Objective To evaluate the clinical efficacy of Shengyang Juxian Acupuncture Method(mainly with the function of elevating yang to cure prolapse syndrome)combined with neuromuscular electrical stimulation(NMES)in treating postpartum diastasis recti abdominis(DRA).Methods A total of 200 patients diagnosed with postpartum DRA at Tangshan Maternal and Child Health Hospital from March 2022 to December 2022 were enrolled and randomly divided into an observation group and a control group using a number list,with 100 cases in each group.The control group received NMES therapy,while the observation group underwent additional Shengyang Juxian Acupuncture Method for 1 month.Clinical efficacy was assessed by comparing inter-recti distance(IRD),abdominal circumference,pelvic floor muscle strength(type Ⅰ and Ⅱ muscle fibers).The Visual Analogue Scale(VAS)scores for low back pain,and 36-Item Short Form Health Survey(SF-36)scores were compared before and after treatment.Results(1)After treatment,both groups showed significant improvements in IRD and abdominal circumference(P＜0.05),with superior outcomes in the observation group,the difference being statistically significant(P＜0.05).(2)Pelvic floor muscle strength of type Ⅰ and Ⅱ fibers markedly improved in both groups,with greater enhancement in the observation group,the difference being statistically significant(P＜0.05).(3)VAS and SF-36 scores significantly improved in both groups(P＜0.05),yet the observation group demonstrated more pronounced improvements,the difference being statistically significant(P＜0.05).(4)The total effective rate was 96.00%(96/100)in the observation group versus 80.00%(80/100)in the control group,with superior outcomes in the observation group,the difference being statistically significant(P＜0.05).Conclusion Shengyang Juxian Acupuncture Method combined with NMES effectively promotes rectus abdominis recovery,enhances pelvic floor muscle strength,alleviates low back pain,and improves quality of life in postpartum DRA patients,demonstrating significant clinical efficacy.

Objective To analyze the relationships of serum microRNA(miR)-15,miR-16,miR-17-5p,and triglyceride-glucose index(TyG)with severity and prognosis of acute ischemic stroke(AIS).Methods A total of 136 AIS patients admitted to the Shijiazhuang Hospital of Traditional Chi-nese Medicine from April 2023 to April 2024 were enrolled.Prognosis and disease severity were as-sessed using modified Rankin Scale(mRS)and National Institutes of Health Stroke Scale(NIHSS),respectively.Based on mRS scores at 3 months post-onset,patients were divided into poor prognosis group(mRS Score＞2,n=42)and good prognosis group(mRS Score ≤ 2,n=94).Serum levels of miR-15,miR-16,miR-17-5p,TyG,and NIHSS scores were compared between two groups.Pearson correlation analysis was used to evaluate associations of these biomarkers with NIHSS scores.Logistic regression was used to identify risk factors for poor prognosis,while receiver operating characteristic(ROC)curves was applied to assess the predictive value of individual and combined biomarkers.Results The poor prognosis group exhibited significantly higher serum miR-15,miR-16,miR-17-5p,TyG,and NIHSS scores than the good prognosis group(t=8.634,13.171,29.018,2.687,26.432;P＜0.05).Positive correlations were observed between these biomarkers and NIHSS scores(r=0.472,0.449,0.492,0.437;P＜0.05).Logistic regression identified miR-15[OR(95％CI):3.526(2.628 to 5.859)],miR-16[1.976(1.226 to 3.017)],miR-17-5p[1.828(1.294 to 3.428)],NIHSS score[1.787(1.105 to 2.896)],TyG[1.886(1.233 to 3.284)],and homo-cysteine[1.906(1.252 to 3.794)]as independent risk factors for poor prognosis(P＜0.05).ROC analysis demonstrated that the combined model(miR-15,miR-16,miR-17-5p,TyG,and NIHSS)achieved superior predictive performance(AUC=0.877;95％CI,0.820 to 0.948)com-pared to individual biomarkers(Z＞2.527,P＜0.05).Conclusion Elevated serum miR-15,miR-16,miR-17-5p,and TyG are closely associated with AIS severity and poor prognosis.The above indicators combined with NIHSS scores can enhance predictive accuracy for unfavorable outcomes.

[摘 要] 目的：探讨溶瘤新城疫病毒（NDV）对IL-6诱导的人胶质母细胞瘤U87MG细胞增殖、迁移和侵袭的作用及其可能的机制。方法：将U87MG细胞分为对照组、IL-6组、NDV组、NDV+IL-6组，其中IL-6组与NDV+IL-6组用75 ng/mL IL-6预处理1 h，其余组用DMEM预处理1 h，后分别用DMEM、75 ng/mL IL-6、1 HU NDV、1 HU NDV+75 ng/mL IL-6处理24 h。MTT法、细胞划痕实验和Transwell侵袭实验分别检测IL-6、NDV对U87MG细胞增殖、迁移和侵袭的影响，WB法检测各组细胞JAK2、p-JAK2、STAT3、p-STAT3和MMP2蛋白的表达水平。结果：与对照组相比，IL-6组细胞迁移率显著升高（P<0.05），侵袭细胞数目显著增多（P<0.01）；与IL-6组相比，NDV+IL-6组U87MG细胞增殖率显著降低（P<0.05），细胞迁移率和侵袭细胞数目均显著降低（均P<0.01）。WB实验结果显示，与对照组相比，IL-6组p-STAT3/STAT3比值显著升高（P<0.01），NDV组p-JAK2/JAK2、p-STAT3/STAT3比值显著降低（P<0.05，P<0.01），MMP-2蛋白表达量显著降低（P<0.01）；与IL-6组相比，NDV+IL-6组p-STAT3/STAT3比值、MMP-2蛋白表达量均显著降低（均P<0.05）。结论：NDV能抑制IL-6对人脑胶质瘤U87MG细胞迁移和侵袭的诱导作用，其机制可能与JAK2/STAT3信号通路的参与调控有关。

Objective:To investigate the correlation between serum sclerostin and sarcopenia-related indicators in chronic kidney disease (CKD) patients, and to find biomarkers and potential therapeutic targets that can take into account both osteoporosis and sarcopenia.Methods:It was a single-centre cross-sectional study. The clinical data of CKD stage 5 patients undergoing maintenance hemodialysis regularly and CKD stage 1-5 non-dialysis inpatients in the Hemodialysis Centre of Guangzhou Red Cross Hospital from March 2021 to March 2023 were collected retrospectively. The enzyme-linked immunosorbent assay was used to detect the level of serum sclerostin. The anthropometric data such as height, weight, upper arm circumference, upper arm muscle circumference, skinfold thickness, pinch strength and handgrip strength were measured. Body composition analyzer was used to measure the body composition. The patients were divided into CKD stage 1-3 group, CKD stage 4-5 group, and stage 5 hemodialysis group. One-way ANOVA, Kruskal-Wallis H test, and chi-square test were used to compare the differences of demographics and clinical characteristics in different stages of CKD. Spearman correlation analysis and multiple linear stepwise regression analysis were utilized to analyze the correlation between serum sclerostin and sarcopenia-related indicators in CKD patients. Results:The study included 104 patients with CKD stage 5 hemodialysis and 104 patients with CKD stage 1-5 non-dialysis patients, with age of (61.8±13.7) years old and 114 males (54.8%). There were 89 patients (42.8%) with diabetic nephropathy and 67 patients (32.2%) with sarcopenia. As renal injury progressed, serum sclerostin levels were 0.4 (0.3, 0.9) ng/L, 0.5 (0.3, 1.1) ng/L, and 1.1 (0.6, 2.3) ng/L in patients with CKD stage 1-3, stage 4-5, and stage 5 undergoing hemodialysis ( χ2=8.934, P<0.001), and the prevalence of sarcopenia was 16.4% (10/61), 34.9% (15/43), and 40.4% (42/104) ( χ2=10.312, P=0.006), respectively. Spearman correlation analysis showed that serum sclerostin was negatively correlated with estimated glomerular filtration rate ( r=-0.314, P<0.001), pinch strength ( r=-0.229, P=0.007), skinfold thickness ( r=-0.254, P<0.001), appendicular skeletal muscle index ( r=-0.169, P=0.010), body cell mass ( r=-0.174, P=0.020), and phase angle ( r=-0.264, P<0.001), and positively correlated with serum phosphorus ( r=0.227, P=0.002) and intact parathyroid hormone ( r=0.297, P<0.001). Multiple linear stepwise regression analysis showed that lg[appendicular skeletal muscle index] was negatively correlated with male ( β=0.330, t=5.675, P<0.001) and serum sclerostin ( β=-0.125, t=-2.143, P=0.033), and positively correlated with body mass index ( β=0.474, t=8.090, P<0.001). Conclusion:Serum sclerostin can be used as a good index and a potential therapeutic target for sarcopenia in CKD patients.

Objective To construct an in vitro"metabolic memory"cell model of HT-22 mouse hippocampal neurons induced by high glucose,and to investigate the effect of"metabolic memory"on apoptosis and histone acetylation in HT-22 cells.Methods HT-22 cells were cultured in high glucose medium(glucose concentration was 55 mmol/L)and conventional glucose medium(glucose concentration was 25 mmol/L),and cells were divided into the control group(NG 4,6 and 8 groups,25 mmol/L glucose was cultured for 4,6 and 8 days,respectively),the high glucose group(HG 4,6 and 8 groups,respectively)and the metabolic memory group(HG2NG2,HG2NG4,HG2NG6,HG4NG2 and HG4NG4 groups,high glucose culture for 2 days to 25 mmol/L glucose culture for 2,4 or 6 days,high glucose culture for 4 days to 25 mmol/L glucose culture for 2 or 4 days).Cell viability was detected by CCK-8 method.The release of lactate dehydrogenase(LDH)in cell culture supernatant was detected,and the optimal time to establish a"metabolic memory"model was selected.Subsequently,cells were divided into the NG4 group,the NG8 group,the HG4 group,the HG4NG4 group and the HG8 group,and the cell morphology of each group was observed by optical microscope.The apoptosis rate was detected by flow cytometry.The activities of deacetylase(HDAC)and histone acetyltransferase(HAT)were detected by enzyme-linked immunosorbent assay(ELISA).Western blot assay was used to detect expression levels of histone deacetylase 4(HDAC4),B lymphocyte tumor 2(Bcl-2),Bcl-2 related X protein(Bax)and Caspase-3 protein.Results The HG4NG4 group was the ideal cell model with high glucose metabolic memory.Cells of the NG4 group and the NG8 group were interwoven into a dense network,growing well,with spindle shaped cells and distinct synaptic structures.However,in the HG4 group and the HG8 group,the cell body became round,synaptic structure disappeared and growth was inhibited.In the HG4NG4 group,the number of cells increased but their morphology was damaged.Results of flow cytometry showed that compared with the NG8 group,the apoptosis rates were significantly increased in the HG8 group and the HG4NG4 group(P＜0.05).ELISA results showed that compared with the NG8 group,the expression levels of HDAC4,Bax,and Caspase-3 proteins increased in the HG8 group and the HG4NG4 group,while the expression level of Bcl-2 protein significantly decreased(P＜0.05).Compared with the HG8 group,there were no significant differences in protein expression levels of HAT and HDAC in the HG4NG4 group.Western blot reslts showed that compared with the NG8 group,the levels of HDAC4,Bax and Caspase-3 protein increased in the HG8 group and the HG4NG4 group(P＜0.05).Compared with the HG8 group,there were no significant differences in protein expression levels in the HG4NG4 group.Conclusion HT-22 mouse hippocampal neurons cultured with 55mmol/L high glucose for 4 days,and then cultured with 25 mmol/L glucose for 4 days are the ideal"metabolic memory"cell model.The mechanism may be related to the increased activity of HDAC,HAT and HDAC4 expression in the hyperglycemic model.

Lipid and glucose metabolism play crucial roles in maintaining energy homeostasis, and their dysregulation can lead to the development of metabolic disorders, such as obesity and diabetes. Studies indicate that the skeleton, involved in lipid and glucose metabolism, functions as an endocrine organ, regulating systemic metabolism through bone-derived molecules. Sclerostin is a protein mainly produced by osteocytes, possessing the ability to inhibit bone formation, and its antibodies have become therapeutic targets for treating osteoporosis. Recent evidence suggests that sclerostin also plays a role in lipid and glucose metabolism disorders. Therefore, through summarizing in vitro and in vivo researches, this article reviews the role of sclerostin in lipid and glucose metabolism, its relationship with obesity and diabetes, and potential role in the treatment of metabolic diseases in the future.

Objective:To evaluate the disinfection effect of high-energy pulse ultraviolet disinfection equipment in medical institution settings.Methods:The disinfection effect was evaluated through field tests and laboratory tests. Among them, 135 high-frequency contact points were selected from nine departments in the field test. Samples were collected before and after disinfection, and the disinfection effects of 75% alcohol wipes wiping disinfection, high-energy pulse ultraviolet disinfection robot disinfection and high-energy pulse ultraviolet handheld disinfection instrument were compared. In the laboratory test, 30 infected areas of the simulated test table were exposed to vertical ultraviolet irradiation and the bacterial-killing rate before and after disinfection was calculated.Results:In the field test, the bacteria-killing rates of 75% alcohol wipes, high-energy pulse ultraviolet disinfection robot and high-energy pulse ultraviolet handheld disinfection instrument were 94.99%, 91.53% and 95.94%, respectively, and the difference was statistically significant. The disinfection effect of the high-energy pulse ultraviolet handheld disinfection instrument was better than that of the high-energy pulse ultraviolet disinfection robot ( P values <0.05). In the laboratory test, the killing log value of Staphylococcus aureus and Escherichia coli on the carrier were both greater than 3.00. In the simulated field test, the killing log value of Staphylococcus aureus on the surface samples were 4.99. Conclusion:Both the high-energy pulse ultraviolet handheld disinfection instrument and the high-energy pulse ultraviolet disinfection robot have good disinfection effects, which are similar to the disinfection effects of conventional 75% alcohol wipes.

Objective:To understand the prevalence of major chronic diseases of diabetes, cardiovascular disease and malignant tumor in people living with HIV in Taizhou.Methods:The data were collected from China Information System for Disease Control and Prevention and Taizhou Chronic Disease Information Management System. A total of 5 126 people living HIV under follow-up in Taizhou from 1998 to 2022 were included in the analysis. Software SAS 9.4 was used for χ 2 test, trend analysis and logistic regression analysis. Results:In the 5 126 people living with HIV, the reported prevalence rates of diabetes,cardiovascular disease and malignant tumor were 10.28% (527/5 126),3.98% (204/5 126) and 6.01% (308/5 126), respectively. 37.00% (195/527) and 48.58% (256/527), 40.20% (82/204) and 48.53% (99/204), 37.66% (116/308) and 48.38% (149/308) were diagnosed as diabetes, cardiovascular disease and malignant tumor before and after confirmation of HIV infection. From 2013 to 2022, the proportion of HIV infected people diagnosed with diabetes, cardiovascular disease and malignant tumor after confirmation increased (trend χ2=79.98, P<0.001; trend χ2=17.44, P<0.001; trend χ2=32.06, P<0.001). Based on the analysis on the factors for complicated chronic diseases in people living with HIV, it was found that women under 60 years old (a OR=0.66, 95% CI: 0.50-0.86) and those with access to antiviral treatment for >5 years before 2016 (a OR=0.54,95% CI:0.37-0.78) were less likely to develop complicated chronic diseases, and those under 60 years old with initial CD4 +T lymphocytes counts <200 cells/μl (a OR=1.32, 95% CI: 1.02-1.70), those aged 40-49 and 50-59 years (a OR=2.88, 95% CI:2.20-3.79; a OR=5.43, 95% CI: 4.10-7.21) as well as those without a record of treatment medication use after 2016 (a OR=1.95,95% CI:1.20-3.16) were more likely to develop complicated chronic diseases. The probability of developing complicated chronic diseases might increase with age in people living with HIV. Conclusions:From 1998 to 2022, there was a certain proportion of complicated chronic diseases among HIV infected individuals in Taizhou, and the proportion of diagnosed cases increased after HIV infection was confirmed. It is necessary to conduct early chronic disease screening, behavior intervention and standardized management in people living with HIV.

Objective:To systematically evaluate the incidence of psychological dysfunction in critically ill patients with post-intensive care syndrome (PICS) .Methods:Computer search was conducted on CNKI, Wanfang Database, VIP, SinoMed, PubMed, Embase, Cochrane Library, Web of Science, CINAHL and Scopus for literature related to the incidence of psychological dysfunction in critically ill PICS patients, and the search period was from establishment of the databases to April 2023. Data extraction and quality evaluation were carried out for the included literatures and Stata 17.0 software was used for meta-analysis.Results:A total of 32 articles were included, with a total of 496 399 patients. Meta-analysis results showed that the incidence of psychological dysfunction in critically ill PICS patients was 31.0% (95% CI: 26%-35%). Subgroup analysis results showed that the incidence of psychological dysfunction in PICS patients was higher in foreign literature, literature on virus-infected ICU patients, and literature published from 2017 to 2023, with incidence of 32.5% (95% CI: 25%-40%), 34.0% (95% CI: 29%-39%), and 32.8% (95% CI: 28%-38%), respectively. Conclusions:The incidence of psychological dysfunction in critically ill patients with post-intensive care syndrome is relatively high. Medical staff should pay more attention to the psychological health of post-ICU patients, strengthen screening of psychological dysfunction in critically ill patients with post-intensive care syndrome and give timely targeted intervention measures.
