1.Relationship between childhood adversity and depression: cumulative effects, sensitive windows, and interactions with later environments

Childhood adversity is a risk factor for developing depressive symptoms or suffering from depressive disorders across the life course. However, existing evidence has focused on the analysis of the association between childhood adversity and depression from a single theoretical perspective, and there is a lack of comprehensive understanding of its multiple action pathways. By systematically summarizing the cumulative risk model, sensitive-period model, recency model, risk chain model, and the stress sensitization, stress amplification, and stress inoculation hypotheses, as well as the emerging pubertal stress recalibration hypothesis, this article attempts to construct an integrated theoretical framework to elucidate the internal logical synergy among these models. Furthermore, by reviewing relevant empirical evidence, this article analyzes the applicability and limitations of different theoretical models in illustrating the underlying psychopathological mechanisms of depression. The current evidence suggests that the impact of childhood adversity on the risk of psychopathology may be driven by a complex dynamic process involving the interaction of exposure timing, cumulative load, response boundaries of biological systems, and modification by later environments. Future research should utilize longitudinal cohorts and multimodal data within a unified analytical framework to comprehensively examine the multiple pathways through which childhood adversity affects the risk of depression. This will provide a reference for precisely identifying high-risk populations, targeting "recalibration windows" represented by adolescence, developing focused intervention strategies, and ultimately blocking the cascading effects of early life adversity as early as possible. 2.Impact of non-optimal temperature on 120 emergency call volume for acute alcohol intoxication: A time-series study in Wuxi City

Background Non-optimal temperatures pose significant threats to public health. Analyzing the association between temperature exposure and the number of emergency cases of acute alcohol intoxication can provide evidence for optimizing emergency resource allocation and response strategies. Objective To analyze the overall impact and lag effects of non-optimal temperatures on the number of 120 emergency calls for acute alcohol intoxication in Wuxi, and to assess the attributable risk, in order to provide empirical evidence for formulating climate-adaptive public health strategies. Methods Call records of acute alcohol intoxication from Wuxi's 120 emergency service, concurrent air pollutant data, and meteorological data (including daily mean temperature) were collected from January 1, 2014 to December 31, 2020. Distributed lag nonlinear modeling was used for time-series analysis, with cross-basis functions to capture the nonlinear relationship and lag effects between temperature and emergency volume. Confounding factors such as long-term trends, humidity, pollutants [ultimately including ozone (O3) and fine particulate matter (PM2.5)], day of the week, and holidays were controlled. The maximum lag period was set to 14 days. Single-day lag and cumulative lag effects of extreme temperatures were analyzed, followed by sensitivity analysis. 3.Analysis on clinical characteristics and mixed infections in 350 children with pertussis in Wuhan

OBJECTIVE To analyze the clinical characteristics and pathogens for mixed infections in children with pertussis,so as to references for clinical diagnosis and treatment.METHODS A retrospective analysis was conduc-ted on the clinical data of 350 children diagnosed with pertussis and hospitalized in Wuhan Children's Hospital from Jan.2022 to Dec.2023.The clinical characteristics,peak white blood cell count,peak lymphocyte ratio,high-sensitivity C-reactive protein or C-reactive protein,procalcitonin and complications were compared between different age groups,as well as between the single infection group and the mixed infection group.RESULTS Mixed infections were observed in 291 children(83.14%),predominantly viral(63.43%,222/350),with human rhinovirus/enterovirus being the most common(32.57%,114/350).For children aged<6 months,6 months to<1 year and 1 to<3 years,the incidence of family contact history of cough,panting,hasty breathing,respira-tory failure,peak white blood cell count and peak lymphocyte ratio were all higher than those aged 3 to<6 years and≥6years(all P<0.05).The incidence of post-cough vomiting was higher in children aged 6 months to<1 year and 1 to<3 years compared to other age groups(P<0.05).The length of hospital stay decreased with in-creasing age(P<0.05).The proportions of children with paroxysmal spasmodic cough,post-cough cyanosis,post-cough flushing,coughing spells,inspiratory three concave sign,pneumonia and pulmonary phlegm rales all generally decreased with age(all P<0.05).Nodding respiration was only observed in children aged<1 year(P<0.05).The incidence of fever,co-infection with bacteria and Mycoplasma pneumoniae infection was higher in children aged 6 months to<1 year,1 to<3 years,3 to<6 years and ≥ 6 years compared to those aged<6 months(all P<0.05).The incidence of fever was higher in the mixed infection group than that in the single in-fection group(P<0.05).Children in the single infection group had longer hospital stays,higher incidence of par-oxysmal spasmodic cough,nocturnal cough,post-cough flushing,coughing spells,pulmonary phlegm rales and higher peak lymphocyte ratio(all P<0.05).The incidence of paroxysmal spasmodic cough was higher in the sin-gle infection group for children aged<3 months(P<0.05).CONCLUSIONS The younger children with pertussis have a higher proportion of family contact history of cough and are more prone to clinical manifestations such as paroxysmal spasmodic cough,post-cough vomiting,post-cough cyanosis,and pneumonia.They also have higher peak white blood cell counts,peak lymphocyte ratios and longer hospital stays.As children'age increased,the in-cidence of fever,bacterial,and mycoplasma infections also increased.The types of pathogens in mixed infections varies with patients' age and pertussis mixed infections may mask typical clinical symptoms.
4.Symptom management experience in patients with acute decompensated heart failure: a Meta-synthesis of qualitative studies

Objective:To integrate the symptom management experiences of patients with acute decompensated heart failure (ADHF), so as to provide a basis for developing symptom management measures.Methods:Qualitative or mixed studies on the symptom management experience of ADHF patients included from establishment of the database to September 2024, were electronically retrieved in PubMed, CINAHL, Cochrane Library, Web of Science, Embase, EBSCO, China National Knowledge Infrastructure, WanFang Data, VIP, China Biomedical Database and other Chinese and English databases and gray literature databases. The quality of the literature was evaluated using the Critical Appraisal Skills Programme developed by the Center for Evidence-Based Medicine at the University of Oxford. The results were synthesized through the aggregative integration method.Results:A total of 14 papers were included. Thirty-four findings were distilled into eight categories and three integrative findings, namely, the multiple challenges posed by symptoms (complex and multiple symptomatic somatic experiences, symptom-induced mood changes, and reduced family and social engagement), the unmet needs of patients (insufficient healthcare resources, insufficient supply of discharge services provided by healthcare organizations, and lack of knowledge), and the co-existence of positive and negative coping styles (negative coping styles in symptomatic distress, positive debugging and diversified coping in symptomatic distress) .Conclusions:ADHF symptoms severely affect patients' physical, psychological, and social function. Healthcare professionals should focus on the unmet needs of patients with ADHF and explore patient-engaged models of active symptom management.
5.Correlation between changes of brain region volume and cognitive function in patients with temporal lobe epilepsy

Objective To observe the volume changes of brain regions and their correlation with cognitive function in temporal lobe epilepsy(TLE)patients.Methods Fifty TLE patients(TLE group)and 34 healthy controls(control group)were prospectively enrolled.Brain 3D-T1WI was collected,brain regions were delineated with automated segmentation technique,and hippocampus was segmented additionally with hippocampal brain template(HBT).The volume of brain regions were compared between groups,and the correlation of brain region volume with Montreal cognitive assessment(MoCA)score were analyzed.Results Compared with control group,in TLE group,bilateral cerebrum white matter,right globus pallidus,left posterior cingulate gyrus,left precentral gyrus,right fusiform gyrus,left cerebellum white matter,corpus callosum,left hippocampus(HBT segmentation and conventional segmentation)and right thalamus volumes decreased significantly(all P<0.05),while right choroid plexus volume increased significantly(P<0.05).The volume of left hippocampus(HBT segmentation)and right fusiform gyrus were both positively correlated with MoCA score in TLE group(r=0.335,0.376,P=0.009,0.007).Conclusion Widespread atrophy of brain regions could be seen in TLE patients,which were correlated with cognitive function.

7.Compliance analysis of new oral anticoagulants in elderly patients with nonvalvular atrial fibrillation

Objective To explore the influencing factors of compliance with new oral anticoagulants(NOACs)in elderly patients with nonvalvular atrial fibrillation(NVAF)and its correlation with social support.Methods The clinical data of 126 patients with NVAF treated by NOACs in Lu 'an People's Hospital of Anhui Province from January 2022 to January 2024 were selected by convenient sampling method.General data of patients were collected.According to Morisky medication compliance scale,patients were divided into two groups:the good compliance group(n=74)and the poor compliance group(n=52).Social support was investigated by Social Support Rating Scale(SSRS).General data of patients were compared between the good compliance group and the poor compliance group.The relationship between medication compliance and social support was analyzed by Pearson correlation.Multivariate Logistic regression was used to analyze the influencing factors of patient medication compliance.Results There were statistically significant differences in educational level,the proportion of patients with anxiety and depression,CHA2DS2-VASc score,HAS-BLED score and SSRS score between the good compliance group and the poor compliance group(P<0.05).The results of the correlation analysis showed that the medication compliance of patients was positively correlated with objective support(r=0.461),subjective support(r=0.432),support utilization(r=0.323)and SSRS score(r=0.596,P<0.05).The results of multivariate Logistic regression analysis showed that low educational level,accompanied by anxiety,low HAS-BLED and SSRS scores were risk factors for poor medication compliance in elderly NVAF patients(P<0.05).Conclusion Improving the social support level of elderly patients with NVAF is conducive to the formation of good medication compliance.
8.Current development status of genomics in nursing

With the advent of the genomic era, genomics has gradually become a part of nursing. The use of genomic data can explore the mechanisms of disease occurrence and progression, accurately identify intervention targets and regulatory pathways of diseases, provide precise and personalized nursing services for patients, and improve their quality of life. This paper reviews the current application status of genomics in nursing, deeply analyzes the development process of genomics in genetic risk assessment, symptom precision nursing, precision nursing decision-making, and nursing education, and proposes corresponding development strategies. Through a comprehensive and in-depth exploration of the combination of genomics and nursing, the aim is to provide strong theoretical guidance for the future development of genomics nursing in China.
