1.Predictive Value of Residual Quantitative Flow Ratio for Long-term Vessel-oriented Composite Endpoints

Objectives:To explore the predictive value of residual Murray's law-based quantitative flow ratio(μQFR)on long-term vessel-oriented composite endpoints(VoCE).Methods:This retrospective study included 3 510 patients from the FAVOR Ⅲ China trial.Offline residual μQFR analysis was performed on all vessels(diameter≥2.5 mm)with 50%-90%stenotic lesions.Patients were stratified into high-,intermediate-,and low-risk groups based on residual μQFR tertiles.The primary endpoint was 3-year VoCE,defined as a composite of cardiac death related to the target vessel,target vessel-related spontaneous myocardial infarction,and ischemia-driven target vessel revascularization.Results:Offline analysis was performed on 5 256 vessels from 3 510 patients.The mean residual μQFR was 0.92±0.75.The high-risk group(residual μQFR≤0.91)with 1 554 patients(1 958 vessels);the intermediate-risk group(residual μQFR 0.92-0.96)with 1 211 patients(1 906 vessels);and the low-risk group(residual μQFR>0.96)with 745 patients(1 392 vessels).Over 3-year follow-up,VoCE occurred in 227 vessels(4.3%).The 3-year VoCE incidence was significantly higher in the high-risk group compared to the intermediate-and low-risk groups(6.2%vs.4.1%vs.2.5%,log-rank P<0.001),primarily driven by ischemia-driven target vessel revascularization(5.0%vs.3.0%vs.1.6%,log-rank P<0.001).Hypertension(OR=0.83,95%CI:0.72-0.96),hypercholesterolemia(OR=0.84,95%CI:0.73-0.97),bifurcation lesions(OR=0.72,95%CI:0.63-0.83),moderate/severe calcification(OR=0.70,95%CI:0.57-0.84),and tandem lesions(OR=0.59,95%CI:0.47-0.75)were independent predictors of lower residual μQFR values.Conclusions:Lower residual μQFR is significantly associated with increased VoCE risk during the 3-year follow up period.
2.Application of Artificial Intelligence in Whole-process Management of Patients With Valvular Heart Disease

This review systematically describes the application of artificial intelligence in the comprehensive management of patients with valvular heart disease(VHD).In population screening,technologies such as artificial intelligence(AI)electrocardiogram(AI-ECG),AI auscultation,and new imaging modalities play important roles,help early detection of VHD.In imaging assessment and diagnosis,AI can add automatic morphological and functional evaluations,assisting in precise typing and risk prediction to accurately determine the timing of VHD intervention.For treatment,especially transcatheter aortic valve replacement(TAVR),AI can assist in image analysis and strategy formulation in a variety of ways throughout the process,improving procedure success rates and avoiding complications.Although the application of AI is limited by factors such as data quality and computational capacity,in the future,multi-center cooperation can be adopted to expand data samples and enhance computational capabilities to improve AI performance,thus further optimizing VHD management.

This article focused on the revision and application of General Rule 0982 Determination of Particle Size and Size Distribution by Light Scattering Method in the Chinese Pharmacopoeia(2025 edition).By referencing domestic and international technical requirements and considering practical applications,the structure and content of this general rule were analyzed,and specific revisions were proposed.Key recommendations included adding sections on method validation and results reporting to the current general rule,incorporating the appendix content into the main text,and revising/supplementing the method name,introduction,instrument requirements,and determination procedures.The revised light scattering method would strengthen China's capacity for pharmaceutical quality control.
4.Key Data Collection Specifications and Variable Standards for Measuring the Clinical Management and Outcomes of Patients With Valvular Heart Disease in China:Based on the Multi-center Clinical Research Innovation Platform of"China-DVD2 Study"

Objectives:Utilizing an advanced and high-capacity data acquisition system tailored for elderly patients with valvular heart disease,we aimed to establish standardized data acquisition protocols and define variable sets for a clinical research big data platform dedicated to this condition.Guided by clinical treatment decisions,we implemented a standardized and uniform evaluation process for valvular heart disease.This initiative might culminate in the creation of the first comprehensive database for elderly valvular heart disease,encompassing clinical information,functional assessments,multimodal imaging,treatment modalities,and follow-up data.Methods:Variables were selected based on past clinical studies and literature,both domestic and international,and considering key guidelines,advancements in valvular heart disease,and China's clinical practices.Standards for data variables,imaging techniques,data acquisition,multi-channel verification,and quality control were established.Variables are then classified by their properties and generation time.Results:This comprehensive set of variables encompassed a total of 567 relevant variables,which were generated throughout the entire diagnosis and treatment process of patients with valvular heart disease,starting from admission and extending to those required for clinical research.The variable set was divided into six distinct parts:clinical information(encompassing demographics,admission details,clinical manifestations,comorbidities,medical history,physical examination,auxiliary examinations,and comprehensive assessment of frailty in the elderly),echocardiography,cardiac computed tomography(CT),treatment(including surgery,valve intervention therapy,non-valve intervention therapy,and medication therapy),clinical outcomes,and follow-up.Specifically,it included 14 variables related to demographic and admission information;22 variables encompassing clinical manifestations and physical examination;63 variables concerning medical history and comorbidities;49 variables for inspection and examination;15 variables for the comprehensive evaluation of frail elderly individuals;55 variables of echocardiography,including complete dynamic images at 4 time points,6 cross-sections per point,and corresponding Doppler spectra);131 variables for cardiac CT,including baseline and postoperative raw image uploads(encompassing non-enhanced electrocardiography[ECG]-gated cardiac CT scans,ECG-gated full-phase CT angiography(CTA)scans,and large-scale non-ECG-gated spiral CTA scans);51 variables for surgical diagnosis and treatment;72 variables for valve intervention therapy;33 variables for non-valvular interventional therapy;27 variables for medication information;and 35 variables for clinical outcomes.The follow-up integration comprises 140 variables,along with the upload of raw echocardiography and CT images.Conclusions:Through the establishment of data collection standards and variable standards for the big data platform of valvular heart disease,the standardization and normalization of comprehensive evaluation,diagnosis and treatment,and clinical research of patients with valvular heart disease can be achieved.As a national level big data research platform that can be sustainably used for clinical research and innovative development of valvular heart disease,it will provide support for long-term monitoring of valvular heart disease,real-world data analysis,clinical research(observational and randomized controlled trials),technological innovation,drug and device development,and post market evaluation.

6.Concept Change of Percutaneous Coronary Intervention for Patients With Stable Coronary Heart Disease

Since the first report of percutaneous transluminal coronary angioplasty for patients with coronary heart disease in 1977,percutaneous coronary intervention(PCI)has been rapidly developed and comprehensively popularized,and is considered to be the most effective method for the treatment of coronary heart disease.Clinical trials have demonstrated that PCI could reduce the risk of major cardiovascular events in patients with acute coronary syndrome.However,the effectiveness of PCI in patients with stable coronary artery disease(SCAD)has been controversial.In addition,the rapid development of drugs,interventional devices and diagnostic techniques has continuously challenged the concept of PCI treatment for patients with SCAD.This article reviews the evidence-based medical evidence of PCI for treatment of SCAD,sorts out the changes in the concept of PCI for SCAD at different stages,and looks forward to the future prospects of PCI for SCAD.

BACKGROUND:Psoralen has a strong anti-osteoporotic activity and may have a restorative effect on chemotherapy-induced osteoporosis. OBJECTIVE:To explore the restorative effect of psoralen on the osteogenic differentiation of bone marrow mesenchymal stem cells in mice inhibited by cyclophosphamide and its mechanism. METHODS:C57BL/6 mouse bone marrow mesenchymal stem cells were isolated and cultured.Effect of psoralen on viability of bone marrow mesenchymal stem cells was detected by MTT assay.Osteogenic induction combined with alkaline phosphatase staining was used to determine the optimal dose of psoralen to restore the osteogenic differentiation of bone marrow mesenchymal stem cells inhibited by cyclophosphamide.The mRNA expression levels of Runx2,alkaline phosphatase,Osteocalcin,osteoprotegerin,and Wnt/β-catenin signaling pathway-related genes Wnt1,Wnt4,Wnt10b,β-catenin,and c-MYC were measured by RT-qPCR at different time points under the intervention with psoralen.The protein expression of osteogenic specific transcription factor Runx2 and Wnt/β-catenin signaling pathway related genes Active β-catenin,DKK1,c-MYC,and Cyclin D1 was determined by western blot assay at different time points under the intervention with psoralen. RESULTS AND CONCLUSION:(1)There was no significant effect of different concentrations of psoralen on the viability of bone marrow mesenchymal stem cells.The best recovery of the inhibition of osteogenic differentiation of bone marrow mesenchymal stem cells caused by cyclophosphamide was under the intervention of psoralen at a concentration of 200 μmol/L.(2)Psoralen reversed the reduction in osteogenic differentiation marker genes Runx2,alkaline phosphatase,Osteocalcin and osteoprotegerin mRNA expression and Runx2 protein expression in bone marrow mesenchymal stem cells caused by cyclophosphamide conditioned medium.(3)Psoralen reversed the decrease in Wnt/β-catenin pathway-related genes Wnt4,β-catenin,c-MYC mRNA and Active β-catenin,c-MYC,and Cyclin D1 protein expression and the increase in DKK1 protein expression in bone marrow mesenchymal stem cells caused by cyclophosphamide conditioned medium.(4)The results showed that cyclophosphamide inhibited osteogenic differentiation of bone marrow mesenchymal stem cells in mice,and psoralen had a restorative effect on it.The best intervention effect was achieved at a concentration of 200 μmol/L psoralen,and this protective effect might be related to the activation of Wnt4/β-catenin signaling pathway by psoralen.
8.The impact of coronary artery calcification on the long-term outcomes after chronic total occlusion percutaneous coronary intervention

Objective:Investigate the impact of calcification on the long-term outcomes of patients with coronary chronic total occlusion (CTO) after percutaneous coronary intervention (PCI).Methods:A retrospective cohort study was conducted. Patients who underwent PCI and had at least one CTO lesion at Fuwai Hospital between January 2010 and December 2013 were consecutively enrolled. Calcification was evaluated by coronary angiography, and patients were divided into two groups: moderate/severe calcification group and non/mild calcification group. Clinical follow-up was completed up to 5 years. Incidence of PCI-related complications and immediate procedural outcomes were compared between two groups, and the primary endpoint was the target lesion failure (TLF) at 5 years after PCI. Clinical follow-up endpoint events were analyzed using Kaplan-Meier survival analysis with log-rank test, and Cox multivariate regression model was used to evaluate the relationship between calcification and TLF.Results:The study included 2 659 CTO patients with an age of (57.2±10.5) years, of whom 442 (16.6%) were female, and among whom 13.5% (360/2 659) had moderate/severe calcification. Compared with the non/mild calcification group, the moderate/severe calcification group had a higher incidence of PCI-related complications (43.2% (156/361) vs. 32.5% (772/2 374), P<0.001) and procedural failure (34.3% (124/361) vs. 24.3% (577/2 374), P<0.001). Additionally, the moderate/severe calcification group showed a higher risk of the primary endpoint event (TLF) during the 5-year follow-up (19.8% vs. 15.3%, log-rank P=0.028). Higher incidence of cardiac death was observed in moderate/severe calcification group (5.7% vs. 2.7%, log-rank P=0.003). Cox multivariate regression analysis revealed that moderate/severe calcified plaques remained an independent risk factor for 5-year TLF after CTO-PCI ( HR=1.34, 95% CI: 1.01-1.79, P=0.043). Conclusion:Compared with CTO patients with non/mild calcification, those with moderate/severe calcification have higher procedural failure and complication rates, as well as poorer long-term prognosis, mainly due to an increase in cardiac death.
9.Transcriptomic analysis and characterization of brain tissue from lambs infected with Enterococcus faecalis

In order to study the pathogenesis of meningitis in lambs caused by Enterococcus faecalis(E.faecalis),and to elucidate the pathogenesis of meningitis from the circRNA level,E.faecalis(named XJ6)was reintroduced into the host to construct a lamb brain tissue injury model,and then the lamb brain tissues were selected for whole genome transcript sequencing at four time in-tervals(24,48,60 and 72 h).Based on the transcriptomics sequencing results,the differential cir-cRNAs were preliminarily screened,and the accuracy of the transcriptomics data was verified by real-time fluorescence quantification.The results showed that the experiments successfully con-structed the brain tissue injury model of E.faecalis infected lambs,and the GO enrichment analy-sis results showed the most significant 10 cellular components,10 biological processes,and 10 mo-lecular functions.KEGG pathway enrichment results revealed that the genes targeted by differenti-al circRNAs were mainly focused on signaling pathways regulating the neuronal cells,the MAPK signaling pathway,the Rap1 signaling pathway,VDEF signaling pathway,Apelin signaling path-way,vascular smooth muscle contraction,and cancer development.The differentially expressed cir-cRNAs validated by real-time quantitative PCR were completely consistent with the transcriptomic results.Among them,novel_circ_0004872 was identified as a candidate circRNA potentially involved in altering the permeability of the blood-brain barrier in lambs,providing a theoretical ref-erence for exploring the regulatory role of circRNAs in blood-brain barrier function.

In recent years, with the continuous development and increasing maturity of interventional techniques, interventional treatment for congenital heart disease (CHD) has been progressively disseminated to county- and city-level hospitals in China. Concurrently, the standardized management of adult CHD (particularly patent foramen ovale) and the lifelong management of complex CHD are gaining increasing clinical attention, while the emergence of new techniques and products continuously advances the discipline. This article aims to review the new progress made in the field of interventional treatment for congenital heart disease in China during 2024. It specifically reviews and analyzes the following key aspects: (1) annual statistics on interventional closure procedures for CHD; (2) recent insights into patent foramen ovale closure; (3) advances in transcatheter pulmonary valve replacement; (4) interventional treatment and lifelong management strategies for complex CHD; (5) new interventional techniques for acquired heart disease; and (6) the application of artificial intelligence in CHD management. Through the synthesis and discussion of these topics, this article seeks to provide a detailed analysis of the current landscape of interventional treatment for CHD in China and project its future development trends.
