1.Development of a questionnaire for residents to evaluate the quality of general practice teaching clinics

Objective:To develop a scientific and practical questionnaire for general practice residents, and to conduct multidimensional and comprehensive evaluation of the quality of general practice teaching clinics.Methods:A preliminary draft of the questionnaire items was formulated based on a literature review and in-depth interviews. The Delphi method was employed to conduct two rounds of consultation with 14 experts. Following revisions, a convenience sampling method was used to invite general practice residents from three standardized residency training bases to test the reliability and validity of the questionnaire.Results:The questionnaire consisted of 23 items, covering the three dimensions of preparation, implementation process, and comprehensive evaluation of the teaching clinics. The response rates for the two rounds of the expert consultation were both 100.00%, with expert authority coefficients of 0.89 and 0.90, respectively. The overall Cronbach's α coefficient of the questionnaire was 0.93, and the correlation coefficients between each item score and the total score were all >0.30. Structural validity analysis revealed that three common factors were extracted from the questionnaire, with a cumulative variance contribution rate of 77.89%. Conclusions:The General Practice Teaching Clinic Quality Evaluation Questionnaire for Residents developed in this study demonstrates high reliability and validity. The questionnaire provides a scientific basis for the standardized assessment of teaching quality in general practice clinics. By incorporating resident feedback on the teaching process, the questionnaire promotes the development of a teaching clinic quality improvement mechanism focused on residents and plays a significant role in enhancing the teaching capabilities of supervising physicians in clinics.
2.A three-party evolutionary game analysis of patient privacy protection in live surgery

With the rapid development of network technology, live surgery has become the new way of surgery teaching. However, the issue of patient privacy protection caused by live surgery has received widespread attention. Based on the evolutionary game theory, this paper constructed an evolutionary game model from the three-party perspectives of doctors, patients, and government and analyzed the game behaviors of the three parties in the process of live surgery. Matlab software was utilized to conduct dynamic simulation and numerical simulation analysis. It was found that the factors affecting the choice of doctors’ strategies included protection costs, the cost of privacy leakage, the benefits of protection, high-traffic benefits, and other aspects; the factors affecting the choice of patient strategies encompassed surgical costs, the risk of privacy leakage, additional benefits, and other aspects; the factors affecting the choice of government strategies embodied regulatory costs and the improvement of credibility. To realize a win-win situation among doctors, patients, and the government, the three parties need to work together to ensure that patient privacy is not violated and find a balance between expanding the influence of medical education and protecting patient privacy.
3.Influencing factors of the comorbidity between inflammatory bowel disease and depression

Objective To investigate the influencing factors associated with the comorbidity of inflammatory bowel disease (IBD) and depression. Methods A case-control study was conducted based on the “Healthcare Big Data Platform” of a tertiary class-A comprehensive hospital in Shandong Province. IBD comorbid with depression was served as the case group and IBD without depression was served as the control group. Propensity score matching (PSM) was performed by matching the case group with the control group in a ratio of 1:2 according to the age and gender of the patients. Conditional logistic regression model was used to explore the influencing factors associated with the comorbidity of IBD and depression. Results A total of 405 patients with IBD were enrolled in this study, including 270 patients without depression and 135 patients comorbid with depression. The results of conditional logistic regression showed that the use of immunosuppressants (OR=2.84, 95% CI: 1.00-8.07) and glucocorticoids (OR=2.05, 95% CI: 1.17-3.58), dementia (OR=5.20, 95% CI:1.59-17.05), cardiovascular disease (OR=3.58, 95% CI: 1.84-6.98) and cancer (OR=2.63, 95% CI: 1.16-5.95) were associated with the comorbidity of depression and IBD. Conclusion Attention should be paid to the use of immunosuppressants and glucocorticoids in the population of IBD comorbid with depression, and the coexistence of physical diseases such as dementia, cardiovascular disease and cancer. Early prevention and targeted treatment measures should be taken for high-risk populations to reduce their risk of depression and improve their quality of life and health.
4.Major changes and implementation considerations of the 2024 version of the Declaration of Helsinki

The World Medical Association approved the latest revision of the Declaration of Helsinki on October 19, 2024, which was revised again after 11 years. As the international ethical principles for medical research involving research participants worldwide, this revision aims to respond to the dynamic and rapidly changing research environment and to ensure its relevance. The main contents of this revision include changing subjects to research participants as partners; adding other researchers as the implementing subjects and requiring individuals, teams, and organizations to comply; adding social value, always for the purpose of improving individual and public health; requiring free and full informed consent and maintaining the autonomy of research participants; advocating fair distribution and encouraging community participation; responsibly integrating vulnerable individuals, groups and communities; supporting the maintenance of independent review and authority, and clarifying the dual ethical review of international collaborative research; strengthening the supervision of biological samples and data to prevent risks; emphasizing that the welfare of research participants should be continuously maintained after the end of the clinical trials; adding environmental sustainability and expanding the scope of attention; requiring scientific integrity and maintaining the research environment; demonstrating the scientific validity of research plans and avoiding research waste; adding ethical principles should be observed during public health emergencies; using placebos cautiously and avoiding abuse; clarifying the conditions for the use of unproven interventions. To implement the revised contents of the Declaration of Helsinki, measures that could be taken include applying to all human-related research to protect research participants; centering on patients to ensure that research meets patients’ needs; implementing responsible inclusion through the collaboration of all parties; strictly supervising biological samples and data to protect privacy and maintain sustainable and healthy development; strengthening the supervision of the use of unproven interventions to safeguard the long-term interests of patients; continuously constructing ethics committees and embracing the support of artificial intelligence; standardizing effective informed consent and implementing the principles of freedom and adequacy. The Declaration of Helsinki, which aims to promote and ensure respect and protection of participants in a rapidly innovating medical research ecosystem, will continue to face revisions brought about by new challenges in the future.
5.Study on medical damage liability dispute cases involving breach of the duty to inform

ObjectiveTo understand the current situation of medical damage liability disputes involving breach of the duty to inform in China, analyze the factors influencing the types of medical staff’s breach of the duty to inform, and explore the notification problems of medical staff in clinical practice. On these foundations, suggestions were proposed to improve the performance of the duty to inform and reduce medical disputes. MethodsUsing public cases from the China Judgements Online as the data source, the relationship between risk points and types of breach of duty to inform was analyzed using the Chi-square test. Logistic regression analysis was performed to explore the influencing factors of the types of breach of the duty to inform, and qualitative research summarized the specific contents of breach of the duty to inform. ResultsThere were differences in the effects of factors, including whether the patient was hospitalized, whether surgery was performed, whether Intensive Care Unit (ICU) treatment was required, the level of the medical institution, and whether a consultation occurred, on the types of breach of the duty to inform (P<0.05). Whether surgery was performed was an influencing factor for the types of breach of the duty to inform. Qualitative research showed that the contents of breach of duty to inform primarily involve risk, treatment plan, and deficiencies in disease notification. ConclusionStrengthening the performance of the medical staff’s duty to inform should mainly focus on the medical side, coordinating with multiple parties and taking measures to improve the performance of the duty to inform, to reduce unnecessary medical disputes.
6.Clinical application of bilateral targeted puncture based on vertebral osteodense zone in percutaneous vertebroplasty.

OBJECTIVE:
To investigate the clinical efficacy of bilateral targeted puncture in percutaneous vertebroplasty(PVP) based on the vertebral osteodense zone.
METHODS:
A retrospective analysis was conducted on 76 patients with fresh symptomatic osteoporotic vertebral compression fractures, characterized by the presence of a dense zone, who underwent percutaneous vertebroplasty (PVP) between January 2021 and December 2021. All patients involved single-level vertebral fractures. There were 19 males and 57 females, aged from 62 to 88 years old, with an average of (68.5±12.5) years old. All patients underwent bilateral transpedicular puncture procedures. Preoperative CT or MRI was utilized to ascertain the relative position of the bone osteodense zone within the vertebral body (specifically, whether this zone is situated in the upper one-third or one-quarter of the left or right sagittal plane). Considering the head and tail regions of the dense zone as puncture targets, the puncture points and paths were meticulously planned, and the working channel was subsequently established. Under continuous monitoring by a C-arm X-ray machine, bone cement was carefully and gradually injected. The operation time, bone cement injection volume, and bone cement leakage were recorded. The visual analogue scale (VAS) and Oswestry disablity index (ODI) were used to evaluate the effectiveness of the operation. ODI and anterior height (AH) of the vertebral body were used to evaluate the efficacy.
RESULTS:
All patients successfully completed the surgery and were followed up for (8.0±1.0) months. The operation time was (36.57±11.25) min, the volume of bone cement injection was(6.07±1.19) ml, and 21 patients of bone cement leakage. There were 3 patients with the VAS exceeded 4 points two days postoperatively, indicating suboptimal pain management. At the three time points of pre-operation, 2 days post-operation and 6 months post-operation, the VAS scores were(7.82±1.29), (2.11±0.44), and (2.04±0.67) respectively;the ODI percentages were(75.65±7.23)%, (29.45±4.16)%, and(28.68±5.62)%;and the AH values were (11.02±1.30), (12.87±3.91), and (12.91±3.86) cm. The differences were all statistically significant(P<0.05). The aforementioned three indices demonstrated significant improvement at both 2 days and 6 months post-operation (all P<0.05). There were no statistically significant differences in these indices between the 2-day and 6-month post-operative periods(P>0.05). The postoperative outcome was satisfactory and durable, with no evidence of vertebral height reduction.
CONCLUSION
Bilateral targeted puncture based on the osteodense dense zone within the vertebral body can achieve bilateral symmetrical and upright full vertebral bone cement reinforcement without increasing bone cement leakage, achieving good early efficacy and preventing late vertebral collapse. This has positive significance for further improving the efficacy of percutaneous vertebroplasty.

7.Synthesis and in vitro antiviral effects against hepatitis C virus of oleanolic acid and ursolic acid derivatives

Objective To design and synthesize derivatives of oleanolic acid and ursolic acid, and investigate their anti-hepatitis C virus (HCV) activity along with that of common triterpenoid acids. To explore the structure-activity relationship and provide a reference for the research of anti-HCV drugs derived from natural products through obtaining compounds with higher activity. Methods Oleanolic acid and ursolic acid were directly reacted with corresponding amines using PyBOP as a condensing agent in the presence of DIEA. Alternatively, the target compounds were prepared through PCC oxidation followed by the Baeyer-Villiger reaction catalyzed by m-CPBA. In vitro anti-HCV activity was tested using the HCVcc infection model. Molecular docking was performed by Autodock software to investigate the interaction between the active compounds and HCV NS5B. Results Oleanolic acid, glycyrrhetinic acid, ursolic acid, and asiatic acid all exhibited certain anti-HCV effects. Specifically, oleanolic acid derivatives OA2-OA4, OA6, and OA7, as well as ursolic acid derivatives UA1 and UA2, demonstrated superior anti-HCV activity compared to their parent compounds. Preliminary structure-activity relationship analysis revealed that introducing a bulky group to 28-COOH of oleanolic acid and ursolic acid enhanced their activity. Molecular docking results demonstrated that the active compounds could stably bind to HCV NS5B, thereby exhibiting antiviral activity. Conclusion Pentacyclic triterpenoids possessed anti-HCV effects, and their derivatives coud be synthesized to obtain more active compounds. The anti-HCV mechanism of these compounds may be associated with their inhibition of NS5B.
8.Effect of supplementing specific nutrients under calorie restriction on exercise performance in mice

Objective To develop a composite nutritional preparation that can effectively improve exercise performance under calorie restriction(CR)condition.Methods A total of 24 male C57BL/6J mice(weighing 23~26 g,8 weeks old)were randomly divided into control group(CON),CR,CR+basal nutrient group(CRN1),and CR+compound nutrient group(CRN2).All groups underwent moderate-intensity running training 5 d per week,for totally 3 weeks.The grip strength of the forelimbs were measured weekly,and in 3 weeks after training,exhaustion and post-exhaustion distance tests were conducted to evaluate exercise performance.Blood biochemical indicators,levels of skeletal muscle and liver redox biomarkers,and histopathological conditions were measured and observed.Results After 21 d of intervention,the CR group and CRN1 group had the post-exhaustion running distance prolonged by 278%and 289%,respectively,reduced blood glucose level,and decreased muscle mass,subcutaneous fat and epididymal fat mass when compared with the CON group(P<0.05).Compared with the CON group,the CRN1 and CRN2 groups demonstrated significantly higher gastrocnemius glycogen content.The CRN2 group obtained even longer post-exhaustion distance(increased by 52%and 36%respectively,compared with the CON group and CRN1 group,P<0.05),enhanced grip strength of the forelimbs(raised by 9%,17%and 15%,respectively than the CON,CR and CRN1 groups,P<0.05),elevated brown fat mass(compared to the CON group and CRN1 group,P<0.05),increased blood glucose level(compared to the CRN1 group,P<0.05),decreased blood low-density lipoprotein cholesterol level(compared to the CON and CR groups,P<0.05),and increased glutathione peroxidase content in the gastrocnemius muscle(compared to the CON group,P<0.05).Conclusion Supplementing with compound nutritional supplements in mice under CR can promote exercise performance,including improving fatigue recovery after exhaustive exercise and enhancing forelimb grip strength.
9.Pharmacokinetics, tissue distribution, and excretion of anthraquinones and their glucuronide metabolites from raw and steamed pieces of Rhei Radix et Rhizoma extract in constipation rats

Background: Rhei Radix et Rhizoma has been traditionally used as a potent laxative for centuries due to its remarkable efficacy. Raw pieces of Rhei Radix et Rhizoma (RP) are known for their strong laxative effects, often accompanied by side effects, while steamed Rhei Radix et Rhizoma pieces (SP) possess a milder laxative effect and are widely used clinically. However, there is a lack of comprehensive evidence examining the mechanisms underlying SP's effectiveness, particularly from a bioavailability perspective. Objective: This study aimed to investigate the impact of the steaming process on the in vivo disposition of RP and SP through pharmacokinetics, tissue distribution, and excretion assays. Methods: An ultra-performance liquid chromatography-tandem mass spectrometry method was developed for the simultaneous quantitative analysis of prototype anthraquinones and their glucuronide metabolites. Pharmacokinetic, tissue distribution, and excretion assays were conducted in constipation rats following oral administration of RP and SP. Blood, tissue, urine, and fecal samples were collected and analyzed to compare the absorption, distribution, metabolism, and excretion profiles of anthraquinones, highlighting differences in bioavailability and safety between RP and SP. Results: Compared with the RP group, the SP group showed significantly reduced area under the plasma concentration-time curve, mean residence time, and half-life time values for rhein-8-O-β-D-glucopyranoside, rhein, emodin, aloe-emodin, and their glucuronide metabolites. The clearance values were significantly increased in the SP group. These results demonstrate that SP led to lower exposure levels and higher elimination rates of these components compared with RP. Additionally, these components were primarily distributed in the large intestine, where they exerted their laxative effects. Glucuronide metabolites were mainly excreted through urination, while prototype components were excreted in both urine and feces. Notably, the cumulative excretion of aloe-emodin, emodin, rhein, and their glucuronide metabolites was significantly higher in both urine and feces after SP administration, indicating that SP enhances the excretion of these components compared with RP. Conclusion: The findings suggest that SP reduced anthraquinone exposure levels while enhancing their excretion, demonstrating that the steaming process significantly promotes the elimination of key components. This study provides a comprehensive analysis of how steaming alters the in vivo disposition of Rhei Radix et Rhizoma, offering a scientific basis for the improved safety and clinical use of SP. These insights not only clarify the mechanistic differences between RP and SP but also contribute to a broader understanding of processing-induced modifications in Chinese medicines. This research paves the way for optimizing Chinese medicine processing techniques to enhance the safety and efficacy of herbal therapies.

Cemental tear is a rare and indetectable condition unless obvious clinical signs present with the involvement of surrounding periodontal and periapical tissues. Due to its clinical manifestations similar to common dental issues, such as vertical root fracture, primary endodontic diseases, and periodontal diseases, as well as the low awareness of cemental tear for clinicians, misdiagnosis often occurs. The critical principle for cemental tear treatment is to remove torn fragments, and overlooking fragments leads to futile therapy, which could deteriorate the conditions of the affected teeth. Therefore, accurate diagnosis and subsequent appropriate interventions are vital for managing cemental tear. Novel diagnostic tools, including cone-beam computed tomography (CBCT), microscopes, and enamel matrix derivatives, have improved early detection and management, enhancing tooth retention. The implementation of standardized diagnostic criteria and treatment protocols, combined with improved clinical awareness among dental professionals, serves to mitigate risks of diagnostic errors and suboptimal therapeutic interventions. This expert consensus reviewed the epidemiology, pathogenesis, potential predisposing factors, clinical manifestations, diagnosis, differential diagnosis, treatment, and prognosis of cemental tear, aiming to provide a clinical guideline and facilitate clinicians to have a better understanding of cemental tear.
