Objective: To investigate the status of heat illness, knowledge awareness and adaptive behaviors of heat wave knowledge among food delivery riders, so as to provide a basis for optimizing heat wave response measures for food delivery riders. Methods: In November 2022, food delivery riders from a large food delivery platform in Ningbo City, Zhejiang Province were selected as survey subjects using a cluster sampling method. A self-designed electronic questionnaire was used to select demographic information, work status, lifestyle behaviors and disease history, heat illness status, knowledge awareness and adaptive behaviors of heatwave. Results: A total of 911 questionnaires were distributed, and 830 valid questionnaires were recovered, resulting in a valid response rate of 91.11%. Among the respondents, 796 (95.90%) were male, and 818 (98.55%) worked full-time. The mean age was (27.75±8.00) years. A total of 470 respondents (56.63%) had a work tenure of less than 1 year. The primary working hours were 8-<12 hours, with 504 people accounting for 60.72%. There were 108 cases of heatstroke, with an occurrence rate of 13.01%. And 286 people reported heat-related symptoms, with an occurrence rate of 34.46%. The overall awareness rate of heat wave knowledge was 73.22%, while the awareness rate of heat warning signal classification was relatively low at 9.04%. The heat wave cognition score was (5.86±1.31) points. There were statistically significant differences in heat wave cognition scores among food delivery riders of different ages, educational levels, family annual income, work tenures, and work durations (all P<0.05). Regarding positive adaptive behaviors, the number of riders paying attention to weather forecasts and actively learning about preventive measures was higher (734 people each, accounting for 88.43%). Regarding negative adaptive behaviors, the number of riders who often drank ice-cold beverages was higher (509 people, accounting for 61.33%). The heat wave adaptive behavior score was (6.88±1.77) points. There were statistically significant differences in adaptive behavior scores among riders with different educational levels, family annual income, work tenures, and smoking frequency (all P<0.05). Conclusions The occurrence rates of heatstroke and heat-related symptoms among food delivery riders are relatively high. The knowledge awareness and adaptive behaviors regarding heat wave are at a moderate level. It is suggested to strengthen health education, reinforce risk cognition of heat wave, and promote positive adaptive behaviors among food delivery riders.

ObjectiveTo evaluate the clinical efficacy of Huangqin Qingre Chubi capsules （HQC） in patients with rheumatoid arthritis （RA） and the syndrome of dampness-heat obstruction and investigate the mechanism by which HQC restore clinical and laboratory parameters in RA patients through regulation of the long non-coding RNA （LncRNA） endogenous Bornavirus-like nucleoprotein 3 pseudogene （EBLN3P）/miR-369-3p/nuclear factor I X（NFIX）-Janus kinase（JAK）/signal transducer and activator of transcription （STAT） signaling pathway，thereby providing new experimental evidence and potential therapeutic targets for the traditional Chinese medicine （TCM） treatment of RA. MethodsA randomized controlled trial design was adopted，with 100 RA patients meeting the criteria assigned into an observation group （n=50） and a control group （n=50）. In addition，50 healthy controls were enrolled. The control group received leflunomide plus meloxicam，while the observation group additionally received HQC. Efficacy was assessed after 4 weeks of treatment. Observation indicators including Western medicine indicators （DAS28，CPRI-RA，SF-36，VAS，PGA，PhGA，and SAS/SDS），TCM syndrome scores （TCMSS-RADHP，TCMSS-RASDP），and laboratory indicators （ESR，CRP，RF，anti-CCP antibody，VEGF，bFGF，MMP-2/9，p-JAK2/STAT3，and LncRNA EBLN3P/miR-369-3p/NFIX expression） were measured to comprehensively explore the therapeutic mechanism of HQC. ResultsThe study included 45 healthy controls （34 males and 11 females） and 87 RA patients （22 males and 65 females）. The median age，median disease duration，and median BMI of patients were 56 years old，10 years，and 21.51 kg·m^-2，respectively. Compared with healthy controls，the RA group exhibited a typical state of immune-inflammatory activation as manifested by elevated levels of ESR，anti-CCP antibodies，CRP，and RF （P<0.01），raised clinical symptom scores （DAS28，VAS，PGA，PhGA，CPRI-RA，and TCM symptoms） （P<0.05，P<0.01），and reduced SF-36 domain scores （P<0.01）. Molecular detection revealed abnormal expression of LncRNA EBLN3P and miR-369-3p，upregulated expression of key proteins in the JAK/STAT pathway （JAK2/p-JAK2 and STAT3/p-STAT3） and angiogenesis markers （VEGF，bFGF，and MMP-2/9），and downregulated expression of NFIX （P<0.05，P<0.01） in the RA group. Spearman correlation analysis further demonstrated associations of key molecules in the LncRNA EBLN3P/miR-369-3p/NFIX-JAK/STAT pathway with immune-inflammatory markers and clinical symptom scores （P<0.05，P<0.01）. Regarding treatment response，both groups showed reductions in ESR，anti-CCP，CRP，and RF levels compared with baseline （P<0.01） and restored scores of DAS28，CPRI-RA，VAS，PGA，PhGA，TCM symptoms，SAS，SDS，and all SF-36 domains （P<0.01）. The observation group outperformed the control group in restoring the scores of SAS，SDS，PGA，TCMSS-SDP，and SF-36 domains （PF，BP，GH，VT，and SF） （P<0.05）. Real-time PCR analysis revealed that post-treatment expression levels of LncRNA EBLN3P，miR-369-3p，NFIX，JAK2，p-JAK2，STAT3，p-STAT3，VEGF，bFGF，MMP-2，and MMP-9 were recovered compared with pre-treatment levels （P<0.01）. Notably，the observation group demonstrated superior regulatory effects on p-JAK2，STAT3，p-STAT3，and MMP-9 compared with the control group （P<0.05）. Apriori association rule analysis revealed that HQC treatment was significantly correlated with reduced TCMSS-RADHP，ESR，SF，DAS28，SAS，and SDS scores，as well as increased VT，GH，and RP scores （confidence >80%，support >50%）. Regarding safety，two patients in the observation group experienced mild gastrointestinal discomfort，and one patient in the control group developed a rash. All the symptoms resolved after symptomatic treatment. ConclusionHQC target and regulate the LncRNA EBLN3P/miR-369-3p/NFIX-JAK/STAT axis to inhibit angiogenesis and inflammatory responses，reduce immune inflammatory markers，alleviate clinical symptoms，and improve the quality of life in RA patients，exerting synergistic effects on symptoms and quality of life.

Objective This study aims to investigate the current status and potential profiles of emotional inhibition in stoma patients with colorectal cancer,to analyze the differences and influencing factors,and thereby provide a basis for the development of intervention strategies.Methods A convenience sample of 348 colorectal cancer patients with ostomies was recruited from colorectal surgery departments and ostomy outpatient clinics of a tertiary A hospital in Anhui Province.Data were collected using the General Information Questionnaire,Emotional Inhibition Scale,Chinese Perceived Stress Scale,10 item Conner Davidson Resilience Scale,and Family Care Index Questionnaire.Latent profile analysis was performed using Mplus 8.3,while univariate analysis and multivariate logistic regression analysis were conducted using SPSS 26.0.Results Valid questionnaires were collected from 336 participants(response rate 96.6％).Colorectal cancer patients with ostomies demonstrated a mean emotional inhibition score of 30.84±10.49.Latent profile analysis identified 3 distinct emotional inhibition patterns:high inhibition-suppressive type(31.25％),moderate inhibition-adaptive type(39.88％),and low inhibition-excessive type(28.87％).Gender,residential location,household income per capita,ostomy self-care proficiency,perceived stress levels,psychological resilience scores and family functioning status emerged as significant determinants of emotional inhibition heterogeneity among colorectal cancer patients with ostomies across distinct latent profiles(P＜0.05).Conclusion Colorectal cancer patients with ostomies exhibit significant heterogeneity in emotional inhibition profiles.Clinicians should conduct early identification of these latent classes through standardized assessments and develop tailored interventions to improve emotional inhibition outcomes in this clinical population.

Objective To explore and analyze the current situation of consumer health activation index and its influencing factors in hospitalized elderly patients with multiple chronic conditions,and to provide reference basis for clinical intervention.Methods Convenience sampling method was used to select 332 hospitalized elderly patients with multiple chronic conditions from a tertiary hospital in Hefei City from July to December 2024 as the study subjects,and general information questionnaire,Consumer Health Activation Index,Health Literacy Management Scale,Scale of the Elderly Self-neglcet(Rural),and Perceived Social Support Scale were used to investigate the current situation of consumer health activation index of hospitalized elderly patients with multiple chronic conditions.Univariate analysis and multiple linear regression analysis were employed to explore the influencing factors of consumer health activation index in patients with multiple chronic conditions.Results A total of 332 questionnaires were distributed,and 325 valid questionnaires were retrieved.The score of the consumer health activation index of hospitalized elderly patients with multiple chronic conditions was(61.68±15.12).The results of multiple linear regression showed that literacy,average monthly household income,number of chronic diseases,medication frequency,self-care ability,health literacy management,perceived social support and self-neglect were the influencing factors for the positivity of hospitalized elderly patients with multiple chronic conditions(P＜0.05),which could explain 59.9％of the variance of the consumer health activation index of hospitalized elderly patients with multiple chronic conditions.Conclusion The consumer health activation index of hospitalized elderly patients with multiple chronic conditions is at a low level.In the future,precise and systematic intervention measures for enhancing positive health status can be formulated by taking into account relevant influencing factors,in order to promote the transformation of patients from passive health to active health,and thereby facilitating positive aging.

Objective To explore the impact and threshold effect of productive engagement on the elderly patients with chronic diseases with demoralization syndrome,and to provide theoretical basis for further improving the elderly patients with chronic diseases with demoralization syndrome.Methods A total of 350 elderly patients with chronic diseases from July to December 2024 in a tertiary hospital in Hefei were selected as the research subjects by convenience sampling method,and the general information questionnaire,Productive Engagement Scale and Demoralization Scale-Ⅱ were used for investigation.Using multiple stratified regression analysis to explore the impact of participation in productive activities on dementia syndrome in elderly patients with chronic diseases,and conducting threshold effect analysis.Results 341 valid questionnaires were collected.The score of demoralization syndrome in elderly patients with chronic diseases was 14.00(11.00,17.00),and the score of productive engagement was 37.00(29.50,46.00).Stratified regression analysis showed that productive engagement could independently explain 8.7％of the variance(P＜0.05).Curve fitting and threshold effect analysis showed that the degree of demoralization syndrome was not affected when the score of productive engagement of elderly patients with chronic diseases was ≤27 points(P＞0.05),and it was decreased with the increasing of productive engagement score＞27 points(P＜0.001).Conclusion The demoralization syndrome of elderly patients with chronic diseases were in the middle level,and productive engagement is negatively correlated with demoralization syndrome.Healthcare professionals can improve the level of demoralization syndrome by encouraging elderly patients with chronic diseases to actively participate in productive engagement activities.

Objective To explore the correlation between triglyceride-glucose(TyG)index and hyperuricemia in men with normal fasting blood glucose(FPG)levels.Methods A total of 309 men with normal FPG who participated in a health examination at the Ninth Medical Center of the People's Liberation Army General Hospital in April 2024 were enrolled in this study.All the subjects were divided into the normal uric acid(NUA,n=218)group and the hyperuricemia(HUA,n=91)group according to serum uric acid(SUA)levels.Results Scr,TG,weight,SBP,DBP,BMI,ALT,AST,γ-GGT,and TyG index were higher in the HUA group than in the NUA group(P<0.05).Pearson and Spearman correlation analysis showed that SUA were positively correlated with Scr,eGFR,TG,weight,SBP,DBP,BMI,ALT,AST,γ-GGT and TyG(P<0.05),and negatively correlated with HDL-C(P<0.05).Logistic regression analysis showed that after adjusting for confounding factors,TyG index remained an important influencing factor for HUA.ROC curve analysis showed that the area under the ROC curve of TyG index predicting hyperuricemia in men with normal FPG was 0.665,with an cutoff value of 8.45.Conclusions TyG index in men with normal FPG are influencing factors for hyperuricemia,indicating that hyperuricemia has a close association with insulin resistance,and is an important component of metabolic syndrome.

Objective:To explore in depth the real experiences of preoperative prehabilitation among colorectal cancer patients undergoing stoma surgery, in order to provide a reference for the development of preoperative prehabilitation protocols.Methods:This study was a descriptive qualitative research. Using purposive sampling, 18 colorectal cancer ostomy patients who were hospitalized in the Department of Anorectal Surgery of the Third Affiliated Hospital of Anhui Medical University from March to August 2024 were selected for semi-structured in-depth interviews. Thematic analysis was conducted using content analysis methods.Results:The experiences of colorectal cancer ostomy patients with preoperative prehabilitation were summarized into four major themes: cognition and attitudes toward prehabilitation (willingness to actively participate, expectations for prehabilitation outcomes, psychological barriers), positive effects of prehabilitation (promoting postoperative recovery, improving adverse outcomes, enhancing stoma adaptation), challenges of prehabilitation (poor physical condition, negative emotions, inadequate preparation time) and suggestions and needs regarding prehabilitation (availability of information resources, refinement of individualized programs, need for psychological support) .Conclusions:Colorectal cancer ostomy patients generally hold a positive attitude toward preoperative prehabilitation, which helps accelerate postoperative recovery. Medical staff should enhance patient compliance and engagement by constructing an information support and shared decision-making model. Meanwhile, a tripartite psychological support system involving hospitals, communities, and families should be established, and a personalized and tiered prehabilitation program should be developed to ensure the safe and effective implementation of prehabilitation protocols, thereby improving the overall effectiveness of prehabilitation.

Objective:To identify the risk factors for postoperative cognitive dysfunction (POCD) in elderly patients undergoing hip fracture surgery.Methods:In this case-control study, 6 differentially expressed genes were screened through gene expression database analysis and protein-protein interaction network analysis: cytochrome c oxidase subunit 7C (COX7C), ubiquinol-cytochrome c reductase complex III subunit VII (UQCRQ), cytochrome c oxidase subunit 7A2 (COX7A2), translocase of outer mitochondrial membrane 7, ubiquinone oxidoreductase subunit S5, and ribosomal protein L31. Elderly patients who underwent hip fracture surgery at Anhui No. 2 Provincial People′s Hospital from April 2022 to April 2024 were recruited. Based on the results of the Mini-Mental State Examination, the patients were divided into POCD group ( n=51) and non-POCD (NPOCD) group ( n=53). The expression of differentially expressed genes in the peripheral blood was detected using the fluorescent quantitative real-time polymerase chain reaction. The 6 differentially expressed genes were subjected to a difference test, and genes with P<0.05 were included in the binary logistic regression analysis to screen for risk factors for POCD. Results:The expression of COX7C, UQCRQ and COX7A2 was significantly down-regulated in POCD group compared with non-POCD group ( P<0.05). The results of logistic regression showed that the low-expression COX7C ( OR=1.926, 95% confidence interval [ CI] 1.604-5.264, P=0.022), UQCRQ ( OR=3.023, 95% CI 1.966-7.156, P=0.001), and COX7A2 ( OR=1.744, 95% CI 1.479-6.127, P=0.013) in peripheral blood were independent risk factors for the occurrence of POCD in elderly patients. Conclusions:Low-expression COX7C, UQCRQ and COX7A2 are risk factors for POCD in elderly patients undergoing hip fracture surgery.

Objective To analyze the deployment of policy instruments and the distribution of stakeholder engagement in provincial policies on nursing assistant training and management.Methods The relevant policy texts on nursing assistant training and management were systematically searched and collected from the official websites of provincial governments and their direct departments,CNKI,and the PKU Law Database.A two-dimensional framework of policy instruments-stakeholder was constructed,and the content analysis was used to classify,encode and quantify policy clauses.Results The study encompassed 20 provincial-level nursing assistant training and management policies,yielding a total of 359 codes.Within the policy instruments dimension,environmental,supply,and demand instruments constituted 66.30％,28.97％,and 4.74％,respectively.In terms of stakeholders,the management-side accounted for 56.55％,providers and trainers for 18.11％each,partners for 5.01％,and demand-side for 2.23％.Both management-side and trainers engaged with 3 policy instruments,providers with 2,and partners and demand-side with one each.Conclusion In the provincial nursing assistant training and management policies,there are differences in the deployment of policy instruments,and the distribution of stakeholders is uneven.Managers should pay attention to publicity guidance and platform construction,improve incentive mechanisms and training programs,and innovate cooperation models with stakeholders,as well as strengthen communication and exchange.

Objective To evaluate the clinical efficacy and safety of Wuwei Wentong Chubi Capsules in patients with rheumatoid arthritis of cold and dampness obstruction syndrome under real world based on clinical medical record information system.Methods Medical history,symptoms and medication information of hospitalized patients with rheumatoid arthritis of cold and dampness obstruction syndrome in the Department of Rheumatology,First Affiliated Hospital of Anhui University of Chinese Medicine were collected from January 2020 to March 2024.According to whether to use Wuwei Wentong Chubi Capsules,patients were divided into treatment group(304 cases)and control group(378 cases).A database was established after improving the information and indicators of two groups of patients.TCM syndrome scores,disease activity scores and laboratory indicators of both groups were compared.Results Compared with before treatment,TCM syndrome score(severe joint pain,morning stiffness,swelling,worsening pain when encountering cold,poor joint flexion and extension,eating less and eating more,lack of breath and lazy speech,weak mouth and thirst,heavy head and body fatigue,loose stools and fatigue)in the treatment group significantly decreased after treatment(P<0.05),the disease activity score,erythrocyte sedimentation rate(ESR),high-sensitivity C-reactive protein(hs-CRP),rheumatoid factor(RF),complement(C3,C4),immunoglobulin(Ig)G,IgM,anti cyclic citrullinated peptide antibody(anti CCP antibody),thyroid stimulating hormone(TSH),anti thyroglobulin antibody(TGAb),and anti thyroid peroxidase antibody(TPOAb)were significantly reduced(P<0.05),and free triiodothyronine(FT3)and free tetraiodothyronine(FT4)increased(P<0.05);after treatment,TCM syndrome score(severe joint pain,morning stiffness,swelling,fatigue and weakness)in the control group significantly decreased,and the disease activity score,ESR,hs-CRP,RF,the levels of anti CCP antibodies,TGAb,and TPOAb were significantly reduced(P<0.05);compared with the control group after treatment,TCM syndrome scores in the treatment group showed more significant improvement(P<0.05)in terms of joint morning stiffness,swelling,poor flexion and extension,exacerbation of cold exposure,eating less and eating more,lack of breath and speech,weak and dry mouth,heavy head and body fatigue and fatigue.Disease activity score,ESR,hs-CRP,RF,the levels of anti CCP antibodies,C3,IgG,TSH and TPOAb decreased more significantly,while FT3 and FT4 increased more significantly(P<0.05);both groups had normal blood routine and liver and kidney function before and after treatment,with no significant difference(P>0.05).According to the analysis of association rules,after using Wuwei Wentong Chubi Capsules,there was a significant improvement in ESR,hs-CRP,RF,C3,IgG,TSH,FT4 and TPOAb indicators,with a degree of improvement greater than 1,indicating a strong association.Conclusion Wuwei Wentong Chubi Capsules can effectively improve the cold and dampness obstruction syndrome and spleen deficiency dampness excess syndrome in patients with rheumatoid arthritis cold dampness obstruction syndrome,reduce disease activity index and immune inflammatory indicators,and have a certain regulatory effect on thyroid immune dysfunction and the clinical application has good safety.
