.6％) and poor ones (5.5％ vs 21.0％) between the 2 groups (P＜0.05).There were 11 patients in the experimental group and 36 patients in the control group,who appeared adverse reactions such as nausea,bad breath,abdominal distention,poor appetite,and defecation habit change during the process of eradicating H.pylori,but the occurrence rate in the experimental group was obviously lower than that in the control group (15.1％ vs 58.1％,P＜0.05).Conclusion:The telephone follow-up cannot increase the H.pylori eradication rate,but it can improve compliance and satisfaction for the patients and relieve adverse effects.

Objective To summarize surgical treatment of Takayasu arteritis,and analysis the drug treatment effect during the perioperative period.Methods Retrospective analysis 46 patients with Takayasu's arteritis disease and received cardiovascular surgery between January 2010 to December 2015,in Anzhen Hospital.By collecting their clinical characteristics,preoperative drug therapy,surgical treatment,pathological examination results to analyze operation conditions,effect of drugs and preoperative conditions.Results The perioperative mortality rate was 2.2％ and the complication rate was 23.9％ in 46 patients.There were 34 patients with symptomatic relief in the perioperative period,11 patients didn't take hormone drugs before operation.There were 11 cases of complications during the perioperative period,of which 7 patients were in active stage and 10 patients had not been used before operation.Conclusion The surgical treatment of patients with Takayasu's arteritis disease can effectively improve symptoms.The patients in Takayasu's arteritis active stage will affect the outcome of the surgery.Rational use of hormone drugs before surgery,can effectively control the patient's condition,improve the rate of remission of symptoms,and effectively reduce the incidence of perioperative complications.
