1.Analysis of Risk Factors and Establishment of Prediction Model for Turbidity Toxicity Accumulation Syndrome in Patients with Chronic Atrophic Gastritis

ObjectiveThis paper aims to explore the risk factors for chronic atrophic gastritis (CAG) with turbidity toxin accumulation syndrome and establish a prediction model. MethodsClinical data of 180 patients with CAG who participated in the "clinical study of Xianglian Huazhuo Particles blocking CAG cancer transformation" of Hebei Sheng Zhong Yi Yuan from July 2021 to March 2022 were collected. After confounding factors were controlled by propensity score matching, patients were divided into a training set (namely dev) and a validation set (namely vad) in a seven to three ratio. The risk factors for CAG with turbidity toxin accumulation syndrome in the training set were investigated by using univariate Logistic regression analysis and least absolute shrinkage and selection operator (namely Lasso) regression algorithms. Subsequently, a model, named model 1se, was developed by using the training set data to predict the risk factors for CAG with turbidity toxin accumulation syndrome. The accuracy of the prediction model was assessed by using various methods, including the receiver operating characteristic (ROC) curve, Hosmer-Lemeshow test (H-L), calibration plot, and decision curve analysis (DCA). ResultsAge, body mass index (BMI), family history of cancer, job and life satisfaction, yellow and greasy fur with slippery pulse, and heavy body sensation were independent risk factors of the model. The prediction model showed excellent predictive value for both the training and validation sets. ConclusionThe established prediction model for CAG with turbidity toxin accumulation syndrome has high discrimination and excellent calibration, which could provide an excellent clinical basis for disease diagnosis and individualized treatment of patients.

3.MiR-338-3p affects proliferation and apoptosis of alveolar bone osteoblasts by targeting receptor activator of nuclear factor-kappaB ligand

BACKGROUND:MiR-338-3p could inhibit osteoclast differentiation,and downregulation of receptor activator of nuclear factor-κB ligand level could promote bone formation.However,it is unclear whether miR-338-3p can affect the proliferation and apoptosis of alveolar bone osteoblasts by regulating the receptor activator of nuclear factor-κB ligand level. OBJECTIVE:To explore the effect and mechanism of miR-338-3p on proliferation and apoptosis of alveolar bone osteoblasts by targeting receptor activator of nuclear factor-κB ligand. METHODS:Human alveolar bone osteoblasts were isolated,transfected and treated with Wnt-C59(Wnt/β-catenin pathway inhibitor),and divided into transfection control group,miR-338-3p group,miR-338-3p+control group,miR-338-3p+receptor activator of nuclear factor-κB ligand group and miR-338-3p+Wnt-C59 group.The dual luciferase report experiment was used to verify the regulatory effect of miR-338-3p on receptor activator of nuclear factor-κB ligand.Cell counting kit-8 and 5-Ethynyl-2'-deoxyuridine staining were used to detect cell proliferation levels.Flow cytometry was used to detect cell cycle and apoptosis levels.RT-qPCR was used to detect miR-338-3p,receptor activator of nuclear factor-κB ligand,Wnt-3a,β-Catenin,glycogen synthase kinase-3β mRNA levels.Western blot was used to detect RANKL,proliferating cell nuclear antigen,Ki67,CyclinD1,B-cell lymphoma/leukemia-2,B-cell lymphoma-2 related X protein,Caspase3,Wnt-3a,β-catenin,glycogen synthase kinase-3β protein levels. RESULTS AND CONCLUSION:miR-338-3p could target the regulation of receptor activator of nuclear factor-κB ligand.After overexpression of miR-338-3p,cell survival rate,5-Ethynyl-2'-deoxyuridine positive cell rate,proportion of S-phase cells were increased,and apoptosis rate was decreased.The mRNA and protein levels of miR-338-3p,proliferating cell nuclear antigen,Ki67,CyclinD1,B-cell lymphoma/leukemia-2,Wnt-3a,and β-catenin were increased,while the mRNA and protein levels of B-cell lymphoma-2 related X protein,Caspase3 protein,receptor activator of nuclear factor-κB ligand,and glycogen synthase kinase-3β were decreased(all P<0.05).Overexpression of receptor activator of nuclear factor-κB ligand or Wnt-C59 could weaken the effects of overexpression of miR-338-3p on cell proliferation and apoptosis(all P<0.05).Overall,miR-338-3p promotes alveolar bone osteoblast proliferation and inhibits apoptosis by targeting receptor activator of nuclear factor-κB ligand,which may act through activation of the Wnt/β-catenin signaling pathway.
4.Perioperative management strategies for hiatal hernia in patients 70 years old or above

Objective:To explore perioperative management strategies for hiatal hernia in patients 70 years old or above.Methods:The perioperative clinical data of 102 patients (≥70 years) undergoing minimally invasive hiatal hernia repair from January 2019 to December 2024 was retrospectively analyzed.Results:The elderly group age ranged from 70 to 95 years,including 32 males and 70 females with disease duration of 0.67-51 years (mean 10.1 years), showing 68.63% comorbidity incidence (70 cases). Postoperative complication rates were 22.55%, with no severe complications occurring within one month after surgery.Conclusions:Hiatal hernia patients 70 years old or above exhibit longer disease histories, higher comorbidity rates, and increased postoperative complications. Comprehensive preoperative assessment, precise minimally invasive intraoperative techniques, and enhanced perioperative management ensure safe anesthesia and surgical implementation, improve surgical safety, and facilitate patient recovery.
5.Characteristics of wh-question syntactic deficits in patients with Chinese non-fluent aphasia

Objective To study the syntactic impairment characteristics of wh-question comprehension and expression in Chinese non-fluent aphasic patients.Methods The differences in comprehension and expression be-tween 25 non-fluent aphasic patients and 25 normal subjects were tested in the form of sentence-figure matching and elicitation-repetition to analyze syntactic impairment characteristics related to sentence patterns,question objects,and question words.Results The patients with non-fluent aphasia had impaired comprehension and expression of specific interrogative sentences,and the order of correct comprehension was:(active interrogative sentence,object pseudo-split interrogative sentence)>(subject pseudo-split interrogative sentence,passive interrogative sentence),and the order of correct expression was:active interrogative sentence>(pseudo-split interrogative sentence,pas-sive interrogative sentence).Conclusion The pattern of impaired comprehension and expression of wh-question was similar in patients with Chinese non-fluent aphasia.The main influences on syntactic structure included the object of questioning(subject-object)and the type of sentence.Obstacles were more pronounced in sentences with wh-move-ment and non-canonical order.

7.Multiple Myeloma Concurrent with Extramedullary Plasmacytoma of the Breast:A Case Report and Literature Review

Extramedullary plasmacytoma(EMP)is a malignant neoplasm characterized by monoclonal plasma cell proliferation originating outside the bone marrow and hematopoietic tissues.The co-occurrence of multiple myeloma(MM)with mammary EMP is clinically rare.This case report describes the diagnostic and therapeutic management of a middle-aged female patient with MM and mammary EMP,accompanied by a litera-ture review.The patient presented with persistent fever and was found to have a right breast mass via PET/CT during routine follow-up for MM.The lesion was biopsy-confirmed as mammary EMP,and the patient subse-quently underwent chemotherapy.This case highlights the critical importance of differential diagnosis between breast cancer and rare hematologic metastatic tumors in early prevention of disease progression.
8.Research progress in the application of artificial intelligence technology in healthcare-associated infection prevention and control

The application of artificial intelligence(AI)technology in healthcare-associated infection prevention and control deepens continuously.It demonstrates a potential for achieving precise prevention and control of infec-tion through data integration,model construction,and dynamic analysis,and provides scientific support for optimi-zation of decision-making in relevant departments.However,the application of related technologies still faces multi-ple challenges,such as data quality,system compatibility,and adaptation to actual scenarios,which limits its com-prehensive promotion in clinical settings.Based on this,this study systematically summarizes the research progress of AI application in infection prediction,diagnosis support,behavior optimization,and resource management in re-cent years,deeply analyzes its key technical advantages and limitations,and proposes improvement strategies for existing problems and future development directions,with a view to providing theoretical support and practical refe-rences for the intelligent transformation of infection prevention and control in hospitals in China.
9.Application of PDCA cycle in critical value management at an occupational disease specialized hospital

Objective To evaluate the effectiveness of the PDCA cycle in critical value management.Methods PDCA cycle method was applied to manage critical values in an occupational disease hospital.Critical value data from October 2021 to March 2022 were analyzed to identify root causes,followed by corrective measures development and implementation.Post-inter-vention data,from July 2022 to November 2022,were compared with the pre-management data to evaluate the implementation effect of the PDCA cycle on critical value management.Results The application of the PDCA cycle led to a statistically signifi-cant increase in the execution compliance rate for emergency value execution in medical and technical departments increased to 100.00%,and in the clinical departments to 96.77%(P<0.05).Conclusion PDCA cycle can improve the qualified rate of the implementation of critical value system,ensure patients safety,significantly reduce medical risks,and advance the level of standardization and precision in critical value management within our hospital.

Congenital heart disease (CHD) may result from various risk factors including genetic, epigenetic, and environmental elements. The parallel and synchronous development of the placenta and fetal heart establishes a regulatory relationship known as the placenta-heart axis. Placental insufficiency may impact fetal cardiac development, while abnormal cardiac development can conversely disrupt placental structure and function. This review examines the association between placental abnormalities and CHD, providing insights into the etiology and underlying mechanisms of CHD.
